BACKGROUND: The aim of the study was to compare health-related quality of life (QoL) and oncological outcome between gynaecological cancer patients undergoing pelvic exenteration (PE) and extended pelvic exenteration (EPE). EPEs were defined as extensive procedures including, in addition to standard PE extent, the resection of internal, external, or common iliac vessels; pelvic side-wall muscles; large pelvic nerves (sciatic or femoral); and/or pelvic bones. METHODS: Data from 74 patients who underwent PE (42) or EPE (32) between 2004 and 2019 at a single tertiary gynae-oncology centre in Prague were analysed. QoL assessment was performed using EORTC QLQ-C30, EORTC CX-24, and QOLPEX questionnaires specifically developed for patients after (E)PE. RESULTS: No significant differences in survival were observed between the groups (P > 0.999), with median overall and disease-specific survival in the whole cohort of 45 and 49 months, respectively. Thirty-one survivors participated in the QoL surveys (20 PE, 11 EPE). No significant differences were observed in global health status (P = 0.951) or in any of the functional scales. The groups were not differing in therapy satisfaction (P = 0.502), and both expressed similar, high willingness to undergo treatment again if they were to decide again (P = 0.317). CONCLUSIONS: EPEs had post-treatment QoL and oncological outcome comparable to traditional PE. These procedures offer a potentially curative treatment option for patients with persistent or recurrent pelvic tumour invading into pelvic wall structures without further compromise of patients ́ QoL.

The term 'out-of-the-box surgery' in gynecologic oncology was recently coined to describe the resection of tumor growing out of the endopelvic cavity. In the specific case of pelvic sidewall involvement, a laterally extended pelvic resection may be required. As previously defined by Höckel, this resection requires the en bloc removal of structures including the pelvic sidewall muscles, bones, nerves, and/or major vessels. This complex radical procedure leads to tumor-free margins in more than 75% of the patients, with reliable functional results. The rate of recurrence and overall survival are directly correlated with clear resection margins. Progress in imaging, surgical techniques, and perioperative care currently offer the opportunity to attempt surgical curative resection in selected patients for whom palliative therapy was the only alternative. However, the procedure is associated with a high rate of major postoperative complications affecting up to 60% of patients. Multidisciplinary expert centers are the most likely to achieve this complex surgery with favorable oncological outcomes. The aim of this review is to summarize the key issues of out-of-the-box surgery in gynecologic cancer.
Pánevní exenterace (PE) je specifická, komplexní a rozsáhlá operace umožňující erudovanému týmu při vhodné selekci pacientky s pánevní recidivou dosáhnout jejího vyléčení. Výkon je radikalitou a specificitou potřebných chirurgických zkušeností výjimečný a vyžaduje multioborový tým a centralizaci.
Pelvic exenteration is specific, complex and extensive operation which allows experienced surgical team with a good patient selection with a pelvic relaps to reach zero residuum resection. Radicality and specificity of such a procedure requires specific surgical skills, multidisciplinary team and centralisation of patients.

Nádory uretry jsou v urologii vzácné a mucinózní adenokarcinom uretry je velmi raritní. Kazuistika popisuje případ tohoto vzácného nádoru, jeho symptomatiku, diagnostiku a chirurgické řešení. Poukazuje na to, že je třeba myslet na možnost výskytu tumoru i v méně obvyklých lokalizacích a že je třeba brát v úvahu při diferenciální diagnostice i relativně málo časté diagnózy. I při lokálně pokročilém tumoru uretry je v některých případech možné radikální chirurgické řešení a pacienti z něho mohou profitovat.
Tumors of the urethra are rare in urology and the mucinous adenocarcinoma of the urethra is very unique. The case report describesa patient with this rare tumor, its symptom, diagnosis and surgical treatment. It points out the need to think about the possibilityof tumor occurrence even in less common localizations and that relatively uncommon diagnosis should be taken into account fordifferential diagnosis. Even in a locally advanced urethra tumor, some surgical solutions are possible and patients can benefit from it.

Úvod: Pánevní exenterace je základem komplexní léčby pokročilých nádorů malé pánve. Taktika vlastní operace je známa již řadu let a je poměrně konsenzuální. Diskutována však dodnes zůstává otázka rozsahu lymfadenektomie. Cílem práce je v přehledném sdělení shrnout strategii výkonu a potřebný rozsah lymfadenektomie. Metody: Přehledová práce, vycházející z dostupné literatury a zkušeností z retrospektivní analýzy vlastního souboru 63 nemocných, operovaných na Chirurgické klinice 1. LF UK a TN v období let 1999–2015. Závěr: Práce popisuje indikace k pánevní exenteraci, strategii výkonu a potřebný rozsah lymfadenektomie.
Introduction: Pelvic exenteration is an essential part of complex treatment of advanced tumours of the small pelvis. The strategy of surgery is well known and consensual. However, the optimal extent of lymphadenectomy is still under discussion. The aim of this paper is to summarize the strategy of surgery and the extent of lymphadenectomy. Methods: Review of the literature and retrospective analysis of 63 patients of our cohort, operated on at the Department of Surgery of the First Faculty of Medicine, Charles University and Thomayer Hospital between 1999–2015. Conclusion: The paper describes indications for pelvic exenteration, the strategy of surgery and the necessary extent of lymphadenectomy.

Prezentujeme kazuistiku pacientky s lokální recidivou dlaždicobuněčného karcinomu děložního čípku po selhání předchozí léčby. V terénu po radikální hysterektomii a adjuvantní radioterapii za rozvíjející se sepse jsme provedli salvage totální exenteraci pánve. Pacientka je po 7 letech v onkologické remisi a vede kvalitní život.
This article is to report on the case of a female patient with local recurrence of squamous cell carcinoma of uterine cervix after response to prior therapy had failed. On the ground of radical hysterectomy and adjuvant radiotherapy and with developing sepsis, salvage total pelvic exenteration was performed. The patient has been in cancer remission for 7 years and she leads a quality life.

OBJECTIVE: To describe the technique and report experiences with pelvic floor reconstruction by modified rectus abdominis myoperitoneal (MRAM) flap after extensive pelvic procedures. METHODS: Surgical technique of MRAM harvest and transposition is carefully described. The patients in whom pelvic floor reconstruction with MRAM after either infralevator pelvic exenteration and/or extended lateral pelvic sidewall excision was carried out were enrolled into the study (MRAM group, n=16). Surgical data, post-operative morbidity, and disease status were retrospectively assessed. The results were compared with a historical cohort of patients, in whom an exenterative procedure without pelvic floor reconstruction was performed at the same institution (control group, n=24). RESULTS: Both groups were balanced in age, BMI, tumor types, and previous treatment. Substantially less patients from the MRAM group required reoperation within 60days of the surgery (25% vs. 50%) which was due to much lower rate of complications potentially related to empty pelvis syndrome (1 vs. 7 reoperations) (p=0.114). Late post-operative complication rate was substantially lower in the MRAM group (any grade: 79% vs. 44%; grade≥3: 37% vs. 6%) (p=0.041). The performance status 6months after the surgery was ≤1 in the majority of patients in MRAM (81%) while in only 38% of patients from the control group (p=0.027). There was one incisional hernia in MRAM group while three cases were reported in the controls. CONCLUSIONS: Pelvic floor reconstruction by MRAM in patients after pelvic exenterative procedures is associated with a substantial decrease in postoperative complications that are potentially related to empty pelvis syndrome.
