BACKGROUND: Reconstruction of the anterior chest wall defect after sternectomy is a challenge for cardiothoracic surgeons. In 2010, the Padua group published the first case of cadaveric sternum transplantation after sternectomy. This multicenter study reports the clinical indications and early and long-term results of sternal chondral allograft transplantation. METHODS: This is a retrospective multicenter study from 7 academic centers. We collected demographic data, surgical indications, technical details, and early postoperative results. The complications, long-term stability, and tolerance of the allografts were also analyzed. RESULTS: Between January 2008 and December 2019, 58 patients underwent sternectomy followed by reconstruction using cadaveric-cryopreserved sternochondral allografts. Thirty-two patients were male, median age 63.5 years (interquartile range, 50-72 years). Indications for sternectomy were secondary sternal tumors (n = 13), primary sternal tumors (n = 15), and nonneoplastic disease (n = 30). Thirty patients underwent total sternectomy, 16 lower-body sternectomy, and 12 upper-body manubrium resection. The 30-day mortality was 5%; overall morbidity was 31%. Six early reoperations were necessary because of bleeding (n = 1), titanium plate dislocation (n = 1), and resuture of the skin in the lower part of the incision (n = 4). Overall, 5-year survival was 74%. In all the survived patients, the reconstructions were stable and free from mechanical or infective complications. CONCLUSIONS: The main indications for sternal allograft implantation were complex poststernotomy dehiscence followed by primary or secondary tumor involvement of the sternum. The collected results demonstrate that sternochondral allograft transplantation is a safe and effective method in reconstructing the anterior chest wall after sternectomy. Further studies to demonstrate the integration of the bone grafts into the patients' sternal wall will be made.

Řešení těžkých osových deformit při totální endoprotéze (TEP) kolenního kloubu je vždy výzvou pro operatéra. Autoři v článku rozebírají možnosti řešení této problematiky. Základní dělení deformit kolenního kloubu rozděluje deformity na extraartikulární a intraartikulární. Extraartikulární deformity vyžadují vždy pečlivé předoperační plánování s ohledem na umístění a tíži deformity, stav pacienta, stav končetiny i samotného kolenního kloubu. V jejich řešení přichází v úvahu tři základní možnosti: 1. osteotomie s odloženou implantací totální endoprotézy, 2. osteotomie a implantace totální endoprotézy v jedné době, 3. intraartikulární řešení extraartikulární deformity. Každá z možností má své indikace a technické aspekty. Intraartikulární deformity v zásadě dělíme na deformity valgózní a varózní. I u těchto deformit je základem úspěchu dobré předoperační plánování následované sekvenčním uvolněním měkkých tkání kolenního kloub s kompenzačními resekcemi kostní tkáně.
Solutions of severe axial deformities in TEP of the knee joint are always a challenge for the surgeon. The authors discuss the possibilities of solving this problem. The basic categorisation the of knee joint deformity divides deformities into extraarticular and intraarticular. Extraarticular deformities always require careful pre-operative planning with regard to the location and severity of the deformity, the condition of the patient, the condition of the limb and the knee itself. There are three basic options in their solution 1. osteotomy with delayed implantation of total endoprosthesis 2. osteotomy and implantation of total endoprosthesis at one time 3. intraarticular solution of extraarticular deformity Each option has its indications and technical aspects. Intraarticular deformities are basically divided into valgus and varrous deformities. Even with these deformities, good pre-operative planning is the basis for success, followed by sequential relaxation of soft tissue of the knee joint with compensatory resections of bone tissue.

The aim of this paper is to review the experimental methods currently being used to evaluate the osseointegration of nanostructured titanium implants using animal models. The material modifications are linked to the biocompatibility of various types of oral implants, such as laser-treated, acid-etched, plasma-coated, and sand-blasted surface modifications. The types of implants are reviewed according to their implantation site (endoosseous, subperiosteal, and transosseous implants). The animal species and target bones used in experimental implantology are carefully compared in terms of the ratio of compact to spongy bone. The surgical technique in animal experiments is briefly described, and all phases of the histological evaluation of osseointegration are described in detail, including harvesting tissue samples, processing undemineralized ground sections, and qualitative and quantitative histological assessment of the bone-implant interface. The results of histological staining methods used in implantology are illustrated and compared. A standardized and reproducible technique for stereological quantification of bone-implant contact is proposed and demonstrated. In conclusion, histological evaluation of the experimental osseointegration of dental implants requires careful selection of the experimental animals, bones, and implantation sites. It is also advisable to use larger animal models and older animals with a slower growth rate rather than small or growing experimental animals. Bones with a similar ratio of compact to spongy bone, such as the human maxilla and mandible, are preferred. A number of practical recommendations for the experimental procedures, harvesting of samples, tissue processing, and quantitative histological evaluations are provided.
Epidermoidní cysty jsou vzácné, benigní a pomalu rostoucí léze. Mohou dosáhnout až enormních velikostí, často bez navození neurologických příznaků. Intradiploické epidermoidní cysty tvoří 25 % ze všech dermoidů. Svůj původ mají v ektodermálních zbytcích, které přetrvávají uvnitř lebečních kostí v průběhu embryonálního vývoje. Mohou být lokalizované v kterékoliv části lebky. Referujeme desetiletou dívku s intradiploickou epidermoidní cystou frontální kosti. Vyšetření pomocí počítačové tomografie prokázalo tumorózní osteolytickou lézi frontální kosti. Tumor byl kompletně odstraněn. Biopsie potvrdila diagnózu epidermoidní cysty s nálezem vrstvy plochých epitelových buněk a zrohovatění.
Epidermoid cysts are uncommon, benign and slow-growing lesions. They can reach an enormous size without producing neurologic symptoms. Intradiploic epidermoid cysts are nearly 25% of all epidermoids. They are derived from ectodermal remnants that stay within the cranial bones during embryonic development and can be located in any part of the skull. We report a 10-year-old girl with an intradiploic epidermoid cyst in the frontal bone. Computed tomography examination revealed the tumor with osteolytic lesion of the frontal bone. The tumor was totally removed. The biopsy confirmed the diagnosis of epidermoid cyst with finding of the squamous epithelium and keratinization.

Charcotova osteoartropatie s hlubokou infekcí a osteomyelitidou u pacientů se syndromem diabetické nohy patří k nejtěžším stavům často indikovaným k amputační léčbě. Zevní fixace se řadí k modalitám přispívajícím k lepším výsledkům „limb salvage surgery” . Mezi indikace pro zevní fixaci u pacientů s hlubokou infekcí patří patologické fraktury nosného skeletu v infikovaném terénu, akutní Charcotova osteoartropatie a/nebo osteomyelitida s defekty, znemožňující fixaci dlahou, septická artritida a selhání konvenčních osteosyntéz. Za kontraindikaci zevní fixace považujeme střední až vážnou periferní arteriální chorobu bez intervence, progresivní sepsi, non-compliance pacienta. Za základní výhody externí fixace proti jiným druhům fixace pokládáme uspokojivou stabilitu, lepší komfort pro ošetření ran (včetně případného využití vakuové terapie) a relativně časnější možnost zátěže. Z technického hlediska doporučujeme zevní rámovou fixaci. Vhodná délka fixace je 12-25 týdnů při pravidelných kontrolách a úpravách fixátoru. Zevní fixace je často opomíjenou modalitou v léčbě Charcotovy osteoartropatie a osteomyelitidy u syndromu diabetické nohy. Při správné indikaci dává naději na prediktabilní a kladné výsledky léčby.
Charcot osteoarthropathy with a deep infection and osteomyelitis in patients with diabetic foot syndrome belongs to the most severe conditions that are often indicated for the amputation. The external fixation is one of the modalities contributing to better outcomes of limb salvage surgery. Pathological fractures of weight bearing skeleton in contaminated tissue, acute Charcot's osteoarthropathy and/or osteomyelitis with defects that prevent plate fixation, septic arthritis and a failure of conventional osteosynthesis are indicated for the external fixation in patients with a deep infection. The moderate and severe peripheral arteriopathy without intervention, progressive sepsis and patient's non-compliance are considered the contraindications of the external fixation. The basic benefits of the external fixation are good stability, better comfort for wounds nursing with possible application of vacuum therapy and relatively faster possibility of weight bearing. From a technical point of view, we recommend the external fixation with frame. The adequate duration of fixation is 12-25 weeks with regular check ups and adjustments of the fixator. The external fixation is a frequently neglected modality in the treatment of Charcot's osteoarthropathy and osteomyelitis in the diabetic foot syndrome. If correctly indicated it gives hope for predictable and positive outcome of the treatment.
