Zavedení thienopyridinů do klinické praxe v polovině devadesátých let 20. století umožnilo revoluci v intervenční kardiologii. Stenty do té doby byly spjaty s 6–8% rizikem subakutní trombózy a s 12–13% rizikem závažných krvácivých komplikací (v důsledku léčby kombinací heparinu s warfarinem a kyselinou acetylsalicylovou, ASA). Zjednodušení antitrombotické medikace po implantaci stentů na duální protidestičkovou léčbu (ASA + ticlopidin, později ASA + clopidogrel) snížilo riziko trombózy stentu na méně než 1 % a riziko krvácivých komplikací po PCI na 1–2 %. Přibližně v téže době studie CAPRIE prokázala o 9 % vyšší antitrombotickou účinnost clopidogrelu ve srovnání s ASA při současném o 30 % nižším výskytu krvácení. Řada následujících klinických studií prosadila clopidogrel do široké medicínské praxe v těchto indikacích: perkutánní koronární intervence, akutní koronární syndrom, cévní mozková příhoda, sekundární prevence infarktu myokardu, sekundární prevence cévní mozkové příhody. V těchto indikacích se clopidogrel podává buď v kombinaci s ASA, nebo jako monoterapie (tam, kde nemocní ASA netolerují). Clopidogrel je tedy dnes protidestičkovým léčivem s nejpříznivějším poměrem účinek/bezpečnost. Neprokázalo se však, že by clopidogrel byl vhodný i pro primární prevenci u rizikových, ale klinicky zatím zdravých osob – zůstává tedy nejúčinnějším léčivem vyhrazeným pro sekundární prevenci aterotrombotických příhod u osob, u kterých se aterotrombóza již klinicky manifestovala.
Introduction of thienopyridines into clinical practice in the last decade allowed the revolution in interventional cardiology. Before this discovery stent used to be connected with the 6–8 % risk of subacute thrombosis and 12–13 % risk of serious hemorrhage complications (as a result of combination of warfarin and acetylsalicylic acid (ASA)). Simplification of antithrombotic medication after implantation of stents to dual antithrombotic therapy (ASA + ticlopidine, ASA + clopidogrel) lowered the risk of stent thrombosis to < 1 % and risk of hemorrhage complication after PCI to 1–2 %. In the relatively same time CAPRIE study proved 9 % higher antithrombotic efficacy of clopidogrel compared to ASA and 30 % lower incidence of hemorrhage complications. Results of several subsequent studies led to implementation of clopidogrel into clinical practice in following indications: percutaneous coronary intervention, acute coronary syndrome, stroke, secondary prevention of myocardial infarction, secondary prevention of stroke. In these indication clopidogrel is prescribed in combination with ASA or alone in case of hypersensitivity to ASA. Nowadays clopidogrel is a antithrombotic substance with the best risk/benefit ratio. Nonetheless, it was shown clopidogrel is not sufficiently effective in primary prevention. Thus, clopidogrel remains the most effectiv e drug for secondary prevention ot atherothrombotic events in patients with clinically manifested atherothrombosis.

Clopidogrel je perorálně podávaný thienopyridinový přípravek s protidestičkovým působením, který je již řadu let dostupný zejména v indikaci prevence trombotických komplikací u nemocných s ischemickou chorobou srdeční, periferními formami ischemie nebo cerebrovaskulární nemocí. Základem působení je ireverzibilní vazba na adenosindifosfátové (ADP) chemoreceptory P2Y12 na povrchu trombocytů. Clopidogrel je indikován zejména u akutních koronárních stavů s intervencemi, kde podáváme nasycovací dávku 300–600 mg před výkonem a následně dlouhodobě 75 mg denně, spolu s kyselinou acetylsalicylovou (ASA); dále je indikován v prevenci aterotrombotických příhod u nemocných, u nichž není možno podávat ASA. S příchodem řady generik clopidogrelu se zvýšila jeho dostupnost pro pacienty a stal se reálnější alternativou ASA. Spolu s ASA tvoří tzv. duální protidestičkovou medikaci, která zlepšuje prognózu zejména u nemocných po infarktech myokardu a po zavedení stentů.
Clopidogrel is an oral thienopyridine antiplatelet agent, available for many years especially for prevention of thrombotic complications in coronary artery disease, peripheral forms of ischemic disease and cerebrovascular disorders. It acts through an irreversible binding to adenosin-diphosphate (ADP) receptors P2Y12 on the platelet surface. Clopidogrel is indicated especially in acute coronary events requiring interventions; initial dose of 300–600 mg before the procedure is used in these cases, followed by a maintenance dose of 75 mg daily combined with acetylsalicylic acid (ASA). It is also indicated for prevention of atherothrombotic events in patients who cannot be treated with ASA. With the advent of many generics, clopidogrel became more easily available and a more realistic alternative to ASA. The combination of clopidogrel with ASA represents a “dual antiplatelet medication” that improves the prognosis of many patients, mainly those with a history of myocardial infarction and/or stent placement.

Studie porovnávající účinnost ticagreloru proti clopidogrelu u nemocných se symptomatickou ischemickou chorobou dolních končetin během 30měsíčního sledování ukázala, že ticagrelor není superiorní ve smyslu snížení kardiovaskulárních událostí, pouze bylo pozorováno snížení počtu ischemických iktů. Velká krvácení se vyskytla ve stejné míře v obou léčebných režimech.
In patients with symptomatic peripheral artery disease, ticagrelor was not shown to be superior to clopidogrel in the reductionof cardiovascular events during a 30-month follow-up period; the only significant difference between the groups was in the rateof ischemic stroke. The rates of major bleeding were similar among the patients in the two trial groups.

BACKGROUND: Gastrointestinal complications are an important problem of antithrombotic therapy. Proton-pump inhibitors (PPIs) are believed to decrease the risk of such complications, though no randomized trial has proved this in patients receiving dual antiplatelet therapy. Recently, concerns have been raised about the potential for PPIs to blunt the efficacy of clopidogrel. METHODS: We randomly assigned patients with an indication for dual antiplatelet therapy to receive clopidogrel in combination with either omeprazole or placebo, in addition to aspirin. The primary gastrointestinal end point was a composite of overt or occult bleeding, symptomatic gastroduodenal ulcers or erosions, obstruction, or perforation. The primary cardiovascular end point was a composite of death from cardiovascular causes, nonfatal myocardial infarction, revascularization, or stroke. The trial was terminated prematurely when the sponsor lost financing. RESULTS: We planned to enroll about 5000 patients; a total of 3873 were randomly assigned and 3761 were included in analyses. In all, 51 patients had a gastrointestinal event; the event rate was 1.1% with omeprazole and 2.9% with placebo at 180 days (hazard ratio with omeprazole, 0.34, 95% confidence interval [CI], 0.18 to 0.63; P<0.001). The rate of overt upper gastrointestinal bleeding was also reduced with omeprazole as compared with placebo (hazard ratio, 0.13; 95% CI, 0.03 to 0.56; P = 0.001). A total of 109 patients had a cardiovascular event, with event rates of 4.9% with omeprazole and 5.7% with placebo (hazard ratio with omeprazole, 0.99; 95% CI, 0.68 to 1.44; P = 0.96); high-risk subgroups did not show significant heterogeneity. The two groups did not differ significantly in the rate of serious adverse events, though the risk of diarrhea was increased with omeprazole. CONCLUSIONS: Among patients receiving aspirin and clopidogrel, prophylactic use of a PPI reduced the rate of upper gastrointestinal bleeding. There was no apparent cardiovascular interaction between clopidogrel and omeprazole, but our results do not rule out a clinically meaningful difference in cardiovascular events due to use of a PPI. It remains unclear whether dual antiplatelet therapy >12 months might carry a better prognosis after percutaneous coronary intervention (PCI) with drug-eluting stents (DESs). To address the hypothesis that in the real world the risk of very late thrombosis after PCI with DESs can be decreased by an extended use of clopidogrel, we set up the Two-Year ClOpidOgrel Need (TYCOON) registry and prospectively investigated the impact on very late thrombosis of 12- versus 24-month dual antiplatelet regimens in an unselected population. The registry enrolled 897 consecutive patients who underwent PCI with stenting from January 1, 2003, to December 31, 2004, and had dual antiplatelet therapy. All patients had a 4-year clinical follow-up. In the 447 patients with DES implantation, the dual antiplatelet regimen after PCI was given for 12 months in the 173 patients treated in 2003 (12-month group) and for 24 months in the 274 patients treated in 2004 (24-month group). Comparison between groups did not reveal any significant difference in baseline clinical characteristics, angiographic and procedural features, and major adverse cardiac events. During follow-up, there were 5 cases of stent thrombosis after PCI in the 12-month DES group and 1 case in the 24-month DES group (p = 0.02). Specifically, there were 2 cases of subacute thrombosis (1 in each group), no case of late thrombosis, and 4 cases of very late thrombosis occurring at 13, 15, 17, and 23 months after DES implantation in the 12-month group only. In conclusion, a 2-year dual antiplatelet regimen with aspirin and clopidogrel can prevent the occurrence of very late stent thrombosis after PCI with DESs.

Dvě nové studie (CLARITY a COMMIT) přinesly nové údaje o roli clopidogrelu v léčbě pacientů s infarktem myokardu s ST elevacemi (STEMI), studie ARMYDA-2 řeší otázku optimalizace úvodní dávky clopidogrelu před intervenčním výkonem. Ve studii CLARITY vedlo přidání clopidogrelu k léčbě kyselinou acetylosalicylovou a trombolytikem ke zlepšení průchodnosti infarktové tepny a k snížení výskytu ischemických komplikací u STEMI pacientů. Studie COMMIT prokázala příznivý vliv časného podání clopidogrelu na mortalitu pacientů se STEMI. Ve studii ARMYDA-2 vedlo předléčení dávkou 600 mg clopidogrelu 4 až 8 hodin před koronární intervencí ve srovnání s konvenční dávkou 300 mg k významnému snížení výskytu periprocedurálního infarktu myokardu. Všechny studie potvrdily bezpečnost podávání clopidogrelu ve vztahu ke krvácení.
Two new trials (CLARITY and COMMIT) have brought new data on the role of clopidogrel in treatment of patients with ST-elevation-myocardial infarction (STEMI). The ARMYDA-2 trial is concerned with optimalization of clopidogrel loading dose before intervention. In the CLARITY trial, the addition of clopidogrel to aspirin plus thrombolysis improved the patency rate of the infarct-related artery and reduced ischaemic complications in STEMI patients. The COMMIT trial has shown a positive effect of early administered clopidogrel on mortality of STEMI patients. In the ARMYDA-2 trial, pretreatment with a 600 mg loading dose of clopidogrel 4 to 8 hours before coronary intervention significantly reduced periprocedural MI as compared to the conventional 300 mg dose. All trials confirmed the safety of clopidogrel in relation to bleeding.

Clopidogrel loading has mostly been studied in clopidogrel-naive patients. Whether clopidogrel-treated patients readmitted for an acute coronary syndrome or percutaneous coronary intervention can benefit from a new load of clopidogrel and at what dose remain unknown. Our aim was to evaluate the impact of 3 different strategies of administration of a loading dose of 900 mg clopidogrel in patients already treated with a maintenance dose of 75 mg clopidogrel for at least 7 days on residual platelet aggregation. METHODS AND RESULTS: Patients treated long term by clopidogrel 75 mg/d were assigned to receive a first loading dose of 300, 600, or 900 mg clopidogrel and 4 hours later a second loading dose of 600, 300, or 0 mg, respectively, to achieve a total loading dose of 900 mg in all patients. Platelet aggregation was evaluated at baseline, at 4 hours after the initial load (and before second load), and at 24 hours using light transmission aggregometry with 20 micromol ADP and the point-of-care assay VerifyNow P2Y(12). The primary objective of the study was to evaluate the inhibition (relative change) of residual platelet aggregation (percentage of IRPA) between 600- and 900-mg first loading at 4 hours. IRPA at 24 hours also was evaluated as a secondary objective, as well as the rate of suboptimal response at 4 hours defined as IRPA <10%. We included 166 consecutive patients with acute coronary syndromes (n=80, 48%) or stable coronary artery disease (n=86, 52%). Baseline characteristics were similar in the 3 dose groups. A significant stepwise increase was found in percentage IRPA assessed at 4 hours in patients initially assigned to 300 versus 600 versus 900 mg (30.7% versus 40.3% versus 64.0%, respectively; P=0.0024). The difference in percentage IRPA at 4 hours was not significant between 300 and 600 mg but was significant between 600 and 900 mg and between 300 and 900 mg. Percentage IRPA assessed at 24 hours when all patients had received 900 mg did not differ between the 3 loading regimens. The rates of suboptimal response (IRPA <10% at 4 hours) were 23.6%, 20.4%, and 5.3% with 300, 600, and 900 mg, respectively (P=0.02 for all). CONCLUSIONS: In patients treated long term with 75 mg clopidogrel, a new loading dose of 900 mg improves IRPA and reduces poor and/or slow response to clopidogrel significantly more than that obtained with 300 or 600 mg.

Existence pozdní trombózy koronárních stentů je aktuálním problémem současné kardiologie. V přehledném článku je diskutována současná definice této jednotky, její časové vymezení a reálný výskyt. Podle údajů z dostupných observačních a randomizovaných studií je posuzována dlouhodobá bezpečnost konvenčních kovových a lékových stentů a vývoj této bezpečnosti v čase od implantace. Délka léčby thienopyridiny po koronární angioplastice (PCI) je rozdílná podle typu použitého stentu. Zatímco implantace konvenčního kovového stentu (BMS) je dlouhodobě bezpečná při 1měsíční léčbě, implantace lékového stentu (DES) by měla být podle údajů observačních studií doplněna až 12měsíčním podáváním klopidogrelu. Tento postoj může být v budoucnu ovlivněn dalším vývojem lékových stentů a prospektivně získanými informacemi o dlouhodobém podávání klopidogrelu těmto nemocným.
