PURPOSE OF THE STUDY To present a retrospective evaluation of the results of our method of open reduction and internal fixation. MATERIAL AND METHODS A total of 35 patients were surgically treated for talar fractures at our department between 2004 and 2008. There were 27 men and eight women, with an average age of 31 years (range, 21 to 65). Talar neck fractures were recorded in 21 and talar body fractures in 14 patients. The most frequent cause of injury was a fall from height (77%); motorcar accidents were less frequent (14%). Open fractures were found in 8.5% of the patients, and talar fractures as a single trauma were recorded in 80% of them. Indication criteria for surgery included displaced talar neck (Hawkins type II to type IV) and body fractures, with a displacement exceeding 1 mm. The traction screw osteosynthesis used was combined with plate fixation in some patients. Full weight-bearing of the extremity was allowed from 12 post-operative months. The patients were followed up at 6 weeks, 3, 6 and 12 months and then at yearly intervals. The American Orthopaedic Foot and Ankle Society (AOFAS) scores were used to evaluate the results. RESULTS Of the 35 fractures, 16 (45.7%) were treated surgically on the day of injury and 19 (54.3%) on subsequent days. The injury-surgery interval ranged from 0 to 12 days (average, 8 days). Primary bone union was recorded in 34 patients (97%) within 16 weeks of surgery; pseudoarthrosis developed in one patient. The results were excellent in eight (23%), good in 11 (31%) and satisfactory in seven (20%) patients. Poor outcome including function was reported by nine (26%) patients. The poor results were mostly due to associated tibial pilon fractures or because of arthrodesis necessary to be performed for management of necrosis or arthritis. Complications were recorded in 22 patients (63%) and included avascular necrosis in six (17%), traumatic arthritis of the tibiotalar and subtalar joints in 14 (40%) patients and pseudoarthrosis in one (3%) patient. This was treated by corti-cocancellous graft implantation and repeated osteosynthesis, and bone union occurred within 6 months. Traumatic arthritis was managed by arthrodesis in seven patients.
PURPOSE OF THE STUDY To assess outcomes of the Ponseti method for treatment of talipes equinovarus in relation to patient age at its start. MATERIAL AND METHODS In the 2006-2010 period, 115 children (163 feet) with talipes equinovarus were treated using the principles of Ponseti. The right foot was affected in 44, the left foot in 23, and the bilateral form was treated in 48 patients. In order to obtain results for at least 3 years of follow-up, we included the patients treated by the Ponseti method between 2006 and 2008, in whom 83 affected feet were analysed (23 isolated right feet, 14 left feet, 23 bilateral forms). According to the Dimeglio scoring system used to assess the severity of clubfoot deformity, there were four feet in group II, 37 feet in group III and 42 feet in group IV. RESULTS Subtalar release was performed in 29 feet (34.9%), in two feet being only posterior. Moreover, foot deviations, e.g. adduction of the forefoot less than 10° (5 feet, 6%), varus deformity of the heel less than 5° (6 feet, 7.2%), and failure to complete the Ponseti treatment (3 feet, 3.6%) were recorded. For assessment of the effect of age at start of casting on the outcome, two patient subgroup were distinguished: group 1, the treatment stared and continued in the first 8 postnatal weeks; group 2, the treatment was initiated and carried out between 9 and 20 weeks postnatally Subsequently, subtalar release was performed in 18 of 61 feet (29.5%) in group 1, and in 11 of 21 feet (52.4 %) in group 2. Using Fischer's exact test, the difference was found statistically significant. (p<0.05). Of the three patients with an unfinished course of initial treatment, two underwent subtalar release later and one was lost to follow-up. DISCUSSSION Contrary to many recent reports, the frequency of Ponseti treatment failure in this study is substantially higher. Nevertheless, we adhered strictly to the Ponseti protocol of treatment, tenotomy was performed under general anaesthesia and Ponseti splinting was maintained properly in all but seven patients (8.4%). CONCLUSIONS Irrespective of rather frequent failure of the initial Ponseti treatment, its contribution for the patient is beneficial as it can reduce the extent of subtalar release required. However, a modern family will hardly accept several years of applying splints every night, irrespective of maximally sophisticated bracing. This becomes critical usually at the age when the child starts walking. Nowadays, families cooperate quite well due to the fact that a new method with high mass-media coverage has been offered to them.

Jedním ze základních principů, ať už konzervativní nebo chirurgické léčby pacientů se spastickou formou DMO, je buď udržení nebo znovu obnovení délky spastického svalu. V posledních dvaceti letech došlo k rozmachu použití botulinum toxinu A k selektivnímu ovlivnění svalového tonu u dětí s DMO. Integrovaný přístup k aplikaci botulinum toxinu A spojuje výhody víceetážové aplikace, redresního sádrování a využití fyzioterapie a ortézování. Celý koncept je postaven na relativně vysokodávkované aplikaci botulinum toxinu A společně s maximalizací terapeutického efektu pomocí adjuvantní terapie. Naše více než desetileté zkušenosti s tímto konceptem potvrzují jeho krátkodobý i dlouhododobý efekt.
One of the basic principles of both conservative and surgical treatment of patients with spastic cerebral palsy is either maintaining or restoring the length of the spastic muscle. In the last twenty years, there has been an expansion in the use of botulinum toxin type A in order to selectively affect the muscle tone in children with cerebral palsy. An integrated approach to administering botulinum toxin type A combines the advantages of administration into multiple sites, redressing plaster cast treatment and the use of physiotherapy and orthoses. The whole concept is based on a relatively high-dose administration of botulinum toxin type A while maximizing the therapeutic effect by means of adjuvant therapy. Our more than ten years of experience with this concept confirm its short-term as well as long-term effect.

CÍL: Článek je přehledem diagnostiky a terapie zlomenin distálního radia založené na zkušenostech s léčbou tohoto poranění na Klinice úrazové chirurgie LF MU a FN Brno. METODIKA: Diagnostika poranění se opírá o RTG či CT nález. Zlomeniny klasifikujeme dle AO dělení. Léčba je dle typu zlomeniny konzervativní nebo operační. Metodou volby operační léčby je osteosyntéza úhlově stabilními dlahami – LCP. VÝSLEDKY: V období 1/2004–12/2008 jsme ošetřili celkem 1260 zlomenin distálního radia. Konzervativně 932 zlomenin (74 %), operačně 328 (26 %). Dlahovou technikou jsme ošetřili celkem 233 zlomenin (71 %). Instrumentárium LCP 3,5 u 110 zlomenin (47 %) a LCP 2,4 ve 123 případech (53 %). LCP implantáty jsme aplikovali volárně ve 124 případech (53 %) a dorzálně ve 109 případech (47 %). Implantát Targon DR jsme použili u 5 zlomenin (1,5 %). DISKUZE: Zlomeniny distálního radia představují komplexní poranění. Prognóza závisí mimo jiné na výskytu a závažnosti přidružených poranění. Jedná se zejména o symptomy z útlaku ner-vus medianus, poškození distálního radioulnárního kloubu (DRUK), poranění karpálních vazů. Komplikace u zlomenin distálního radia pozorujeme dle literárních údajů u 20-30 % pacientů. Jsou to zejména – redislokace, Sudeckova algoneurodystrofie, léze cév, šlach či nervové poškození, možný výskyt infekčních komplikací, omezení funkce poraněné oblasti a poúrazová artróza radioulnární či radiokarpální. ZÁVĚR: Léčení zlomenin distálního radia je rutinní záležitostí každodenní praxe úrazového chirurga či traumatologa. Nesprávná léčba může mít pro pacienta vážné následky. Pozornost je třeba věnovat i přidruženým poraněním a možným komplikacím.
AIM: The article concentrates on diagnostics and therapy of distal radius fractures. It is based on doctors´experience gained during their work at the Department of Trauma Surgery, University Hospital and Faculty of Medicine, Brno. METHODOLOGY: The diagnostics of injury is based on X-ray and CT examination. Fractures are classified after the AO classification. Treatment according to the type of fracture is either conservative or operative. The operative treatment is performed as osteosynthesis with the LCP implants. RESULTS: From January 2004 to December 2008 1260 distal radius fractures were treated. 932 fractures were treatedin the conservative way (74 %), 328 fractures in the operative way (26 %). 233 fractures were treated with the use of splints (71 %). LCP implants 3,5mm in 110 fractures (47 %) a LCP 2,4mm in 123 fractures (53 %). LCP implants were from volar approach in 124 cases (53 %) and from dorsal approach in 109 cases (47 %) implanted. The implant Targon DR was used in 5 cases (1,5 %). DISCUSSION: The distal radius fracture is a complex injury. The prognosis depends on the occurence and the gravity of attached injuries, as symptoms which resulted from the oppression of nervus medianus, distal radioulnar joint damage (DRUK), injury of carpal ligaments. According to literary facts, 20-30 % patients suffer from complication occuring at distal radius fractures. It is mainly redislocation, Sudeck algoneurody-strophy, vessels, tendons or nerve damage, the possible occurence of infectious complication, the functional restriction in the injured area and posttraumatis arthrosis of distal radioulnar or radiocarpal joint. CONCLUSION: The treatment of distal radius fractures is a routine procedure for trauma surgeons, occuring almost every day. Incorrect treatment may have serious effects on the patients´ health. It is necessary to pay attention to attached injuries and to possible complications.

Cílem této studie bylo provést komplexní analýzu krátkodobého účinku vysokodávkovaného botulinum toxinu-A (BTX-A) aplikovaného společně s redresním sádrováním z důvodu ekvinózního postavení nohy při chůzi u pacientů s dětskou mozkovou obrnou (DMO). Dalším cílem bylo zhodnotit vliv této léčby na délku musculus triceps surae. Materiál a metodika: Jedná se o prospektivní studii, do které bylo zařazeno 10 pacientů (17 končetin) se spastickou formou DMO a ekvinózním postavením nohy. K hodnocení byla použita metoda trojdimenzionální kinematické a kinetické analýzy chůze společně s počítačovou modelací délky musculus triceps surae. Pacienti byli vyšetřeni před a 10 týdnů po léčbě. Ke statistické analýze byl použit párový T-test. Výsledky: Rychlost chůze a délka kroku se po aplikaci BTX-A nezměnila. Maximální dorziflexe na konci fáze jedné opory se zvýšila (p = 0,045), což vedlo k normalizaci "slope quotient" (p = 0,001). Došlo také k odstranění patologické generace síly během první poloviny fáze jedné opory (p = 0,012). Oproti očekávání, síla plantiflexorů nebyla aplikací BTX-A ovlivněna (p = 0,486). Neprokázali jsme prodloužení musculus triceps surae. Závěr: Aplikace BTX-A společně s redresním sádrováním vede ke zlepšení kinematiky a kinetiky chůze pacientů s ekvinózní deformitou a DMO. Výsledky naší studie upozorňují na možnost rozdílného působení BTX-A na intrafuzální a extrafuzální svalová vlákna.
The aim of the study was to perform a comprehensive analysis of the short-term effects of a standardized high dose of BTX-A and serial casting on spastic equinus in children with cerebral palsy. A second goal was to evaluate this short-term effect on muscle lengths of the triceps surae. Materials and methodology: Prospective study on 10 children (17 limbs) with spastic cerebral palsy and equinus gait pattern. Three-dimensional kinematic and kinetic gait analysis with musclulo-skeletal modelling was used to evaluate pre- and 10 weeks post-treatment results. The paired Student?s t-test was used for data analysis. Results: Walking velocity and stride length did not change after BTX-A application. The maximal ankle angle at the end of single support increased (p = 0.045) which led to a change of the ankle angle slope quotient towards normal positive values (p = 0.001). Abnormal ankle power generation in the first part of single stance changed towards normal (p = 0.012). Notably, the ankle plantar-flexor power was not reduced 10 weeks after BTX-A injection (p = 0.486). No lengthening of the triceps surae muscles compared to the baseline assessment was found. Conclusions: Beneficial functional changes in ankle kinematics and kinetics after the application of BTX-A when combined with serial casting were demonstrated. The results of our study underline the possible differences in effect of BTX-A on intrafusal and extrafusal muscle fibres.
