Účinnost stimulace různých spoušťových extraspinálních zón na blokády v segmentech C2-C3 a C3-C4 není neměnnou vlastností. Maximální nenucená rotace hlavy a krční páteře napravo tuto účinnost potlačuje, maximální nenucený nebo nucený úklon tuto účinnost nastoluje. Mimoto existuje složitý systém tlumivých a povzbuzujících vlivů stimulace různých zón na účinnost následné stimulace jiných homo- i heterolaterálních zón. Toto předběžné sdělení má za úkol upozornit na existenci těchto jevů.
The effect of mechanical stimulation of various extraspinal trigger zones in abolition of blockages at C2-C3 and C3-C4 segments is not a permanent property, but it can be suppressed by means of maximum unforced rotations of the head and neck to the right side, while maximum unforced or forced inclination to the right is capable to establish such efficacy in hitherto ineffective zones. Moreover, there exists a complicated system of facilitatory and inhibitory impulses generated by previous stimulation of another zone, on the same or on the opposite side of the body. This preliminary report is intended to draw attention to the existence of these phenomena.
Cíl: Mechanická trombektomie (MT) symptomatického uzávěru mozkové tepny u pacientů s akutní ischemickou cévní mozkovou příhodou (iCMP) se stala standardní léčbou. Cílem studie METRICS (MechanicalThrombectomy Quality Indicators Study in Czech Stroke Centers) bylo ověřit, zda centra v České republice provádějící endovaskulámí léčbu iCMP plní indikátory kvality navržené multioborovým konsenzem, a tak v mezinárodním kontextu ověřit kvalitu péče napříč naší republikou. Metodika: Národní prospektivní observační studie METRICS probíhala v České republice v roce 2019. Studie se zúčastnilo 12 z 15 center provádějících mechanickou trombektomii. Všechna technická a klinická data byla sbírána prospektivně a následně byla srovnána s doporučenými hodnotamijednotlivých parametrů multioborového konsenzu. Výsledky: V práci jsou analyzována data z 12 center, ve kterých bylo mechanickou trombektomii ošetřeno celkem 1178 pacientů (50,3% mužů) s mediánem věku 72 roky (18-98 let). Intravenóznitrombolýzou (IVT) bylo před MT léčeno 827 nemocných (70,2%) a 445 (37,8%) bylo sekundárně transponováno k MT z jiného centra. Získaná data z národní prospektivní observační studie METRICS byla hodnocena pro jednotlivá centra. Shoda výsledků péče s doporučeními navrženými multioborovým konsenzem byla velmi vysoká. Pět center splnilo parametry všech doporučení. Ostatní centra nesplnila pouze ojedinělá kritéria a ve většině případů jen hraničně. Závěr: Výsledky studie METRICS ukazují, že lze splnit doporučované hodnoty parametrů uvedených v mezinárodním multioborovém konsenzu i v podmínkách běžné klinické praxe napříč Českou republikou.
Aim: Mechanical thrombectomy (MT) of symptomatic occlusion of cerebral artery has become a standard treatment in acute ischemic stroke (IS) patients. The aim of the presented study was to evaluate if centers performing endovascular treatment of acute ischemic stroke meet the recommendations established by the international multi-society consensus. Methods: The national prospective observational multicenter study METRICS (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers) was conducted in Czech Republic in year 2019. Twelve of 15 centers performing mechanicaltrombectomy were participating on study. Alltechnicaland clinical data were collected prospectively and achieved results were subsequently compared with the recommendations established by international multi-society consensus. Results: In 12 centers 1178 (86%) patients (50.3% males, median of age 72 years, range 18-98 years) by MT were treated in 2019 and the collected data of these patients were analyzed. IV thrombolysis prior MT was performed in 827 (70.2%) patients and 445 (37.8%) patients were transferred secondarily for MT from primary stroke centers to dedicated comprehensive centers. Achieved data from the study METRICS were subsequently evaluated for individual participating centers. The concordance of results with the recommendations of multi-society consensus was very high. Five centers met parameters of all recommendations. Remaining centers did not meet sporadic criteria only and most parameters had borderline values. Conclusion: The results of METRICS study showed that it is possible meet the recommended values of parameters stated in the international multi-society consensus even in a real world practice across the Czech Republic.

Background and Objectives Automatic detection of breathing disorders plays an important role in the early signalization of respiratory diseases. Measuring methods can be based on electrocardiogram (ECG), sound, oximetry, or respiratory analysis. However, these approaches require devices placed on the human body or they are prone to disturbance by environmental influences. To solve these problems, we proposed a heart contraction mechanical trigger for unobtrusive detection of respiratory disorders from the mechanical measurement of cardiac contractions. We designed a novel method to calculate this mechanical trigger purely from measured mechanical signals without the use of ECG. Methods The approach is a built-on calculation of the so-called euclidean arc length from the signals. In comparison to previous researches, this system does not require any equipment attached to a person. This is achieved by locating the tensometers on the bed. Data from sensors are fused by the Cartan curvatures method to beat-to-beat vector input for the Convolutional neural network (CNN) classifier. Results In sum, 2281 disordered and 5130 normal breathing samples was collected for analysis. The experiments with use of 10-fold cross validation show that accuracy, sensitivity, and specificity reach values of 96.37%, 92.46%, and 98.11% respectively. Conclusions By the approach for detection, the system offers a novel way for a completely unobtrusive diagnosis of breathing-related health problems. The proposed solution can effectively be deployed in all clinical or home environments.

PURPOSE: Cancer initialization can be explained as a result of parasitic virus energy consumption leading to randomized genome chemical bonding. MATERIALS AND METHODS: Analysis of experimental data on cell-mediated immunity (CMI) containing about 12,000 cases of healthy humans, cancer patients and patients with precancerous cervical lesions disclosed that the specific cancer and the non-specific lactate dehydrogenase-elevating (LDH) virus antigen elicit similar responses. The specific antigen is effective only in cancer type of its origin but the non-specific antigen in all examined cancers. CMI results of CIN patients display both healthy and cancer state. The ribonucleic acid (RNA) of the LDH virus parasitizing on energy reduces the ratio of coherent/random oscillations. Decreased effect of coherent cellular electromagnetic field on bonding electrons in biological macromolecules leads to elevating probability of random genome reactions. RESULTS: Overlapping of wave functions in biological macromolecules depends on energy of the cellular electromagnetic field which supplies energy to bonding electrons for selective chemical bonds. CMI responses of cancer and LDH virus antigens in all examined healthy, precancerous and cancer cases point to energy mechanism in cancer initiation. CONCLUSIONS: Dependence of the rate of biochemical reactions on biological electromagnetic field explains yet unknown mechanism of genome mutation.

N6-Methyladenosine (m6A) is the predominant internal RNA modification in eukaryotic messenger RNAs (mRNAs) and plays a crucial role in mRNA stability. Here, using human cells, we reveal that m6A sites in the coding sequence (CDS) trigger CDS-m6A decay (CMD), a pathway that is distinct from previously reported m6A-dependent degradation mechanisms. Importantly, CDS m6A sites act considerably faster and more efficiently than those in the 3' untranslated region, which to date have been considered the main effectors. Mechanistically, CMD depends on translation, whereby m6A deposition in the CDS triggers ribosome pausing and transcript destabilization. The subsequent decay involves the translocation of the CMD target transcripts to processing bodies (P-bodies) and recruitment of the m6A reader protein YT521-B homology domain family protein 2 (YTHDF2). Our findings highlight CMD as a previously unknown pathway, which is particularly important for controlling the expression of developmental regulators and retrogenes.

We show the influence of osteoblastic SAOS-2 cells on the transfer characteristics of nanocrystalline diamond solution-gated field-effect transistors (SGFET) prepared on glass substrates. Channels of these fully transparent SGFETs are realized by hydrogen termination of undoped diamond film. After cell cultivation, the transistors exhibit about 100× increased leakage currents (up to 10nA). During and after the cell delamination, the transistors return to original gate currents. We propose a mechanism where this triggering effect is attributed to ions released from adhered cells, which depends on the cell adhesion morphology, and could be used for cell culture monitoring.

BACKGROUND AND PURPOSE: Rigorous and regular evaluation of defined quality indicators is crucial for further improvement of both technical and clinical results after mechanical thrombectomy (MT) for acute ischemic stroke (AIS). Following the recent international multi-society consensus quality indicators, we aimed to assess trend in these indicators on national level. MATERIAL AND METHODS: The prospective multicenter study (METRICS) was conducted in Czech Republic (CR) in year 2019. All participating centers collected technical and clinical data including defined quality indicators and results were subsequently compared with those from year 2016. RESULTS: In the 2019, 1375 MT were performed in the CR and 1178 (86%) patients (50.3% males, mean age 70.5 ± 13.0 years) were analyzed. Recanalization (TICI 2b-3) was achieved in 83.7% of patients and 46.2% of patients had good 3-month clinical outcome. Following time intervals were shortened in comparison to 2016: "hospital arrival - GP" (77 vs. 53 min; p<0.0001), "hospital arrival - maximal achieved recanalization" (122 vs. 93 min; p<0.0001), and "stroke onset - maximal achieved recanalization" (240 vs. 229 min; p p<0.0001). More patients with tandem occlusion were treated in 2019 (7.8 vs. 16.5%; p<0.0001) and more secondary transports were in 2019 (31.3 vs. 37.8%; p=0.002). No difference was found in 3-month clinical outcome and in the rate of periprocedural complications. Results of the METRICS study met all criteria of multi-society consensus quality indicators. CONCLUSION: Nationwide comparison between 2016 and 2019 showed improvement in the key time intervals, but without better overall clinical outcomes after MT.

BACKGROUND & AIMS: Interleukin (IL)-6 cytokine family members contribute to inflammatory and regenerative processes. Engagement of the signaling receptor subunit gp130 is common to almost all members of the family. In the liver, all major cell types respond to IL-6-type cytokines, making it difficult to delineate cell type-specific effects. We therefore generated mouse models for liver cell type-specific analysis of IL-6 signaling. METHODS: We produced mice with a Cre-inducible expression cassette encoding a designed pre-dimerized constitutive active gp130 variant. We bred these mice to different Cre-drivers to induce transgenic gp130 signaling in distinct liver cell types: hepatic stellate cells, cholangiocytes/liver progenitor cells or hepatocytes. We phenotyped these mice using multi-omics approaches, immunophenotyping and a bacterial infection model. RESULTS: Hepatocyte-specific gp130 activation led to the upregulation of innate immune system components, including acute-phase proteins. Consequently, we observed peripheral mobilization and recruitment of myeloid cells to the liver. Hepatic myeloid cells, including liver-resident Kupffer cells were instructed to adopt a bactericidal phenotype which ultimately conferred enhanced resistance to bacterial infection in these mice. We demonstrate that persistent hepatocyte-specific gp130 activation resulted in amyloid A amyloidosis in aged mice. In contrast, we did not observe overt effects of hepatic stellate cell- or cholangiocyte/liver progenitor cell-specific transgenic gp130 signaling. CONCLUSIONS: Hepatocyte-specific gp130 activation alone is sufficient to trigger a robust innate immune response in the absence of NF-κB activation. We therefore conclude that gp130 engagement, e.g. by IL-6 trans-signaling, represents a safe-guard mechanism in innate immunity. LAY SUMMARY: Members of the interleukin-6 cytokine family signal via the receptor subunit gp130 and are involved in multiple processes in the liver. However, as several liver cell types respond to interleukin-6 family cytokines, it is difficult to delineate cell type-specific effects. Using a novel mouse model, we provide evidence that hepatocyte-specific gp130 activation is sufficient to trigger a robust systemic innate immune response.
