Kyselina hyaluronová (KH) je lineární, přirozeně se vyskytující polysacharid o vysoké molekulové hmotnosti. Je součástí řady bio logických procesů, zejména regulačních, protizánětlivých, imunosupresivních nebo antiproliferativních. Významně váže vodu a podílí se na udržování viskoelasticity tkání, na supramolekulárním prostorovém uspořádání proteoglykanů v extracelulární matrix a na regulaci osmotického tlaku, což významně ovliňuje homogenitu tkáně a její homeostatickou integritu. Díky těmto vlastnostem lze neimunogenní KH použít v mnoha klinických aplikacích. V otorinolaryngologii jsou možnosti jejího využití široké zejména v otologii, laryngologii, rinologii nebo při léčbě zánětlivý stavů a keloidních jizev. Ně kte ré potenciální oblasti využití jsou stále ve stadiu experimentu (genová terapie).
Hyaluronic acid (HA) is a linear, naturally occurring polysaccharide of high molecular weight. It is part of a number of bio logical processes, especially regulatory, anti-inflammatory, immunosuppressive or antiproliferative. It significantly binds water and participates in the maintenance of tissue viscoelasticity, in the supramolecular spatial arrangement of proteoglycans in the extracellular matrix and in the regulation of osmotic pressure, which significantly affects tissue homogeneity and its homeostatic integrity. Due to these properties, non-immunogenic HA can be used in many clinical applications. In otorhinolaryngology, the possibilities of its use are wide, especially in otology, laryngology, rhinology or in the treatment of inflammatory conditions and keloid scars. Some potential areas of use are still in the experimental stage (gene therapy).
Intrauterinní adheze jsou závažnou komplikací, která vzniká po intrauterinních výkonech, nejčastěji v souvislosti s graviditou. Projevy jako amenorea, pellipatia a neplodnost jsou pro ženu závažné obzvláště v reprodukčním věku a nazýváme je spolu s intrauterinními adhezemi Ashermanovým syndromem. Důležitá je primární prevence po intrauterinních výkonech. Z publikovaných studií vyplývá, že použití gelu kyseliny hyaluronové především po abortech vede k prevenci vzniku středních a těžkých intrauterinních adhezí a zvyšuje také míru otěhotnění.
Intrauterine adhesions are a serious complication that occurs after intrauterine procedures, most often in connection with pregnancy. Manifestations such as amenorrhea, pelvic pain, and infertility for a woman, especially in reproductive age, are serious and together with intrauterine adhesions we call them Asherman’s syndrome. Primary prevention after intrauterine procedures is important. Published studies show that the use of hyaluronic acid gel, especially after abortions, leads to the prevention of moderate and severe intrauterine adhesions and also increases the pregnancy rate.

Kyselina hyaluronová je polysacharid s mnohostranným uplatněním. V urologii našla své místo v mnoha využitích. Uplatňuje se v rámci prevence recidiv infekcí dolních cest močových, léčby post-radiační cystitis či syndromu chronické bolesti močového měchýře. Dále má využití v andrologii i v dětské urologii.
Hyaluronic acid is a polysaccharide with various utlization. In urology, hyaluronic acid found its place in many uses. It is applied in prevention of recurrence of lower urinary tract infections, treatment of post-radiation cystitis or in therapy of primary bladder pain syndrome. It is also used in andrology or pediatric urology.

Background: Cartilage intermediate layer protein 2 (CILP2) is a monomeric glycoprotein that is mostly expressed in the intermediate zone of articular cartilage and can be detected in the extracellular matrix (ECM). The level of CILP2 in serum and its quantity on the articular cartilage surface and throughout the articular cartilage indicates the potential utility of CILP2 for investigation as a biomarker for determining cartilage deterioration in joint disorders. The CILP2 measurement of changes in cartilage biomarkers may be an effective and sensitive tool for detecting the early development of knee osteoarthritis (KOA) in people at risk for KOA. Changes in CILP2 levels may be beneficial for the early diagnosis of KOA, as CILP2 appears to be linked to cartilage thickness reduction in people who are more likely to develop KOA. Platelet rich plasma (PRP) effectiveness of transforming growth factor, platelet-derived growth factor, and the vascular endothelial growth factor is thought to be related to their release. Because of their capacity to increase matrix formation,growth factors have been widely researched for OA and cartilage regeneration. Hyaluronic acid (HA) is a glycosaminoglycan polymer composed of N-acetyl glucosamine and D-glucuronic acid disaccharide molecules. Early embryonic development, inflammatory, wound repair, cell differentiation, and viscoelasticity are all affected by HA, and other biological processes The study was aimed to evaluate effecte of CILP2 levels after intraarticular injecting pure platelet-rich plasma and hyaluronic acid into patients with early knee osteoarthritis.Materials and Methods: BT LAB kits were used to assess the serum CILP2 level. The experimental investigation included 18 control groups, 10 HA injections, and 21 pure PRP injections. age ranged from 35 to 75. The study was exluded all individuals with advanced osteoarthritis in the knee, COVID-19, diabetes, and autoimmune diseases. the time frame running from November 2021 until June 2022. Other variables in our research were age, gender, family history, use of antihypertensive medications or medications for other disorders, and body mass index (BMI). Results: The findings of this study demonstrate elevated CILP2 levels in patients with early KOA before treatments injection compared with the healthy control. After treatments injection, the level of CILP2 was decreased compared with before injection.

Kyselinu hyaluronovou bychom dnes bez nadsázky mohli označit jako renesanční molekulu. Důvodem k tomu je její pestré medicinální využití a současně i její pleiotropní účinek, který se teprve učíme chápat. Zdaleka totiž nejde o její schopnost na sebe ve velkém množství vázat vodu a zlepšovat tak hydrataci cílové tkáně. K dispozici je dnes v řadě lékových forem, s různě podloženou účinností. Následující stručný text na tuto molekulu pohlíží optikou možného přínosu z pohledu dermatologie, resp. dermokosmetiky, při perorálním způsobu suplementace.
Nowadays, hyaluronic acid could be described without exaggeration as a renaissance molecule. The reason for this is its varied medical use and at the same time its pleiotropic effect, which we are just learning to understand. It is far from its ability to bind water in large quantities and thus improve the hydration of the target tissue. Currently, it is available in a number of dosage forms, with different efficacies. The following brief text looks at this molecule from the perspective of possible benefits from the point of view of dermatology / dermocosmetics, in the oral route of its supplementation.

INTRODUCTION: M89, containing 89% of Vichy mineralizing water and hyaluronic acid, has been developed to reinforce the skin barrier and to improve skin quality. AIM: To assess efficacy and tolerability of M89. METHOD: Observational survey of subjects with facial dermatoses or after esthetic procedures. M89 served as adjunct to conventional therapy. Clinician assessments of erythema, desquamation, irritation and patient-reported dryness, burning, itching, and stinging/tingling were conducted at baseline and 4 weeks. At 1 week and 4 weeks, patient assessed tolerance and satisfaction with M89. RESULTS: A total of 1630 subjects participated; 92.5% were females. Mean age was 41.1 ± 11.3 years. Dermatological indications accounted for 32.5%, procedures for 67.5%. At 4 weeks, in subjects with dermatoses, erythema had resolved or improved in 68.0%, desquamation in 83.4%, and irritation in 93.3%. Dryness, burning, itching, and stinging/tingling scores had decreased by 63.8%, 81.8%, 70.9%, and 85.2%, respectively (all P ≤ .0001); 75.7% considered that their skin was sufficiently hydrated. In the procedure group, erythema had resolved or improved in 72.5%, desquamation in 75.2%, and irritation in 88.1%. Dryness, burning, itching, and stinging/tingling scores had decreased by 62.1%, 78.8%, 70.0%, and 84.2%, respectively (all P ≤ .0001); 74.1% considered that their skin was sufficiently hydrated. Almost all subjects reported soothed skin and satisfaction with product texture. Subject and investigator satisfaction was very high. CONCLUSION: M89 significantly improves skin signs and symptoms after 4 weeks of continued use with no tolerance issues in subjects with dermatological indications. Moreover, subjects who have had recently undergone esthetic procedures M89 allowed a satisfying skin recovery.

In our previous research, we concluded that polymeric micelles based on hyaluronic acid are able to penetrate into the deeper layers of skin tissue. The aim of this work was to characterize the mechanisms involved in the uptake by skin cells, which is important for understanding the influence of the carrier composition on the drug penetration. To reach this goal, we used micelles encapsulating curcumin made of oleyl-hyaluronan (HAC18:1) and hexyl-hyaluronan (HAC6) covalently linked with fluorescent Nile Blue. This labeling enabled us to track the micelle-forming derivative and also micelle payload into the keratinocytes and fibroblasts by fluorescent microscopy and flow cytometry. The regulation of both the passive and active cellular uptake was used to determine the mechanism of micelle internalization. Furthermore, the changes of membrane fluidity were measured for these derivatives by FRAP. Using these methods we concluded that carriers entered the cells using both active and passive transport. Passive transport was facilitated by the affinity of the carrier to the cell membrane, especially in the case of HAC18:1 carrier, which changed significantly the membrane fluidity. The active transport was dependent on cell type, but mainly driven by the clathrin-mediated endocytosis and macropinocytosis. Surprisingly, the main HA receptor, CD44, was not involved in the uptake. We can conclude that these carrier systems could be used for the local transport of active substances or hydrophobic drugs into the skin cells using the advantage of passive transport of oleyl-HA derivative.
