Rekonštrukcia močových ciest je integrálnou súčasťou transplantácie obličky. Cieľom rekonštrukcie močových ciest je snaha dosiahnuť optimálnu a sociálne najprijateľnejšiu drenáž moču z transplantovanej obličky. Anatomický a funkčný stav močových ciest významne ovplyvňuje nielen kvalitu života pacienta po transplantácii obličky, ale aj funkciu a prežitie transplantovanej obličky i samotných pacientov. Komplikácie urologického charakteru, ktoré majú súvislosť s rekonštrukciou močových ciest tvoria samostatnú kapitolu medzi chirurgickými komplikáciami po transplantácii obličky a patria medzi najzávažnejšie komplikácie po transplantácii obličky. Môžu byť príčinou straty transplantovanej obličky, ale môžu pacientov aj priamo ohrozovať na živote. V prípade urologických komplikácií sa indikuje promptná diagnostika a razantné chirurgické riešenie a to individuálne podľa charakteru komplikácie.
Urinary tract reconstruction is an integral part of kidney transplantation. The aim of urinary tract reconstruction is to obtain optimum and socially acceptable drainage of urine from the transplanted kidney. The anatomical and functional status of the urinary tract significantly affects not only the patient´s quality of life after kidney transplantation, but also the function and survival of the transplanted kidney, and of the patients themselves. Urological complications, related to urinary tract reconstruction, create a specific family among surgical complications after kidney transplantation, and belong to the most severe complications after kidney transplantation. They can cause loss of the transplanted kidney, and endanger the patient´s life. In the case of urological complications, immediate precise diagnostics and appropriate surgical therapy, based on individual characteristics of the complication, is indicated.

Chirurgické techniky používané při transplantacích zahrnují odběrové operace u dárců i vlastní transplantaci. Samostatnou kapitolou je provedení odběru ledviny od žijícího dárce, které má svá charakteristická pravidla. Transplantace ledviny sestává ze dvou fází - cévní anastomózy a implantace močovodu. Cílem všech operací spojených s transplantací je obnovení funkce ledviny v těle příjemce a zároveň zajištění podmínek pro její dlouhodobé dobré fungování.
Surgical techniques utilized in transplantations include harvesting procedures and the transplantation itself. Living-donor nephrectomy with its specific principles represents a special chapter. Kidney transplantation consists of two phases – vascular anastomosis and ureteral implantation. The aim of all ope - rations associated with the transplantation is a kidney function recovery in the recipient body and ensurance of the optimal conditions for long-term graft function.

Je prezentován popis případu 37letého muže s transplantovanou ledvinou. Ledvina byla odebrána z kadaverózního dárce věku 3,5 let, štěp měl dvě tepny, které byly transplantovány na aortálním terčíku na zevní iliackou tepnu příjemce. Čtyři měsíce po transplantaci se dobře rozvinutá funkce štěpu začala zhoršovat, společně s rozvojem hypertenze. Byla diagnostikována stenóza obou tepen štěpu a indikována angioplastika. Obě zúžení špatně reagovala na angioplastiku, která musela být v krátké době opakovaná. Ani po druhé angioplastice nebyl nález ideální a nakonec došlo k perifernímu uzávěru horní a současně menší z obou ledvinných tepen, funkce štěpu se ale zlepšila a byla v podstatě stabilní po další čtyři roky, hypertenze přetrvávala a byla obtížně kontrolovatelná. Pátý rok po transplantaci se funkce štěpu opět zhoršila a byla prokázána těsná restenóza jediné zbylé tepny štěpu. Zúžení špatně reagovalo na dilataci, krátce po ní došlo k úplnému selhání funkce štěpu a dekompenzaci hypertenze s následným levostranným selháním. Pacient byl opakovaně dialyzován, další pokus o angioplastiku byl proveden s použitím řezacího balónkového katétru (Cutting Balloon). Tato angioplastika dobře rozšířila tepnu štěpu, během několika dnů došlo k úplnému návratu funkce štěpu při hodnotách SCr 140 až 160 umol/1, GFR (CCr) 0,65 ml/sec. Funkce štěpu zůstává stabilní již více než dva roky, přetrvává obtížně kontrolovatelná hypertenze.
This study is a report of 37 year old male with a transplanted kidney. The kidney cadaverous donor was a 3.5 year old child - the graft had two arteries which were transplanted with an aortic patch to an external iliac artery. Four months after transplantation, the graft function was deteriorating together with the development of hypertension. Stenosis of both graft arteries was detected and the patient was referred to angioplasty. The stenoses poorly responded to dilatation and it had to be repeated. Although the completion angio-graphy after second PTRA was not ideal and the smaller of both graft arteries eventually occluded, the graft function improved and was more or less stable (SCr 160-200 umol/1) for four years along with persistent difficult to control hypertension. Five years after transplantation, the graft function impaired again and a tight graft artery restenosis was detected. The restenosis poorly responded to dilatation, graft function failed, hypertension decompensated and LV failure developed. The patient required dialysis. An attempt to open the artery using a cutting balloon was performed. The angioplasty opened the artery and kidney function was restored after a few days and SCr level dropped to levels 140-160 umol/1, GFR (CCr) 0.65 ml/s. The graft function has been stable for more than two years, however, the hypertension is still difficult to control.
