OBJECTIVES: Mindfulness-based interventions (MBIs) for persons with dementia (PwD) have yielded mixed results, possibly attributable to the fact that little is known about the validity and reliability of trait mindfulness self-report measures in PwD. This narrative review sought to identify studies involving self-reported trait mindfulness and other clinical measures that may hold information on the convergent validity and reliability of these measures in PwD. METHODS: Scientific databases were searched for studies involving PwD and mindfulness assessments. RESULTS: N = 426 studies from PubMed and N = 156 from PsychInfo databases were reviewed. Four cross-sectional studies were identified that allowed inferences about the validity of mindfulness measures. A qualitative review indicated that convergent validity with other measures varied with sample heterogeneity and cognitive impairment. Merely one MBI included self-reported trait mindfulness, however without reporting sample-specific validity or reliability. CONCLUSIONS: Despite efforts to implement MBIs in PwD, information on basic methodological psychometric issues is minimal. Future studies ought to address the validity and reliability of self-reported mindfulness in detail across different stages of dementia. CLINICAL IMPLICATIONS: Results of MBIs need to be considered cautiously. Basic information about psychometric properties of mindfulness self-report measures is required and these measures need to be included systematically in MBIs.

OBJECTIVE: Research suggests that disrupted interoception contributes to the development and maintenance of functional neurological disorder (FND); however, no functional neuroimaging studies have examined the processing of interoceptive signals in patients with FND. METHODS: The authors examined univariate and multivariate functional MRI neural responses of 38 patients with mixed FND and 38 healthy control individuals (HCs) during a task exploring goal-directed attention to cardiac interoception-versus-control (exteroception or rest) conditions. The relationships between interoception-related neural responses, heartbeat-counting accuracy, and interoceptive trait prediction error (ITPE) were also investigated for FND patients. RESULTS: When attention was directed to heartbeat signals versus exteroception or rest tasks, FND patients showed decreased neural activations (and reduced coactivations) in the right anterior insula and bilateral dorsal anterior cingulate cortices (among other areas), compared with HCs. For FND patients, heartbeat-counting accuracy was positively correlated with right anterior insula and ventromedial prefrontal activations during interoception versus rest. Cardiac interoceptive accuracy was also correlated with bilateral dorsal anterior cingulate activations in the interoception-versus-exteroception contrast, and neural activations were correlated with ITPE scores, showing inverse relationships to those observed for heartbeat-counting accuracy. CONCLUSIONS: This study identified state and trait interoceptive disruptions in FND patients. Convergent between- and within-group findings contextualize the pathophysiological role of cingulo-insular (salience network) areas across the spectrum of functional seizures and functional movement disorder. These findings provide a starting point for the future development of comprehensive neurophysiological assessments of interoception for FND patients, features that also warrant research as potential prognostic and monitoring biomarkers.

Cognitive flexibility (CF) is the ability to adapt cognitive strategies according to the changing environment. The deficit in CF has often been linked to various neurological and psychiatric disorders including schizophrenia. However, the operationalization and assessment of CF have not been unified and the current research suggests that the available instruments measure different aspects of CF. The main objective of the present study was to compare three frequently used neuropsychological measures of CF-Wisconsin Card Sorting Test (WCST), Trail Making Test (TMT) and Stroop Color and Word Test (SCWT) in a population of patients (N = 220) with first-episode schizophrenia spectrum disorders in order to evaluate their convergent validity. The hypothesis of an underlying latent construct was tested via a confirmatory factor analysis. We used a one-factor CF model with scores from WCST, SCWT and TMT as observed variables. The established model showed a good fit to the data (χ2 = 1.67, p = 0.43, SRMR = 0.02, RMSEA = 0.0, CFI = 1.00). The highest factor loading was found in WCST as CF explained most of the variance in this neuropsychological measure compared to the other instruments. On the other hand, a TMT ratio index and a SCWT interference demonstrated lowest loadings in the model. The findings suggest that not all the frequently used measures share an underlying factor of CF or may capture different aspects of this construct.

Úvod: Primární tumory a infiltrativní procesy očnice zahrnují poměrně širokou škálu dia gnóz. Nádory a infiltrace, které vycházejí primárně z tkání prostoru očnice ohraničeného periorbitou, označujeme jako primární, zatímco sekundární tumory zasahují do orbity z okolních tkání vně od periorbity. Speciální skupinu tvoří léze metastatické. Cíl: Retrospektivní observační analýza souboru pacientů diagnostikovaných pro primární tumor a primární infiltraci očnice na Klinice otorinolaryngologie a chirurgie hlavy a krku v Nemocnici u sv. Anny v Brně (KOCHHK) v letech 2000– 2023. Metodika: Zařazeni byli pacienti ve věku ≥ 18 let, kteří absolvovali otorinolaryngologické a oftalmologické vyšetření, zobrazovací vyšetření (CT/ MR) a podstoupili stanovenou léčbu. Hodnoceny byly demografické parametry, symptomatologie, diagnostický a terapeutický přístup, histologická charakteristika procesů a efekt léčby. Výsledky: Ve sledovaném souboru byl nejčastějším benigním procesem pseudotumor očnice (12 pacientů, 33 %), z toho jedenáct pacientů podstoupilo kortikoidní terapii. U sedmi pacientů došlo ke kompletní regresi, u jednoho pacienta k parciální regresi, u čtyř byla pozorována recidiva. Z maligních infiltrací byl nejvíce zastoupen lymfom (10 pacientů, 27 %), přičemž u šesti pacientů se jednalo o MALT-lymfom. U devíti pacientů došlo ke kompletní remisi, u jednoho pacienta k remisi parciální. Závěr: Dia gnostika a terapie primárních lézí očnice nevyhnutelně vyžaduje mezioborovou spolupráci oftalmologa, neurochirurga, otorinolaryngologa, radiologa, histopatologa, event. hematoonkologa a dalších. Pro diagnostiku a adekvátní léčbu je klíčové zobrazovací vyšetření doplněno zpravidla i o histologickou verifikaci. Její provedení a event. chirurgické odstranění léze je značně limitováno lokalizací. Prognóza závisí nejen na maligním potenciálu léze, ale významně také na jejím vztahu k okolí a k důležitým strukturám v očnici.
Introduction: Primary tumors and infiltrative processes of the orbit include a fairly wide range of diagnoses. Tumors and infiltrations that arise primarily from the tissues of the orbital space bounded by the periorbita are termed primary, while secondary tumors encroach into the orbit from surrounding tissues outside the periorbita. A special group consists of metastatic lesions. Aim: Retrospective observational analysis of a cohort of patients diagnosed for a primary tumor or infiltration of the orbit at the Department of Otorhinolaryngology and Head and Neck Surgery at St. Anne‘s Hospital in Brno (KOCHHK) between 2000 and 2023. Methods: Patients aged ≥ 18 years who underwent otorhinolaryngological and ophthalmological examinations, imaging (CT/MRI), and treatment were included. Demographic parameters, symptomatology, diagnostic and therapeutic approaches, histological characteristics of the processes, and treatment effect were evaluated. Results: In the study group, the most common benign process was a pseudotumour of the orbit (12 patients, 33%), of which 11 patients underwent corticosteroid therapy. Complete regression was observed in 7 patients, partial regression was in 1 patient, and recurrence was in 4 patients. Lymphoma was the most common malignant infiltration (10 patients, 27%), with 6 patients having MALT-lymphoma. Nine patients had complete remission and 1 patient had partial remission. Conclusion: Diagnosis and therapy of primary lesions of the orbit inevitably require interdisciplinary cooperation of the ophthalmologist, neurosurgeon, otorhinolaryngologist, radiologist, histopathologist, hematooncologist, and others. For diagnosis and adequate treatment, imaging examination is crucial, usually supplemented by histological verification. Its performance, and if necessary, surgical removal of the lesion is limited by localization. Prognosis depends not only on the malignant potential of the lesion, but also significantly on its relationship to the surrounding area and to important structures in the orbit.

Membrane contact sites harbor a distinct set of proteins with varying biological functions, thereby emerging as hubs for localized signaling nanodomains underlying adequate cell function. Here, we will focus on mitochondria-associated endoplasmic reticulum membranes (MAMs), which serve as hotspots for Ca2+ signaling, redox regulation, lipid exchange, mitochondrial quality and unfolded protein response pathway. A network of MAM-resident proteins contributes to the structural integrity and adequate function of MAMs. Beyond endoplasmic reticulum (ER)-mitochondrial tethering proteins, MAMs contain several multi-protein complexes that mediate the transfer of or are influenced by Ca2+, reactive oxygen species and lipids. Particularly, IP3 receptors, intracellular Ca2+-release channels, and Sigma-1 receptors (S1Rs), ligand-operated chaperones, serve as important platforms that recruit different accessory proteins and intersect with these local signaling processes. Furthermore, many of these proteins are directly implicated in pathophysiological conditions, where their dysregulation or mutation is not only causing diseases such as cancer and neurodegeneration, but also rare genetic diseases, for example familial Parkinson's disease (PINK1, Parkin, DJ-1), familial Amyotrophic lateral sclerosis (TDP43), Wolfram syndrome1/2 (WFS1 and CISD2), Harel-Yoon syndrome (ATAD3A). In this review, we will discuss the current state-of-the-art regarding the molecular components, protein platforms and signaling networks underlying MAM integrity and function in cell function and how their dysregulation impacts MAMs, thereby driving pathogenesis and/or impacting disease burden. We will highlight how these insights can generate novel, potentially therapeutically relevant, strategies to tackle disease outcomes by improving the integrity of MAMs and the signaling processes occurring at these membrane contact sites.

Couples often resemble each other in characteristics like depression, but the reasons for this homogamy (i.e., similarity) remain unclear. We investigated two potential mechanisms: preference for a self-similar partner and convergence (i.e., increasing similarity) over time. In a nationally representative sample of 2,793 Czech individuals who we surveyed three times in one year, we examined self-reports of participants', their ideal partners', and their actual partners' "pessimism and depressiveness". Participants preferred partners less depressive than themselves, yet their actual partners were more depressive than desired. Those who ended their relationships showed a greater ideal-versus-actual partner discrepancy than those who stayed together. In stable relationships, individuals adjusted their ideal preferences to align more closely with their actual partners over time. We identified four relationship classes with latent class growth modeling based on self and partner evaluations: both non-depressive, both depressive, self depressive and partner non-depressive, and self non-depressive and partner depressive. Romantic relationships were most stable when both partners were non-depressive and most likely to dissolve when both were depressive. While we failed to detect convergence overall, we found it within heterogamous (i.e., dissimilar) classes. Overall, our findings suggest that homogamy and heterogamy in depressiveness are complexly associated with relationship maintenance.

Since cell dying in heart failure (HF) may vary based on the aetiology, we examined the main forms of regulated necrosis, such as necroptosis and pyroptosis, in the hearts damaged due to myocardial infarction (MI) or pressure overload. We also investigated the effects of a drug inhibiting RIP3, a proposed convergent point for both these necrosis-like cell death modes. In rat hearts, left ventricular function, remodelling, pro-cell death, and pro-inflammatory events were investigated, and the pharmacodynamic action of RIP3 inhibitor (GSK'872) was assessed. Regardless of the HF aetiology, the heart cells were dying due to necroptosis, albeit the upstream signals may be different. Pyroptosis was observed only in post-MI HF. The dysregulated miRNAs in post-MI hearts were accompanied by higher levels of a predicted target, HMGB1, its receptors (TLRs), as well as the exacerbation of inflammation likely originating from macrophages. The RIP3 inhibitor suppressed necroptosis, unlike pyroptosis, normalised the dysregulated miRNAs and tended to decrease collagen content and affect macrophage infiltration without affecting cardiac function or structure. The drug also mitigated the local heart inflammation and normalised the higher circulating HMGB1 in rats with post-MI HF. Elevated serum levels of HMGB1 were also detected in HF patients and positively correlated with C-reactive protein, highlighting pro-inflammatory axis. In conclusion, in MI-, but not pressure overload-induced HF, both necroptosis and pyroptosis operate and might underlie HF pathogenesis. The RIP3-targeting pharmacological intervention might protect the heart by preventing pro-death and pro-inflammatory mechanisms, however, additional strategies targeting multiple pro-death pathways may exhibit greater cardioprotection.

Lower risk (LR) myelodysplastic syndromes (MDS) are heterogeneous hematopoietic stem and progenitor disorders caused by the accumulation of somatic mutations in various genes including epigenetic regulators that may produce convergent DNA methylation patterns driving specific gene expression profiles. The integration of genomic, epigenomic, and transcriptomic profiling has the potential to spotlight distinct LR-MDS categories on the basis of pathophysiological mechanisms. We performed a comprehensive study of somatic mutations and DNA methylation in a large and clinically well-annotated cohort of treatment-naive patients with LR-MDS at diagnosis from the EUMDS registry (ClinicalTrials.gov.NCT00600860). Unsupervised clustering analyses identified six clusters based on genetic profiling that concentrate into four clusters on the basis of genome-wide methylation profiling with significant overlap between the two clustering modes. The four methylation clusters showed distinct clinical and genetic features and distinct methylation landscape. All clusters shared hypermethylated enhancers enriched in binding motifs for ETS and bZIP (C/EBP) transcription factor families, involved in the regulation of myeloid cell differentiation. By contrast, one cluster gathering patients with early leukemic evolution exhibited a specific pattern of hypermethylated promoters and, distinctly from other clusters, the upregulation of AP-1 complex members FOS/FOSL2 together with the absence of hypermethylation of their binding motif at target gene enhancers, which is of relevance for leukemic initiation. Among MDS patients with lower-risk IPSS-M, this cluster displayed a significantly inferior overall survival (p < 0.0001). Our study showed that genetic and DNA methylation features of LR-MDS at early stages may refine risk stratification, therefore offering the frame for a precocious therapeutic intervention.

Bipolar disorder is a leading contributor to the global burden of disease1. Despite high heritability (60-80%), the majority of the underlying genetic determinants remain unknown2. We analysed data from participants of European, East Asian, African American and Latino ancestries (n = 158,036 cases with bipolar disorder, 2.8 million controls), combining clinical, community and self-reported samples. We identified 298 genome-wide significant loci in the multi-ancestry meta-analysis, a fourfold increase over previous findings3, and identified an ancestry-specific association in the East Asian cohort. Integrating results from fine-mapping and other variant-to-gene mapping approaches identified 36 credible genes in the aetiology of bipolar disorder. Genes prioritized through fine-mapping were enriched for ultra-rare damaging missense and protein-truncating variations in cases with bipolar disorder4, highlighting convergence of common and rare variant signals. We report differences in the genetic architecture of bipolar disorder depending on the source of patient ascertainment and on bipolar disorder subtype (type I or type II). Several analyses implicate specific cell types in the pathophysiology of bipolar disorder, including GABAergic interneurons and medium spiny neurons. Together, these analyses provide additional insights into the genetic architecture and biological underpinnings of bipolar disorder.

OBJECTIVES: Research questions about how and why health trends differ between populations require decisions about data analytic procedure. The objective was to document and compare the information returned from stratified, fixed effect and random effect approaches to data modelling for two prototypical descriptive research questions about comparative trends in toothbrushing. METHODS: Data included five cycles of the Health Behaviour in School-aged Children 2006 to 2022, which provided a sample of 980192 11- to 15- year olds from 35 countries. Using logistic regression models and generalized linear mixed models, toothbrushing daily was regressed on time, following the three approaches to analysis of trends. RESULTS: The stratified approach suggested a positive but non-linear trend in toothbrushing from 2006 to 2022 in most countries but provided no statistical inference on the variation. The fixed effect and the random effect approach converged on a positive but flattening overall trend, with a statistically significant country variation in trends. CONCLUSION: Only the fixed effect approach and the random effects approach provided clear answers to the research question. Additional methodological considerations for making an informed choice of analytical approach are discussed.
