Neuronal voltage-gated calcium channels play a pivotal role in the conversion of electrical signals into calcium entry into nerve endings that is required for the release of neurotransmitters. They are under the control of a number of cellular signaling pathways that serve to fine tune synaptic activities, including G-protein coupled receptors (GPCRs) and the opioid system. Besides modulating channel activity via activation of second messengers, GPCRs also physically associate with calcium channels to regulate their function and expression at the plasma membrane. In this mini review, we discuss the mechanisms by which calcium channels are regulated by classical opioid and nociceptin receptors. We highlight the importance of this regulation in the control of neuronal functions and their implication in the development of disease conditions. Finally, we present recent literature concerning the use of novel μ-opioid receptor/nociceptin receptor modulators and discuss their use as potential drug candidates for the treatment of pain.

Orofacial pain disorders are frequent in the general population and their pharmacological treatment is difficult and controversial. Therefore, the search for novel, safe and efficient analgesics is an important but still elusive goal for contemporary medicine. In the recent years, the antinociceptive potential of endocannabinoids and opioids has been emphasized. However, concerns for the safety of their use limit their clinical applications. the possibility of modulating the activity of endocannabinoids by regulation of their synthesis and/or degradation offers an innovative approach to the treatment of pain. A rat model of trigeminal pain, utilizing tongue jerks evoked by electrical tooth pulp stimulation during perfusion of the cerebral ventricles with various neurotransmitter solutions can be used in the pharmacological studies of nociception in the orofacial area. The aim of this review is to present the effects of pharmacological activity of opioids and endocannabinoids affecting the transmission of the sensory information from the orofacial area on the example of trigemino-hypoglossal reflex in rats.

Discovery of somatic and mental disorders in opioid drug addicts on methadone maintenance treatment (MMT) with a goal for prevention and treatment. Clearing influence of opioids on health status. Monitoring before and during MMT. Except investigation with usuall methods for classification of health status direction to liver diseases, metabolic and nutritional diosorders, bone turnover or other disturbances ie neurological and others. The effects of MMT will be summarized in standards and guidelines forsystematical diagnostic and treatment procedures.
Odkrytí poruch somatického a duševního zdraví u toxikomanů závislých na opiátech léčených metadonem s cílem prevence a léčení. Objasnění vlivu opiátů na zdravotní stav. Pacienti budou vyšetřeni před zahájením a v průběhu léčby. Kromě běžných vyšetření vprůběhu 3 roků zvl sledování jaterních poruch, metabolických a nutričních poruch, kostního metabolismu a jiných poruch (např. neurologických apod.). Zhodnocení efektu metadonu a návrh standardních léčebných postupů.

Východisko. Sledování zdravotního stavu pacientů na opiátech je zdůvodněno vysokým stupněm morbidity těchto abuzérů. Cílem práce bylo zjistit aktuální zdravotní stav a rozsah i stupeň postižení před zahájením substituční léčby agonistou opioidních receptorů – metadonem. Metody a výsledky. Soubor abuzérů tvořila skupina 101 osob (věkový průměr 28,5 roků, variační šíře 20–47 roků, 77 mužů a 24 žen). Z abnormálních nálezů se nejčastěji projevily patologické výsledky hodnot jaterních enzymů ALT, AST (35 a 31 %), GMT (9 %), snížené hodnoty hemoglobinu (24 %), zvýšené hodnoty IgM (38 %) a CRP (25 %). Při současné pozitivitě testů na hepatitidy (C a B, 70 % a 61 %) je jejich alterace pochopitelná. Snížené hodnoty testosteronu u mužů (63 %) ukazují na dysfunkci hypofýzo-hypotalamo-gonadální osy, zvýšené hodnoty tyroxinu (14 %), P (20 %) a aterogenního indexu (19 %) zůstávají bez vysvětlení. CRP signalizuje probíhající (skrytou) infekci. Řada laboratorních – biochemických ukazatelů nevykázala signifikantní patologické hodnoty. Zobrazovací metody zjistily hepatomegalii u 28 % a splenomegalii u 27 %, echotextura signalizující steatózu byla nalezena u 15 % vyšetřovaných. Závěr. U chronických abuzérů je možné (před zahájením substituční léčby metadonem) očekávat ve vysokém stupni především alteraci jaterních funkcí, pozitivitu testů na hepatitidy B a C, alteraci imunoglobulinů a deficit testosteronu (u mužů).
Background. Examination of the health status of patients abusing opiates can be substantiated by their high morbidity. The aim of the work was to describe the actual health status and the extent and seriousness of the affliction before the substitution therapy with opioid receptors agonist – methadone. Methods and Results. The group of drug abusers consisted of 101 persons (average age was 28.5 years, within the range of 20 to 47 years, 77 men and 24 women). Among the pathological findings, values of the levels of hepatic enzymes ALT, AST (35 and 31 %), GMT (9 %), low haemoglobin levels (24 %) and elevated values of IgM (38 %) and CRP (25 %) belonged to the most frequent ones. Because of the simultaneously positive tests for hepatitis (C and B, 70 % and 61 %), the alteration is fully obvious. Low testosterone levels in males (63 %) indicate the dysfunction of the hypothalamus-hypophysis-gonadas axis. Increased values of thyroxin (14 %), P (20 %) and atherogenity index (19 %) remain unexplained. Several other laboratory-biochemical parameters remained within the physiological range. Imaging methods revealed hepatomegaly in 28 % and splenomegaly in 27 %, echotexture indicating steatosis was found in 15 % of studied persons. Conclusions. In chronic drug abusers before the onset of the substitution therapy, it is possible to expect frequent alterations, namely that of hepatic functions, positive test for hepatitis B and C, alteration of immunoglobulines level and testosterone deficits (in males).
