S pacientmi po lumbálnej punkcii a po neuroaxiálnej anestézii sa stretávame naprieč mnohými odbormi. Medzi najčastejšie komplikácie daných výkonov patrí postpunkčná cefalea, pričom subdurálny hematóm patrí medzi ich zriedkavé komplikácie s možným závažným až letálnym dopadom. Vzhľadom na rovnakú, resp. podobnú manifestáciu týchto dvoch diagnóz môže dôjsť často k oneskorenému rozpoznaniu SDH a jeho liečbe, eventuálne aj k jeho prehliadnutiu. Na túto diagnózu treba pomýšľať pri prolongovaných bolestiach hlavy nereagujúcich na klasickú analgetickú liečbu, pri strate väzby na ortostatickú polohu, prípadne pri pridružených parézach hlavových nervov či poruche vedomia. Nasledujúci článok dokumentuje kazuistiku bilaterálneho subdurálneho hematómu po epidurálnej pôrodnej analgézii a poskytuje literárny prehľad o danej problematike.
We come across with patients after lumbar puncture and neuraxial anesthesia athwart many disciplines. Among the most frequent complications of these procedures are post-operative cephalea, whereas subdural hematoma is one of its rarest complications with a potentially severe or even lethal impact. Given the same resp. a similar manifestation of the two diagnoses there may often be a delay in the recognition of SDH and its treatment, eventually to its overlooking. This diagnosis should be considered with prolonged headaches unresponsive to classical analgesic treatment, loss of binding to the orthostatic position, prospectively associated paresis of the cranial nerves or consciousness disorder. The following article documents the case study of bilateral subdural hematoma following epidural birth analgesia and provides a literally overview of the issue concerned.

BACKGROUND: Rocuronium for cesarean delivery under general anesthesia is an alternative to succinylcholine for rapid-sequence induction of anesthesia because of the availability of sugammadex for reversal of neuromuscular blockade. However, there are no large well-controlled studies in women undergoing general anesthesia for cesarean delivery. The aim of this noninferiority trial was to determine whether rocuronium and sugammadex confer benefit in time to tracheal intubation (primary outcome) and other neuromuscular blockade outcomes compared with succinylcholine, rocuronium, and neostigmine in women undergoing general anesthesia for cesarean delivery. METHODS: We aimed to enroll all women undergoing general anesthesia for cesarean delivery in the 2 participating university hospitals (Brno, Olomouc, Czech Republic) in this single-blinded, randomized, controlled study. Women were randomly assigned to the ROC group (muscle relaxation induced with rocuronium 1 mg/kg and reversed with sugammadex 2-4 mg/kg) or the SUX group (succinylcholine 1 mg/kg for induction, rocuronium 0.3 mg/kg for maintenance, and neostigmine 0.03 mg/kg for reversal of the neuromuscular blockade). The interval from the end of propofol administration to tracheal intubation was the primary end point with a noninferiority margin of 20 seconds. We recorded intubating conditions (modified Viby-Mogensen score), neonatal outcome (Apgar score <7; umbilical artery pH), anesthesia complications, and subjective patient complaints 24 hours after surgery. RESULTS: We enrolled 240 parturients. The mean time to tracheal intubation was 2.9 seconds longer in the ROC group (95% confidence interval, -5.3 to 11.2 seconds), noninferior compared with the SUX group. Absence of laryngoscopy resistance was greater in the ROC than in the SUX groups (ROC, 87.5%; SUX, 74.2%; P = 0.019), but there were no differences in vocal cord position (P = 0.45) or intubation response (P = 0.31) between groups. No statistically significant differences in incidence of anesthesia complications or in neonatal outcome were found (10-minute Apgar score <7, P = 0.07; umbilical artery pH, P = 0.43). The incidence of postpartum myalgia was greater in the SUX group (ROC 0%; SUX 6.7%; P = 0.007). The incidence of subjective complaints was lower in the ROC group (ROC, 21.4%; SUX, 37.5%; P = 0.007). CONCLUSIONS: We conclude that rocuronium for rapid-sequence induction is noninferior for time to tracheal intubation and is accompanied by more frequent absence of laryngoscopy resistance and lower incidence of myalgia in comparison with succinylcholine for cesarean delivery under general anesthesia.

BACKGROUND: The purpose of this national survey was to determine current anesthesia practices for cesarean delivery in the Czech Republic. METHODS: In November 2011, we invited all departments of obstetric anesthesia in the Czech Republic to participate in a prospective study to monitor consecutive peripartum obstetric anesthesia procedures. Data were recorded online in the TrialDB database (Yale University, New Haven, CT). RESULTS: The response rate was 51% (49 of 97 departments); participating centers represented 60% of all births in the country during the study period. There were 1943 cases of peripartum anesthesia care, of which 1166 cases (60%) were anesthesia for cesarean delivery. Estimates were weighted based on population distribution of cesarean delivery among types of participating centers. Neuraxial anesthesia was used in 55.6% (95% confidence interval [CI], 52.8%-58.5%); the distribution of anesthesia techniques differed among type of participating center. The rate of neuraxial anesthesia in university hospitals was 55.6% (95% CI, 51.5%-59.6%), 32.4% (95% CI, 26.4%-39.0%) in regional hospitals, and 60.7% (95% CI, 55.2%-66.0%) in local hospitals. The reasons for cesarean delivery under general anesthesia were emergency procedure (67%), refusal of neuraxial blockade by parturient (30%), failure of neuraxial anesthesia (6%), and preoperative administration of low-molecular-weight heparin (3%). Postcesarean analgesia was primarily provided by systemic opioid (66%) and nonopioid analgesics (61%), solely or in combination. Epidural postoperative analgesia was used in 14% of cases. Compared with national neuraxial anesthesia rate data published in the 1990s (6.7% in 1993), there has been an upward trend in the use of neuraxial anesthesia for cesarean delivery during the 21st century (40.5% in 2000) in the Czech Republic. CONCLUSIONS: The rate of neuraxial anesthesia use for cesarean delivery has increased in the Czech Republic in the last 2 decades. However, the current rate of general anesthesia is high compared with other Western countries.

Hypotonie/atonie děložní je nejčastější příčinou peripartálního život ohrožujícího krvácení. Cílem článku je podat informace o současných možnostech léčby této závažné komplikace. Podrobněji jsou rozebrány farmakologické vlastnosti uterotonik, jejichž nesprávné podání může paradoxně vést ke zhoršení kritického stavu. Současně je zdůrazněna nutnost mezioborové spolupráce při řešení hypotonie/atonie děložní.
Uterine hypotony/atony is the most common cause of life-threatening peripartum bleeding. The article presents information on the current treatment approaches for this serious complication. The pharmacological properties of the uterotonic agents are discussed in detail. Their incorrect administration may paradoxically lead to a deterioration in the critical condition. We also emphasize the need for concurrent interdisciplinary co-operation in the management of uterine hypotony/atony.

Epidurální analgezie je stále považována za „zlatý“ standard tlumení porodní bolesti, a to zejména pro svoji účinnost. Přesto je u části rodiček její aplikace kontraindikována. V těchto případech je možnou alternativou systémová aplikace remifentanilu. Cílem tohoto článku je přinést aktuální souhrn informací o účinnosti a bezpečnosti remifentanilu u porodu. Dalším cílem bylo poukázat na možné způsoby aplikace, dávkování a případné nežádoucí účinky pro rodičku, plod či novorozence. Některé z těchto otázek zůstávají nadále otevřené a pro jejich zodpovězení je potřeba dalších randomizovaných studií.
Epidural analgesia is still considered the “gold standard“ for labour analgesia, especially because of its efficacy. In case of its contraindication, systemic administration of remifentanil is possible. The aim of this article is to provide a summary of current information on the efficacy and safety of remifentanil during labour. There are further described possible ways of application, dosage and side effects for the mother, fetus or newborn. Some of these questions remain open and further randomized trials are needed.

Predikce intenzity porodních bolestí, stejně jako průběhu porodu, je velmi obtížná a obzvláště u prvorodiček téměř nemožná. Některé ženy porodní bolest tolerují dobře, jiné vyžadují její účinné tlumení. Někdy pomohou zcela jednoduché postupy, jako jsou přítomnost partnera, relaxační prostředky, masáže, chůze, jindy je třeba pomoc porodní asistentky či lékaře-porodníka, nebo dokonce anesteziologa. Výběr postupu nebo metody by měly záviset vždy na aktuální situaci a na volbě/individuálním rozhodnutí rodičky. V této souvislosti byl roce 2013 založen v České republice Program INKA. Název vznikl z akronymuI (informovaná těhotná), N (nabídka služeb/servis), K (kvalita poskytovaných služeb) a A (analýza systému poskytované péče). Smyslem tohoto programu je rozšířit informovanost pro nejširší populaci těhotných o současných možnostech v tlumení porodních bolestí. Informace musí objektivně a srozumitelně popisovat výhody a nevýhody každé analgetické metody a nesmí žádnou z metod upřednostňovat ani potlačovat. V této souvislosti byl na konci roku 2013 vydán tiskem materiál, který pod názvem „Porod nemusí až tak bolet“ umožňuje informovat laickou veřejnost o všech dostupných metodách analgezie u porodu v naší zemi. Materiál byl oficiálně schválen i výbory ČGPS a ČSARIM ČLS JEP. Na personál porodnických zařízení jsou v rámci programu INKA kladeny požadavky, aby vznikal co nejvíce individuální přístup k potřebám každé rodičky. I ženám s původně negativním postojem vůči porodnické analgezii by měla být kdykoli k dispozici možnost nabídnout adekvátní a účinnou pomoc v případech, že se porod nevyvíjí podle jejich původních představ.
Setting: Department of Obsterics and Gynecology 1st Faculty of Medicine, Charles University and General Faculty Hospital in Prague. Objective: Predicting both the course of childbirth and the intensity of pain felt during childbirth is difficult, especially with first-time mothers. Some women tolerate birthing pains well, others require effective pain relief. Sometimes simple methods are enough, such as the presence of a partner, a relaxing atmosphere, massages, or walking, while other times the services of a birthing assistant or physician or even an anaesthesiologist are required. Choosing the right approach or methods should always be dependent on the actual situation and the individual choices and preferences of the mother. In line with this, the INKA Program was initiated in the Czech Republic in 2013. The name is an acronym derived from the Czech words for informed (as in the expectant mother), choice (of services), quality (of services offered) and analysis (of the system of care offered). The aim of this program is to distribute information about current possibilities for decreasing the pain of childbirth to the widest possible population of expectant mothers. The information must be objective and clearly explain the benefits and risks of each analgesic method, and must not encourage or discourage any particular method. As part of this program, a brochure titled “Birth doesn’t have to be that painful” was distributed at the end of 2013, to inform the lay public about all available methods of analgesia for childbirth in the Czech Republic. The contents of the brochure were approved by the Czech Gynecological and Obstetrical Society and the Czech Society of Anaesthesiology and Intensive Care Medicine. The INKA program encourages the personnel of birthing centres to arrange the most individual approaches possible for the needs of each expectant mother. Even women that originally had negative attitudes toward obstetric analgesics should have the opportunity for adequate and effective help at any time, if the birth does not proceed according to their original expectations.

Postpunkční cefalea (PDPH – postdural puncture headache) je v porodnictví poměrně častá (okolo 1 % všech neuro-axiálních punkcí) iatrogenní komplikace. Důvodem jejího častějšího výskytu u rodiček oproti běžné pacientské populaci jsou především fyziologicko-patofyziologické rozdíly těhotných žen, ale i stoupající počet aplikovaných neuroaxiálních blokád. Protože neléčený postpunkční syndrom může vyústit až ve fatální komplikace, je hlavní zásadou léčby komplexní a multimodální přístup skládající se nejen z využití všech prostředků konzervativní i invazivní terapie, ale též odpovídající komunikace a psychické podpory pacientky. Cílem tohoto článku je podat přehled a současný pohled na patofyziologii a léčbu této komplikace.
Postdural puncture headache (PDPH) is a relatively common iatrogenic complication (about 1% of all neuroaxial punctures) in obstetrics. The reasons for higher incidence in pregnant women compared to the general patient population are mainly the physiological and pathophysiological differences in pregnant women, but also the increasing number of neuroaxial blockades used. Because untreated postdural puncture syndrome can result in fatal complications, the basic principle of treatment is a complex multimodal approach consisting of all available conservative and invasive therapies, adequate communication and psychological support of the patient. The aim of this article is to provide an overview and current views on the pathophysiology and treatment of this complication.
