Společně s vývojem medicíny se neustále vyvíjejí také možnosti transplantace tkání a orgánů, což s sebou přináší řadu etických konotací. V transplantační medicíně je nutné dodržovat vzájemně propojené legislativní požadavky a etické principy. Transplantace tkání a orgánů přináší etické aspekty i do běžné praxe. S potřebou řešení aktuálních etických souvislostí se setkáváme při vývoji technologií umožňujících rozvoj xenotransplantací. Uvažuje se také využití technologií umělé inteligence pro etické rozhodování v transplantační medicíně.
Along with the development of medicine and new therapeutic methods, the options of tissue and organ transplantation are constantly evolving, which brings a whole range of ethical connotations. The paper presents the legislative requirements and ethical principles that should be followed in transplant medicine, as well as their mutual connection. The article focuses on the ethical aspects which occur in connection with tissue and organ procurement and transplantation in everyday practice. The article also deals with modern methods that are currently being intensively developed and, in the future, could be helpful in solving contemporary issues in the field of transplantation. This concerns the ethical context of xenotransplantation, and the possibility of using artificial intelligence technologies for ethical decision-making in transplant medicine.

S rychlým rozvojem oborů transfuzní, transplantační a regenerační medicíny se pojí nutnost informovat o připravovaných legislativních změnách na úrovni Evropské unie (EU) a stručně popsat a porovnat náročnost některých dárcovských procedur z pohledu živého dárce i s jejich riziky, včetně psychosociálních. Tato práce je postavena na kvalitativním expertním šetření. Porovnání náročnosti procedur z pohledu dárců je důležité například pro nastavení kompenzací. Z tabulek vyplývá, že současné kompenzace jsou v nepoměru.
Transfusion, transplantation, and regenerative medicine are rapidly developing fields. The authors of the text want to inform about upcoming legislative changes at the EU level and briefly describe and compare the difficulty of some donation procedures from the point of view of a living donor, as well as their risks, including psychosocial risks. The study is based on a qualitative expert investigation. Comparing the complexity of procedures from the perspective of donors is important, for example, for setting compensations. The tables show that the current compensations are disproportionate.

Těla zemřelých jsou od pradávna nedílnou součástí výuky anatomie, potažmo studia medicíny. Bez nich by nemohly být realizovány kvalitní výuka anatomie a klinické anatomie ani mnoho výzkumných projektů. V současnosti se v evropských zemích používají k anatomické pitvě pouze těla dárců. V poslední době jsme zaznamenali vzrůstající trend potřeb těl nejen pro výuku mediků, ale také z hlediska klinické anatomie a popromočního vzdělávání. Také vyvstala otázka vhodnosti použití COVID-19 pozitivních dárců nebo legislativní možnosti získání těl při nedostatku dárců v dárcovském programu. Naše sdělení řeší aktuální otázky dárcovství těla pro výukové a výzkumné účely a jejich využití v Česku.
Since time immemorial, bodies of deceased have been an integral part of teaching anatomy, and therefore the study of medicine. Without them, the teaching of anatomy, clinical anatomy and many research projects could not be realized. Nowadays, the European countries allow to use exclusively bodies of the deceased donors. Recently, we have registered a growing trend in the needs of the bodies not only for the purposes of medical education, but also for those of clinical anatomy. The question also arose of the suitability of using COVID-19 positive donors or the legislative possibility of obtaining bodies in the absence of donors in the donor program. Our communication addresses current issues of body donation for teaching and research purposes and their use in the Czech Republic.

BACKGROUND: In recent decades gamete donation has received growing attention. Data from the Czech National Registry of Assisted Reproduction show that the number of cycles using donated oocytes has been increasing every year. According to Czech law, gamete donation is anonymous. Since 2011, some members of the Czech parliament have repeatedly made requests to revoke the anonymity but anonymity is one of the preconditions for such donation in this country. The aim of this study was to find out how the gamete recipients feel towards informing their child about the circumstances of their conception and their access to the identity of the donor. METHODS: A total of 195 recipients (122 women undergoing treatment - 43 Czechs, 79 foreigners (Western Europe and the USA) and 73 male partners - 28 Czechs, 45 foreigners) participated in this survey. The data were obtained by anonymous questionnaire. RESULTS: A significant difference between the attitude of the future Czech and foreign parents regarding disclosing the mode of conception was found (P = 0.003). The vast majority of Czechs were against disclosure. The foreign recipients were somewhat more divided. Regarding the donor's identity, there was no difference in atttitude between the groups. Recipients rarely consider that the knowledge of the donor's identity will be important for their child. The recipients overall, were convinced that the psychological aspects of parenting are far more important to the child than genetics, and see no reasons for disclosing the donor´s identity. CONCLUSION: While the the foreign recipients were less adamant about non-disclosure, the overall finding was in accord with the current Czech law on anonymity and not in agreement with the proposed abolition. The recipient's attitudes towards disclosing were also culturally determined. The fact that some countries have revised their rules towards open idendity is not a rationale for such change in the Czech Republic.

Transplantační medicína zaznamenává v posledních letech nebývalý rozvoj, počet nemocných léčených orgánovou transplantací setrvale narůstá. Dárcovský program je pak úhelným kamenem celé transplantační medicíny. V České republice představují dárci se smrtí mozku většinový zdroj orgánů pro potřeby transplantace. Legislativní a organizační rámec je v našich podmínkách definován transplantačním zákonem a souvisejícími předpisy. V následujícím přehledu dále shrnujeme patofyziologické pochody v souvislosti s mozkovou smrtí, diagnostický postup k potvrzení smrti mozku a základní opatření v péči o hemodynamiku, ventilaci a vnitřní prostředí potenciálního dárce orgánů na jednotce intenzivní péče.
Transplantation medicine is undergoing an extraordinary progress nowadays. The number of organ transplants has been permanently increasing. The organ donation program represents a cornerstone of transplantation medicine. The majority of organs for transplantation in the Czech Republic is harvested from brain-dead donors. The legislative and organisational framework has been defined by the Transplantation Act and related documents. In this atricle we review the basics of brain death pathofysiology, brain death diagnostics and the complex management of the organ donor in intensive care units.
