Remote detection of hydrogen, without the utilization of electronic component or elevated temperature, is one of the hot topics in the hydrogen technology and safety. In this work, the design and realization of the optical fiber-based hydrogen sensor with unique characteristics are proposed. The proposed sensor is based on the gold-coated multimode fiber, providing the plasmon properties, decorated by the IRMOF-20 layer with high selectivity and affinity toward hydrogen. The IRMOF-20 layer was grown by a surface-assisted technique, and its formation and properties were studied using X-ray photoelectron spectroscopy, Raman, X-ray diffraction, and Brunauer-Emmett-Teller techniques. Simultaneous ellipsometry results indicate the apparent changes of the refractive index of the IRMOF-20 layer due to hydrogen sorption. As results, the presence of hydrogen led to the pronounced changes of plasmon band wavelength position as well as its intensity increase. The proposed hydrogen sensors were favorably distinguished by a high response/recovery rate, excellent selectivity toward the hydrogen, very low temperature dependency, functionality at room or lower temperature, insensitivity toward the humidity, and the presence of CO2, CO, or NO2. Additionally, the proposed hydrogen sensor showed good reversibility, reproducibility, and long-term stability.
OBJECTIVE: As a standard method, periodic limb movements in sleep (PLMS) are detected by means of polysomnography (PSG). Actigraphic detection of PLMS is a recently developed method for performing of multiple-night recordings in an outpatient setting. The aim of our study was to assess sensitivity-related parameters of actigraphic detection of PLMS in comparison to the PSG, when mounted at ankles and at the base of the big toe. METHODS: We simultaneously performed PSG and actigraphic recordings at both ankles and at the bases of both big toes (Cambridge Actiwatch AW-64) for 40 nights. The PLM index (PLMI), number of periodic movements per hour of sleep, was the primary output of both methods. RESULTS: We have proven significant correlation of all resulting parameters when comparing actigraphy at either position to the PSG; however, the PLMI values obtained by the actigraphy at toes were significantly higher than by PSG (sign test, p<0.0001). At bases of the big toes, threshold of PLMI=7.6 was used as a cut off for positivity, while PLMI=5 at ankles was used for PSG. Comparing ankle versus toe actigraphic placement, sensitivity was 67% versus 94%, specificity was 95% versus 91% and negative predictive value was 78% versus 95%. The correlation of the results from actigraphy and PSG was not affected by presence of respiratory events. DISCUSSION: Our results suggest good validity of actigraphic PLM evaluation at the base of big toe using AW-64 devices and therefore, actigraphy seems suitable for screening purposes in both clinical and research usage.

Souhrn Hodnocení potřeb terminálně nemocných v domácí péči je důležitým ukazatelem kvality poskytované péče. Cílem výzkumu bylo zjistit bio-psycho-sociální a spirituální potřeby terminálně nemocných v domácí péči v souvislosti s kvalitou života. Dále pak zjistit míru důležitosti potřeb a míru jejich saturace zdravotnickými pracovníky. Ke sběru dat byly použity dotazníky: pro hodnocení potřeb – PNPC (Problems and Needs in Paliative Care questionnaire), kvality života – EORTC QOL – C30, a psychického stavu – škála HADS (Hospital Anxiety and Depressed Scale). Výzkumu se zúčastnilo 37 terminálně nemocných pacientů z 6-ti agentur domácí péče v Ostravě, kteří měli Karnofský index <50. Pacienti uvedli jako důležité a současně nedostatečně saturované tyto potřeby: únava, problémy se spánkem, bolest, schopnost soustředit se, vyrovnat se s nemocí a se změnami v těle, mít naplněný den aktivitou. Statisticky významný rozdíl v hodnocení důležitosti potřeb byl zjištěn pouze v oblasti potřeb informací, kterou považovali za důležitější věřící pacienti (p = 0,0219). Souvislost mezi saturací potřeb a kvalitou života byla zjištěna pouze v oblasti fyzických potřeb (p<0,05). Pacienti, kteří považovali fyzické potřeby za nedostatečně saturované, současně uvedli nižší celkovou kvalitu života.
Evaluation of the needs of terminally ill patients in home care is an important indicator of the quality of care that is being provided. The aim of the research was to determine the bio-psycho-social and spiritual needs of terminally ill patients in home care with regards to their quality of life. Furthermore to determine the degree of importance of needs and level of their saturation by health workers. Questionnaires were used for data collection: – for needs assessment – PNPC (Problems and Needs in Paliative Care questionnaire), – for quality of life – EORTC QOL – C30, and – psychic state – HADS scale – (Hospital anxiety and Depressed Scale). A total of 37 terminally ill patients with Karnofsky index <50 from 6 home care agencies in Ostrava participated in the research. Patients reported the following needs to be important and also insufficiently addressed: – fatigue, – sleeping problems, – pain, – ability to concentrate, – to cope with the disease and with changes in the body, – to have an activity-filled day. A statistically significant difference in the assessment of importance of needs was only detected in the area of information needs, which was considered as more important in patients with religious faith (p = 0.0219). The relationship between the saturation of needs and quality of life was only detected in the physical needs (p<0.05). Patients who felt that physical needs are insufficiently saturated simultaneously reported a lower overall quality of life.
