This study looked for correlations between molecular identification, clinical manifestation, and morphology for Trichophyton interdigitale and Trichophyton mentagrophytes. For this purpose, a total of 110 isolates were obtained from Czech patients with various clinical manifestations of dermatophytosis. Phenotypic characters were analyzed, and the strains were characterized using multilocus sequence typing. Among the 12 measured/scored phenotypic features, statistically significant differences were found only in growth rates at 37 °C and in the production of spiral hyphae, but none of these features is diagnostic. Correlations were found between T. interdigitale and higher age of patients and between clinical manifestations such as tinea pedis or onychomychosis. The MLST approach showed that internal transcribed spacer (ITS) genotyping of T. mentagrophytes isolates has limited practical benefits because of extensive gene flow between sublineages. Based on our results and previous studies, there are few taxonomic arguments for preserving both species names. The species show a lack of monophyly and unique morphology. On the other hand, some genotypes are associated with predominant clinical manifestations and sources of infections, which keep those names alive. This practice is questionable because the use of both names confuses identification, leading to difficulty in comparing epidemiological studies. The current identification method using ITS genotyping is ambiguous for some isolates and is not user-friendly. Additionally, identification tools such as matrix-assisted laser desorption/ionization time-of-flight mass spectrometry fail to distinguish these species. To avoid further confusion and to simplify identification in practice, we recommend using the name T. mentagrophytes for the entire complex. When clear differentiation of populations corresponding to T. interdigitale and Trichophyton indotineae is possible based on molecular data, we recommend optionally using a variety rank: T. mentagrophytes var. interdigitale and T. mentagrophytes var. indotineae.

Toenail onychomycosis caused by dermatophytes is a significant medical and financial worldwide burden. Relatively scant research has been undertaken as to the predominant species and strains causing this condition in Australia, which is a unique isolated continent with diverse geographical, climatic and population regions. Four regions were selected in Eastern Australia: Far North Queensland, Rural Victoria, Melbourne Metropolitan and Tasmania. From each of these areas, communal nail dust bags from podiatric physicians' work were collected and analysed. A total of 32 dust bags were collected: 10 from Far North Queensland, 8 from Melbourne Metropolitan, 8 from Rural Victoria and 6 from Tasmania. Dermatophyte test medium was used to isolate dermatophytes from the dust, and the colonies were subcultured to Potato Dextrose Agar. Of the bags collected, in total 69% were positive for dermatophytes: 40% from Far North Queensland, 75% from Melbourne Metropolitan, 88% from Rural Victoria and 83% from Tasmania. The internal transcribed spacer (ITS) region of ribosomal DNA was used to identify and compare isolates. A total of 148 dermatophyte strains were identified. The predominant species isolated was Trichophyton interdigitale (125 isolates), which was found in all four regions. This species was further subdivided into four ITS genotypes: the first two were present in all regions, but the third was found only in the Melbourne Metropolitan area and the fourth only in Tasmania. Only one strain of Trichophyton rubrum was found and only in Rural Victoria. Eighteen isolates of Arthroderma quadrifidum were cultured from Rural Victoria and Tasmania and were further classified into three ITS genotypes. Some isolates rarely reported in clinical material were identified as Paraphyton cookei, Arthroderma tuberculatum and Arthroderma crocatum. A potentially new species of Arthroderma was also found in Melbourne Metropolitan. These findings reveal a unique dermatophyte fingerprint in toenails for Eastern Australia.

Uvádíme pět kazuistik kožních mykotických infekcí typu tinea faciei a tinea corporis u čtyř mladých žen a chlapce vyvolaných zoofilním druhem Trichophyton erinacei. Všichni pacienti doma chovali ježka bělobřichého (Atelerix albiventris) a také shodně uvedli, že chovaní ježci neměli žádné viditelné klinické projevy infekce. Jedná se o první detailně zdokumentované případy v České republice. Ložisko infekce na obličeji první pacientky bylo nejprve léčeno lokálně ciklopiroxolaminem s regresí nálezu, následně byla terapie doplněna o systémově podaný terbinafin a úspěšně vyléčena. Celkem tři pacienti se dostavili s lézemi na předloktí a byli ve dvou případech úspěšně léčeni systémově podaným terbinafinem, nebo lokální aplikací ciklopiroxolaminu; údaje o zbývajícím případu jsou neúplné. Pátý pacient s lézí na boku byl nejprve léčen kombinací lokálního ciklopiroxolaminu a kyseliny fusidové a později z důvodu znovuobjevení infekce došlo k vyléčení po aplikaci krému s ekonazolem. Trichophyton erinacei je poměrně vzácným původcem dermatofytóz člověka, obvykle ve spojitosti s kontaktem pacienta s ježky. Vzhledem k morfologické podobnosti s druhem T. interdigitale je možné tyto druhy v laboratoři zaměnit, čímž unikne skutečný zdroj infekce a může docházet k jejímu dalšímu šíření. Ověření identifikace pomocí molekulárních metod, například sekvenací oblasti ITS rDNA, která byla použita v této práci, je tedy velmi žádoucí.
We report five cases of tinea faciei and tinea corporis in four young women and a boy, caused by zoophilic species Trichophyton erinacei. All patients kept hedgehogs (Atelerix albiventris) at home and they consistently stated that animals did not show any visible signs of the infection. These are the first cases in the Czech Republic described in detail. Facial lesion of the 1st patient was treated with ciclopiroxolamine at the beginning with a visible regression and subsequently, the therapy was supplemented with systemic terbinafine. In total, three patients presented with lesions on the forearm, two of them were successfully treated by systemic terbinafine and local ciclopiroxolamine, respectively; information about the remaining case are incomplete. Fifth patient was initially treated by combination of local ciclopiroxolamine and acidum fusidicum; because of the relaps of the infection, the therapy was changed to econazol cream with good clinical effect. Trichophyton erinacei is a rare cause of human dermatophytosis, usually in connection with contact of the patient with a hedgehog. Due to the morphological similarity with the T. interdigitale, these strains can be easily confused in the laboratory. This may lead to the misidentification of the infection source and its further spread. Verification of identification using molecular methods, such as the sequencing of ITS rDNA used in this work, is therefore highly desirable.

Cílem studie bylo otestovat použitelnost metody PCR v kombinaci s High Resolution Melting Analysis (HRMA) pro detekci a identifikaci dermatofytů přímo z klinických vzorků kůže a jejích adnex. Metodika by měla vést ke zrychlení a zvýšení výtěžnosti diagnostiky dermatomykóz a ke zkvalitnění péče o pacienta. Ve studii bylo analyzováno 128 klinických vzorků od pacientů s podezřením na dermatomykózu. Pro izolaci DNA dermatofytů z klinického materiálu byl použit izolační KIT ZR Fungal/Bacterial DNA MiniPrepTM. Pro detekci DNA oblastí kódujících ribozomální RNA byly využity dvě sady primerů zachycující široké spektrum dermatofytů. Pro druhovou identifikaci dermatofytů byla použita real-time PCR metoda v kombinaci s High Resolution Melting Analysis (PCR-HRMA). Dermatofytóza byla potvrzena u 42 ze 128 zkoumaných vzorků (pozitivní přímá mikroskopie, kultivace, detekce PCR-HRMA, nebo kombinace metod). Výtěžnost PCR detekce byla 74 % pro obě sady primerů, metoda HRMA umožnila ve všech případech druhovou identifikaci detekovaných dermatofytů pomocí PCR. Naproti tomu kultivace byla pozitivní pouze u 52 % pacientů s dermatofytózou. Mikroskopie ze vzorku byla pozitivní v 90 % pozitivních dermatofytóz. Při současném použití mikroskopie, kultivace a PCR-HRMA bylo úspěšně identifikováno na úroveň druhu 90 % dermatofytů. Nejčastější druhy dermatofytů (Trichophyton rubrum, T. interdigitale, T. benhamiae) byly touto metodikou spolehlivě zachyceny. PCR detekce rDNA přímo z klinického materiálu s využitím HRMA zvýšila výtěžnost diagnostiky dermatofytóz zvláště tím, že přispěla k dosažení druhové identifikace u výrazně vyššího počtu případů v porovnání s konvenčními metodami. Kombinace konvenčních a molekulárně biologických vyšetření se zdá být vhodná pro rychlou a spolehlivou diagnostiku dermatofytóz bez nutnosti výrazného navýšení nákladů.
The aim of the study was to test the utility of the PCR method in combination with High Resolution Melting Analysis (HRMA) for the detection and identification of dermatophytes directly from clinical skin and adnexa samples. The methodology should reduce the time between sampling and diagnosis, increase the diagnostic sensitivity, and overall improve patient care. In the study, 128 clinical samples from patients with suspected dermatomycosis were analyzed. To isolate fungal DNA from the clinical specimens, a KIT ZR Fungal/Bacterial DNA MiniPrepTM was used. Two sets of primers specific for a wide range of dermatophytes were used to detect ribosomal DNA regions. The real-time PCR High Resolution Melting Analysis (PCR-HRMA) method was used for dermatophyte species identification. The PCR detection success was 74% for both sets of primers, the PCR-HRMA method enabled dermatophyte species identification in all PCR positive cases. In contrast, only 52% of patients with dermatophytosis were culture positive. Microscopy from the sample was positive in 90% of patients with proven dermatophytosis. 90% of dermatophytes were successfully identified using microscopy, culture and PCR-HRMA. The most common dermatophyte species (Trichophyton rubrum, T. interdigitale, T. benhamiae) were reliably detected by this methodology. PCR detection of rDNA directly from clinical material using HRMA increased the number of species identificacions in the diagnosis of dermatophytosis. The combination of classical and molecular biologic examinations appears to be a suitable method for the rapid and reliable diagnosis of dermatophytosis.

BACKGROUND: Antifungal drug resistance in dermatophytes was first reported shortly after the turn of the millennium and has today been reported in Trichophyton and occasionally in Microsporum, but not in Epidermophyton species. Although drug resistance in dermatophytes is not routinely investigated, resistance in Trichophyton spp. is increasingly reported worldwide. The highest rates are observed in India (36% and 68% for terbinafine (MIC ≥4 mg/L) and fluconazole (MICs ≥16 mg/L), respectively), and apparently involve the spread of a unique clade related to the Trichophyton mentagrophytes/Trichophyton interdigitale complex. OBJECTIVES: The European Committee on Antimicrobial Susceptibility Testing Subcommittee on Antifungal Susceptibility Testing (EUCAST-AFST) has released a new method (E.Def 11.0) for antifungal susceptibility testing against microconidia-forming dermatophytes including tentative MIC ranges for quality control strains and tentative breakpoints against Trichophyton rubrum and T. interdigitale. Here, the details of the new procedure E.Def 11.0 are described. SOURCES: This technical note is based on the multicentre validation of the EUCAST dermatophyte antifungal susceptibility testing method, the mould testing method (E.Def 9.3.2) and the updated quality control tables for antifungal susceptibility testing document, v 5.0 (available on the EUCAST website). CONTENTS: The method is based on the EUCAST microdilution method for moulds but significant differences include: (a) an altered test medium selective for dermatophytes; (b) an altered incubation time and temperature; and (c) a different end-point criterion (spectrophotometric determination) of fungal growth. It can easily be implemented in laboratories already performing EUCAST microdilution methods and has been validated for terbinafine, voriconazole, itraconazole and amorolfine against T. rubrum and T. interdigitale. IMPLICATIONS: This standardized procedure with automated end-point reading will allow broader implementation of susceptibility testing of dermatophytes and so facilitate earlier appropriate therapy. This is important, as resistance is rapidly emerging and largely underdiagnosed.
Taxonomie dermatofytů prošla v minulosti řadou výrazných změn a nikdy nebyla příliš stabilní. Poslední rozsáhlejší změny proběhly v souvislosti s revizí skupiny molekulárně genetickými metodami. Hlavním cílem této práce je seznámit čtenáře s pokroky v taxonomii dermatofytů a prostřednictvím seznamu doporučených jmen sjednotit pojmenovávání druhů napříč českými pracovišti. To by mělo nejen usnadnit komunikaci, ale také porovnávání výsledků se zahraniční literaturou. Jedním z doporučení je například ukončení používání jména Trichophyton mentagrophytes v klinické praxi. V úvodu textu se diskutuje o problematickém konceptu druhu u dermatofytů, jehož rozdílná aplikace může mít velký vliv na počet uznávaných druhů. Závěrečná část je věnována budoucím změnám souvisejícím s novým vydáním Mezinárodního kódu nomenklatury řas, hub a rostlin. Jedná se především o ukončení duální nomenklatury hub a vybrání jediného správného jména pro každý druh. Obecně se dá říci, že je vhodné preferovat jméno nepohlavního stadia (Trichophyton, Microsporum, Epidermophyton), s výjimkou případů, kde toto jméno není k dispozici (např. Arthroderma benhamiae). Taxonomy of dermatophytes has changed significantly in the past and it has never been stable. Last extensive changes appeared after revision of the group by molecular genetic methods. This paper introduces the advances in taxonomy of dermatophytes and helps to unify the species names in use among the Czech laboratories to simplify their communication and comparison of the results with foreign literature. The complicated species concept of dermatophytes that might be applied differently and might influence the number of accepted species is discussed in the beginning. The last part is dedicated to future changes related to a new edition of the International Code of Nomenclature for algae, fungi, and plants namely to termination of a dual nomenclature of fungi and choosing the one correct name for each species. A case of dermatomycosis caused by zoophilic strain of Trichophyton interdigitale was treated by low-temperature plasma produced by direct current (DC) cometary discharge. The shortening of skin lesion persistence along with suppression of subjective discomfort and etiological agent was observed.
Trichophyton onychocola is a recently described geophilic dermatophyte that has been isolated from a toenail of Czech patient with a history of onychomycosis due to T. rubrum and clinical suspicion of relapse. In this study, we report a similar case from Denmark in an otherwise healthy 56-year-old man. The patient had a history of great toenail infection caused by T. rubrum in 2004 and presented with suspected relapse in 2011 and 2013. Trichophyton onychocola was the only microbial agent isolated at the second visit in 2013 and the identification was confirmed by DNA sequencing. Direct microscopic nail examination was positive for hyphae, however the etiological significance of T. onychocola was not supported by repeated isolation of the fungus. This new species may be an overlooked geophilic species due to the resemblance to some common species, for example, zoophilic T. interdigitale or some species of geophilic dermatophytes. We included differential diagnosis with phenotypically similar species; however, it is recommended that molecular methods are used for correct identification. The MAT locus of Danish strain was of opposite mating type than in the previously isolated Czech strain and the two isolates were successfully mated. The mating experiments with related heterothallic species T. thuringiense and Arthroderma melis were negative. The sexual state showed all typical signs of arthroderma-morph and is described by using optical as well as scanning electron microscopy. The sexual state was induced on a set of agar media, however low cultivation temperature and the presence of keratin source were crucial for the success rather than formulation of medium.

Mykotická onemocnění kůže patří mezi nejčastější kožní infekční onemocnění. Diagnostika a léčba povrchových mykotických infekcí kůže může být někdy obtížná u určitých skupin populace, jako jsou děti. Popisujeme případ dvouleté dívky s klinickou jednotkou tinea corporis včetně klinického obrazu, diferenciálně diagnostické úvahy, kultivačního nálezu Trichophyton interdigitale/Trichophyton mentagrophytes a léčby. Uvádíme přehled současné literatury týkající se doporučené lokální a celkové antimykotické terapie této jednotky u dětí.
Fungal skin diseases belong to the most common infectious disorders of the skin. Diagnosis and therapy of superficial fungal skin infections may sometimes become difficult in certain population groups such as in childhood. We describe the case of a 2-year old girl with tinea corporis including clinical picture, differential diagnostic considering, cultivation finding of Trichophyton interdigitale/Trichophyton mentagrophytes and therapy. We summarize current literature recommendations of topical and systemic antifungal treatment of this entity in children.
