Chemoterapie (CHT) je nepostradatelnou součástí léčby pacientů s nemalobuněčným plicním karcinomem (nonsmall-cell lung cancer, NSCLC). Cílem naší studie bylo nalézt prediktory efektu této léčebné modality. Stanovili jsme hladiny mRNA genů účastnících se na opravách poškozené DNA a určili vztah hladin mRNA k bezpříznakovému období (disease-free interval, DFI) a celkovému přežití (overall survival, OS) u pacientů, kteří podstoupili resekci nádorové tkáně a následnou adjuvantní CHT. Konkrétně se jednalo o expresi genů ERCC1 (excision repair cross-complementary group 1), RRM1 (ribonucleotide reductase subunit M1) a BRCA1 (breast cancer 1) přímo v nádorové tkáni. Zajímalo nás, zda potenciální reziduální nádorové buňky po chirurgické resekci odrážejí vlastnosti primárního tumoru, na což bychom usuzovali dle léčebné odpovědi. Stanovení exprese genů jsme provedli u 90 pacientů s NSCLC kvantitativně metodou real-time RT PCR, v nádorové i normální plicní tkáni. Vztah k DFI a OS byl analyzován u souboru 59 pacientů. Prokázali jsme signifikantně nižší expresi mRNA ERCC1 a RRM1 u tkáně NSCLC ve srovnání s normální plicní tkání. Vyšší exprese ERCC1, RRM1 i BRCA1 pak byla zaznamenána u adenokarcinomu (ACC) ve srovnání se squamózním karcinomem (SCC). Rozdíly v DFI a OS byly nalezeny pouze ve specifických podskupinách pacientů podle typu nádoru a jeho stadia. Celkové přežití pacientů bylo u skupiny ACC ve vztahu k hladině mRNA RRM1, u skupiny SCC pak ve vztahu k mRNA BRCA1. DFI u stadia III pak bylo delší u pacientů s vyššími hladinami RRM1 a ERCC1. Zjistili jsme, že stanovení exprese genů ERCC1, RRM1 a BRCA1 má prognostický význam, ale nepredikuje efekt chemoterapie. U našeho souboru pacientů léčených adjuvantní CHT byla vyšší exprese opravných genů spojena s lepší prognózou.
Chemotherapy represents an integral modality in the treatment of non-small cell lung cancer (NSCLC). However, adjuvant chemotherapy in patients with resected tumors is still not optimally effective. Therefore, markers that would increase its benefit for individual patients are searched. The study dealt with the predictive and prognostic effects of mRNA levels of selected DNA repair genes (ERCCl, RRMl and BRCAl) in adjuvant chemotherapy. Also investigated were differences between normal lung tissue and cancer tissue in NSCLC. A significant relationship was found. Furthermore, there was a significant association between mRNA expression of the selected repair genes and patients' prognosis; higher expression of the repaiir genes was cormected with better prognosis. Nevertheless, no predictive relationship to adjuvant chemotherapy was manifested.

Cíl Hodnotili jsme účinnost gemcitabinu ve srovnání s kombinací gemcitabin plus carboplatina u pacientů s pokročilým nemalobuněčným karcinomem plic (non–small-cell lung cancer, NSCLC) a performance status (stav výkonnosti, PS) 2 a zkoumali jsme, zda úroveň exprese proteinů RRM1 a ERCC1 v nádoru předpovídají odpověď na léčbu. Pacienti a metody Byla provedena randomizovaná studie fáze III v komunitních onkologických zařízeních. Vzorky nádorů byly odebrány předem a odeslány do jediné laboratoře k slepě provedenému stanovení úrovní exprese proteinů RRM1 a ERCC1 in situ automatizovanou kvantitativní imunofluorescenční technikou. Výsledky Náhodným způsobem bylo zařazeno sto sedmdesát pacientů. Celkový medián přežití byl 5,1 měsíce pro gemcitabin a 6,7 měsíce pro kombinaci gemcitabin plus carboplatina (p = 0,24). Hodnoty RRM1 (rozpětí 5,3–105,6; medián 34,1) a ERCC1 (rozpětí 5,2–131,3; medián, 34,7) byly ve vzájemném vztahu významné a inverzní korelace s odpovědí onemocnění (r = - 0,41; p = 0,001 pro RRM1; r = - 0,39; p = 0,003 pro ERCC1; to znamená, že odpovědi byly lepší u pacientů s nízkou úrovní exprese). Model predikce odpovědi, který zahrnoval RRM1, ERCC1 a léčebné rameno, předpovídal léčebnou odpověď s vysokou pravděpodobností (p = 0,0005). Nepozorovali jsme statisticky významné spojení mezi přežitím a úrovněmi exprese RRM1 nebo RCC1. Závěr Standardem léčby pacientů s pokročilým NSCLC a špatným PS zůstává chemoterapie jedním lékem. Kvantitativní analýza exprese proteinů RRM1 a ERCC1 v běžně odebíraných vzorcích z komunitních onkologických zařízení předpovídá odpověď na terapii gemcitabinem a kombinací gemcitabin plus carboplatina. Onkologové by měli zvážit možnost zařadit do svého rozhodování o léčbě analýzu exprese těchto proteinů in situ.
We evaluated the efficacy of gemcitabine versus gemcitabine and carboplatin in patients with advanced non-small-cell lung cancer (NSCLC) and a performance status (PS) of 2 and assessed if tumoral RRM1 and ERCC1 protein levels are predictive of response to therapy. PATIENTS AND METHODS: A randomized phase III trial was conducted in community-based oncology practices. Tumor specimens were collected a priori and shipped to a single laboratory for blinded determination of in situ RRM1 and ERCC1 protein expression levels by an automated quantitative immunofluorescent-based technology. RESULTS: One hundred seventy patients were randomly assigned. Overall median survival was 5.1 months for gemcitabine and 6.7 months for gemcitabine and carboplatin (P = .24). RRM1 (range, 5.3 to 105.6; median, 34.1) and ERCC1 (range, 5.2 to 131.3; median, 34.7) values were significantly and inversely correlated with disease response (r = -0.41; P = .001 for RRM1; r = -0.39; P = .003 for ERCC1; ie, response was better for patients with low levels of expression). A model for response prediction that included RRM1, ERCC1, and treatment arm, was highly predictive of the treatment response observed (P = .0005). We did not find statistically significant associations between survival and RRM1 or ERCC1 levels. CONCLUSION: Single-agent chemotherapy remains the standard of care for patients with advanced NSCLC and poor PS. Quantitative analysis of RRM1 and ERCC1 protein expression in routinely collected tumor specimens in community oncology practices is predictive of response to gemcitabine and gemcitabine and carboplatin therapy. Oncologists should consider including in situ expression analysis for these proteins into their therapeutic decisions.

UNLABELLED: Chemotherapy is an important modality of treatment for non-small cell lung cancer (NSCLC). Recent studies have shown that assessment of predictive molecular markers could be helpful for estimation of the response rate to chemotherapy. The aim of our study was to assess the relation of mRNA levels of DNA repair genes excision repair cross-complementary group 1 (ERCC1), ribonucleotide reductase subunit M1 (RRM1) and breast cancer 1 (BRCA1), in surgically-resected tumor tissues from patients who underwent adjuvant chemotherapy, to the disease-free interval (DFI) and overall survival (OS). We investigated if potential residual tumor cells after resection reflect properties of the primary tumor and response to chemotherapy according to the level of predictive markers with respect to current knowledge. PATIENTS AND METHODS: We studied a group of 90 patients with NSCLC who had undergone curative lung resection; 59 of them were subsequently treated with adjuvant chemotherapy, DFI and OS were evaluated only in this subgroup. Quantitative estimation of mRNA of selected genes in paired (tumor and control)-lung tissue samples was performed by real-time reverse transcription-polymerase chain reaction (RT-PCR). RESULTS: We found a significantly lower mRNA expression of ERCC1 (p<0.001) and RRM1 (p=0.023) in NSCLC tumor tissues compared to normal lung tissues. Comparing expression in histological subtypes, we recorded higher mRNA expression of ERCC1 (p=0.021), RRM1 (p=0.011) and BRCA1 (p=0.011) in adenocarcinoma than in squamous cell carcinoma (SCC). Differences in DFI and OS were found only in specific subgroups according to tumor type and stage. We found longer OS for patients with adenocarcinoma with higher expression of the RRM1 mRNA (p=0.002), and for patients with SCC with higher expression of the BRCA1 mRNA (p=0.041). In patients with NSCLC of stage III, we found longer DFI in those with higher expression of RRM1 (p=0.004) and ERCC1 (p=0.038). CONCLUSION: Patients who had been treated with adjuvant chemotherapy and had shown lower expression of repair genes had adverse prognosis. We observed that the assessment of DNA repair gene level in primary tumors treated by surgical resection had prognostic significance and did not predict response to adjuvant chemotherapy.

The human prototypical SR protein SRSF1 is an oncoprotein that contains two RRMs and plays a pivotal role in RNA metabolism. We determined the structure of the RRM1 bound to RNA and found that the domain binds preferentially to a CN motif (N is for any nucleotide). Based on this solution structure, we engineered a protein containing a single glutamate to asparagine mutation (E87N), which gains the ability to bind to uridines and thereby activates SMN exon7 inclusion, a strategy that is used to cure spinal muscular atrophy. Finally, we revealed that the flexible inter-RRM linker of SRSF1 allows RRM1 to bind RNA on both sides of RRM2 binding site. Besides revealing an unexpected bimodal mode of interaction of SRSF1 with RNA, which will be of interest to design new therapeutic strategies, this study brings a new perspective on the mode of action of SRSF1 in cells.

Human antigen R (HuR) is a key regulator of cellular mRNAs containing adenylate/uridylate-rich elements (AU-rich elements; AREs). These are a major class of cis elements within 3' untranslated regions, targeting these mRNAs for rapid degradation. HuR contains three RNA recognition motifs (RRMs): a tandem RRM1 and 2, followed by a flexible linker and a C-terminal RRM3. While RRM1 and 2 are structurally characterized, little is known about RRM3. Here we present a 1.9-Å-resolution crystal structure of RRM3 bound to different ARE motifs. This structure together with biophysical methods and cell-culture assays revealed the mechanism of RRM3 ARE recognition and dimerization. While multiple RNA motifs can be bound, recognition of the canonical AUUUA pentameric motif is possible by binding to two registers. Additionally, RRM3 forms homodimers to increase its RNA binding affinity. Finally, although HuR stabilizes ARE-containing RNAs, we found that RRM3 counteracts this effect, as shown in a cell-based ARE reporter assay and by qPCR with native HuR mRNA targets containing multiple AUUUA motifs, possibly by competing with RRM12.

MicroRNAs have the potential to become valuable predictive markers for gastric cancer. Samples of biopsy tissue, routinely taken from gastric cancer patients undergoing palliative chemotherapy, constitute suitable material for microRNA profiling with the aim of predicting the effect of chemotherapy. Our study group consisted of 54 patients, all of whom underwent palliative chemotherapy based on 5-fluorouracil (5-FU) or 5-FU in combination with platinum derivatives between 2000 and 2013. The expression of 29 selected microRNAs and genes BRCA1, ERCC1, RRM1 and TS, in gastric cancer tissue macrodissected from FFPE tissue samples, was measured by quantitative RT-PCR. The relationship between gene expression levels and time to progression (TTP) and overall survival (OS) was analysed. From the set of the 29 microRNAs of interest, we found high expression of miR-150, miR-342-3p, miR-181b, miR-221, miR-224 and low levels of miR-520h relate to shorter TTP. High levels of miR-150, miR-192, miR-224, miR-375 and miR-342-3p related to shorter OS. In routinely available FFPE tissue samples, we found 6 miRNAs with a relation to TTP, which may serve as predictors of the effectiveness of palliative treatment in gastric cancer patients. These miRNAs could also help in deciding whether to indicate palliative chemotherapy.
