INTRODUCTION: The prognostic value of B-cell lymphoma 2 (BCL2) expression in de novo diffuse large B-cell lymphoma (DLBCL) treated with immunochemotherapy is of interest to define a target patient population for clinical development of BCL2 inhibitors. We aimed to develop a reproducible immunohistochemistry algorithm and assay to determine BCL2 protein expression and assess the prognostic value of BCL2 in newly diagnosed DLBCL cohorts. PATIENTS AND METHODS: The prospectively defined algorithm incorporated BCL2 staining intensity and percentage of BCL2-positive cells. Functionally relevant cutoffs were based on the sensitivity of lymphoma cell lines to venetoclax. This assay was highly reproducible across laboratories. The prognostic impact of BCL2 expression was assessed in DLBCL patients from the phase 3 MAIN (n = 230) and GOYA (n = 366) trials, and a population-based registry (n = 310). RESULTS: Approximately 50% of tumors were BCL2 positive, with a higher frequency in high International Prognostic Index (IPI) and activated B-cell-like DLBCL subgroups. BCL2 expression was associated with poorer progression-free survival in the MAIN study (hazard ratio [HR], 1.66; 95% confidence interval [CI], 0.81-3.40; multivariate Cox regression adjusted for IPI and cell of origin). This trend was confirmed in the GOYA and registry cohorts in adjusted multivariate analyses (GOYA: HR, 1.72; 95% CI, 1.05-2.82; registry: HR, 1.89; 95% CI, 1.29-2.78). Patients with BCL2 immunohistochemistry-positive and IPI-high disease had the poorest prognosis: 3-year progression-free survival rates were 51% (GOYA) and 37% (registry). CONCLUSION: Findings support use of our BCL2 immunohistochemistry scoring system and assay to select patients with BCL2-positive tumors for future studies.

Pleomorfný adenóm morfologicky charakterizuje bifázický vzorec s obsahom epiteliálnej a mezenchymálnej komponenty. V súčastnosti je akceptovaný jeho čisto epiteliálny pôvod. Onkogenéza tohoto nádoru, rekapitulujúca rast a obnovu dukto-acinárnej jednotky normálnej slinnej žľazy, vychádza z nádorovej proliferácie a diferenciácie jedinej bunky dávajúcej vznik elementom oboch zložiek. Potvrdením tejto pluripotentnej unicelulárnej teórie je priekaz kmeňových buniek, náchádzajúcich sa v analogických lokalizáciách v tkanive pleomorfného adenómu a normálnej žľazy. Jedným z markerov kmeňových, apoptóze nepodliehajúcich, buniek, je bcl-2 onkoproteín. V retrospektívnej štúdii autori sledovali jeho imunoexpresiu a distribúciu v jednotlivých bunkových populáciách 24 primárnych pleomorfných adenómov. Bcl-2 pozitivita bola prítomná v 95,8 % nádorov (91,7 % v epiteliálnej a v 62,5 % v myxoidnej komponente). Pravidelná a najintenzívnejšia pozitivita bola v bunkách bazálnej vrstvy štruktúr napodobňujúcich exkretorické vývody, ich luminálne bunky boli vždy negatívne. Na ich periférii a v intermediárnych bunkách bola slabá až stredná pozitivita. Väčšina jednotlivých, vretenovitých a hviezdicovitých buniek myxoidnej komponenty javila negatívnu, menšia časť slabú či strednú, a len ojedinele silnú bcl-2 imunoreakciu. Imunoreakcia vždy úplne chýbala v oblastiach sporadicky pozorovanej skvamocelulárnej metaplázie. Výsledky štúdie sú potvrdením pluripotentnej unicelulárnej teórie tumorigenézy pleomorfného adenómu. Autori považujú bcl-2 gén za kľúčový pre vznik pleomorfného adenómu za účasti ďalších, tu nešpecifikovaných antiapoptotických mechanizmov.
Pleomorphic adenoma is morphologically characterized as biphasic tumour, including epithelial and mesenchymal components. At present its epithelial origin is accepted. Pluripotent unicellular theory is the most plausible tumorigenesis theory. According this theory, the steam cell is the origin for renewal of mature salivary gland tissue as well as for tumour growth. This cell is the basis for epithelial as well as mesenchymal tumour components. Bcl-2 (b-cell lymphoma 2) gene is the steam cell marker that protects cell against apoptosis. In retrospective study, the authors followed up immunoexpression and distribution of bcl-2 oncoprotein in single cell population of 24 primary pleomorphic adenomas. Bcl-2 positivity was detected in 95.8% pleomorphic adenomas (91.7% in epithelial and 62.5% in mesenchymal components). The most intensive immunostaning was found in the outer cells of well differentiated excretory duct resembling formations. The cells of the inner layer were always negative. Week to moderate reaction was present in cells at periphery of excretory-like ductal formations as well as in “intermediate” cells in some unstructured solid regions. The majority of mesenchymal cells were negative, minority showed week to moderate positivity, only occasionally with strong immunoreaction. The occasional foci of squamous cell metaplasia were bcl-2 negative. The study confirmed the pluripotent unicellular theory. Authors consider bcl-2 gene as the key element in pleomorphic adenoma genesis, in cooperation with other antiapoptotic mechanisms.

AIM: To evaluate the expression of epithelial markers of colorectal carcinogenesis in patients with long-term ulcerative colitis (UC) and primary sclerosing cholangitis (PSC) before and after transplantation. METHODS: Eight patients with UC and PSC prior to liver transplantation (PSC-UC), 22 patients with UC after liver transplantation for PSC (OLT), 9 patients with active ulcerative colitis without PSC (UCA), 7 patients with UC in remission (UCR) and 10 controls (N) underwent colonoscopy with multiple biopsies. Specimens were analysed histologically and semi-quantitatively immunohistochemically for p53, Bcl-2 and cyclooxygenase-2 (COX-2) markers. Statistical analysis was performed by Kruskal-Wallis and Fisher's exact tests. RESULTS: PSC-UC had a statistically significantly higher expression of p53 in the nondysplastic mucosa as compared to OLT, UCA, UCR and N (P < 0.05). We also found a statistically significant positive correlation between the incidence of PSC and the expression of p53 (P < 0.001). UCA had a higher p53 expression as compared to UCR. OLT had a significantly lower expression of p53 as compared with PSC-UC (P < 0.001). Bcl-2 had a significant higher bcl-2 expression as compared with controls. No difference in COX-2 expression between PSC-UC, UCR and UCA was found. UCA had higher COX-2 expression as compared to UCR. We also found a statistically significant positive correlation between the expression of COX-2 and p53. Patients after liver transplantation for PSC had a statistically significantly lower expression of the p53 compared with PSC-UC (P < 0.001). PSC-UC had the same inflammatory endoscopic activity as OLT and UCR when evaluated with the Mayo score. CONCLUSION: Our study shows that the nondysplatic mucosa of UC patients with PSC is characterised by a higher expression of the tumour suppressor gene p53, suggesting a higher susceptibility of cancer. This p53 overexpression correlates with the presence of PSC whilst it is not present in patients with UC after liver transplantation for PSC.

Cíl studie: Studium nových vybraných molekulárně genetických imunomarkerů (p53, c-erbB-2, Ki 67, bcl-2), hormonálních receptorů (ER, PR) a změn v buněčné ploiditě nádorových buněk endometriálního karcinomu a jejich komparace s hlavními prognostickými faktory. Typ studie: Prospektivní studie. Název a sídlo pracoviště: Gynekologicko-porodnická klinika LF UP a FN Olomouc, Laboratoř experimentální medicíny, Ústav molekulární a transplantační medicíny LF UP a FN Olomouc, Ústav patologie LF UP a FN Olomouc. Metodika: Studovaný soubor tvoří 88 nemocných operovaných primárně pro karcinom endometria. Peroperačně získaný bioptický materiál z tumoru byl klasicky histopatologicky zpracován a následně vyšetřen imunohistochemicky ke stanovení vybraných molekulárních markerů. Stejný materiál byl po mikrodisekci nádorové tkáně vyšetřen průtokovou cytometrií ke stanovení změn v buněčné ploiditě. Výsledky byly komparovány s hlavními prognostickými faktory, zejména klinickým stadiem onemocnění, gradingem a histopatologickým typem nádoru. Výsledky: U 71 % biologicky nezralých endometriálních tumorů (G3) se špatnou prognózou jsme prokázali přítomnost aneuploidních karyotypů, zatímco u nádorů G1 a G2 jen ve 47 %. Ze studovaných imunomarkerů měly nejvyšší expresi u prognosticky nepříznivých případů p53, Ki 67 a c-erbB-2. U stejné skupiny tumorů jsme prokázali přítomnost steroidních receptorů jen v 58 %. Závěr: Stanovení DNA ploidity je významným prognostickým faktorem. Ze sledovaných histopatologických imunomarkerů jsme prokázali zvýšenou expresi p53, Ki 67 a c-erbB-2 u prognosticky nepříznivých tumorů. Snížená přítomnost steroidních receptorů pak byla prokázána u stejné skupiny nádorů
Objective: The aim of the study was the analysis of the new molecular genetic immunomarkers (p53, c-erbB-2, Ki 67, bcl-2) hormonal receptors (ER, PR) and ploidy disturbances and their relation to the most important prognostic factors for endometrial cancer. Design: Prospective study. Setting: Dept. of Gynaecology and Obsterics, Laboratory of Experimental Medicine, Institute of Pathology, Institute of Molecular and Transplational Medicine, Medical Faculty and University Hospital, Olomouc. Methods: The study group consisted of 88 endometrial cancer patients. The biopsies of the tumours obtained at operations were routinely histopathologically examined. Subsequently, the immunohistochemical tumormarkers were determined. The same biopsies were examined by microdissection and flow cytometric ploidy analysis and karyotyping. The findings were compared with the most important prognostic factors for endometrial cancer, mainly with clinical stage of the disease, grade and histopathological type. Results: Aneuploidy was found in 71% in the group of poorly differentiated endometrial cancers (G3) in contrast to 47% in the group of G1 and G2 tumours. High expression of p53, Ki 67, c-erbB-2 and low rate of sex hormone receptors was found in the prognostically unfavourable group (G3). Conclusions: Aneuploidy seems to be an important prognostical factor for endometrial cancer patients. Identification of p53, Ki 67, c-erbB-2, ER a PR is a useful tool to specify a group of prognostically unfavourable patients.

Chromosomal rearrangements and copy number variation are frequently observed in cancer cells, including multiple myeloma (MM). Karyotypic abnormalities seen in MM cells correlate with the disease stage and drug responses. Here, we investigate the nuclear arrangement of the 1q21 region; amplification of this region is an important diagnostic and prognostic marker of MM. We examined the lymphoblastoid cell line CD138- ARH-77, multiple myeloma CD138+ MOLP-8 cells, and the CD138+ bone marrow fraction of patients diagnosed with MM. In this experimental system, we observed that gamma-radiation and selected cytostatic drugs such as melphalan and dexamethasone did not significantly alter the nuclear radial arrangement of the 1q21 region and other relevant regions of chromosome 1. Similarly, conserved nuclear radial positioning after cytostatic treatment was observed for the c-myc, TP53, CCND1, and IgH loci. When analyzed Mcl-1, a protein encoded by a gene mapped to the 1q21 region, we found that the variant Mcl1S is highly expressed in multiple myeloma MOLP-8 cells, but not in peripheral blood lymphocytes of healthy donors or lymphoblastoid ARH-77 cells; this is in contrast to the expression pattern of the Mcl-1L variant. On the basis of these observations we suggest that the 1q21 region is an important diagnostic marker of MM, particularly the gene encoding the Mcl-1S variant, which can be easily detected by western analysis.

Cíl studie: Zjistit pomocí imunohistochemického vyšetření expresi p53, bcl-2, c-erb-2, Ki-67, estrogenového a progesteronového receptoru u pacientek s karcinomem endometria. Typ studie: Experimentální studie. Název a sídlo pracoviště: Porodnicko-gynekologická klinika, Ústav lékařské genetiky, Ústav patologie, LF UP a FN Olomouc. Metodika: Studovali jsme 103 případy primárně neléčeného karcinomu endometria, u nichž jsme pomocí imunohistochemického vyšetření stanovovali přítomnost p53, bcl-2, c-erb-2, Ki-67, estrogenového a progesteronového receptoru. Stanovili jsme korelace mezi imunohistochemickým barvením zhodnoceným pomocí histoskóre, věkem, grade, hloubkou invaze, klinickým stadiem a metastatickým šířením. Výsledky: Průměrný věk byl 67 let (rozmezí 35-90). p53, bcl-2, c-erb-2, Ki-67, estrogenový a progesteronový receptor byly pozitivní u 49 (48 %), 81 (79 %). 18 (17 %), 99 (96 %), 73 (70 %) a 87 (84 %) pacientek. Nebyla nalezena jasná závislost mezi immunohistochemickými parametry a věkem pacientek. Zvýšená exprese p53 a Ki-67 korelovala se špatnou diferenciací, hlubokou invazí do myometria, pokročilým klinickým stadiem a metastatickým šířením. Imunohistochemické barvení pro bcl-2 korelovalo inverzně s FIGO stadiem, zatímco c-erb-2 bylo zvýšené exprimováno u nádorů s hlubokou myometrální invazí. Nádory s pozitivními estrogenovými a progesteronovými receptory vykazovaly další klinickopatologické parametry spojené s lepší prognózou. Závěr: Zvýšená exprese p53 a Ki-67 zřejmě indikuje malignější fenotyp, zatímco bcl-2 a c-erb-2 mohou mít do určité míry omezený význam pro identifikaci vysoce-rizikových nádorů.
To assess the immunohistochemical expression of p53, bcl-2, c-erb-2, Ki-67, estrogen and progesterone receptors in endometrial cancer patients. DESIGN: Experimental study. SETTING: Department of Obstetrics and Gynecology, Institute of Human Genetics, Department of Pathology, Palacky University Medical School and University Hospital, Olomouc. METHODS: We studied 103 cases of primary untreated endometrial carcinoma in which the p53, bcl-2, c-erb-2, Ki-67, estrogen and progesterone receptor antigens were investigated by an immunohistochemical method. We evaluated the correlations among the immunohistochemical staining assessed by histoscore, and the age, grading, depth of invasion, stage of the neoplasia and extrauterine disease. RESULTS: Mean age was 67 years (range 35-90). p53, bcl-2, c-erb-2, Ki-67, estrogen and progesterone receptors were positive in 49 (48%), 81 (79%). 18 (17%), 99 (96%), 73 (70%) and 87 (84%) patients respectively. There was no clear association between immunohistochemical parameters and the age of patients. p53 and Ki-67 overexpression was found to be related to poor grade of differentiation, deeper myometrial invasion, advanced stage of neoplasia and extrauterine spread of disease. Immunostaining for bcl-2 correlated inversely with FIGO stage, while c-erb-2 was overexpressed in tumors with deeper myometrial invasion. Estrogen and progesterone receptor positive tumors showed a statistically significant association with clinicopathological parameters of better clinical outcome. CONCLUSION: The overexpression of p53 and Ki-67 seems to indicate more malignant phenotype, while bcl-2 and c-erb-2 may have a limited role in the identification of high-risk tumors.

We report an unusual example of trichoepithelioma containing giant and multinucleated cells in the epithelial compartment. The patient was a 52-year-old woman who presented with a solitary nodule on the scalp measuring 2 cm of apparently long duration. The biopsy revealed a typical trichoepithelioma. The unusual feature was the presence in some epithelial nodules of large epithelial cells with hyperchromatic nuclei and no visible nucleoli that exceed 3 to 5 times the adjacent follicular germinative cells. Often, multinucleated cells were seen. Rare nodules were almost entirely composed of giant/multinucleated cells. The majority of the nodules containing giant cells were situated in the deeper portion of the neoplasm. No mitoses were seen in these giant cells. No enlarged or multinucleated cells were seen in the stroma. The neoplastic epithelial cells were diffusely reactive for bcl-2, including the giant and multinucleated cells. The proliferating rate was low; Ki-67 stained some giant cells. CD34 stained the stroma. We believe that the focal presence of pleomorphic giant cells in trichoepithelioma has no clinical or prognostic implications and does not denote its "malignant transformation".

BACKGROUND: The aim of this study was to analyse the usefulness of detecting important apoptosis and proliferation markers in assessing the biological potential of odontogenic keratocysts (OKC) and thus selecting the optimal diagnostic algorithm for these lesions. METHODS: Indirect immunohistochemistry and relevant statistical methods were used for analysis of formalin-fixed and paraffin-embedded samples from 98 patients. RESULTS: Nevoid basal cell carcinoma syndrome (NBCCS) keratocysts were characterized by higher expression of Bcl-2, p27Kip1 and c-erbB-2 as well as by lower proliferative activity measured by Ki-67 in basal cell epithelium and by a lower inflammatory response in comparison with sporadic keratocysts. Dentigerous, radicular and non-specified odontogenic cysts differed from both NBCCS and sporadic keratocysts in a wide spectrum of apoptosis and/or cell cycle-related protein expressions, higher proliferation in the basal cell layer, and vice versa, lower proliferation in the suprabasal cell layer. CONCLUSIONS: The NBCCS keratocysts have a different immunophenotype from sporadic keratocysts and both types are distinguishable from dentigerous, radicular and non-specified odontogenic cysts. These findings confirm the separate biological potential of these lesions and the results of the immunohistochemical analysis have diagnostic and prognostic implications.

In last few years, numerous groups of proteins participating in the regulation of cell proliferation, differentiation and death during ontogenesis have been described. In this study we compared the occurrence of Bcl-2, p53 and myc protein families with the level of proliferative activity and apoptosis during development of duodenal epithelium. Paraffin embedded tissues of eight human embryos and foetuses aged from the 6th-18th week of IUD were used. For the detection of apoptotic cells the TUNEL method was performed, the proliferative marker PCNA and all the proteins studied were detected by means of indirect three-step immunohistochemical method. In the 6th and 8th week of intrauterine development we observed isolated TUNEL positive epithelial cells only and this was accompanied by the disperse presence of PCNA as well as by all the studied proteins: Bcl-2, Bax, Bcl-XL, c-myc, N-myc, p53, p63 and p73. In the early foetal period of duodenal development we registered changes in PCNA and TUNEL positivity in accordance with the constitution of the stem cell pool on base of villi, where more numerous Bcl-2 positive cells were also found. The separation of primitive crypts and villi was not accompanied by any differences in distribution of Bax, Bcl-XL, c-myc, N-myc, p63 and p73 proteins between those compartments: all the studied proteins showed dispersed character. P53 rapidly decreased in this period. In the 18th week of intrauterine development the balance between proliferation in crypts and apoptosis of villi epithelium was well established and no p53 positive cells were found. In the presence of Bcl-2, Bax, Bcl-XL, p63 and p73 we did not find any dramatic changes. The myc proteins were restricted within the epithelium of the Lieberkuhn crypts only.
