It has been suggested that sympathetic activity, measured as changes in electrical skin impedance (SI), can be used to assess the adequacy of general anesthesia. Our prospective study investigated if measurements of skin impedance can determine levels of sedation induced by midazolam. Twenty-seven patients scheduled for arthroscopy requiring general anesthesia were served as their own control. These were blinded to the order of injections by telling them that they will be randomly administered a placebo (saline) orsedative agent. A DM 3900 multimeter was used for SI measurements. The degree of sedation was measured using the modified Observer's Assessment of Alertness and Sedation (mOAAS) scale. Resting SI values were noted, and all participants were then administered the placebo followed 5 min later by midazolam 2 mg i.v. Five min after that, patients were administered standard general anesthesia with propofol, oxygen, nitrous oxide 60 %, and isoflurane 1 MAC via a laryngeal mask, and sufentanil 5 - 10 μg. SI significantly increased after administration of midazolam and induction of anesthesia. There were no significant differences between pre-administration (baseline) and placebo and end of surgery and end of anesthesia with closed eyes. There were highly significant differences (p<0.001) between pre-administration vs. midazolam, placebo vs. midazolam, pre-administration vs. induction of anesthesia. We found slight correlation between mOAAS and SI. There were no significant changes between the end of surgery and the end of anesthesia with closed eyes, but SI significantly decreased (p<0.01) after eyes opened.

Diabetic peripheral neuropathy (DPN) is a common complication of diabetes with potential severe consequences. Its pathogenesis involves hyperglycemia-linked mechanisms, which may include changes in the expression of neurotrophic growth factors. We analyzed the expression of 29 factors potentially related to nerve degeneration and regeneration in skin biopsies from 13 type 1 diabetic pancreas and kidney recipients with severe DPN including severe depletion of intraepidermal nerve fibers (IENF) in lower limb skin biopsies (group Tx1 1st examination). The investigation was repeated after a median 28-month period of normoglycemia achieved by pancreas transplantation (group Tx1 2nd examination). The same tests were performed in 13 stable normoglycemic pancreas and kidney recipients 6-12 years posttransplantation (group Tx2), in 12 matched healthy controls (group HC), and in 12 type 1 diabetic subjects without severe DPN (group DM). Compared to DM and HC groups, we found a significantly higher (p < 0.05-0.001) expression of NGF (nerve growth factor), NGFR (NGF receptor), NTRK1 (neurotrophic receptor tyrosine kinase 1), GDNF (glial cell-derived neurotrophic factor), GFRA1 (GDNF family receptor alpha 1), and GFAP (glial fibrillary acidic protein) in both transplant groups (Tx1 and Tx2). Enhanced expression of these factors was not normalized following the median 28-month period of normoglycemia (Tx1 2nd examination) and negatively correlated with IENF density and with electrophysiological indices of DPN (vibration perception threshold, electromyography, and autonomic tests). In contrast to our expectation, the expression of most of 29 selected factors related to neural regeneration was comparable in subjects with severe peripheral nerve fiber depletion and healthy controls and the expression of six factors was significantly upregulated. These findings may be important for better understanding the pathophysiology of nerve regeneration and for the development of intervention strategies.

Úvod: Testování termických prahů je nedílnou součástí neurofyziologické diagnostiky neuropatie tenkých vláken, jejíž nejčastější příčinou je diabetes mellitus. Hlavním cílem bylo stanovit, existují-li signifikantní rozdíly termických prahů mezi diabetiky 1. typu a kontrolní skupinou a zhodnotit vývoj termických prahů v závislosti na délce trvání diabetu. Soubor a metodika: Vyšetřeno bylo 45 diabetiků 1. typu (27 žen, 18 mužů, průměrný věk 45,1 let). Soubor jsme rozdělili do dvou skupin podle doby trvání diabetu do 25 let a nad 25 let. Termické prahy byly stanoveny teplotním stimulátorem firmy Somedic na horních a dolních končetinách metodou „Limity“. Výsledky: Mezi kontrolní skupinou a diabetiky byl prokázán statisticky významný rozdíl mezi termickými prahy na dolních končetinách/tibie (práh pro detekci chladu: p < 0,001, práh pro detekci tepla: p = 0,002), nárt (prahy pro detekci chladu, tepla: p < 0,001). Mezi dvěma podskupinami diabetiků jsme zjistili statisticky významný rozdíl pro průměrné hodnoty prahů v oblasti nártu (chlad: p = 0,024, teplo: p = 0,049). Prokázán byl statisticky významný vztah mezi hodnotami termických prahů na nártu a délkou trvání diabetu, stanoveny byly jednotlivé regresní rovnice. Závěr: Pro skríningové vyšetření má význam vyšetřovat termické prahy zejména distálně na dolních končetinách v situacích, kdy diabetik popisuje neuropatické symptomy a elektromyografické vyšetření neprokazuje abnormitu.
Background: Thermal threshold testing represents an integral part of neurophysiological diagnostics of small fiber neuropathy. Diabetes mellitus is the most common cause of SFN. The aim of our study was to evaluate changes of thermal thresholds depending on the duration of diabetes. Material and methods: 45 patients with type 1 diabetes mellitus (27 women, 18 men, mean age 45,1 years) were enrolled in the study. The group was divided into two subgroups according to the duration of diabetes (≤ 25 years, > 25 years). We used a thermal stimulator (Somedic) and determined cold detection thresholds (CDT) and warm detection thresholds (WDT) in thenar, tibia and dorsum of the foot. Non-randomized method of reaction time was used as an investigative algorithm. Results: We demonstrated a statistically significant difference between the thermal thresholds in lower extremities/tibia (CDT p < 0.001, WDT p = 0.002), dorsum of the foot (CDT, WDT p < 0.001). In diabetics, CDTs are lower and WDTs are higher in comparison with controls. An abnormality of CDT occurs earlier and more frequently. We proved statistically significant difference for the mean values of CDT (p = 0.024) and for the mean values of WDT (p = 0.049) in the dorsum of the foot between the two subgroups. Linear regression demonstrated a statistically significant relationship between the thermal thresholds in the dorsum of the foot and the duration of diabetes and determined individual regression equations. Conclusion: Thermal threshold testing in the distal part of the leg appears to be a good screening tool for detection of small fiber neuropathy in type 1 diabetic patients, especially in diabetics with neuropatic symptoms and normal nerve conduction studies and needle electromyography.

Úvod: Traumatické léze brachiálního plexu patří mezi závažná postižení periferních nervů. Základní diagnostickou metodou je podrobné klinické neurologické vyšetření, CT myelografie nebo MR, které prokážou vytržené míšní kořeny. K upřesnění tíže poruchy se používají neurofyziologická vyšetření – elektromyografie a evokované potenciály. Cílem práce bylo ukázat, zda při diagnostice traumatické léze brachiálního plexu, vč. avulze krčních míšních kořenů může pomoci i vyšetření míšního inhibičního reflexu (kožní perioda útlumu), který hodnotí funkci tenkých A-delta vláken. Materiál a metodika: Vyšetřili jsme 23 nemocných s traumatickým poraněním brachiálního plexu (16 mužů, věk 18–62 let). Provedli jsme podrobné neurologické vyšetření, zobrazovací metody (CT myelografie nebo MR) a neurofyziologické vyšetření vč. jehlové EMG, kondukčních studií a kožní periody útlumu po stimulaci 1., 3. a 5. prstu (dermatomy C6–C8). Výsledky: U všech nemocných byla prokázána léze brachiálního plexu s maximálním postižením horního trunku nebo s avulzí kořenů C5 a C6. U 16 nemocných byla kožní perioda útlumu přesto dobře výbavná. U sedmi osob jsme kožní periodu útlumu nevybavili alespoň v jednom dermatomu. Všichni tito nemocní měli zároveň těžkou kořenovou senzitivní lézi. Závěr: U většiny nemocných s těžkou lézí brachiálního plexu byla kožní perioda útlumu zachována. Ukazuje to existenci plurisegmentální inervace s reziduální funkcí A-delta vláken se zachováním míšních protektivních reflexů. I přes těžkou traumatickou lézi je patrna výraznější odolnost A-delta vláken vůči traumatu např. ve srovnání s motorickými vlákny.
Objectives: Traumatic lesions of brachial plexus are serious periferal nerve injuries. Neurological examination and CT myelography or MRI are the basic examination methods that can confirm spinal root avulsion. To specify severity of the injury – electromyography and evoked potentials are used. The objective of this study was to determine whether implementation of cutaneous silent period that asseses function of small diameter A-delta fibers, is useful as a diagnostic tool in cervical root avulsion and brachial plexus injury. Material and methods: Clinical examination, imaging studies (CT myelography or MRI) and neurophysiological examination were performed in 23 patients with traumatic brachial plexus injury (16 males, age 18–62 years). Needle EMG was obtained from muscles supplied by C5–T1 myotomes. CSP was recorded after painful stimuli were delivered to the thumb (C6 dermatome), middle (C7) and little (C8) fingers while subjects maintained voluntary contraction of intrinsic hand muscles. Results: Electrodiagnostic and CT/MRI studies confirmed brachial plexopathy involving mainly the upper trunk or corresponding C5, C6 roots in all patients. However, well defined CSP was still present in 16 subjects. CSP was absent in at least one of the dermatomes in the remaining seven patients. All these patients had severe plurisegmental sensitive lesion. Conclusion: CSP was still present, although not absolutely normal, in the majority of patients with severe brachial plexus injury. This suggests there are plurisegmental innervations with residual function of A-delta fibers and the presence of spinal inhibitory reflexes. Resistance of A-delta fibers seems to be higher compared to motor fibers despite of severe traumatic lesion.

Whether nerve fiber loss, a prominent feature of advanced diabetic neuropathy, can be reversed by reestablishment of normal glucose control remains questionable. We present 8-year follow-up data on epidermal nerve fiber (ENF) density and neurological function in patients with type 1 diabetes after simultaneous pancreas and kidney transplantation (SPK) with long-term normoglycemia. Distal thigh skin biopsies with ENF counts, vibration perception thresholds (VPTs), autonomic function testing (AFT) and electrophysiological examinations were performed at time of SPK and 2.5 and 8 years after SPK in 12 patients with type 1 diabetes. In comparison to controls, baseline ENF density, VPT and AFT results of patients indicated severe neuropathy. At follow-up, all SPK recipients were insulin independent with excellent glycemic control and kidney graft function; however, the severe ENF depletion present at baseline had not improved, with total ENF absence in 11 patients at 8-year follow-up. Similarly, no amelioration occurred in the VPT and AFT results. Numerical improvement was seen in some electrophysiological parameters; however, statistical significance was achieved only in median motor nerve conduction velocity. ENF loss and functional deficits in advanced diabetic peripheral neuropathy are rarely reversible, even by long-term normoglycemia, which underscores the importance of neuropathy prevention by early optimal glycemic control.

Standard cutaneous innervation maps show strict midline demarcation. Although authors of these maps accept variability of peripheral nerve distribution or occasionally even the midline overlap of cutaneous nerves, this concept seems to be neglected by many other anatomists. To support the statement that such transmedian overlap exists, we performed an extensive literature search and found ample evidence for all regions (head/neck, thorax/abdomen, back, perineum, and genitalia) that peripheral nerves cross the midline or communicate across the midline. This concept has substantial clinical implications, most notably in anesthesia and perineural tumor spread. This article serves as a springboard for future anatomical, clinical, and experimental research.
