Po dobu téměř celého století se jako zlatý standard operací pro benigní hyperplazii prostaty (BHP) etablovaly metody omezené velikostí prostaty. Pro žlázy nad 80 g otevřená prostatektomie (OP), pro menší transuretrální prostatektomie (TURP). Přes nesporný efekt přinášejí vysokou míru morbidity. Technologický rozvoj 90. let 20. století umožnil nové operační postupy, z nichž se jako metoda nezávislá na velikosti prostaty ustavuje enukleace prostaty holmiovým laserem (HoLEP). Snažíme se podat přehled o vývoji metody, alternativních postupech a nových technologických možnostech. Zároveň přinášíme porovnání v různých parametrech s doposud standardními postupy. Stále více literárních pramenů považuje Ho - LEP za efektní, bezpečnou a dlouhodobě účinnou metodu operační léčby BHP. Na základě toho v doporučených postupech postupně nahrazuje TURP a OP jako nová metoda volby. HoLEP je nadále nutno zvažovat jako možnost číslo jedna při plánování operací na prostatě, bez ohledu na její velikost. Lze jej nabídnout i pacientům s vyšším rizikem krvácení, po biopsii i po předchozích výkonech na prostatě. Rozvoj technologií slibuje další zrychlení a zvýšení bezpečnosti výkonu, který se stává novým zlatým standardem.
For nearly a century, methods limited by the size of the prostate have been established as the gold standard of surgery for benign prostatic hyperplasia (BPH). For glands over 80g open prostatectomy (OP) and for smaller ones transurethral prostatectomy (TURP). Despite their indisputable effect, they bring a high rate of morbidity. The technological development of the 90s of the 20th century enabled new surgical procedures of which Holmium laser enucleation of the prostate (HoLEP) is established as a size-independent method. We try to provide an overview of the development of the method, alternative procedures and new technological possibilities. At the same time we bring a comparison of various parameters with standard procedures up to now. More and more literary sources consider HoLEP to be an effective, safe and long-term effective method of surgical treatment of BPH. Based on this, it is gradually replacing TURP and OP as the new method of choice in the guidelines. HoLEP should be considered as the number one option when planning prostate surgery regardless of prostate size. It can also be offered to patients with a higher risk of bleeding, after a biopsy or after previous procedures on the prostate. The development of technologies promises further speed-up and better safety profile of the procedure which is becoming the new gold standard in BHP surgery.

Benigní hyperplazie prostaty (BHP) je jednou z nejčastějších urologických diagnóz u stárnoucích mužů. Postihuje téměř 80 % mužů straších 70 let. Je nutno konstatovat, že i přes dramatický technologický pokrok při chirurgickém řešení je stále považována za diagnózu obávanou a velká část populace stárnoucích mužů raději volí konzervativní, farmakologickou léčbu. Ta často není schopna trvale vyřešit symptomy dolních močových cest (LUTS) a v nezanedbatelném počtu případů končí trvalou permanentní katetrizací nešťastných mužů, když promeškají vhodné období k absolvování operace a jejich celkový zdravotní stav znemožňuje bezpečné provedení chirurgického výkonu. Posledních přibližně 70 let byla považována za zlatý standard v léčbě LUTS při BHP transuretrální resekce prostaty (TURP). Přesto, že původní monopolární technologie je dnes již prakticky nahrazena daleko bezpečnější technologií bipolární, nelze TURP úspěšně používat u hyperplazií nadměrného objemu. Pro tyto případy je stále aktuální metodou řešení otevřená prostatektomie. V současné době moderní literatura poukazuje na to, že Holmium laserová enukleace prostaty (HoLEP) nahradila TURP a otevřenou prostatektomii a je považována za zlatý standard v léčbě BHP. V posledních letech se objevily další technologie, umožňující transuretrální enukleaci prostaty (TUEP), které však nevyužívají laserovou technologii a prezentují podobné výsledky, jako je tomu u HoLEP. V tomto článku představujeme stávající transuretrální enukleační metody k řešení BHP, které jsou dle našich vědomostí prováděné v České republice. Diskutujeme operační principy a specifika jednotlivých technologií stejně jako bezpečnost výkonů, vedlejší efekty a dlouhodobé výsledky.
As one of the most common urological diagnosis of aging men, benign prostatic hyperplasia (BPH) affects almost 80% of males over 70. Despite dramatic progress in technological and surgical treatment options, it is still considered as feared diagnosis among patients. Majority of aging men choose conservative, pharmacological treatment which in many cases is unable to permanently solve the lower urinary tract symptoms (LUTS) and in many cases ends up with permanent catheterization of those unfortunate ones who missed a surgery while having a suitable performance status. For the past 70 years transurethral resection of the prostate (TURP) has been considered a gold standard in LUTS/BPH treatment. Although original monopolar technology is now replaced which much safer bipolar technology, TURP cannot be successfully used for prostate hyperplasias of excessive volumes. For these cases, open prostatectomy still remains a standardized option. Currently, modern literature points out that Holmium laser enucleation (HoLEP) has replaced TURP and open prostatectomy and is considered the new gold standard in BPH treatment. However, other technologies and techniques evolved, allowing transurethral bipolar enucleation of the prostate (TUBEP) without using laser technology and delivering similar results as HoLEP. We present an overview of existing transurethral enucleation methods for the treatment of BPH which are based on our knowledge performed in the Czech Republic, this article discusses the surgical principles and specifics of individual technologies as well as performance safety, side effects and long-term results.

Na českém a zahraničním trhu jsme doslova zavaleni možnostmi, jak rychle a jednoduše vypnout pleť a vrátit tak několik let zpátky. Vedle široké škály přístrojů a technik tak lze narazit i na širokou škálu účinnosti od nulové až po významné. Nejenom pro pacienty, ale i pro odborníky začíná být těžké se vyznat, která metoda je skutečně účinná a „ta pravá“. Předpoklady pro funkční metodu na omlazení jsou: minimální invazivita, rychlá rekonvalescence, bezpečnost ověřená klinickými studiemi, efektivita ověřená klinickými studiemi a samozřejmě cena. Autorka představuje kazuistiku s novou metodou Endolift®, jejímž hlavním účinkem je vypnutí kůže pomocí neokolagenogeneze, aktivace metabolických funkcí extracelulární matrix a rozpouštění tukových depozit. Toto lékařské ošetření je prováděno pomocí speciálních laserových mikrooptických vláken, která se zavedou pod kůži. Tím se přesouvá klasické laserové ošetření o vlnové délce 1 470 nm přímo do podkoží, kde vykoná svou nejlepší funkci: laserový paprsek zde specificky interaguje s vodou a tukem.
Currently, we are literally overwhelmed with the possibilities on the market of how to quickly and easily tighten the skin and turn back several years. In addition to a wide range of devices and techniques, there is also a wide range of in/efficiencies. It is becoming more and more difficult for patients and even professionals to understand which method is effective and "the right one". Preconditions for a functional method for rejuvenation are: minimal invasiveness, rapid recovery, safety verified by clinical studies, effectiveness verified by clinical studies and, of course, the cost. The author presents here a case report with the new Endolift® method, the main effect of which is skin tightening by neocolagenogenesis, activation of metabolic functions of the extracellular matrix and dissolution of fat deposits. This medical treatment is performed using special laser micro-optical fibers, which are temporarily inserted under the skin. This removes the classic laser treatment with a wavelength of 1,470 nm directly into the subcutaneous tissue, where it performs its best function: the laser beam specifically interacts with water and fat here.

PURPOSE: To compare the efficacy and safety of anterior capsulotomy creation with a new selective laser device (CAPSULaser) with those of manual capsulotomies. SETTING: GEMINI Eye Clinic, Zlin, Czech Republic. DESIGN: Prospective case series. METHODS: Patients were placed in cohorts based on age and cataract grade and randomly allocated to have laser capsulotomy or manual continuous curvilinear capsulorhexis (CCC). The anterior capsule was stained with microfiltered trypan blue 0.4%. The anterior capsulotomy was created with the laser device focused on the anterior capsule through a custom patient interface lens. Intraoperative video analysis with the use of an intraocular ruler and postoperative examinations were used to assess safety and efficacy (accuracy of capsulotomy size, circularity, centration). RESULTS: No intraoperative complications occurred in the laser group or the manual group. All capsulotomies in the laser group were free-floating with no tags or tears. The mean capsulotomy diameter was 5.03 mm overall (range 4.8 to 5.2 mm, laser group; 4.4 to 5.8 mm, manual group). In the laser group, all the capsulotomies were within 0.1 mm ± 0.1 (SD) of the target. The circularity accuracy was greater than 99.0% ± 1.0%; the mean centration of the capsulotomy in relation to the intraocular lens (IOL) was 0.1 ± 0.1 mm. All parameters were statistically significant (P < .01). The IOL-capsulotomy overlap was 360 degrees in all laser cases. CONCLUSIONS: Selective laser capsulotomy using a new proprietary trypan blue formulation was safe and effective in cataract surgery. The sizing, circularity, and centration of the laser capsulotomy were more accurate than those of the manual CCC, resulting in consistent 360-degree IOL coverage.

BACKGROUND: To compare baseline characteristics and outcomes of patients undergoing GreenLight laser vaporization (GL) or transurethral resection of the prostate (TURP) in a real life setting. METHODS: In this prospective observational cohort, the Clinical Research Office of the Endourological Society (CROES) collected data of consecutive GL or TURP treated patients. Treatment involved one of three GL laser powers (80 W, 120 W or 180 W) based on availability in each participating centre, or TURP. Data on baseline characteristics as well as functional measures were collected at three time points: 6-12 weeks, 6, and 12months after surgery. Functional measures included urinary flow parameters, perceived prostate function (IPSS), perceived erectile function (IIEF-5) and complications. RESULTS: Seven hundred thirteen patients underwent GL, and 234 patients underwent TURP. Overall, patients treated with GL show higher BMI, IIEF and medication use, together with lower urinary function (voided volume, incontinence, urinary retention) at baseline. After the procedure, despite higher antibiotic and antimuscarinic use and shorter hospital stay, readmission rates, PVR, PSA were higher, but Qmax, and IIEF were lower in the GL group. The rate of post-operative complications was 10.3% and 5.2% for the TURP and GL group, respectively (P=0.006). CONCLUSIONS: We were unable to categorically state which procedure is superior. This observational study confirms that treatment decision for either TURP or GL is not based on patient characteristics.
