Hereditárny angioedém (HAE) je zriedkavé geneticky podmienené ochorenie charakterizované rekurentnými epizódami lokalizovaného, bradykinínom sprostredkovaného opuchu hlbokých tkanív kože, dýchacieho a gastrointestinálneho traktu, ktorý môže ohroziť pacientov život. Vo väčšine prípadov je HAE spôsobený nedostatkom inhibítora C1-esterázy (C1-inhibítora), najčastejšie vplyvom mutácie v géne SERPING1. Symptómy sa obvykle začínajú v detstve alebo adolescencii a smerom k dospelosti sa zhoršujú. Diagnostiku komplikuje značná variabilita klinických príznakov a/alebo prítomnosť sprievodných najmä alergických ochorení. V článku uvádzame súbor 6 pacientov z 3 rodín s geneticky potvrdeným HAE a ich klinický fenotyp.
Hereditary angioedema (HAE) is a rare genetic disorder characterized by recurrent episodes of localized, bradykinin-mediated swelling of the deep tissues of the skin, respiratory and gastrointestinal tract, which can be life-threatening. In most cases, HAE is caused by C1-esterase inhibitor (C1-inhibitor) deficiency, most commonly due to a mutation in the SERPING1 gene. Symptoms usually begin in childhood or adolescence and worsen towards adulthood. Diagnosis is complicated by the considerable variability of clinical symptoms and/or the presence of concomitant, mainly allergic diseases. In this article, we describe a series of 6 patients from 3 families with genetically confirmed HAE and their clinical phenotype.

Hereditární angioedém (HAE) je vzácné dědičné onemocnění aktuálně klasifikované do dvou základních kategorií na HAE s deficiencí C1 inhibitoru (HAE-C1-INH) a HAE s normální hladinou a funkcí C1 inhibitoru (HAE nC1-INH). Klinicky se manifestuje recidivami lokalizovaných otoků podkoží a sliznic. Z toho plyne velmi široká klinická manifestace a pacient s HAE se může objevit v ordinaci nejrůznějších odborností. Příčinou otoků je nekontrolovaná aktivace komplementového a kininového systému. Diagnostika HAE vychází z klinických projevů, anamnestických údajů a laboratorní diagnostiky. Strategie léčby zahrnuje krátkodobou a dlouhodobou profylaxi a léčbu atak.
Hereditary angioedema (HAE) is a rare, hereditary disorder classified to HAE with C1 inhibitor deficiency (HAE-C1-INH) and HAE with normal C1 inhibitor level and function (HAE nC1-INH). HAE is characterized by recurrent attacks of subcutaneous or submucosal oedema. These results in a very wide clinical manifestation and a patient with HAE may visit an outpatient clinic of various specialties. The cause of oedema is uncontrolled complement and kinin system activation. The diagnosis of HAE is based on clinical manifestation, anamnestic data and laboratory diagnostics. Treatment strategies include short-term and long-term prophylaxis and on demand treatment.

INTRODUCTION: Acquired angioedema with C1 inhibitor deficiency (AAE-C1-INH) is rare but a potentially life-threatening disease. There are no official prevalence data, nor approved therapies for this condition. OBJECTIVE: In this study, we aimed to collect and analyze clinical data on patients with AAE-C1-INH in the Czech Republic. METHODS: We have conducted a retrospective analysis of AAE-C1-INH patients from Czech referral centers for the treatment of hereditary angioedema with C1 inhibitor deficiency. The inclusion criteria involved recurrent episodes of angioedema with the first manifestation at or after the age of 40, negative family history of angioedema, and C1 inhibitor function 50% or less. RESULTS: A total of 14 patients (7 males and 7 females) met the inclusion criteria for AAE-C1-INH. The median age of the symptom onset was 59.5 years, and the median diagnosis delay was 1 year. The most common clinical manifestation was facial edema (100%) and upper airway swelling (85.7%). All patients responded to the acute attack treatment with icatibant and plasma-derived or recombinant C1 inhibitor concentrate. Lymphoid malignancy was identified in 9 patients (64%), monoclonal gammopathy of uncertain significance in 3 (21%), and in 1 patient autoimmune disease (ulcerative colitis) was considered causative (7%). We were not able to identify any underlying disease only in 1 patient (7%). In 6 of 7 patients (86%) treated for lymphoma, either a reduction in the frequency of angioedema attacks or both angioedema symptoms' disappearance and complement parameter normalization was observed. CONCLUSIONS: The prevalence of AAE-C1-INH in the Czech Republic is about 1:760,000. This rare condition occurs in approximately 8% of the patients with angioedema with C1 inhibitor deficiency. AAE-C1-INH is strongly associated with lymphoproliferative disorders, and treating these conditions may improve the control of angioedema symptoms.

Oproti běžným představám se problematika primárních imunodeficiencí zdaleka netýká pouze pediatrie, velký počet primárně imunodeficitních pacientů je nejen léčen, ale i diagnostikován v dospělém věku. Nejdůležitější manifestací těchto chorob jsou závažné, neobvyklé nebo špatně léčitelné infekce. Některé choroby z primární imunodeficience se často manifestují až v dospělém věku - jedná se především o hypogamaglobulinemii označovanou jako běžná variabilní imunodeficience (CVID) a Goodův syndrom (hypogamaglobulinemie s tymomem). Díky úspěšné léčbě se dospělosti dožívají (a často vedou "normální" život) i ti nemocní, jejichž manifestace začala v časném dětství. I mezi nimi jsou nejčastější nemocní s poruchami tvorby protilátek - např. s X-vázanou agamaglobulinemií, ale také nemocní s Di Georgeovým syndromem (porucha vývoje tymu, srdce, paratyreoidey). Důležitou skupinou jsou i nemocní s hereditárním angioedémem (deficit C1-INH), tito nemocní netrpí zvýšeným výskytem infekcí, objevují se u nich však otoky podkoží a sliznic.
In contrast to general opinion, the issue of primary immunodeficiency is far from just a pediatric medicine; a large number of patients with primary immunodefciencies are not only treated, but also diagnosed in adulthood. The most important manifestation of these diseases are serious, unusual or ill-treatable infections. Some primary immune deficiency diseases manifest themselves in adulthood - mainly common variable immunodeficiency (CVID) and Good´s syndrome (hypogammaglobulinemia with thymoma). Due to successful treatment, adults also suffer from illnesses manifestation of which begin in early infancy. Among these, the most common are patients with disorders of antibody production - for example, X-linked agammaglobulinemia or T-cell defect - Di George syndrome (thymus deficiency, morphological heart abnormalities, hypoparathyroidism). Another important group of primary immunodeficiencies are patients with hereditary angioedema (C1-INH deficiency). These patients do not suffer from an increased incidence of infections, but from swelling of the subcutaneous and submucosal tissues.
