BACKGROUND: The long-term efficacy and safety of alcohol septal ablation (ASA) has recently been demonstrated. However, there is still debate about the outcome of younger patients who should be treated using myectomy, according to American College of Cardiology Foundation/American Heart Association guidelines. The aim of this study was to evaluate the long-term outcome of patients ≤ 50 years of age after ASA for hypertrophic obstructive cardiomyopathy (HOCM). METHODS: We retrospectively evaluated consecutive, highly symptomatic patients aged ≤ 50 years with HOCM who underwent ASA. RESULTS: Institutional databases of 3 cardiovascular centres identified 290 patients with HOCM who underwent ASA; 75 (26%) of them were aged ≤ 50 years at the time of their first ASA. Median duration of follow-up was 5.1 years (range, 0.1-15.4 years). Four patients (5%) died during the study period (438 patient-years; the annual mortality rate was 0.91%; 95% confidence interval [CI], 0.25-2.34%; the annual mortality rate combined with the first appropriate implantable cardioverter-defibrillator discharge was 1.43%; 95% CI, 0.52-3.10%). Survival free of all-cause mortality at 1, 5, and 10 years was 97% (95% CI, 89-99%), 94% (95% CI, 84-98%), and 94% (95% CI, 84-98%), respectively. CONCLUSIONS: Results of this first study focused on HOCM patients aged ≤ 50 years who underwent ASA suggest a low risk of all-cause death or appropriate implantable cardioverter-defibrillator discharge in the long-term follow-up.

Byla purifikována a charakterizována protease stabilni v organických rozpoštědlech. Její molekulová hmotnost byla stanovena pomocí SDS-PAGE a SEC na 26 kDa. Purifikovaná proteasa měla nejvyšší aktivitu při 70°C a její pH optimum bylo velmi široké (5,0 – 12.0). Po ošetření 5 mM EDTA a β-mercaptoethanolem zůstával enzym aktivní. Proteasa měla po hodinové inkubaci při 30°C v přítomnosti 20, 40 a 60 % (v/v) organických rozpouštědel, jako jsou například DMSO, DMF, aceton, ethanol, iso-propanol a toluen vyšší aktivitu a stabilitu. Proteolytická aktivita byla v přítomnosti Mn2+ iontů prokazatelně vyšší a v přítomnosti 5, 10 a 15 mM Pb2+, Zn2+, K+, Fe2+, Co2+, Cd2+, Mg2+, Ca2+, Fe3+ iontů po dobu 1h při 30°C si zachovávala více než 90 % aktivity.
In the present research, stable in organic solvent protease from a new thermophilic actinomycete isolate, was purified and characterized. The approximate molecular mass of 26 kDa was determined by SDS-PAGE and SEC. The purified protease showed maximum activity at 70°C and exhibited broad pH optimum (5.0 – 12.0). After treatment with 5 mM EDTA and β-mercaptoethanol the enzyme remained fully active. The protease showed an increased activity and stability in the presence of 20, 40 and 60 % (v/v) organic solvents such as DMSO, DMF, acetone, ethanol, iso-propanol and toluene when incubated for 1h at 30°C. The proteolytic activity was significantly enhanced in presence of Mn2+ and remained more than 90 % active in the presence of 5, 10 and 15 mM Pb2+, Zn2+, K+, Fe2+, Co2+, Cd2+, Mg2+, Ca2+, Fe3+ ions for 1h at 30°C. The kinetic constants were also determined.
BACKGROUND: Highly symptomatic patients with obstructive hypertrophic cardiomyopathy (HCM) are candidates for alcohol septal ablation (ASA). We wanted to determine long-term (>60 months) clinical and echocardiographic outcomes of patients treated with low (1-2 mL) or high (>2 mL) doses of alcohol. METHODS: Seventy-six patients were randomized into 2 arms in a 1:1 ratio, and subsequently were treated by ASA with a low (1-2 mL) or high (>2 mL) dose of alcohol. Clinical and echocardiographic examinations were performed at baseline, 1 year after the procedure, and at the end of follow-up (at least 60 months after ASA). RESULTS: Both groups of patients matched in all baseline clinical and echocardiographic data. In a total of 76 patients, 86 septal branches were ablated in 80 ASA procedures (2 repeat procedures in each group). There were no differences in postprocedural complications. Seven patients (4 vs 3 patients; not significant) died during follow-up (60-138 months; median 85 months). Pressure gradients decreased significantly in both groups (from 74±36 to 24±32 mm Hg in the low-dose group and from 74±39 mm Hg to 18±20 mm Hg in the high-dose group). There were no significant differences between the groups, and all main hemodynamic and echocardiographic changes occurred in the first postprocedural year. At final examination, there were no patients with New York Heart Association class>2 dyspnea in either group. CONCLUSIONS: This study demonstrates that ASA for obstructive hypertrophic cardiomyopathy is safe and effective in long-term follow-up. No differences in long-term efficacy and safety were found between low and high doses of alcohol.

Látky mohou být inhalovány ve formě plynů, aerosolů nebo prachových částic. Absorpce z plic a nástup účinku látek po inhalačním podání je velmi rychlý. Zřejmě nejčastěji jsou zneužívána cestou inhalace solvencia, látky uhlovodíkové povahy vyráběné často z nafty a zemního plynu. Motivace zneužívání těkavých látek je determinována euforizujícími, halucinogenními a anestetickými účinky. Při nižších dávkách látky připomíná akutní intoxikace chování intoxikovaného v lehčím stupni opilosti, se stupňováním toxického účinku drogy dochází k nepravidelnostem srdečního rytmu až s nebezpečím srdečního selhání (syndrom „sudden sniffing death“). Pravidelné zneužívání těkavých látek vede především k ireverzibilnímu poškození CNS, někdy i periferních nervů, dále k těžkému poškození jater nebo ledvin. U chronických konzumentů zneužívání solvencií způsobuje psychickou závislost. Lze prokázat aktivaci mozkového dopaminergního systému. Při náhlém odnětí drogy se rozvíjí anxiózní stav, ze somatických projevů je dominantní nauzea a tremor.
Substances may be inhaled in the form of gases, aerosols, or dust particles. Absorption from the lungs and the onset of effect after inhalation are very rapid. The substances most widely abused by inhalation include solvents, substances of carbohydrate nature often made from oil and natural gas. The motivation to abuse volatile substances is determined by euphorising, hallucinogenic, and anaesthetic effects. With lower doses, acute intoxication resembles behaviour in mild alcohol intoxication while with an increasing toxic effect irregularities in the heart rhythm occur with a potential risk of sudden sniffing death syndrome. Regular abuse of solvents results in irreversible CNS damage, sometimes even damage of the peripheral nerves, as well as in severe liver or kidney damage. In chronic abusers, solvent abuse produces psychic dependence. In addition, activation of the brain dopaminergic system may be shown. Sudden substance withdrawal results in a state of axiety, somatic manifestations mainly include nausea and tremor.
