OBJECTIVES: The vascular supply to the neck and body of the pancreas is highly variable. The dorsal pancreatic artery is the dominant artery feeding this area. The aim of this study was to describe the vascular supply of postresection pancreatic remnants after pancreaticoduodenectomy. Patients with hazardous anatomical arrangement may be at a higher risk of postresection remnant ischemia and postoperative pancreatic fistula development. METHODS: The modified Whipple procedure was performed on 20 cadaveric donors. The macroscopic anatomical supply of the postresection pancreatic remnant of each donor was evaluated. RESULTS: The arterial supply of the postresection remnant was highly variable. In 30% of cases (6/20), the dorsal pancreatic artery was cut during the pancreatoduodenectomy or it was missing. In these cases, the area of the pancreaticojejunostomy construction was fed only through anastomoses between the transverse pancreatic artery and the pancreatic branches of the splenic artery. CONCLUSIONS: In 30% of cases, the arterial supply of the postresection pancreatic remnant was dependent on inconstant intraparenchymal arterial anastomoses. These patients may be at a higher risk of postoperative pancreatic fistula development.

Gastroduodenální vředová choroba je charakterizována vředovým postižením žaludku či dvanáctníku, přičemž častěji dochází k výskytu vředových lézí v oblasti duodena. Mezi závažné komplikace tohoto onemocnění patří krvácení, perforace vředu či penetrace do okolního orgánu. Naše kazuistika prezentuje případ staršího muže, u něhož došlo k penetraci žaludečního vředu do slinivky břišní. Ta nebyla diagnostikována a pacient se do péče našeho pracoviště dostal pro recidivující hematemézu bez prokázaného zdroje krvácení. V průběhu diagnostického procesu bylo vysloveno podezření na přítomnost intramurálně uloženého tumoru žaludku. Na naší klinice byla úspěšně provedena excize suspektní léze s následným histologickým potvrzením diagnózy penetrujícího vředu. Pooperační průběh byl komplikován rozvojem pankreatické píštěle, avšak konzervativním postupem bylo dosaženo úplného zotavení pacienta.
Gastroduodenal ulcer disease is characterized by ulceration of the stomach or duodenum, with ulcer lesions occurring more frequently in the duodenum. Serious complications of this disease include bleeding, perforation of the ulcer and penetration into surrounding organ. Our report presents the case of an older man who had a gastric ulcer penetrating into the pancreas. This was not diagnosed at the time of the penetration. The patient came to our care for recurrent hematemesis without a proven source of bleeding. During the diagnostic process, the presence of an intramural gastric tumor was suspected. An excision of the suspected lesion was successfully performed at our clinic, followed by histological confirmation of perforated ulcer. The postoperative course was complicated by the development of a pancreatic fistula. However, conservative treatment resulted in complete recovery of the patient.
Formou kazuistiky upozorňujeme na riziko poranění pankreatu při urologické operaci a zároveň prezentujeme možnosti řešení těchto komplikací.
The article highlights possible risk of pancreatic injury during urologic surgery in the form of a case report and presents therapeutic options for the management of such a complication.

BACKGROUND: This dual-center, randomized controlled trial aimed to compare 2 types of intra-abdominal drains after pancreatic resection and their effect on the development of pancreatic fistulae and postoperative complications. METHODS: Patients undergoing pancreatic resection were randomized to receive either a closed-suction drain or a closed, passive gravity drain. The primary endpoint was the rate of postoperative pancreatic fistula. A secondary endpoint was postoperative morbidity during follow-up of 3 months. The planned sample size was 223 patients. RESULTS: A total of 294 patients were assessed for eligibility, 223 of whom were randomly allocated. One patient was lost during follow-up, and 111 patients in each group were analyzed. The rate of postoperative pancreatic fistula (closed-suction 43.2%, passive 36.9%, P = .47) and overall morbidity (closed-suction 51.4%, passive 40.5%, P = .43) were not different between the groups. We did not find any differences between the groups in reoperation rate (P = .45), readmission rate (P = .27), hospital stay (P = .68), or postoperative hemorrhage (P = .11). We found a significantly lesser amount of drain fluid in the passive gravity drains between the second and fifth postoperative days and also on the day of drain removal compared with closed-suction drains. CONCLUSION: The type of drain (passive versus closed suction) had no influence on the rate of postoperative pancreatic fistulae. The closed-suction drains did not increase the rate of postoperative complications. We found that the passive gravity drains are more at risk for obstruction, whereas the closed-suction drains kept their patency for greater duration.

Pankreatoduodenektomie (PD) je výkonem volby v léčbě resekabilních periampulárních a pankreatických karcinomů a u některých pacientů s chronickou pankreatitidou. PD je jedním z nejnáročnějších GI/ HPB chirurgických výkonů, který zahrnuje resekci několika orgánů a rekonstrukci s několika anastomózami. Přestože mortalita se snížila na méně než 5 %, morbidita je neustále vysoká. Pacienti podstupující PD jsou většinou starší s přidruženými komorbiditami– všeobecné komplikace po takto náročném výkonu, např. ranné, hrudní, kardiální a venózní trombembolické jsou časté. Hlavní peroperační morbiditou PD je krvácení, které může být z vícero míst, jako například z lůžka žlučníku, choledochálních žil, gastrokolického trunku, pankreatoduodenálních žil, jejunálních žil, uncinátních žil a z resekované plochy pankreatu. Při preparaci ductus choledochus může dojít k poranění aberantní arteria hepatica dextra (z arteria mesenterica superior). Pankreatický únik je definován jako přítomnost tekutiny bohaté na amylázu (>3x sérová amyláza) ve drénech po dobu delší než 3 dny. Rozděluje se na typy A (bez klinického dopadu), B (perzistující, infikovaná) a C (systémová sepse, reoperace). Vyšetřením volby je počítačová tomografie; lokalizovaná kolekce se může perkutánně zadrénovat. Velký únik si vyžaduje revizi s provedením laváže a drenáže; neměla by se pokoušet korekce či předělání anastomózy. Měkký pankreas a nedilatovaný vývod jsou dva nejdůležitější rizikové faktory pro únik z pankreatické anastomózy; role oktreotidu v prevenci úniku je diskutabilní. Předoperační drenáž žlučových cest použitím endoskopicky zavedeného stentu může snížit krvácivé komplikace regulací koagulopatie, ale zvyšuje riziko infekčních komplikací. Pooperační krvácení může být časné nebo pozdní a intraluminální nebo intraabdominální. Časné intraabdominální krvácení má chirurgickou příčinu – buď jde o uvolněnou ligaturu např. z gastroduodenální arterie, nebo jedné z žil, nebo pankreatoduodenálního lůžka; měla by se provést revize a ošetření krvácení pokud je závažné. Časné intraluminální krvácení bývá z jedné z anastomóz- nejčastěji pankreatické. UGIE vylučuje krvácení z gastroduodenální anastomózy (pokud se vyskytne, lze řešit endoskopicky). Krvácení z pankreatického pahýlu se řeší reoperací − snesením přední vrstvy anastomózy nebo provedením jejunotomie. Pozdní intraabdominální krvácení bývá z pseudoaneuryzmatu tepny, obvykle gastroduodenální, způsobeného erozí abscesem sekundárně z úniku; léčbou volby je angioembolizace. Pozdní intraluminální krvácení bývá způsobené stresovým vředem při systémové sepsi, většinou sekundárně po úniku a intraabdominální sepsi. Opožděné vyprazdňování žaludku a akutní pankreatitida jsou dalšími častými komplikacemi. Reoperace po PD jsou četné, nejčastější příčinou je krvácení, poté únik; mortalita po revizi je vysoká. Mezi dlouhodobé komplikace patří stenóza anastomózy (PJ/ HJ), endokrinní a exokrinní nedostatečnost, neschopnost přibrat na váze a špatná kvalita života. Pancreato-duodenectomy (PD) is the procedure of choice for management of resectable periampullary and pancreatic cancers and some patients with chronic pancreatitis. PD is one of the most major GI/ HPB surgical procedures performed involving resection of multiple organs and reconstruction with multiple anastomoses. While mortality of PD has been brought down to less than 5% morbidity still remains high. Patients undergoing PD are usually elderly with comorbidities – general complications of a major operation e.g. wound, chest, cardiac and venous thrombo-embolism, are common. The major intra-operative morbidity of PD is bleeding which can be from multiple sites viz. gall bladder bed, choledochal veins, gastro-colic trunk, pancreato-duodenal veins, jejunal veins, uncinate veins and cut surface of pancreas. An aberrant right hepatic artery (from the superior mesenteric artery) can be injured while dissecting the common bile duct. Pancreatic leak is defined as presence of amylase rich (>3x serum amylase) fluid in drain beyond 3 days. It is graded as A (no clinical impact), B (persists, infected) or C (systemic sepsis, reoperation). Investigation of choice is computed tomography; a localized collection can be drained percutaneously. Major leak requires reexploration at which lavage and drainage should be performed; no attempt should be made to repair or redo the anastomosis. Soft pancreas and undilated duct are the two most important risk factors for pancreatic anastomotic leak; role of octreotide to prevent the leak is debatable. Preoperative biliary drainage in the form of endoscopic stenting may reduce bleeding complications by controlling coagulopathy but increases the risk of infective complications. Postoperative bleed can be early or delayed and intra-luminal or intra-abdominal. Early intra-abdominal bleed is surgical – either a slipped ligature e.g. of the gastro-duodenal artery or one of the veins or from the pancreato-duodenal bed; reexploration should be done to control it if it is severe. Early intra-luminal bleed is from one of the anastomoses – commonest being the pancreatic. UGIE rules out bleed from the gastro/duodenal anastomosis (which if present, can be controlled endoscopically). Control of pancreatic stump bleed requires reoperation - taking down anterior layer of the anastomosis or a jejunotomy. Delayed intra-abdominal bleed is from a pseudo-aneurysm of an artery, usually gastro-duodenal caused by erosion by an abscess secondary to a leak; treatment of choice is angio-embolization. Delayed intra-luminal bleed is because of stress ulcers caused by systemic sepsis usually secondary to a leak and intra-abdominal sepsis. Other common complications include delayed gastric emptying and acute pancreatitis. Reoperations after PD are frequent – commonest cause is bleed, followed by leak; mortality of reexploration is high. Long term complications include anastomotic (PJ/ HJ) stricture, endocrine and exocrine insufficiency and inability to regain weight and poor quality of life. BACKGROUND: The morbidity of pancreatic resection remains high, with pancreatic fistula being the most common cause. The important question is whether any postoperative treatment adjustment may prevent the development of clinically significant postoperative pancreatic fistulae. Recent studies have shown that intraabdominal drains and manipulation using them are of great importance. Although authors of a few retrospective reports have described good results of pancreatic resection without the use of intraabdominal drains, a recent prospective randomized trial showed that routine elimination of drains in pancreaticoduodenectomy is associated with poor outcome. An important issue arises as to which type of drain is most suitable for pancreatic resection. Two types of surgical drains exist: open drains and closed drains. Open drains are considered obsolete nowadays because of frequent retrograde infection. Closed drains include two types: passive gravity drains and closed-suction drains. Closed-suction drains are more effective, as they remove fluid from the abdominal cavity under light pressure. However, some surgeons believe that closed-suction drains represent a potential hazard to patients and that negative pressure might increase the risk of pancreatic fistulae. Nobody has yet specifically dealt with the question of which kind of drainage is most appropriate in pancreatic surgery. METHODS/DESIGN: The aim of the DRAins in PAncreatic surgery (DRAPA) trial is to compare the closed-suction drain versus the closed passive gravity drain in pancreatic resection. DRAPA is a dual-centre, prospective, randomized controlled trial. The primary endpoint is the rate of postoperative pancreatic fistula; the secondary endpoint is postoperative morbidity with follow-up of 3 months. DISCUSSION: No study to date has compared different types of drains in pancreatic surgery. This study is designed to answer the question whether any particular type of drain might lower the rate of postoperative pancreatic fistula or other complications. AIM: To study all the aspects of drain management in pancreatic surgery. METHODS: We conducted a systematic review according to the PRISMA guidelines. We searched the Cochrane Central Registry of Controlled Trials, EMBASE, Web of Science, and PubMed (MEDLINE) for relevant articles on drain management in pancreatic surgery. The reference lists of relevant studies were screened to retrieve any further studies. We included all articles that reported clinical studies on human subjects with elective pancreatic resection and that compared various strategies of intra-abdominal drain management, such as drain vs no drain, selective drain use, early vs late drain extraction, and the use of different types of drains. RESULTS: A total of 19 studies concerned with drain management in pancreatic surgery involving 4194 patients were selected for this systematic review. We included studies analyzing the outcomes of pancreatic resection with and without intra-abdominal drains, studies comparing early vs late drain removal and studies analyzing different types of drains. The majority of the studies reporting equal or superior results for pancreatic resection without drains were retrospective and observational with significant selection bias. One recent randomized trial reported higher postoperative morbidity and mortality with routine omission of intra-abdominal drains. With respect to the timing of drain removal, all of the included studies reported superior results with early drain removal. Regarding the various types of drains, there is insufficient evidence to determine which type of drain is more suitable following pancreatic resection. CONCLUSION: The prophylactic use of drains remains controversial. When drains are used, early removal is recommended. Further trials comparing types of drains are ongoing.

OBJECTIVE: Pancreatic islet autotransplantation (IAT) has a potential to prevent brittle diabetes in patients after total pancreatectomy. Because of the fear of tumor spread, IAT has rarely been used in case of malignancy. We report our experience with patients who underwent hemipancreatoduodenectomy for carcinoma and later completion pancreatectomy for pancreatic fistula with islet autotransplantation at our institution. METHODS: From August 2007 to December 2012, 5 patients underwent IAT after completion pancreatectomy for pancreatic fistula after hemipancreatoduodenectomy for carcinoma. Islets were isolated from the pancreatic tail with the use of digestion with collagenase. Nonpurified islet suspension was infused into the portal vein during surgery. RESULTS: The median number of islets transplanted was 175,000 islet equivalents (range, 70,000-365,000). One patient died after surgery for reasons unrelated to IAT. Another 3 patients had stable diabetes with partial graft function (fasting C-peptide levels 0.23, 0.41, and 0.61 nmol/L and HbA1c 4.8%, 4.6%, and 6.9% at 24, 24 and 9 months after IAT, respectively). The 1st patient, with pancreatic head carcinoma, was alive 28 months after IAT with lymph node and liver recurrence since 18 months after IAT. The 2nd patient, with gall bladder and distal bile duct carcinoma, died 47 months after IAT with tumor recurrence. The 3rd patient, with ampullary carcinoma, died 12 months after IAT with local recurrence and solitary liver metastasis. The last patient had been off insulin 9 months after IAT without tumor recurrence (fasting C-peptide, 0.89 nmol/L; HbA1c, 4.2%). CONCLUSIONS: Autotransplantation of pancreatic islets isolated from the residual pancreatic tissue in patients who previously underwent hemipancreatoduodenectomy for cancer may provide stable glucose control and thus improve quality of life. In this small series we did not observe early development of multiple liver metastases caused by islet suspension contamination with malignant cells. Oncologic outcome of the patients was not worse than what would be expected without IAT.

BACKGROUND: Postoperative pancreatic fistula is the main cause of morbidity after pancreatic resection. This study aimed to quantify the clinical and economic consequences of pancreatic fistula in a medium-volume pancreatic surgery center. METHODS: Hospital records from patients who had undergone elective pancreatic resection in our department were identified. Pancreatic fistula was defined according to the International Study Group on Pancreatic Fistula (ISGPF). The consequences of pancreatic fistula were determined by treatment cost, hospital stay, and out-patient follow-up until the pancreatic fistula was completely healed. All costs of the treatment are calculated in Euros. The cost increase index was calculated for pancreatic fistula of grades A, B, and C as multiples of the total cost for the no fistula group. RESULTS: In 54 months, 102 patients underwent elective pancreatic resections. Forty patients (39.2%) developed pancreatic fistula, and 54 patients (52.9%) had one or more complications. The median length of hospital stay for the no fistula, grades A, B, and C fistula groups was 12.5, 14, 20, and 59 days, respectively. The hospital stay of patients with fistula of grades B and C was significantly longer than that of patients with no fistula (P<0.001). The median total cost of the treatment was 4952, 4679, 8239, and 30 820 Euros in the no fistula, grades A, B, and C fistula groups, respectively. CONCLUSIONS: The grading recommended by the ISGPF is useful for comparing the clinical severity of fistula and for analyzing the clinical and economic consequences of pancreatic fistula. Pancreatic fistula prolongs the hospital stay and increases the cost of treatment in proportion to the severity of the fistula.

Pankreatická píštěl je poměrně častou komplikací po resekci pankreatu. Její incidence se udává mezi 10 a 30 %. Rozdíly incidence mezi jednotlivými studiemi jsou dány především rozdílnými definicemi píštěle. Pankreatická píštěl podle ISGPF je definována jako výdej jakéhokoliv měřitelného množství tekutiny peroperačně zavedenými drény (nebo drény perkutánně zavedenými následně po operaci) po třetím pooperačním dnu, pokud je koncentrace amyláz v tekutině alespoň 3krát vyšší, než je normální sérová koncentrace. Pankreatická píštěl se dělí do 3 stupňů podle klinické závažnosti (A, B, C). Jsou známé tři různé kategorie rizikových faktorů pro vznik pankreatické píštěle: související s onemocněním pankreatu, související s pacientem a související s operačním výkonem. Většinu rizikových faktorů vzniku pankreatické píštěle nelze ovlivnit před operací ani během operace. Možnosti prevence vzniku pankreatické píštěle jsou v zásadě dvě: farmakologická intervence (podání somatostatinu nebo jeho derivátů) a technické modifikace ošetření zbytkového parenchymu pankreatu. Z dosažitelné literatury vyplývá, že rutinní podání somatostatinu nebo jeho derivátů u všech operací pankreatu není doporučeno. Spíše je vhodné jeho selektivní podání v případech, kdy je vyšší riziko vzniku pankreatické píštěle. Druhou možností snížení četnosti pankreatické píštěle jsou různé modifikace ošetření zbytkového parenchymu po resekci pankreatu. Většina studií, které testovaly různé způsoby ošetření, byla retrospektivních s nižším stupněm průkaznosti. Dobře vedených randomizovaných studií bylo pouze několik a většina z nich neprokázala výraznou výhodu určité techniky nad jinou. Ukázalo se, že z velké části záleží na zkušenostech centra a především individuálních zkušenostech chirurga, který resekci pankreatu provádí. Terapie pankreatické píštěle se řídí klinickou závažností. Ve většině případů se postupuje konzervativně. V případech, kdy je nutná reoperace, jsou možné dvě strategie: chirurgická otevřená drenáž kolekcí nebo dokončení pankreatektomie. V minulosti bylo preferováno dokončení pankreatektomie, které je však technicky velmi náročné, je zatíženo vysokou morbiditou a letalitou. V současnosti se většina autorů přiklání k chirurgické drenáži. Tento postup je technicky méně náročný, má nižší letalitu, je zachována endokrinní funkce pankreatu a ve většině případů stav nevyžaduje další reoperace. Pancreatic fistula is a common complication after pancreatic resections. Its incidence oscillates between 10 and 30%. The differences in the incidence cited in the studies are due to the various fistula definitions. According to ISGPF, pancreatic fistula is an output -via an operatively placed drain (or a subsequently placed percutaneous drain) - of any measurable volume of drain fluid on or after postoperative day 3, with an amylase content higher than 3 times the upper normal serum value. The fistula is then classified according to the clinical impact in grades A, B, and C. There are known three risk factor categories for the development of pancreatic fistula: the risk related to the pancreatic disease, to the patient, and to the surgical procedure. Most of the risk factors for the development of pancreatic fistula cannot be influenced either prior to or during the surgery. There are two basic options for the prevention of pancreatic fistula: pharmacological intervention (administration of somatostatin and its analogues) and technical modifications of the pancreatic remnant treatment. However, the routine administration of somatostatin and its analogues is not advisable in all pancreatic surgical procedures. In high risk cases the selective administration is preferred. The second option is modification of pancreatic remnant treatment. Most of the studies dealing with various modifications of the pancreatic remnant treatment were retrospective with lower level of evidence. There were only a few properly designed randomized trials, and most of them did not prove benefit of one method over another. It has been shown that the results depend on the experience of a given surgical department, and above all on the experience of an individual surgeon who performs the pancreatic resection. The therapy of pancreatic fistula is based on the clinical severity. Conservative approach is warranted in most patients. In cases when reoperation is required, there are two basic strategies: surgical drainage of the collections, and completion of total pancreatectomy. Total pancreatectomy was preferred in the past, however, this procedure is technically very demanding with mortality up to 80 per cent. Nowadays, most of the authors prefer surgical drainage; this procedure is technically less demanding, has lower mortality, the endocrine function of pancreas is protected, and the patients usually need no further interventions.
