Onychomycosis is one of the most common nail disorders. Its current treatment is not satisfactorily effective and often causes adverse side effects. This study aims to determine the optimal conditions for non-thermal plasma (NTP) inactivation of the most common dermatophytes in vitro and to apply it in patient`s therapy. The in vitro exposure to NTP produced by negative DC corona discharge caused full inactivation of Trichophyton spp. if applied during the early growth phases. This effect decreased to negligible inactivation with the exposure applied six days after inoculation. In a group of 40 patients with onychomycosis, NTP therapy was combined with nail plate abrasion and refreshment (NPAR) or treatment with antimycotics. The cohort included 17 patients treated with NPAR combined with NTP, 11 patients treated with antimycotics and NTP, and 12 patients treated with NPAR alone. The combination of NPAR and NTP resulted in clinical cure in more than 70% of patients. The synergistic effect of NPAR and NTP caused 85.7% improvement of mycological cure confirmed by negative microscopy and culture of the affected nail plate. We conclude that NTP can significantly improve the treatment of onychomycosis.

Onychomykóza je infekční onemocnění nehtů způsobené zejména dermatofyty. Přes po-kroky ve farmacii je stále obtížně léčitelná. U systémových antimykotik je riziko hepatotoxicity a léko-vých interakcí, lokální antimykotika mají jen omezenou účinnost vzhledem k nedostatečnému průniku do nehtové ploténky. Výzkum se zaměřuje na nové lokální přípravky, fyzikální terapii a různé metody kombinované léčby, které by zlepšily účinnost a bezpečnost léčby onychomykóz.
Onychomycosis – topical therapy. Onychomycosis is an infectious disease of the nails caused mainly by dermatophytes. In spite of progress in pharmaceuticals, it is still difficult to treat. Sys-temic antifungals are associated with the risk of hepatotoxicity and drug interaction, while topical antifun-gals have limited efficacy due to poor penetration into the nail plate. New topical formulations, physical treatments and various combination therapies are explored in order to improve the efficacy and safety of therapy for onychomycosis

Inaktivace vláknitých hub včetně dermatofytů aplikací nízkoteplotního plazmatu byla již dříve prokázána řadou experimetálních in vitro i in vivo studií. V tomto příspěvku je na klinických případech doložena použitelnost této jednoduché a levné metody pro léčbu lidské onychomykózy.
Inactivation of fungi including dermatophytes by the action of non-thermal plasma has already been verified by numerous in vitro and in vivo studies. Feasibility of this simple and cheap method for therapy of human onychomycosis is documented here on clinical case reports.

Onychomykóza postihuje asi 5,5 % světové populace a představuje 50 % onemocnění nehtů. Existuje velké množství medikamentózních postupů topických či systémových, ale žádný nesplňuje všechny potřebné parametry. Proto vznikají další alternativní léčebné postupy. Velké množství studií po roce 2010 je věnováno využití nejrůznějších typů laserů, jejichž nasazení bylo následně schváleno a je součástí klinické praxe. S odstupem času můžeme analyzovat studie. Stále nemáme konsenzus na optimálním typu a nastavení laseru. Léčebné výsledky jsou z hlediska medicínského nedostatečné, z hlediska estetického uspokojující. Lasery nemohou být nyní považovány za vhodnou monoterapii konkurující výsledky medikamentózní léčbě. S řadou výhrad jsou využitelné k dosažení dočasných a estetických cílů. Své místo mají v druhé terapeutické vlně a asi v kombinované léčbě, kde však jsou studie příliš heterogenní ve smyslu parametrů a použitých medikamentů.
Onychomycosis affects for about 5.5 % of the world's population and accounts for 50 % of nail diseases. There are a large number of topical or systemic medication procedures, but none of them meets all the necessary parameters. That is why other alternative treatments are created. A large number of studies after the year 2010 are dedicated to the use of various types of lasers, the deployment of which has subsequently been approved and is part of clinical practice. With time, we can analyse the results of many studies. We still have no consensus on the optimal laser type and setting. The therapeutic results are medically insufficient, aesthetically satisfactory. Lasers can not now be considered as a suitable monotherapy that competes with drug treatment results. With some reservations, they can be used to achieve temporary and aesthetic goals. They have a place in the second therapeutic wave and perhaps in combination therapy, but the studies are too heterogeneous regarding parameters and used medications.
Onychomykóza je časté infekční onemocnění nehtové jednotky způsobené mikroskopickými houbami. Většina onychomykóz je způsobena dermatofyty, méně často se etiologicky uplatňují kvasinky a oportunní hyfomycety. Tato infekce tvoří až 50 % všech onemocnění nehtů. Osoby léčené pro diabetes mellitus, cévní onemocnění s trofickými změnami na periferii končetin, obezitu, HIV a imunosuprimovaní pacienti jsou predisponováni ke vzniku tohoto onemocnění. Současná léčba onychomykóz je založena na systémovém podávání či lokální aplikaci antimykotik, doplněné o mechanické či chemické odstranění postižené části nehtu. V posledních letech se alternativně uplatňují ošetření laserem či fotodynamická terapie.
Onychomycosis is a common fungal infection of the nail unit. In most cases, onychomycosis is caused by dermatophyte, occasionally can be caused by yeast and nondermathophyte fungi. Onychomycosis accounts for 50 % of all nail disease cases. Diabetes mellitus, peripheral vascular disease, HIV, obesity and immunosuppression are predisposing factors. Current treatment of onychomycosis includes oral or topical antifungal agents supplemented by chemical or mechanical debridement of the infected nail. Alternative treatment includes lasers and photodynamic therapy.
