INTRODUCTION: Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by deficits in social communication and the presence of restricted interests and repetitive behaviors. Transient hyperphosphatasemia of infancy and early childhood (THI) is a benign laboratory disorder characterized by transiently extremely elevated activity of serum alkaline phosphatase (S-ALP). CASE REPORT: We present a 21-month-old girl with a right leg limp, most probably due to reactive arthritis after febrile viral infection, and deterioration of psychomotor development with concomitant transient elevation of S-ALP (61.74 μkat/L; normal 2.36-7.68 μkat/L). Normal values of serum creatinine, aspartate-aminotransferase, alanin-aminotransferase, calcium, phosphate, together with normal wrist X-ray ruled out rickets or other bone or hepatic cause of high S-ALP. The S-ALP gradually decreased within 3 months, thus fulfilling the THI criteria. Screening for inborn errors of metabolism was negative and meticulous neurologic, psychologic and psychiatric assessment pointed to the diagnosis of autism spectrum disorder (ASD). There was no causal relationship between THI and ASD, as high S-ALP was an accidental and transient finding within the routine laboratory assessment. However, when THI occurs in a child with an onset of a new disorder, or with a pre-existing bone or liver disease, it might seriously concern the physician. CONCLUSION: Children with THI should be spared from extensive evaluations and unnecessary blood draws.

V posledních dvou dekádách se pozornost odborné veřejnosti výrazněji obrátila směrem k poruchám autistického spektra (PAS). V mnoha zemích včetně České republiky byl zaveden včasný záchyt PAS. Také v důsledku povinného screeningu se počet diagnostikovaných dětí s PAS neustále zvyšuje. Ze současných výzkumů vyplývá, že některou z forem autismu trpí 1 až 2 % populace. Pro zlepšení prognózy dítěte s autismem je rozhodující včasné zachycení prvních příznaků, následná diagnostika a neprodlené zahájení efektivní intervence zaměřené na zmírnění jádrových symptomů PAS a rozvoj klíčových dovedností dítěte. Článek představuje průběh včasného záchytu PAS v ordinacích praktických lékařů pro děti a dorost, který byl v České republice zaveden v roce 2016, a navazující diagnostický proces prováděný dětským a dorostovým psychiatrem. Reaguje na odborníky a rodiči pociťovaný nedostatek efektivní včasné intervence a seznamuje s behaviorálními přístupy při rozvoji dovedností dítěte s PAS v raném věku. Věnuje se zejména včasné intenzivní behaviorální intervenci, která je založena na principech aplikované behaviorální analýzy a patří mezi doporučené postupy psychiatrické péče vydané Psychiatrickou společností ČLS JEP i dalšími odbornými organizacemi. V současné době se jedná o jednu z mála behaviorálních a psychoterapeutických intervencí u autismu, které se zaměřují na rozvoj klíčových komunikačních a sociálních dovedností dětí s neurovývojovým postižením, a její efektivita je podložena dostatečným množstvím kvalitní vědecké evidence.
In the last two decades, the professional community focused its attention on autism spectrum disorders (ASD). Many countries including the Czech Republic started to administer early screeing of ASD. The prevalence of ASD is steadily increasing. Current research suggests that between 1 % and 2 % of the population suffer from some form of autism. Screening of early symptoms, subsequent diagnostics and prompt initiation of effective intervention aimed at the core symptoms of ASD and developing the child ́s key skills are critical for improving the prognosis ofa child with autism. This article presents the process of ASD screening at general practitioners ́ offices, which was initiated in the Czech Republic in 2016, and the follow-up diagnostic process performed by a child and adolescent psychiatrist. It responds to the lack of effective early intervention perceived by professionals and parents and introduces approaches for developing critical skills of a child with ASD at an early age. It focuses on Early Intensive Behavioral Intervention, which is based on the principles of applied behavior analysis and is one of the recommended practices of the Czech Psychiatric Society and other renown professional organizations. Currently, it is one of the few behavioral and psychotherapeutic interventions for autism that focuses on the development of key communication and social skills in children with neurodevelopmental disabilities and its effectiveness is supported by a sufficient amount of high-quality scientific evidence.

The review about the autism spectrum disorders includes the recent knowledges about their prevalence, etiopathogenesis and a description of their clinical findings. It gives the basic attributes about diagnosis, course, prognosis and the overview of the PAS therapy. It mentions the key role of pediatricians, highlighting their ability to identify early signs, symptoms and to provide an early diagnostics.
Vývojová dysfázie je charakterizována narušením vývoje jazyka. Systémově je zařazena mezi novou kategorii jedenácté revize Mezinárodní klasifikace nemocí s názvem Neurovývojové poruchy. Jejich koncept vznikl na podkladě poznání obdobné etiopatogeneze: silné genetické pozadí vede ke strukturálním změnám mozku. Mezi další společné znaky patří komorbidní charakter se vzájemným překrýváním, tendence přetrvávat a důsledky v sociální, emocionální a akademické oblasti. Poruchy mají významně odlišné publikační indexy, přičemž vývojová dysfázie, přestože patří k poruchám s nejvyšší prevalencí, je stále méně známá. Navíc po dlouhá léta nebyla sjednocená její terminologie a symptomatologie. V letech 2016 a 2017 došlo ke konsenzu anglofonních zemí v terminologii a identifikaci poruchy, upřesňuje se diagnostika, nejméně studií s prokázanými výsledky pochází ze samotné léčby. V ČR dosud neexistovalo standardní opatření pro stanovení diagnózy a navržení intervence. Z toho důvodu předkládáme doporučení, které vychází jednak ze současných zahraničních poznatků a jednak ze současného stavu českého diagnostického materiálu i českých studií a obecně českého prostředí, neboť diagnostika poruchy je vázána na jazykové a kulturní prostředí. Hlavním oborem v procesu diagnostiky a léčby vývojové dysfázie je klinická logopedie. Nezbytným oborem je rovněž dětská neurologie. Příspěvek seznamuje s postupem diagnostiky, diferenciální diagnostiky, doplňujících vyšetření pro suspekce komorbidních chorob, specifikuje víceoborový tým a doporučuje průběh terapie.
Developmental dysphasia is characterized by a disruption in language development. It is systematically classified as a new category of the 11th revision of the International Classification of Diseases called Neurodevelopmental Disorders. Their concept was developed on the basis of the recognition of a similar etiopathogenesis: a strong genetic background leads to structural changes in the brain. Other common features include a comorbid nature with mutual overlap, a tendency to persist and consequences in the social, emotional and academic domains. The disorders have significantly different publication indices, and developmental dysphasia, although one of the disorders with the highest prevalence, is still less well known. Moreover, for many years, its terminology and symptomatology have not been unified. In 2016 and 2017, there has been a consensus among English-speaking countries in terminology and identification of the disorder, the diagnosis is being refined, and the fewest studies with proven results come from the treatment itself. In the Czech Republic, there has not yet been a standard measure for making a diagnosis and proposing an intervention. For this reason, we present recommendations that are based both on current international knowledge and on the current state of Czech diagnostic material and Czech studies and the Czech environment in general, as the diagnosis of the disorder is linked to the linguistic and cultural environment. The main field in the process of diagnosis and treatment of developmental dysphasia is clinical speech therapy. Pediatric neurology is also an indispensable field. This manuscript introduces the process of diagnosis, differential diagnosis, and additional examinations for suspected comorbid conditions, specifies the multidisciplinary team and recommends a course of therapy.
Kanne and Bishop's (2020) Editorial Perspective 'The Autism waitlist crisis and remembering what families need' offers a strong argument to provide greater access to high-quality assessments for Autism Spectrum Disorder (ASD). They note, correctly, that due to increasing numbers of referrals practitioners are under increasing pressure to provide quicker or abbreviated evaluations, that some cases are extremely complex and require considerable expertise to assess, and that a good assessment is a good investment in effective intervention. I agree with these points but also want to highlight some difficulties and dilemmas associated with the assessment of ASD; and to argue that improving access to assessments and interventions through the use of nonspecialists and new technologies may be a promising direction.

Autism spectrum disorder (ASD) represents a serious neurodevelopmental disorder associated with autonomic nervous system dysregulation. The aim was to study complex cardiovascular autonomic regulation using heart rate variability (HRV) and systolic blood pressure variability (SBPV) linear/non-linear analysis at rest and during orthostasis, and to assess plasma levels of epidermal growth factor (EGF) and vascular endothelial growth factor (VEGF) in autistic children. Twenty-five ASD boys and 25 age and gender-matched children at the age 7-15 years were examined. After venous blood taking, continuous ECG and blood pressure biosignals were recorded at rest and during orthostasis. Evaluated parameters: RR intervals, high- and low-frequency band of HRV spectral analysis (HF-HRV, LF-HRV), symbolic dynamics parameters 0V%, 1V%, 2LV%, 2UV%, low- and high-frequency band of SBPV (LF-SBPV, HF-SBPV), systolic, diastolic, mean blood pressure, EGF, VEGF plasma levels. RR intervals were significantly shortened and the HF-HRV, LF-SBPV, HF-SBPV parameters were significantly lower at rest, the HF-HRV and LF-SBPV remained lower during orthostasis in autistic children compared to controls (p<0.05). EGF plasma levels were significantly lower in ASD compared to controls (p=0.046). No significant differences were found in remaining parameters. Our study revealed tachycardia, cardiovagal underactivity, and blunted sympathetic vasomotor regulation at rest and during orthostasis in autistic children. Additionally, complex heart rate dynamics are similar in autistic children than controls. Furthermore, EGF was reduced in autistic children without significant correlations with any autonomic parameters. We suggest that the abnormal complex cardiovascular reflex control could contribute to understanding the pathway linking autonomic features and autism.

Recent research has brought mixed results about differences in attention allocation measured by eye tracking between autistic and neurotypical individuals. There are several reasons that might be at the core of this confusion, including the given task. We tested 10 neurotypical adults in our experiment: they filled out a questionnaire based on self-evaluation assessing their position on the autistic spectrum. Their eye movements were recorded while viewing several complex visual scenes in two conditions: free viewing and completing a task. We qualitatively compared the visual strategies of two participants scoring on extreme poles of the spectrum in the analysis. Our findings indicate that the task might play a crucial role in research on attention allocation in autism.
