Cíl: Posoudit léčbu suprakondylických zlomenin distálního humeru u souboru dětí s tímto poraněním, léčených v KNTB Zlín. Zhodnotit výsledky léčby, včetně procentuálního zastoupení jednotlivých komplikací. Úvod: Shrnuje současný pohled na problematiku suprakondylické zlomeniny. Metodika: Retrospektivní studie souboru dětí, ošetřených na Traumatologickém oddělení KNTB Zlín v letech 2014-2018 s tímto poraněním. Výsledky: V daném období bylo ošetřeno celkem 228 pacientů, z toho čtyři repozicí a sádrovou fixací v celkové anestezii a 95 repozicí a osteosyntézou retrográdně zavedenými Ki dráty. Většina pacientů (94,7 %) byla operována urgentně (do šesti hodin od příchodu do ambulance). Preferovanou metodou na našem pracovišti je zkřížená perkutánní fixace Ki dráty (CRCPP). U 12 dětí (12,6 % operovaných) bylo nutno přistoupit k repozici otevřené, prováděné převážně z limitovaného zadního přístupu. Ve skupině operovaných dětí bylo zaznamenáno 20 případů (21,5 %) neurologických komplikací, z toho nejčastěji poranění n. ulnaris (12), případně n. ulnaris i n. medianus (4), nebo samostatně n. medianus (3), u dvou případů byla vzhledem k přetrvávající poruše prokrvení periferie indikována otevřená repozice s revizí NC svazku z předního přístupu. Žádné z ošetřených dětí nevykazovalo závažné trvalé následky. Nejčastějším následkem bylo lehké omezení hybnosti, vyjádřené lehkým omezením flexe, lehkou hyperextenzí lokte, nebo kombinací obojího (celkově 14 případů). V celkovém souboru pacientů jsme pozorovali dva případy poruchy růstu ve smyslu cubitus varus. Nebyl zaznamenán kompartment syndrom ani Volkmannova kontraktura končetiny. Závěr: Suprakondylická zlomenina humeru je častý úraz s relativně vysokým rizikem komplikací. Některé otázky ohledně optimálního managementu této zlomeniny stále nebyly přesvědčivě zodpovězeny a zaslouží si další zkoumání.
Objective: To assess the treatment of supracondylar fractures of the distal humerus in children, which were treated in Tomas Bata Hospital, Zlin. To evaluate the results in the context of contemporary studies. Introduction: Study presents the supracondylar fracture. Method: Retrospective study of a group of 228 patients with this type of fracture, treated between the years 2014-2018 in Traumatology department of Tomas Bata Hospital, Zlin. Results: In this group, 4 children underwent closed reduction and splinting in general anesthesia, 95 were operated on, from which 94,5 % urgently (less than 6 hours after first contact with the patient). The prefered method was closed reduction and percutaneous pinning by crossed Ki wires (CRCPP). In 12 cases (12,6 % of the operations) it was necessary to perform an open reduction, mostly from the limited posterior access. In the group of operatively treated children, there have been noticed 20 cases of some neural injury, mostly n. ulnaris (12), both n. ulnaris and medianus (4), n. medianus only (3). There were 2 cases of pale-pulsless hand after the reduction, which led to the revision of the neurovascular structures from the anterior access. None of the children displayed permanent consequences envolving their everyday lives. In minority of cases (14) at the end of the follow-up, persisted slight deficiency of the flexion, or slight hyperextension of the elbow, or the combination of both. We have recorded 2 cases of the cubitus varus deformity. In this cohort, there was no incidence of the compartment syndrome nor the Volkmann contracture of the extremity. Conclusion: Supracondylar fracture is a frequent injury with relatively high risk of complications. Some questions of the optimal treatment have still not been answered and need further research.
A rare case of a fracture of the anatomical neck of the humerus associated with a posterior dislocation of the shoulder in a 43-year-old man is presented. It was managed by immediate surgery using locking plate osteosynthesis. The shoulder was immobilised for 5 weeks in a Gilchrist brace. The outcome was very good with a full range of motion and a stable shoulder at 1 year after injury, with no signs of avascular necrosis of the humeral head. The aetiology of this injury and possibilities of its treatment are discussed. Key words: shoulder dislocation, humeral fractures.

PURPOSE OF THE STUDY Evaluation of outcome after surgical treatment of humerus metastases with a focus on tumour and patient derived factors, timing and strategy of intervention, surgical outcome and complications. MATERIAL AND METHODS Sixty-fie patients with a mean age of 64.3 years (range 25-89) with 66 metastases of the humerus were surgically treated in a 7-year time-period and retrospectively reviewed. RESULTS Renal cell carcinoma and breast cancer were the most abundant types of primary tumour. The mean time from diagnosis of primary tumour to fist metastasis was 14.5 months (range 0-173). The mean time from diagnosis of metastasis to surgery was 21.4 months (range 0-173). 38/28 intramedullary nails/locking plates were used for 58/8 manifest/impending pathological fractures. Mean cumulative survival was 16.3 months and implant failure rate was 6.1% with a mean time from initial surgery to revision of 22.2-20.6 months. CONCLUSIONS Our data indicate that treatment with intramedullary fiation or cement augmented plate osteosynthesis is successful for the vast majority of patients, but thorough clinical evaluation and precise decision making adapted to the patient's estimated life expectancy must be applied to avoid overtreatment or risk of implant failure. PURPOSE OF THE STUDY: The aim of this study was to evaluate comparatively the outcomes of pediatric displaced supracondylar fractures of humerus which were treated with either closed reduction and percutaneous pinning (CRPP) or open reduction and percutaneous pinning (ORPP). MATERIAL AND METHODS: The study included 100 children with displaced supracondylar fractures of the humerus, who were treated with either CRPP (group 1) or ORPP (group 2); the numbers of subjects in the study groups were equal. The treatment outcomes were assessed clinically and radiologically. RESULTS: Ninety-six percent excellent or good cosmetic results were achieved in both groups, and 94% / 90% excellent or good functional results were achieved in groups 1 and 2 (respectively). There was no signifiant difference between Baumann and humero-capitellar angles of intact and operated sides in both groups, but there was an average carrying angle difference of 2,96 degrees in the group 1 and 1,52 degrees in the group 2 and these differences were statistically signifiant. Five cases (10%) from each group had superfiial pin tract infection. Hypertrophic incision scar occurred in 6 (12%) patients performed ORPP. CONCLUSION: Both CRPP and ORPP are successful treatment methods in the management of non-complicated and non-comminuted displaced supracondylar fractures of the humerus in pediatric ages and their outcomes are similar. Incision scar and the long duration of operation are the disadvantages of open surgery. For fiing the fracture, placement of two K-wires from the medial and lateral aspects which cross each other is enough to achieve a good stability. In ORPP practices, lateral incision is a simple and reliable approach despite of the dissatisfying scar tissue formation.

BACKGROUND AND PURPOSE: Long term outcome of the treatment of displaced complex fractures of humeral head is rare in the literature especially in greater cohorts. Main purpose of our study was the assessment of long term results of intramedullary nailing of 3-4 part fractures. PATIENTS AND METHODS: 137 patients with 137 three or four-part fractures of the humeral head treated by intramedullary nailing were reassessed after an average follow-up time of 57 (27-93) months. The whole cohort consisted of 85 three-part fractures, 38 four-part fractures and 14 fracture-dislocations. Radiographic and clinical outcome - absolute Constant score (CS(abs)) and relative Constant score (compared to the contralateral side - CS(rel)) were evaluated. The rate of complications was recorded. Analysis of the influence of quality of achieved reduction on final functional result and on the development of complications was performed. RESULTS: 125 fractures healed uneventfully. Mean long term CS(rel) was 81% of the unaffected side. Mean CS(rel) was 85% in 3-part fractures, 73% in four-part fractures, 80% in three-part fracture-dislocations and 70% in four-part fracture-dislocations. 96 (70%) patients achieved excellent or good results (CS(rel) higher than 80%), 17 (12.4%) satisfactory results and 24 (17.5%) poor results (CS(rel) less than 60%). No significant difference was observed in functional results between different age groups. No incidence of non-union was observed, 4 losses of reduction were encountered. We observed 17 cases of complete humeral head necrosis, 14 of them in the group of 4-part fractures. Reduction quality strongly influenced functional result and development of head necrosis. In the group of excellent reduction mean CS(rel) was 88% and the rate of necrosis was 2%. Moderate reduction quality deteriorated CS(rel) to 70% and head necrosis rate rose up to 28%. If reduction was poor, mean CS(rel) was 52% and the rate of complete necrosis rose to 60%. CONCLUSION: Long term results confirmed nailing as appropriate treatment strategy for all types of humeral head fractures with limitation of excellent reduction in every age group. Moderate or even poor reduction significantly deteriorates functional results and increases rate of complete necrosis of the humeral head. If good reduction cannot be achieved, treatment strategy should be changed.

ÚVOD: Intraartikulární zlomeniny mají vysoké rizi - ko osteonekrózy hlavice humeru. S věkem dochází i k redukci kostní hmoty v oblasti hlavice humeru a stupeň osteoporózy limituje možnosti spolehlivé fixace osteosyntetickým materiálem. METODIKA: Do naší studie jsme zařadili celkem 29 pacientů s intraartikulárními zlomeninami proxi - málního humeru chirurgicky ošetřovaných LCP dla - hou (PHILOS®) na našem pracovišti v letech 2009 -2013. Pacienti absolvovali RTG a CT vyšetření a byli klasifikováni dle AO klasifikace. Prediktory ische - mie hlavice humeru dle studie Hertela z roku 2004 (délka metafyzárního fragmentu (calcar) menší než 8 mm, poranění mediálního závěsu, zlomeniny v ob - lasti anatomického krčku, 4úlomková zlomenina, angulace hlavice nad 45 stupňů, dislokace hrbolků nad 10 mm, glenohumerální luxace a fragmentace hlavice) byly porovnávány u pacientů s následnou osteonekrózou (parciální nebo kompletní) hlavice. VÝSLEDKY: Osteonekróza hlavice humeru se vy - skytla v 10 případech (34.5 %). Kompletní osteo - nekróza hlavice s jejím kolapsem a resorbcí jsme zaznamenali v 1 případu (3.4 %). Osteonekróza hlavice se vyskytovala u C3 zlomenin v 55.6 % pří - padů, u C2 zlomenin v 27.8 % případů. U C1 zlome - niny jsme nezaznamenali ostenekrózu hlavice ani u jednoho pacienta. Prediktory ischemie hlavice, tj. délka metafyzárního fragmentu menší než 8 mm, poranění mediálního závěsu, zlomenina v oblasti anatomického krčku a 4úlomková zlomenina byly přítomny u kompletní osteonekrózy hlavice. Vět - šina těchto prediktorů (70-100 %) byla přítomna u parciální osteonekrózy hlavice. ZÁVĚR: Riziko vzniku osteonekrózy je podstatně vyšší u C zlomenin. Pacienti s osteonekrózou hlavi - ce humeru měli podstatně horší funkční výsledky a vyžadovali revizní operaci v daleko větším počtu. Po prognózu hojení zlomeniny a riziko vzniku osteo- nekrózy hlavice je žádoucí zhodnocení prediktorů ischemie hlavice. U zlomenin s vysokým rizikem vzniku osteonekrózy hlavice by měla být zvážena primární implantace hemiartroplastiky.
INTRODUCTION: Intra-articular humeral head fractures face a high risk of humeral head osteo - necrosis development. Reduced bone quality in the area of humeral head and osteoporosis in older pati - ents limit the options of successful osteosynthesis. MATERIAL AND METHODS: A total of twenty-ni - ne patients with intra-articular proximal humerus fractures treated surgically by LCP plate (PHILOS®) at our department from 2009 to 2013 were inclu - ded in our study. All patients underwent X-ray and CT examination and the fractures were classified according to the AO classifiication. Predictors of the humeral head ischemia defined by Hertel in 2004 (length of the metaphyseal calcar segment less than 8 mm, disruption of the medial hinge, fractu - res of the anatomical neck, four-fragments fractu - res, head angulation over 45 degrees, displacement of tuberosities over 10 mm, glenohumeral disloca - tion and head-split component) were used for fur - ther assessment of patients with humeral head os - teonecrosis (partial or complete). RESULTS: Humeral head osteonecrosis occurred in 10 out of 29 cases (34.5 %). Complete head osteo - necrosis with the collapse and resorption was no - ted only in 1 case (3.4 %). C3 fractures were affected by osteonecrosis in 55.6 % of cases, C2 fractures in 27.8 %. No avascular necrosis was noted in the C1 fracture group. Predictors of humeral head ische - mia, i.e. length of the metaphyseal calcar segment less than 8mm, disruption of the medial hinge, fractures of the anatomical neck and four-frag - ments fracture were found in the case of complete humeral head osteonecrosis. Most of the predictors (70-100%) have occurred in fractures with develo - ped partial head osteonecrosis. CONCLUSION: The risk of osteonecrosis is considerably higher in C type fractures. Patients with hu - meral head osteonecrosis reached a significantly worse functional outcome and required more re - vision operations. Evaluation of humeral head is - chemia predictors is important for the prognosis of fracture healing and for assessing the risk of head osteonecrosis. Fractures with highly predictable humeral head necrosis should be considered for primary hemiarthroplasty.
