Úvod: Torpédo makulopatia je vzácna, vrodená ložisková lézia sietnice. Klinický obraz je typický unilaterálnou, ohraničenou, oválnou, hypopigmentovou léziou v inferotemporálnej časti makuly. Vo väčšine prípadov sa lézia nachádza pozdĺž horizontálneho rafé, má tvar torpéda a nazálny okraj smeruje do foveoly. Diagnóza je stanovená na základe jej charakteristického tvaru, lokalizácie a nálezu na optickej koherentnej tomografii (OCT). Etiológia a patogenéza vzniku torpédo makulopatie je nejasná, ale predpokladá sa, že ide o vrodený defekt retinálneho pigmentového epitelu (RPE). Cieľom publikácie je poukázať na túto diagnózu a odprezentovať nález torpédo makulopatie u dospelej pacientky. Kazuistika: 30-ročná pacientka sa dostavila na rutinné očné vyšetrenie. Vyšetrenie očného pozadia pravého oka odhalilo inferotemporálne od fovey oválnu hypopigmentovú léziu veľkosti 1 priemeru disku, na ktorú nadväzovalo satelitné ložisko v rovnakej osi smerujúcej do foveoly. Na základe OCT, OCT angiografie, fundusautofluorescencie, odberu anamnézy, typického tvaru a lokality lézie bola pacientke diagnostikovaná torpédo makulopatia na pravom oku. Záver: Vo všeobecnosti je torpédo makulopatia asymptomatická, vrodená, benígna lézia sietnice. Väčšinou je náhodne diagnostikovaná pri vyšetrení očného pozadia. Ide o stabilný nález s minimálnym rizikom poškodenia zrakových funkcií, ktorý nevyžaduje žiadnu liečbu. Napriek tomu sa vzhľadom na malé riziko vzniku choroidálnej neovaskulárnej membrány odporúča sledovanie pacientov raz ročne. Na túto diagnózu je nutné myslieť pri náleze unilaterálneho hypopigmentového ložiska inferotemporálne od fovey a v rámci diferenciálnej diagnostiky ho odlíšiť od chorioretinálnej atrofie, jazvy, kolobómu, viteliformnej dystrofie, či iných lézií RPE.
Aim: Torpedo maculopathy is an incidental, congenital retinal lesion. The typical clinical finding is a unilateral, symmetric, oval, hypopigmented lesion in the inferotemporal macula. In most cases, the lesion is along the horizontal raphe, is torpedo-shaped, and the nasal edge is directed into the foveola. The diagnosis is determined on the basis of its characteristic shape, localization and findings on optical coherence tomography (OCT). The etiology and pathogenesis of torpedo maculopathy is unclear, but it is believed to be a congenital defect of the retinal pigment epithelium (RPE). The aim of this publication is highlight this diagnosis and to present an incidental finding of torpedo maculopathy in an adult patient. Case report: A 30-year-old female patient reported for a routine eye examination. Fundus examination of the right eye revealed an oval hypopigmented lesion with a size of 1 disk diameter inferotemporally from the fovea, which was followed by a satellite lesion in the same axis directed into the foveola. Based on OCT, OCT angiography, fundus autofluorescence, and the typical shape and location of the lesion, the patient was diagnosed with torpedo maculopathy in the right eye. Conclusion: In general, torpedo maculopathy is an asymptomatic, congenital, benign retinal lesion, which is mostly diagnosed accidentally during a routine fundus examination. TM is non-progressive retinal finding with a minimal risk of deterioration of visual functions, which does not require any treatment. Nevertheless, due to the rare risk of a choroidal neovascular membrane, it is recommended to examine patients once a year. It is necessary to consider this diagnosis when a unilateral hypopigmented lesion is found inferotemporally from the fovea, and to distinguish it from chorioretinal atrophy, scar, vitelliform dystrophy, or other RPE lesions as part of the differential diagnosis.

Cíl: Práce seznamuje čtenáře s inovativními postupy v léčbě sítnicových one-mocnění, které by se mohly v následujících letech dostat do klinické praxe. Po-psány jsou retinální protézy, transplantace retinálního pigmentového epitelu (RPE), genová terapie a optogenetika.Metodika: Literární rešerše zaměřená na charakteristiky a mechanismy jednotlivých druhů terapií. Výsledky: Retinální protézy, transplantace RPE, genová terapie a optogenetika nabízejí dosud plně neprobádané možnosti a jsou považovány za budoucnost léčby sítnicových onemocnění tam, kde klasická farmakoterapie či chirurgické možnosti léčby nestačí. Nicméně všechny tyto metody jsou výzvou nejen pro samotné inovativní technické provedení, ale také pro etickou, administrativní a ekonomickou náročnost. Závěr: V léčbě sítnicových onemocnění nás čeká zajímavá budoucnost a nelze nyní odhadnout, která modalita léčby bude dominantní.
Objective: The aim of this comprehensive paper is to acquaint the readers with innovative approaches in the treatment of retinal diseases, which could in the coming years to get into clinical practice. Retinal prostheses, retinal pigment epithelial (RPE) transplantation, gene therapy and optogenetics will be described in this paper. Methodology: Describing the basic characteristics and mechanisms of different types of therapy and subsequently literary minireview clarifying the current state of knowledge in the area. Results: Retinal prostheses, RPE transplantation, gene therapy and optogenetics offer yet unexplored possibilities and are considered as the future of treatment of retinal diseases where classical pharmacotherapy or surgical treatment are no longer sufficient. However, all these methods challenge not only in the innovative technical implementation itself, but also for the ethical, administrative and economic demands. Conclusion: There will be certainly interesting development in the treatment of retinal diseases, but it is not possible to fully estimate which modality of treatment will be dominant in the future.

INTRODUCTION: Combinatory strategies using pharmacology and stem cell therapy have emerged due to their potential in the treatment of retinal pigment epithelium (RPE) cell related diseases, and a variety of different stem cell sources have been evaluated both in animal models and in humans. RPE cells derived from human embryonic stem cells (hESCs) and human induced pluripotent cells (hiPSCs) are already in clinical trials, holding great promise for the treatment of age-related macular disease (AMD) and hereditary RPE-related retinal dystrophies. Highly efficient protocol for RPE generations have been developed, but they are still time-consuming and laborious. Areas covered: The authors review RPE related diseases, as well as the known functions of RPE cells in retinal homeostasis. The authors also discuss small molecules that target RPE in vivo as well as in vitro to aid RPE differentiation from pluripotent stem cells clinically. The authors base this review on literature searches performed through PubMed. Expert opinion: Using high-throughput systems, technology will provide the possibility of identifying and optimizing molecules/drugs that could lead to faster and simpler protocols for RPE differentiation. This could be crucial in moving forward to create safer and more efficient RPE-based personalized therapies.

BACKGROUND: Despite the significant impact of retinal diseases such as wet age-related macular degeneration (wAMD) and diabetic macular edema (DME), there is a limited understanding of how these conditions are managed in Central and Eastern Europe (CEE). OBJECTIVES: To provide a comprehensive overview of the clinical and economic burden of wAMD and DME in CEE and the status quo associated with their management. METHODS: A narrative literature review was undertaken to identify existing data on wAMD and DME, including epidemiology, economic burden, clinical guidelines, and available and reimbursed treatments. Data were collected from relevant sources such as PubMed, ophthalmology associations, national statistical offices, and government agency websites; practical viewpoints were provided by local ophthalmologists and healthcare economics experts in CEE. RESULTS: Epidemiological data on wAMD and DME are limited in CEE, and intercountry comparison is difficult because of differences in data collection methodologies. There are effective treatment options for wAMD and DME, and international guidelines advocate the use of intravitreal anti-vascular endothelial growth factor injections as first-line therapy. Local expert organizations broadly support these recommendations; nevertheless, no clinical practice guidelines exist on the treatment of wAMD and DME in CEE. Access to and reimbursement of anti-vascular endothelial growth factor agents vary significantly in the region and, as a result, many patients remain untreated or inadequately treated. CONCLUSIONS: There is an urgent need for the creation of a wAMD/DME treatment program in CEE to ensure that patients have timely access to the most appropriate treatments.
