PURPOSE: Localizing breast lesions by marking tumors and their detection using probes during surgery is a common part of clinical practice. Various nonwire localization systems were intended to be compared from different perspectives. METHODS: Various measurement experiments were performed. Localization techniques, including radioactive seed (RSLS), magnetically guided (MGLS), or radar (SLS), were compared in signal propagation in water and tissue environments, signal interference by surgical instruments, and the practical experience of surgeons. Individual experiments were thoroughly prospectively planned. RESULTS: The RSLS signal was detectable at the largest evaluated distance, ie, 60 mm. The SLS and MGLS signal detection was shorter, up to 25 mm to 45 mm and 30 mm, respectively. The signal intensity and the maximum detection distance in water differed slightly depending on the localization marker orientation to the probe, especially for SLS and MGLS. Signal propagation in the tissue was noted to a depth of 60 mm for RSLS, 50 mm for SLS, and 20 mm for MGLS. Except for the expected signal interferences by approaching surgical instruments from any direction for MGLS, the signal interruption for RSLS and SLS was observed only by inserting instruments directly between the localization marker and probe. Moreover, the SLS signal interference by instrument touch was noted. Based on surgeons' results, individual systems did not differ significantly for most measurement condition settings. CONCLUSION: Apparent differences noted among localization systems can help experts choose an appropriate system for a specific situation or reveal small nuances that have not yet been observed in clinical practice.

Úvod: Neoadjuvantní terapie (NT) je jedna z možných strategií onkologické léčby u malignit prsní žlázy. Cílem je downstaging nádorového postižení prsu a axily a tím možnost konverze mastektomie na záchovný výkon prsu a provedení šetrnější cílené operace axilárních uzlin. Úlohou radiologa je provést pomocí zobrazovacích metod přesný lokální staging malignity před podáním NT, hodnotit efekt léčby v jejím průběhu a po ukončení léčby provést restaging nádorového postižení prsu a axily. Kazuistiky: Autoři prezentují ve třech kazuistikách pacientek s neoadjuvantní chemoterapií (NCHT) diagnostický postup a zobrazování při stanovení lokálního stagingu malignity před léčbou, při sledování v průběhu podávání a při provedení restagingu malignity po ukončení NCHT. Radiologická odpověď po ukončení NCHT je korelována s patologickou odpovědí. Závěr: Správné stanovení rozsahu nádorového postižení prsu a axily radiologem před léčbou a přesné histologické posouzení nádoru patologem jsou zásadní pro rozhodnutí o následné terapii u pacientů v mamárním týmu.
Introduction: Neoadjuvant therapy (NT) is one of the possible oncological treatment strategies for breast cancer. Its aim is to achieve down-staging of the tumour in the breast and axilla and thus the possibility of converting mastectomy to a breast-conserving procedure, and also to allow for a less burdensome and more targeted operation of the axillary lymph nodes. The role of the radiologist is to utilise imaging procedures for precise local staging of the malignancy prior to NT, to evaluate the effect of treatment during its course and upon its completion, and to perform restaging of the cancer in the breast and axilla. Case reports: The authors present three case reports of female patients with breast cancer who underwent neoadjuvant chemotherapy (NCT). They describe the diagnostic procedure and imaging methods used to establish local staging of the cancer prior to treatment, to monitor the disease during the course of treatment, and to perform restaging of the cancer after completing NCT. The radiological response after NCT completion was correlated with the pathological response. Conclusion: Correct determination of the extent of the cancer in the breast and axilla by the radiologist before NT and precise histological analysis of the tumour by the pathologist are fundamental for selecting the appropriate treatment for patients at the multidisciplinary breast tumour board.

Záchovné operace prsu jsou vyhrazeny pro pacienty s příznivým poměrem mezi velikostí nádoru a velikostí prsu. Vložení místního laloku z laterální oblasti hrudníku rozšířilo indikace pro záchovné operace prsu tím, že umožnilo chirurgovi provést širší excizi a zajistit tak čisté okraje excize a dobrý kosmetický výsledek. My používáme laterální interkostální perforátorové laloky a axiální laloky na arteria thoracica lateralis u pacientek s malými prsy a příznivým poměrem velikosti prsu a nádoru. Od května 2015 do října 2016 bylo 19 pacientek s nádory prsu léčeno s využitím záchovné operace a okamžité náhrady objemu pomocí stopkovaných perforátorových laloků z laterální oblasti hrudníku. U 15 pacientek byly použity laterální interkostální arteriální perforátorové laloky nebo perforátorové laloky na arteria thoracica lateralis po kvadrantektomii nebo široké lokální excizi, u tří pacientek jako náhrada objemu po mastektomii, u jedné pacientky po mastektotomii po předchozí augmentační mamaplastice. U všech pacientek byla dosažena dobrá symetrie prsu, bez žádných velkých komplikací. Po pooperační radioterapii byla u hubených pacientek nebo u laloků s malým množstvím podkožního tuku pozorována fibróza laloku a reziduálního prsního parenchymu s redukcí objemu. Perforátorové laloky z laterální oblasti hrudníku by se měly stát zlatým standardem pro rekonstrukci po záchovných operacích prsu zahrnujících méně než 20 % objemu prsu nebo po mastektomii u pacientek s malými prsy. Operační výkon je bezpečný, rychlý a umožňuje zachování musculus latissimus dorsi a tím minimalizuje morbiditu donorského místa a má také excelentní estetický výsledek.
Breast conserving surgery has been reserved for patients with favorable proportion between tumor dimensions and breast size. Introduction of local flaps from the lateral thoracic region has widened the indications for breast conserving surgery, by allowing surgeons to perform wider excisions, thus yet be able to ensure tumor-free surgical margins and a good aesthetic result. We have used lateral intercostal perforator flaps and flaps harvested on the lateral thoracic artery and lateral thoracic artery axial flap in patients with small breasts and an unfavorable tumor to breast size proportion. From May 2015 to October 2016, 19 patients with breast tumors have been treated with BCS and immediate volume replacement reconstruction by pedicle perforator flaps from the lateral thoracic region. In 15 patients lateral intercostal artery perforator flaps or lateral thoracic artery perforator flaps were used after quadrantectomy or wide local excision, in 3 patients as volume replacement after mastectomy and in 1 patient after mastectomy following previous augmentation mammoplasty. In all patients, good breast symmetry was achieved, with no major complications. Fibrosis of the flap and residual breast parenchyma, with volume reduction were noticed after postoperative radiotherapy in thin patients or flaps with little subcutaneous fat. Perforator flaps from the lateral thoracic region should become the gold standard for reconstructions after breast conserving surgery involving less than 20% of the breast volume or after mastectomy in patients with small breasts. The operating procedure is safe, quick and allows sparing of the latissimus dorsi muscle and thus minimal donor site morbidity, as well as an excellent aesthetic result.
PURPOSE: To assess the feasibility of high-dose-rate perioperative multicatheter interstitial brachytherapy to deliver accelerated partial breast irradiation (APBI) in selected patients with early breast cancer. METHODS AND MATERIALS: Perioperative multicatheter interstitial brachytherapy for APBI has been used at our department since 2012 for patients with low-risk breast cancer. Interstitial catheters were inserted perioperatively via hollow needles immediately following tumorectomy with sentinel node biopsy. APBI started on Day 6 after surgery. The prescribed dose was 34 Gy (10 fractions of 3.4 Gy bid). Hormonal therapy was prescribed in all cases. RESULTS: Between June 2012 and December 2017, 125 patients were scheduled for APBI. Of these, APBI was not performed in 12 patients (9.6%) due to adverse prognostic factors identified on the definitive biopsy. We observed wound dehiscence in 2/113 cases (1.8%), inflammatory complications requiring antibiotics in 7/113 cases (6.2%), transient Grade I radiodermatitis in 6/113 patients (4.4%), and seroma which resolved spontaneously in 3/113 patients (2.7%). With median followup of 39 months (range 3.3-75.3) no relapses were observed. No late complications in Radiation Therapy Oncology Group Grade 3 or higher were documented. Cosmetic outcome in patients with followup > 2 years was excellent or good in 92%. CONCLUSION: Our preliminary results show that the perioperative multicatheter interstitial high-dose-rate brachytherapy for APBI in selected patients with early breast cancer is feasible. This treatment schedule reduces treatment duration, spares the patients of repeated anesthesia, and enables precise application of the afterloading tubes under direct visual control.

BACKGROUND: In a phase 3, randomised, non-inferiority trial, accelerated partial breast irradiation (APBI) for patients with stage 0, I, and IIA breast cancer who underwent breast-conserving treatment was compared with whole-breast irradiation. Here, we present 5-year follow-up results. METHODS: We did a phase 3, randomised, non-inferiority trial at 16 hospitals and medical centres in seven European countries. 1184 patients with low-risk invasive and ductal carcinoma in situ treated with breast-conserving surgery were centrally randomised to either whole-breast irradiation or APBI using multicatheter brachytherapy. The primary endpoint was local recurrence. Analysis was done according to treatment received. This trial is registered with ClinicalTrials.gov, number NCT00402519. FINDINGS: Between April 20, 2004, and July 30, 2009, 551 patients had whole-breast irradiation with tumour-bed boost and 633 patients received APBI using interstitial multicatheter brachytherapy. At 5-year follow-up, nine patients treated with APBI and five patients receiving whole-breast irradiation had a local recurrence; the cumulative incidence of local recurrence was 1.44% (95% CI 0.51-2.38) with APBI and 0.92% (0.12-1.73) with whole-breast irradiation (difference 0.52%, 95% CI -0.72 to 1.75; p=0.42). No grade 4 late side-effects were reported. The 5-year risk of grade 2-3 late side-effects to the skin was 3.2% with APBI versus 5.7% with whole-breast irradiation (p=0.08), and 5-year risk of grade 2-3 subcutaneous tissue late side-effects was 7.6% versus 6.3% (p=0.53). The risk of severe (grade 3) fibrosis at 5 years was 0.2% with whole-breast irradiation and 0% with APBI (p=0.46). INTERPRETATION: The difference between treatments was below the relevance margin of 3 percentage points. Therefore, adjuvant APBI using multicatheter brachytherapy after breast-conserving surgery in patients with early breast cancer is not inferior to adjuvant whole-breast irradiation with respect to 5-year local control, disease-free survival, and overall survival. Následující článek pojednává o metastáze karcinomu prsu do močového měchýře. Navzdory tomu, že metastázy karcinomu prsu jsou běžné, zaznamenaná incidence metastáz karcinomu prsu do močového měchýře je asi 3 % (1–3). Ve většině případů je metastáza do měchýře spojena ze symptomy dolních močových cest a s dalšími metastázami do pánevních orgánů (3).
The article describes the case of metastasis of breast cancer to the urinary bladder. However, metastases of breast cancer are common, the documented incidence of bladder metastases is roughly 3%. In majority of cases, the breast cancer metastasis to the bladder is associated with lower urinary tract symptoms and other pelvic organ metastases.

Prs záchovné operace se v posledních letech stávají nejčastěji používanou metodou léčby časných stadií karcinomu prsu. Kompletní odstranění nádorového ložiska je při použití této metody statisticky významným parametrem, který ovlivňuje vznik lokální recidivy a celkové přežití. Stanovení kompletnosti resekce a zejména vzdálenosti nádorového ložiska od okrajů resekátu však přináší řadu problémů, které vyplývají jak z charakteru tkáně s převahou tukového stromatu, tak z charakteru vlastní nádorové tkáně, dané jejím způsobem růstu a multifokalitou. Přitom stanovení velikosti okraje, který považujeme za bezpečný, zůstává kontroverzním tématem. V článku probíráme současné pohledy na hodnocení volných, resp. bezpečných okrajů a shrnujeme technické možnosti stanovení resekčních okrajů v mamární onkochirurgii.
In recent years breast-conserving therapy has become the most frequently used therapy for early stages of breast cancer. The complete excision of a tumour using this protocol is a statistically significant parameter which influences the incidence of local recurrence and overall survival. Evaluation of the completeness of the resection and especially the distance of the tumour from the margins of the lumpectomy, however, brings a number of problems which arise not only from the character of the tissue with a majority of adipose stroma, but also from the character of the tumour tissue itself regarding growth pattern and multifocality. In fact, establishing the dimension of the margins which we consider safe, remains rather controversial. In this article we discuss contemporary views on the free or safe margins evaluation and summarise technical possibilities for examination of the resection margins in mammary oncosurgery.
