Evidence-based guidelines on the management of pain associated with first-trimester medical abortion are lacking. Most published clinical trials have failed to report on this important aspect of the procedure. The aim of this comprehensive work was to provide clinical advice based on a comprehensive literature review, supplemented by the clinical experience of a group of European experts in case no evidence is available. Pain level ranged from 5 to 8 in 80% of studies where pain was measured on a 0-10 visual analogue scale; severe pain was reported by 20-80% of women. Pain assessment was rarely reported in studies. Pain treatment should be preventive and avoidance of unnecessary uterine contractions should be considered. Analgesic treatment should follow the WHO three-step ladder, starting with the use of NSAIDs and allowing for easily available back-up treatment with weak opioids.

Etoposide is a well-known and widely used anticancer drug that displays several side effects. In addition, tumors often acquire resistance to this drug. Our aim is to develop a combination therapy that would augment toxicity of etoposide in malignant cells. Based on literature and our experiments, we selected mifepristone (RU486) as a potential supporting molecule that is able to enhance etoposide toxicity against cancer cells. All experiments were performed with Hep G2 cells, a well-known and described human hepatocellular carcinoma cell line. By using xCELLigence system, we demonstrated that mifepristone enhances toxicity of etoposide in a dose dependent manner with concomitant caspase-3 activity. We evaluated upregulation of Bax because mifepristone was demonstrated to modulate proapoptotic Bax protein expression. Our data show only weak and not statistically significant increase of Bax expression. On the other hand, we show that mifepristone increases etoposide toxicity via inhibition of ABC transporters, coupled with significant increase of intracellular etoposide concentration. In conclusion, we demonstrate that mifepristone has a synergistic effect with etoposide treatment in the Hep G2 cells and that the effect is related to ABC transporters inhibition.

Fertilization of the mammalian oocyte requires interactions between spermatozoa and expanded cumulus extracellular matrix (ECM) that surrounds the oocyte. This review focuses on key molecules that play an important role in the formation of the cumulus ECM, generated by the oocyte-cumulus complex. In particular, the specific inhibitors (AG1478, lapatinib, indomethacin and MG132) and progesterone receptor antagonist (RU486) exerting their effects through the remodeling of the ECM of the cumulus cells surrounding the oocyte have been described. After gonadotropin stimulus, cumulus cells expand and form hyaluronan (HA)-rich cumulus ECM. In pigs, the proper structure of the cumulus ECM depends on the interaction between HA and serum-derived proteins of the inter-alpha-trypsin inhibitor (IαI) protein family. We have demonstrated the synthesis of HA by cumulus cells, and the presence of the IαI, tumor necrosis factor-alpha-induced protein 6 and pentraxin 3 in expanding oocyte-cumulus complexes (OCC). We have evaluated the covalent linkage of heavy chains of IαI proteins to HA, as the principal component of the expanded HA-rich cumulus ECM, in porcine OCC cultured in medium with specific inhibitors: AG1478 and lapatinib (both inhibitors of epidermal growth factor receptor tyrosine kinase activity); MG132 (a specific proteasomal inhibitor), indomethacin (cyclooxygenase inhibitor); and progesterone receptor antagonist (RU486). We have found that both RU486 and indomethacin does not disrupt the formation of the covalent linkage between the heavy chains of IαI to HA in the expanded OCC. In contrast, the inhibitors AG1478 and lapatinib prevent gonadotropin-induced cumulus expansion. Finally, the formation of oocyte-cumulus ECM relying on the covalent transfer of heavy chains of IαI molecules to HA has been inhibited in the presence of MG132.

AIM: The reactivity of the circadian clock in the suprachiasmatic nuclei (SCN) to stressful stimuli has been controversial but most studies have confirmed the resilience of the SCN to stress. We tested the hypothesis that during a critical period shortly after birth, the developing SCN clock is affected by glucocorticoids. METHODS: Mothers of 2 rat strains with different sensitivities to stress, that is Wistar rats and spontaneously hypertensive rats (SHR), and their pups were exposed to stressful stimuli every day from delivery, and clock gene expression profiles were detected in the 4-day-old pups' SCN. Levels of glucocorticoids in plasma were measured by LC-MS/MS. The glucocorticoid receptors antagonist mifepristone was administered to pups to block the effect of the glucocorticoids. RESULTS: The glucocorticoid receptors were detected at the mRNA and protein levels in the SCN of 4-day-old pups. The exposure of mothers to stressful stimuli elevated their plasma glucocorticoid levels. In Wistar rat pups, combination of daily maternal stress with their manipulation increased the plasma glucocorticoid levels and shifted the Bmal1 rhythm in the SCN which was completely blocked by mifepristone. In contrast, in SHR pups, maternal stress on its own caused phase shift of the Bmal1 expression rhythm in the SCN but the effect was mediated via glucocorticoid-independent mechanism. The Per1 and Per2 expression profiles remained phase-locked to the light/dark cycle. CONCLUSION: The results demonstrate that the SCN is sensitive to stressful stimuli early after birth in pups maintained under light/dark conditions and the effect is mediated via glucocorticoid-dependent pathways.

Cíl studie: V České republice (ČR) lze ženě ženě uměle ukončit těhotenství v I. trimestru Farmakologickou metodou od června roku 2014, pokud o to písemně požádá a je-li při ultrazvukovém vyšetření prokázáno nitroděložní jednočetné prosperující těhotenství od 42. do 49. dne sekundární amenorey, temeno-kostrční délka (crown-rump length, CRL) zárodku 2–9 mm. Cílem práce je analýza managementu umělého ukončení těhotenství Farmakologickou metodou (UUT-F), nepřesahuje-li těhotenství 7 týdnů, v pěti centrech v ČR. Typ studie: Multicentrická kohortová (prospektivní) studie. Název a sídlo pracoviště: Porodnicko-gynekologická klinika LF UP a FN Olomouc; Ústav pro péči o matku a dítě 3. LF UK, Praha; Gynekologicko-porodnická klinika 1. LF UK a VFN, Praha; Gynekologicko-porodnická klinika 1. LF UK a Nemocnice Na Bulovce, Praha; Gynekologicko-porodnická klinika LF MU a FN, Brno. Metodika: V letech 2014–2016 přišlo požádat o UUT-F celkem 1820 těhotných žen, diagnóza nitroděložního jednočetného prosperujícího těhotenství byla stanovena při ultrazvukovém vyšetření transvaginální sondou, CRL 2–9 mm. UUT-F bylo provedeno kombinací podání mifepristonu (600 mg perorálně) a misoprostolu (400 mcg perorálně) s odstupem 48 hodin. Kontrolní ultrazvukové vyšetření k vyloučení pokračování těhotenství bylo provedeno za 2–3 týdny. Výsledky: Celkem u 11,0 % žen (201/1820) bylo diagnostikováno CRL > 9 mm, neprosperující, vícečetné nebo ektopické těhotenství. U 1619 žen bylo diagnostikováno těhotenství nitroděložní jednočetné prosperující CRL 2–9 mm a výkon byl proveden, ale v 221 případech (13,7 %) byla nutná alespoň jedna klinická návštěva navíc, než bylo možné stanovit diagnózu, v 19 případech dvě návštěvy (1,2 %) a v 5 případech dokonce návštěvy tři (0,3 %). Délka sekundární amenorey v den zahájení výkonu byla 42–49 dní (průměr 47,1; medián 47), věk žen byl 14–47 let (průměr 30,7; medián 30). Celkem 20,8 % žen (336/1619) se nedostavilo na kontrolní ultrazvukové vyšetření k vyloučení pokračování těhotenství a hodnocení výsledku tudíž mohlo být provedeno pouze u 1283 žen. Selhání metody „Pokračující těhotenství“ bylo diagnostikováno u 1,6 % žen (24/1283), „Neúplný potrat“ u 6,5 % žen (83/1283) a „Kompletní potrat“ u 91,9 % (1179/1283). Následná chirurgická intervence byla provedena celkem u 7,1 % žen (91/1283). Závěr: Zdravotnické zařízení provádějící farmakologické ukončení těhotenství v I. trimestru by si mělo vypracovat vlastní metodický postup v souladu s platnou legislativou, souhrnem údajů o přípravcích a doporučením odborné společnosti. Součástí metodického postupu by měl být rovněž způsob hodnocení výsledku a management. Následná chirurgická intervence by měla být provedena pouze v indikovaných případech. Hlavním cílem kontrolního vyšetření je vyloučení selhání metody „Pokračování těhotenství“ a pacientka by měla být podrobně informována o rizicích a možnostech řešení, nutný je informovaný souhlas.
Objective: In the Czech Republic (CR), it is possible, to carry out Medical Termination of Pregnancy (MToP) in the 1st trimester since June 2014, in case a woman submits a written request for it and in case the ultrasound examination confirms an intrauterine singleton prosperous pregnancy, between day 42 and 49 of gestation, crown-rump length (CRL) of the embryo 2–9 mm. The aim of the study is to analyze the management of MToP up until the 7th week of gestation in five centres in the CR. Design: Multicenter cohort (prospective) study. Setting: Department of Obstetrics and Gynecology, Palacky University Olomouc, Faculty of Medicine and Dentistry, University Hospital Olomouc; The Institute for the Care of Mother and Child, Charles University in Prague, Third faculty of Medicine; Department of Gynecology and Obstetrics, Charles University in Prague, First faculty of Medicine, General University Hospital in Prague; Department of Gynecology and Obstetrics, Charles University in Prague, First faculty of Medicine, Hospital Na Bulovce, Prague; Department of Gynecology and Obstetrics, Masaryk University, Faculty of Medicine, University Hospital Brno. Methods: In 2014–2016, a total of 1820 pregnant women requested MToP. The diagnosis of an intrauterine singleton prosperous pregnancy was set by transvaginal ultrasound, CRL 2–9 mm. MToP was carried out by combination of mifepristone (600 mg orally) and misoprostol (400 mcg orally) within 48 hours. MToP follow up (exclusion of ongoing pregnancy) after 2–3 weeks was carried out by transvaginal ultrasound as well. Results: In 11.0% of women (201/1820) who requested MToP, CRL > 9 mm, unprosperous, multiple or ectopic pregnancy was diagnosed. In the remaining 1619 women MToP was carried out, but in 221 cases (13.7%) at least one additional pre-first visit was needed before the diagnosis of intrauterine singleton prosperous pregnancy CRL 2–9 mm could be established, in 19 cases (1.2%) two pre-first visits and in 5 cases (0.3%) even three. Gestational age was 42–49 days (average 47.1, median 47), the women were 14–47 years of age (average 30.7, median 30). In 20.8% of women (336/1619) MToP follow up was missed and of the remaining 1283 women, ongoing pregnancy (MToP failure) was diagnosed in 1.6% (24/1283), incomplete abortion in 6.5% (83/1283) and complete abortion in 91.9% (1179/1283). A subsequent surgical intervention was carried out in 7.1 % of women (91/1283). Conclusion: A medical facility performing MToP in the 1st trimester should develop its own methodology in accordance with the legislation in force, Summaries of Product Characteristics, and recommendations of professional associations. The methodology should also include a method of evaluation of the result and management. The subsequent surgical intervention should only be performed in indicated cases. The main goal of MToP follow up is to exclude ongoing pregnancy (MToP failure), and the patient should be informed in detail about the risks involved and possibilities of their solution, it is necessary to obtain an informed consent.

Farmakologické ukončení těhotenství a evakuace děložní dutiny je šetrnější než instrumentální metody. Je popsán mechanizmus působení mifepristonu a misoprostolu, rizika instrumentálních metod a farmakologických metod, postup použití přípravků a současný stav použití metody v České republice.
Pharmacological pregnancy termination and uterus evacuation is more careful than the surgical methods. Mechanism ot the mifepristone and misoprostol action is described, as well as the riscs of the surgical and pharmacologic methods, the proces of the drugs application and current state of application in the Czech Republic.
