Neexistuje žádný univerzální imunohistochemický panel, který by byl v diagnostice nádorů měkkých tkání užitečný za každé situace. Přesto je možné doporučit základní sadu markerů, které jsou vhodné pro použití zejména u nepříliš charakteristických vřetenobuněčných lézí, kdy na základě morfologie nelze jednoznačně určit nejen konkrétní diagnózu, ale často ani směr diferenciace. V tomto přehledovém článku budou shrnuty poznatky o hlavních a letitou praxí osvědčených imunohistochemických markerech pro diagnostiku jednotlivých nádorových skupin v této části patologie se vyskytující: CD34, desmin, epiteliální membránový antigen, širokospektré cytokeratiny, S100 protein a hladkosvalový aktin. Dále bude diskutována problematika spojená jak s využitím imunohistochemického barvení s protilátkou MDM2, tak s metodou fluorescenční in situ hybridizace pro detekci amplifikace genu MDM2 v lipomatózních tumorech.
There is no universal immunohistochemical panel which would be useful for the diagnosis of soft tissue tumors in all circumstances. Nevertheless, especially when faced with an uncharacteristic spindle cell neoplasm, a basic immunohistochemical panel can be recommended consisting of CD34, desmin, epithelial membrane antigen, broad-spectrum cytokeratins, S100 protein and smooth muscle actin. This review will address the utility and pitfalls of this panel. The use of MDM2 immunohistochemistry and fluorescence in situ hybridization in the diagnosis of lipomatous tumors will be discussed as well.

Dědičná nádorová onemocnění tvoří malou, ale klinicky významnou část onkologických onemocnění, v České republice se jedná ročně o několik tisíc osob. Identifikace kauzální mutace v nádorových predispozičních genech má u těchto nemocných zásadní prognostický a v některých případech i prediktivní význam. Mimo to je podmínkou cílené preventivní péče o asymptomatické nosiče mutací v rodinách se zvýšeným rizikem vzniku nádorového onemocnění. Do současné doby bylo charakterizováno více než 150 nádorových predispozičních genů. Mutace většiny z nich se vyskytují vzácně, s výraznou populační specifičností a jejich klinická interpretace je často obtížná. Diagnostiku raritních variant technicky zjednodušují postupy využívající sekvenování nové generace, které umožňují vyšetření rozsáhlých sad genů. Za účelem racionalizace diagnostiky hereditárních nádorových syndromů v České republice jsme navrhli sekvenační panel „CZECANCA“, který cílí na vyšetření 219 genů asociovaných s dědičnými nádorovými onemocněními. Panel obsahuje přes 50 klinicky významných genů vysokého a středního rizika, zbývající geny tvoří málo prozkoumané a kandidátní predispoziční geny, jejichž vrozené mutace mají nejasnou klinickou interpretaci. Společně s návrhem panelu byl optimalizován postup vlastního sekvenování a bioinformatického zpracování sekvenačních dat pro tvorbu jednotné databáze genotypů analyzovaných vzorků. Cílem projektu je nabídnout použití sekvenačního panelu včetně optimalizovaného postupu sekvenování nové generace diagnostickým laboratořím v České republice a zajistit sdílení genotypů a klinických údajů o vyšetřovaných pacientech ve společné databázi za účelem zlepšení možnosti klinické interpretace vzácných mutací u vysoce rizikových osob.
Individuals with hereditary cancer syndromes form a minor but clinically important subgroup of oncology patients, comprising several thousand cases in the Czech Republic annually. In these patients, the identification of pathogenic mutations in cancer susceptibility genes has an important predictive and, in some cases, prognostic value. It also enables rational preventive strategies in asymptomatic carriers from affected families. More than 150 cancer susceptibility genes have been described so far; however, mutations in most of them are very rare, occurring with substantial population variability, and hence their clinical interpretation is very complicated. Diagnostics of mutations in cancer susceptibility genes have benefited from the broad availability of next-generation sequencing analyses using targeted gene panels. In order to rationalize the diagnostics of hereditary cancer syndromes in the Czech Republic, we have prepared the sequence capture panel “CZECANCA”, targeting 219 cancer susceptibility genes. Besides more than 50 clinically important high- and moderate-penetrance susceptibility genes, the panel also targets less common candidate genes with uncertain clinical relevance. Alongside the panel design, we have optimized the analytical and bioinformatics pipeline, which will facilitate establishing a collective nationwide database of genotypes and clinical data from the analyzed individuals. The key objective of this project is to provide diagnostic laboratories in the Czech Republic with a reliable procedure and collective database improving the clinical utility of next-generation sequencing analyses in high-risk patients, which would help improve the interpretation of rare or population-specific variants in cancer susceptibility genes. I přesto, že se pětileté přežívání pacientů s karcinomem žaludku za posledních 20 let zlepšilo jen minimálně, existují specifické podskupiny pacientů, které přežívají výrazně déle. Významným tématem se tak stává hledání markerů na úrovni genových poruch umožňujících individualizovaný odhad prognózy pacientů. Cílem projektu bylo provést profilování somatických bodových mutací a genových amplifikací pro zmapování výskytu v české populaci a případně identifikace potenciálních prognostických markerů karcinomu žaludku. Materiál a metody: Soubor vyšetřených čítal 70 pacientů s histologicky potvrzeným karcinomem žaludku. Vzorky byly získány ve formě FFPE bločků z endoskopických tkáňových biopsií nebo resekátů u pacientů podstupujících operaci. Byla provedena vyšetření mutací genů KRAS, BRAF, PIK3CA, EGFR, CTNNB1, TP53 a APC a panelu amplifikací 29 genů zahrnujících nejčastější (proto)onkogeny a tumor supresorové geny procesu vzniku a progrese nádoru. Výsledky byly korelovány s klinickými daty vč. stadia, lokalizace a histopatologického podtypu karcinomu a přežití pacientů. Korelace amplifikací a mutací s přežitím byla analyzována Kaplan-Meierovou metodikou, relativní poměry rizik (HR – hazard ratio) byly analyzovány na základě vícerozměrného testu dle Coxova modelu proporcionálního rizika. Výsledky: Četnosti poruch byly nižší ve srovnání s celosvětovou populací. Shluková analýza identifikovala specifickou skupinu profilů se zhoršeným přežitím (HR = 7,36; 95% CI 1,34–40,4; p = 0,022). Při analýze amplifikací pro tuto skupinu, PDGFRB, ERBB2, RET, EGFR, CCND1 a CDKN1B (p27/Kip1), byl zjištěn zvýšený HR u pacientů s amplifikací u minimálně dvou genů (HR = 4,66; 95% CI 1,5–14,43; p = 0,008). Závěr: V této studii byla určena frekvence somatických mutací a genových amplifikací u českých pacientů s karcinomem žaludku. Dále byla identifikována skupina genových amplifikací, které by mohly sloužit k odhadu prognózy pacientů. Pro ověření výsledků bude nutno provést validaci na rozšířeném souboru pacientů, pravděpodobně kombinací typicky malých souborů vzorků z více pracovišť. V případě potvrzení by vyšetřování této skupiny markerů mohlo najít uplatnění při racionálním rozhodování o léčbě, a tím zajistit celkové zlepšení přežívání pacientů s karcinomem žaludku.
Although the 5-year survival of patients with gastric cancer has improved only marginally over the past 20 years, some specific subgroups of patients survive significantly longer. Finding molecular markers for individualized prognosis has therefore become a major issue. The aim of this project was to profile somatic point mutations and gene amplifications to map their frequency in Czech gastric cancer patients and to identify potential prognostic markers. Material and methods: The group included 70 patients with histologically confirmed gastric cancer. FFPE samples were obtained from either endoscopic tissue biopsies or resections from patients undergoing surgery. The study included searching for somatic mutations in KRAS, BRAF, PIK3CA, EGFR, CTNNB1, TP53 and APC, and gene amplifications in a panel of 29 genes, including common (proto)oncogenes and tumor suppressors implicated in tumor initiation and progression. The results were correlated with clinical data including stage, localization, and histopathological subtype of carcinoma, as well as patient survival. Correlation with survival was evaluated by using the Kaplan-Meier method. The relative hazard ratios (HRs) were tested by means of multivariate analysis using Cox proportional hazards model. Results: Frequencies of aberrations were lower compared with those in the global gastric cancer population. Cluster analysis identified a group of profiles that correlated with poor survival (HR = 7.36; 95% CI 1.34–40.4; p = 0.022). Analysis within this group characterized by amplifications of PDGFRB, ERBB2, RET, EGFR, CCND1 and CDKN1B (p27/Kip1) revealed an increased HR for patients with amplification of at least two of these genes (HR = 4.66; 95% CI 1.5–14.43; p = 0.008). Conclusion: In this study, the frequencies of somatic mutations and gene amplifications in Czech patients with gastric cancer were determined. Furthermore, a group of gene amplifications that could be used to determine prognosis was identified. The results should be validated in an expanded group of patients, possibly by combining typical smaller sample sets from multiple centers. If confirmed, tests employing these markers could have potential for arriving at rational decisions about patient treatment, resulting in an overall improvement in the survival of patients with gastric cancer.

AIMS: Amyloidosis is caused by deposition of abnormal protein fibrils, leading to damage of organ function. Hereditary amyloidosis represents a monogenic disease caused by germline mutations in 11 amyloidogenic precursor protein genes. One of the important but non-specific symptoms of amyloidosis is hypertrophic cardiomyopathy. Diagnostics of hereditary amyloidosis is complicated and the real cause can remain overlooked. We aimed to design hereditary amyloidosis gene panel and to introduce new next-generation sequencing (NGS) approach to investigate hereditary amyloidosis in a cohort of patients with hypertrophic cardiomyopathy of unknown significance. METHODS: Design of target enrichment DNA library preparation using Haloplex Custom Kit containing 11 amyloidogenic genes was followed by MiSeq Illumina sequencing and bioinformatics identification of germline variants using tool VarScan in a cohort of 40 patients. RESULTS: We present design of NGS panel for 11 genes (TTR, FGA, APOA1, APOA2, LYZ, GSN, CST3, PRNP, APP, B2M, ITM2B) connected to various forms of amyloidosis. We detected one mutation, which is responsible for hereditary amyloidosis. Some other single nucleotide variants are so far undescribed or rare variants or represent common polymorphisms in European population. CONCLUSIONS: We report one positive case of hereditary amyloidosis in a cohort of patients with hypertrophic cardiomyopathy of unknown significance and set up first panel for NGS in hereditary amyloidosis. This work may facilitate successful implementation of the NGS method by other researchers or clinicians and may improve the diagnostic process after validation.

BACKGROUND: We applied a training and testing approach to develop and validate a plasma metabolite panel for the detection of early-stage pancreatic ductal adenocarcinoma (PDAC) alone and in combination with a previously validated protein panel for early-stage PDAC. METHODS: A comprehensive metabolomics platform was initially applied to plasmas collected from 20 PDAC cases and 80 controls. Candidate markers were filtered based on a second independent cohort that included nine invasive intraductal papillary mucinous neoplasm cases and 51 benign pancreatic cysts. Blinded validation of the resulting metabolite panel was performed in an independent test cohort consisting of 39 resectable PDAC cases and 82 matched healthy controls. The additive value of combining the metabolite panel with a previously validated protein panel was evaluated. RESULTS: Five metabolites (acetylspermidine, diacetylspermine, an indole-derivative, and two lysophosphatidylcholines) were selected as a panel based on filtering criteria. A combination rule was developed for distinguishing between PDAC and healthy controls using the Training Set. In the blinded validation study with early-stage PDAC samples and controls, the five metabolites yielded areas under the curve (AUCs) ranging from 0.726 to 0.842, and the combined metabolite model yielded an AUC of 0.892 (95% confidence interval [CI] = 0.828 to 0.956). Performance was further statistically significantly improved by combining the metabolite panel with a previously validated protein marker panel consisting of CA 19-9, LRG1, and TIMP1 (AUC = 0.924, 95% CI = 0.864 to 0.983, comparison DeLong test one-sided P= .02). CONCLUSIONS: A metabolite panel in combination with CA19-9, TIMP1, and LRG1 exhibited substantially improved performance in the detection of early-stage PDAC compared with a protein panel alone.

OBJECTIVE: Developmental and epileptic encephalopathies (DEEs) are a group of severe, early-onset epilepsies characterised by refractory seizures, developmental delay, or regression and generally poor prognosis. DEE are now known to have an identifiable molecular genetic basis and are usually examined using a gene panel. However, for many patients, the genetic cause has still not been identified. The aims of this study were to identify causal variants for DEE in patients for whom the previous examination with a gene panel did not determine their genetic diagnosis. It also aims for a detailed description and broadening of the phenotypic spectrum of several rare DEEs. METHODS: In the last five years (2015-2020), 141 patients from all over the Czech Republic were referred to our department for genetic testing in association with their diagnosis of epilepsy. All patients underwent custom-designed gene panel testing prior to enrolment into the study, and their results were inconclusive. We opted for whole exome sequencing (WES) to identify the cause of their disorder. If a causal or potentially causal variant was identified, we performed a detailed clinical evaluation and phenotype-genotype correlation study to better describe the specific rare subtypes. RESULTS: Explanatory causative variants were detected in 20 patients (14%), likely pathogenic variants that explain the epilepsy in 5 patients (3.5%) and likely pathogenic variants that do not fully explain the epilepsy in 11 patients (7.5%), and variants in candidate genes in 4 patients (3%). Variants were mostly de novo 29/40 (72.5%). SIGNIFICANCE: WES enables us to identify the cause of the disease in additional patients, even after gene panel testing. It is very important to perform a WES in DEE patients as soon as possible, since it will spare the patients and their families many years of a diagnostic odyssey. In particular, patients with rare epilepsies might significantly benefit from this approach, and we propose using WES as a new standard in the diagnosis of DEE instead of targeted gene panel testing.

The aim of the project is to detect circulating tumor cells (CTC) in peripheral blood of patients with metastatic prostate cancer before and during the cytotoxic therapy. Detection will be performed by the method of immunomagnetic separation and RNA isolation from this fraction. Then we will perform the cultivation of the detected cells including the cultivation in milieu of cytotoxic agents. In the next stage the genetic profile of the detected cells will be assessed (a panel of 24 genes) and a similar profile will be evaluated also in histological specimen of a primary tumor. Analysis results will be correlated to the clinical course of disease and the prognostic factors of treatment effect on CTC count decline and patients‘ survival will be determined.
Cílem projektu je detekovat cirkulující nádorové buňky (CTC) v periferní krvi u pacientů s metastatickým karcinomem prostaty před cytotoxickou léčbou a v jejím průběhu. Detekce bude provedena metodou imunomagnetické separace buněk a izolací RNA z této frakce. Dále proběhne kultivace části detekovaných buněk včetně kultivace v prostředí cytotoxických látek. V další fázi bude hodnocen genetický profil zjištěných buněk (stanovení panelu 24 genů) a obdobný profil také u histologického preparátu primárního tumoru. Výsledky analýz budou vztaženy na klinický průběh onemocnění a budou stanoveny prognostické faktory účinku léčby na pokles CTC a přežití pacientů.

Diagnosis of salivary gland neoplasms is often challenging due to their high morphological diversity and overlaps. Several recurrent molecular alterations have been described recently, which can serve as powerful diagnostic tools and potential therapeutic targets (e.g. NTRK or RET fusions). However, current sequential molecular testing can be expensive and time consuming. In order to facilitate the diagnosis of salivary gland neoplasms, we designed an all-in-one RNA-based next generation sequencing panel suitable for the detection of mutations, fusions and gene expression levels (including NR4A3) of 27 genes involved in salivary gland neoplasms. Here we present the validation of the "SalvGlandDx" panel on FFPE histological specimen including fine needle aspiration (FNA) cell block material, against the standard methods currently used at our institution. In a second part we describe selected unique cases in which the SalvGlandDx panel allowed proper diagnosis and new insights into special molecular characteristics of selected salivary gland tumors. We characterize a unique salivary gland adenocarcinoma harboring a ZCCHC7-NTRK2 fusion, a highly uncommon spindle cell and pseudoangiomatoid adenoid-cystic carcinoma with MYBL1-NFIB fusion, and a purely oncocytic mucoepidermoid carcinoma, whereas diagnosis could be made by detection of a CRTC3-MAML2 rearrangement on the cell block specimen of the FNA. Further, a rare case of a SS18-ZBTB7A rearranged low-grade adenocarcinoma previously described as potential spectrum of microsecretory adenocarcinoma, is reported. In addition, features of six cases within the spectrum of polymorphous adenocarcinoma / cribriform adenocarcinoma of salivary gland including PRKD1 p.E710D mutations and novel fusions involving PRKAR2A-PRKD1, SNX9-PRKD1 and ATL2-PRKD3, are described.
