This study aimed to implement a toxigenic culture as an optional third diagnostic step for glutamate dehydrogenase (GDH)-positive and toxin A/B-negative diarrheal stool samples into a diagnostic algorithm for Clostridioides (Clostridium) difficile infection (CDI), and to characterise C. difficile isolates for epidemiological purposes. During the 5-month study, 481 diarrhoeal stool samples from three Slovak hospitals were investigated and 66 non-duplicated GDH-positive stool samples were found. Of them, 36 were also toxin A/B-positive. Twenty-three GDH-positive and toxin A/B-negative stool samples were shown subsequently to be positive following toxigenic culture (TC). Molecular characterisation of C. difficile isolates showed the predominance of PCR ribotype (RT) 001 (n = 37, 56.1%) and the occurrence of RT 176 (n = 3, 4.5%). C. difficile RT 001 isolates clustered to eight clonal complexes (CCs) using multiple-locus variable-number tandem repeats analysis (MLVA). Interestingly, one third of RT 001 isolates clustering in these CCs were cultured from toxin A/B-negative stool samples. Our observations highlight the need of use multiple step diagnostic algorithm in CDI diagnosis in order to detect all CDI cases and to avoid the spread of C. difficile in healthcare settings.

PURPOSE: To characterise and compare twenty-eight Finnish Clostridium difficile RT027-like isolates, selected based on the presence of 18 bp deletion in the tcdC gene and toxin gene profile (A, B, binary), with eleven RT027 isolates from different Finnish geographical areas and time periods. METHODS: Twenty-eight C. difficile RT027-like isolates and 11 RT027 comparative strains were characterised by capillary-electrophoresis (CE) ribotyping, multi-locus variable tandem-repeats analysis (MLVA), multi-locus sequence typing (MLST), and sequencing of tcdC and gyrA gene fragments. Susceptibility to moxifloxacin was determined by E-test. RESULTS: Of 28 RT027-like isolates, seven RTs (016, 034, 075, 080, 153, 176 and 328), three WEBRIBO types (411, 475, AI-78) and three new profiles (F1-F3) were identified. MLVA revealed six clonal complexes (RTs 016, 027, 176 and F3). MLST showed eleven sequence types (1, 41, 47, 67, 95, 191,192, 223, 229, 264 and new ST). Twenty-two isolates (RTs 016, 080, 176, 328, F1, F2, F3 and WRTAI-78) carried Δ117 in the tcdC gene. Isolates of RTs 016, 027 and 176 were moxifloxacin resistant and harboured Thr82Ile in the GyrA. CONCLUSION: Our results show a high diversity within 28 Finnish RT027-like C. difficile isolates, with twelve CE-ribotyping profiles and eleven STs. MLVA revealed the regional spread of RTs 016, 027, 176 and F3. The presence of Δ117 in the tcdC gene in eight non-027 RTs highlights the importance of careful interpretation of the results from molecular systems targeting this site in the genome of C. difficile and the need of strain typing for epidemiological purposes.

Střevní infekce vyvolané bakterií Clostridium difficile (CDI) v současnosti závažný medicínský problém. Patří mezi nejčastěji se vyskytující nozokomiální infekce a v některých zemích výrazně přibývá i komunitních onemocnění. Klinicky se může infekce manifestovat jako nezávažné průjmové onemocnění, ale také jako život ohrožující stav provázený paralytickým ileem a bolestivým rozepnutím tračníku přecházejícím do sekundární sepse. Rekurentní formy s opakovanými atakami průjmů jsou relativně častými obtížně řešitelnými komplikacemi choroby. Závažnost průběhu infekce může být ovlivněna virulencí vyvolávajícího kmene. S těžšími průběhy infekce jsou často spojované ribotypy 027,078, 001 a řada dalších. V ČR v posledních letech převládají ribotypy 001 a 176. Základní laboratorní diagnostika zahrnuje vyšetření stolice pacienta cílené na průkaz klostridiové glutamátdehydrogenázy a klostridiových toxinů (A a B), případně průkaz C. difficile anaerobní kultivací. Léky volby v terapii klostridiové kolitidy jsou metronidazol, vankomycin a fidaxomicin. Při opakovaných rekurencích je doporučována fekální bakterioterapie. Chirurgické řešení je indikováno u progredujících komplikovaných forem, které nereagují na medikamentózní léčbu a pacient se dostává do kritickém stavu.
Intestinal infections caused by the Clostridium difficile (CDI) bacterium currently represent a serious medical problem. They belong to the most frequent nosocomial infections and, in some countries, a community-acquired disease with a significantly increased incidence of community associated CDI is reported. The infection can manifest as mind diarrhea, but also as a life-threatening illness accompanied by paralytic ileus and painful distension of the colon, developing into secondary sepsis. Recurrent forms difficult to manage are a relatively common complication of the disease. Severity of infection may be influenced by the virulence of the causative strain. Severe course of the disease is associated with ribotypes 027, 078, 001. In the Czech Republic, ribotypes 001 and 176 have predominated over the last years. Laboratory diagnosis is based on the detection of C. difficile glutamate dehydrogenase and free clostridium toxins (A,B) in a diarrheal stool sample or culture of C. difficile in anaerobic conditions. Metronidazole, vancomycin and fidaxomicin are the drugs of choice in the treatment of aC. difficile with administration according to the actual treatment guidelines. Fecal bacteriotherapy is recommended in treatment and prevention of recurrent CDI. Surgery is indicated in progressive complicated forms when no response to medication is achieved and the patient is in a critical condition.

In 2014, 18 hospitals in the Czech Republic participated in a survey of the incidence of Clostridium difficile infections (CDI) in the country. The mean CDI incidence was 6.1 (standard deviation (SD):7.2) cases per 10,000 patient bed-days and 37.8 cases (SD: 41.4) per 10,000 admissions. The mean CDI testing frequency was 39.5 tests (SD: 25.4) per 10,000 patient bed-days and 255.8 tests (SD: 164.0) per 10,000 admissions. A total of 774 C. difficile isolates were investigated, of which 225 (29%) belonged to PCR ribotype 176, and 184 isolates (24%) belonged to PCR ribotype 001. Multilocus variable-number tandem repeat analysis (MLVA) revealed 27 clonal complexes formed by 84% (190/225) of PCR ribotype 176 isolates, and 14 clonal complexes formed by 77% (141/184) of PCR ribotype 001 isolates. Clonal clusters of PCR ribotypes 176 and 001 were observed in 11 and 7 hospitals, respectively. Our data demonstrate the spread of two C. difficile PCR ribotypes within 18 hospitals in the Czech Republic, stressing the importance of standardising CDI testing protocols and implementing mandatory CDI surveillance in the country.

Clostridium difficile je závažným nozokomiálním patogenem současnosti uváděným hlavně v souvislosti s postantibiotickými průjmy. Bližší určení klinicky významných izolátů C. difficile přispívá ke zmapování epidemiologické situace v ČR. Celkem 624 izolátů C. difficile z 11 mikrobiologických pracovišť bylo typizováno pomocí ribotypizace založené na kapilární elektroforéze. Celkem bylo identifikováno 75 různých ribotypů. 40% izolátů příslušelo k ribotypu 176. Dalšími častěji se opakujícími ribotypy byly: 001, 012, 014, 017, 020 a 078.
Clostridium difficile is a major nosocomial pathogen reported mainly in the context of post-antibiotic diarrhoea. Further identification of clinically significant isolates of C. difficile contributes to the mapping of the epidemiological situation in the Czech Republic. A total of 624 C. difficile isolates from 11 microbiological centres were characterized using capillary gel electrophoresis-based PCR ribotyping. Forty percent of the isolates belonged to ribotype 176. Other frequently identified ribotypes were: 001, 012, 014, 017, 020, and 078.

A set of 415 clinical samples isolated from 294 patients suspected of having syphilis collected in the Czech Republic between 2004 and 2010 was tested for the presence of treponemal DNA. Standard serological tests showed that 197 patients were syphilis-seropositive and 97 patients were syphilis-seronegative. In each sample, PCR tests for polA (TP0105), tmpC (TP0319), TP0136, TP0548 and 23S rRNA genes were performed. Samples taken from 91 patients were PCR-positive. Molecular typing of treponemal DNA was based on the sequencing of TP0136, TP0548 and 23S rRNA genes. Treponemal DNA was typeable in samples taken from 64 PCR-positive patients and 9 different genotypes were found. The proportion of treponemal strains resistant to macrolide antibiotics was 37.3%. In the DNA samples taken from 39 patients, a parallel treponemal typing approved by Centers for Disease Control and Prevention was performed. The variants of arp and tpr genes appear to combine independently with sequence variants of TP0136, TP0548 and 23S rRNA genes.

This pilot study was carried out to evaluate the occurrence of Clostridium difficile in marine environments and in edible shellfish. Samples of seawater, sediment, and zooplankton were collected at five sampling stations in the Gulf of Naples. Six samples of edible shellfish, furthermore, were obtained: two from mussel farms and four from wholesalers. The isolation and the characterization of C. difficile strains were carried out using selective media and molecular techniques, respectively. C. difficile was isolated from nine of the 21 samples investigated. Shellfish and zooplankton showed the highest prevalence of positive samples. No C. difficile was detected in marine sediment. Majority of the C. difficile isolates were toxin A/B positive. Six known different PCR ribotypes (003, 005, 009, 010, 056, and 066) were identified, whereas one strain may represent a new PCR ribotype. C. difficile may be present in the marine environment in Southern Italy, including shellfish and zooplankton. This study is reporting the isolation of C. difficile from zooplankton, clams, and mussels and pointing out a new possible route to exposure to C. difficile of healthy individuals in the community.

A group of 67 Lactobacillus spp. strains containing Lactobacillus casei/paracasei, Lactobacillus fermentum, Lactobacillus gasseri, Lactobacillus plantarum, Lactobacillus rhamnosus and Lactobacillus salivarius species isolated from early childhood caries and identified to the species level in a previous study (Svec et al., Folia Microbiol 54:53-58, 2009) was characterized by automated ribotyping performed by the RiboPrinter microbial characterization system and by randomly amplified polymorphic DNA fingerprinting (RAPD-PCR) with M13 primer to evaluate these techniques for characterization of lactobacilli associated with dental caries. Ribotyping revealed 55 riboprints among the analysed group. The automatic identification process performed by the RiboPrinter system identified 18 strains to the species level, however cluster analysis divided obtained ribotype patterns into individual clusters mostly corresponding to the species assignment of particular strains. RAPD-PCR fingerprints revealed by the individual Lactobacillus spp. showed higher variability than the ribotype patterns and the fingerprint profiles generated by the analysed species were distributed among one to four clusters. In conclusion, ribotyping is shown to be more convenient for the identification purposes while RAPD-PCR fingerprinting results indicate this method is a better tool for typing of Lactobacillus spp. strains occurring in dental caries.

Vaginální ekosystém je komplexní a složitě regulovaný systém, který slouží na ochranu pochvy před vstupem infekce. Komplexní problematika poševního mikroprostředí a přetrvávající terapeutické rozpaky při léčbě žen s chronicky porušeným vaginálním ekosystémem jsou předmětem výzkumu vaginální flóry s cílem vytvoření nového terapeutického preparátu na obnovu a stabilizaci poševní eumikrobie. V experimentální etapě bylo vyšetřeno 35 vzorků - vaginálních stěrů asymptomatických žen reprodukčního věku. Vzorky bakteriálních kmenů byly přesně identifikovány a po zhodnocení růstových křivek a vitality bylo vybráno deset bakteriálních kmenů, jejichž specifické vlastnosti byly dále podrobně testovány. Výzkum se zaměřil na testování imunomodulační aktivity (stanovení fagocytové aktivity, fagocytového indexu, lysozymové aktivity a peroxidázové aktivity), baktericidní a kandidacidní aktivity vybraných kmenů, schopnost adherence k epiteliálním buňkám (metoda testování lipofility), stanovení odolnosti kmenů vůči ATB (antibiogram) a stanovení produkce kyseliny mléčné a peroxidu vodíku. Po absolvování detailního testování je za ideální součást generovaného probiotického preparátu považován kmen 16A/ 1 Lactobacilus fermentatum (ideální imunostimulačními účinky, vysoká baktericidní a kandidacidní aktivita), kmen 17A Lactobacillus crispatus, 29B Lactobacillus acidophilus (stimuluce fagocytární a peroxidázové aktivity leukocytů) a kmen 8A Lactobacillus gasseri (vysoká rezistence vůči ATB). Testování optimální kombinace probiotických kmenů endogenní vaginální flóry v terapii poševních infekcí a při obnovení a stabilizaci poševního prostředí v novém probiotickém vaginálním preparátu bude předmětem následného klinického hodnocení.
