INTRODUCTION: The impact of bleeding for women with bleeding disorders (WBD) is of increasing focus and importance. Despite this, optimal management strategies are unclear and knowledge gaps persist. AIM: To examine practices and define research priorities on diagnosis and management of WBD in Europe. METHODS: An electronic survey on clinical management of WBD was sent to 136 European haemophilia treatment centres (HTCs), including open questions on knowledge gaps and research priorities. RESULTS: Fifty-nine HTCs from 12 Western (WE) and 13 Central/Eastern European (CEE) countries completed the survey. Less than half runs a joint clinic (24 HTCs, 42%). Most centres without a joint clinic have a named obstetrician (81%) and/or gynaecologist (75%) available for collaboration. Overall 18/54 (33%) European HTCs do not offer preimplantation genetic diagnosis. Third trimester amniocentesis to guide obstetric management is available 28/54 HTCs (52%), less frequent in CEE compared to WE countries (5/17 vs 23/37, P = .03). 53% of HTCs (28/53) reported that only 0%-25% of WBD seek medical advice for heavy menstrual bleeding (HMB). An algorithm managing acute HMB in WBD is lacking in 22/53 (42%) HTCs. The main reported knowledge and research gaps are lack of awareness & education on WBD among patients and caregivers, optimal diagnostic strategies and effective multidisciplinary management of pregnancy & HMB. CONCLUSION: Joint clinics, prenatal diagnostics and algorithms for managing acute HMB are lacking in many European HTCs. HMB may be an underestimated issue. This survey highlights the need to prioritize improvement of knowledge and patient care for WBD across Europe.

Úvod: Spolupráce zdravotnické záchranné služby a traumacentra a její vyhodnocení je pro kvalitní péči o pacienty s úrazem nezbytné. Metodika:Retrospektivně jsme zpracovali 160 pacientů s ISS (Injury Severity Score) > 16, předaných leteckou a pozemní záchrannou službou do traumacentra (TC). Sledovali jsme tyto parametry: čas na místě a dobu transportu, volumoterapii, koagulopathie v souvislosti s volumoterapií, v TC hladina laktátu, hemoglobinu a ISS, v přednemocniční fázi zajištění dýchacích cest v souvislosti s hypoxií a v roce 2014 i tělesnou teplotu při příjmu pacienta v TC. Výsledky: Nejvýhodnějším transportem je pro pacienty s traumatem primární aktivace LZS, aktivace na základě požadavku pozemní skupiny je nevýhodná, prodlužuje čas ošetření. Objemy tekutin nad 1500 ml zvyšujíc riziko vzniku koagulopatie, volumoterapie ovlivňuje i snížení tělesné teploty pacientů. Přesto jsme v roce 2014 pozorovali vyšší přežití pacientů se vstupní hodnotou laktátu oproti roku 2013 (78 % versus 31 %). Diskuze: zjistili jsme souvislost s množstvím podaných náhradních roztoků s výskytem koagulopatie (medián 1500 ml), přímá souvislost je i se snížením tělesné teploty pacienta a množstvím podaných náhradních roztoků. Pro pacienta v PNP s traumatem ISS > 16 je nejvýhodnější zajištění a transport primárně LZS. Potvrdili jsme souvislost hladiny laktátu jako predikční hodnotu šokového stavu. Hladina hemoglobinu souvisí se vznikem koagulopatie, ale není to jediná příčina. Významně se zlepšilo přežívání pacientů se zavedením novým metod a postupů TC – ROTEM, TEG. Závěr: Vypracovali jsme novou metodiku spolupráce – traumaprotokol, který zlepší vyhodnocení spolupráce ZS a TC. Dále rozšiřujeme spolupráci i s traumacentrem ÚVN Praha Střešovice.
Introduction: Cooperation between Emergency medical service and traumacenter and its evaluation is essential for quality of care of patients with trauma. Methods: A retrospective analysis of 160 patients with ISS > 16 transported by helicopter and ground emergency medical service and admitted to traumacenter is presented in this article. We have analysed these parameters: time on scene and transport time, volumotherapy, koagulopathy associated with the amount of fluids, in traumacenter we have analysed lactate and hemoglobin levels and ISS (Injury Severity Score), in the prehospital phase airway management associated with hypoxia and in the year 2014 body temperature on admission, too. Results: The most advantageous type of transport for patients is the primary HEMS run, activation by ground EMS team is disadvantageous and extends the time on scene and transport time. Fluid volume over 1500 ml increases the risk of coagulopathy, there is also an influence of volumotherapy on patient´s body temperature decrease. Nevertheless we have observed higher surival of patients with lacate level over 5 on admission in the year 2014 compared to 2013 (78 % versus 31 %). Discussion: We have found association between fluids administered to the patients and coagulopathy occurence (median 1500 ml) and there is also a clear association between amount of administered fluids and patient´s body temperature decrease. There is profit for patients with ISS > 16 in case of primary HEMS transport compared to other types of transport. We have confirmed the lactate level as a shock state predictor. Hemoglobin level is associated with coagulopathy but not as a sole cause. New methods and procedures included in the trauma management has increased the patients´ survival. Conclusion: We have prepared a new traumaprotocol which helps to improve evaluation of traumacenter and EMS cooperation. We have also started cooperation with Military hospital in Prague Střešovice.

Celiac disease (CeD) is an immune-mediated enteropathy triggered by ingestion of gluten in genetically susceptible individuals. CeD is a global disease and estimated to affect approximately one percent of the global population. With advent of simple serological tests for the diagnosis, the number of individuals diagnosed with CeD is increasing exponentially. It was initially thought that gluten hypersensitivity in CeD is limited to small intestine only and all other features are secondary to malabsorption, but it is now recognized that the hypersensitivity to gluten is not limited to small intestine alone and may affect other organs such as skin, brain, and bones independent of intestinal involvement. CeD is now considered a multi-system disorder and their clinical presentation may be with gastrointestinal symptoms, called “classical CeD” or more often with non-gastrointestinal symptoms called “non-classical CeD”. These patients may present with short stature, anemia, liver dysfunction (asymptomatic increase in transaminases, chronic liver disease, autoimmune hepatitis), cutaneous manifestations (dermatitis herpetiformis, oral ulcers), reproductive diseases (infertility, recurrent abortions), neurological manifestations (ataxia, peripheral neuropathy), and metabolic disorders (osteopenia/osteoporosis). What determines these variable phenotypes remain unclear but likely is a result of genetic as well as environmental factors. Many of these patients with non-classical CeD are likely to report to specialists other than gastroenterologists such as hematologists, endocrinologists, rheumatologists or gynecologists. Unfortunately, the awareness about non-classical presentations of CeD amongst health care professionals remains low. There is an urgent need to increase awareness among health care professionals about varied manifestations of CeD in order to decrease the burden of undiagnosed CeD.

Cieľ: Vyhodnotenie vzťahu parametrov koagulácie (protrombínový čas, medzinárodný normalizovaný pomer, aktivovaný parciálny tromboplastínový čas, trombínový čas a počet trombocytov) u detí a mladistvých po izolovanom kraniocerebrálnom poranení k neurologickému stavu po úraze, typu poranenia a prognóze. Súbor a metodika: Do retrospektívnej štúdie sme zahrnuli 51 detí a mladistvých (33 chlapcov a 18 dievčat) vo veku 1–17 rokov, ktorí boli operovaní na našom pracovisku od 1. 1. 2005 do 1. 3. 2011. Parametre koagulácie, ktoré boli vyšetrené do 6 hodín po úraze, sme porovnali pri rôznych typoch kraniocerebrálneho poranenia a štatisticky korelovali so vstupným a výstupným neurologickým stavom. Výsledky: Parametre koagulácie sa štatisticky významne líšili pri rôznych typoch kraniocerebrálneho poranenia a korelovali so vstupným a výstupným neurologickým stavom. Záver: Porucha koagulácie, ktorá vzniká po izolovanom kraniocerebrálnom poranení u detí a mladistvých, je nepriaznivým prognostickým faktorom, ktorý vyžaduje rýchlu a agresívnu liečbu.
