OBJECTIVE: The aim of our study is to examine if the dissociation, can influence intensity of psychopathology in patients suffering with dissociative disorders compare the level of dissociation of the patients with the data of healthy subjects. METHODS: 32 patients suffering with various types of dissociative disorder (9 males and 23 females) and 93 healthy controls (29 males and 64 females) were included into the study. The patients were psychiatrically assessed and the subjective intensity of symptoms was evaluated by Beck Anxiety Inventory and Beck Depression Inventory. All participants were assessed with the Dissociative Experiences Scale (DES). RESULTS: The two groups didn't differ in demographic variables like age, gender and education. Level of the psychological dissociation assessed with the DES was correlated with the severity of subjective anxiety symptoms (p<0.0001), and with the severity of depression symptoms (p<0.0001). Patients had statistically significant higher mean score of DES than healthy controls (p<0.0001). CONCLUSIONS: Our results suggest that the level of psychological dissociation in dissociative disorder patients is associated with the severity of anxiety and depressive symptoms.

The aim of our study is to examine if the dissociation, can influence the intensity of psychopathology in patients suffering from panic disorder with or without agoraphobia and compare the levels of dissociation in the patients and healthy subjects. METHODS: 41 patients suffering from panic disorder (70.1% females) and 66 healthy controls (77.3% females) were included in the study. The patients were psychiatrically assessed and the subjective intensity of symptoms was evaluated using Beck Anxiety Inventory and Beck Depression Inventory. All participants were assessed with the Dissociative Experiences Scale (DES). RESULTS: The two groups didn't differ in demographic variables like age, gender and education. Level of the psychological dissociation assessed with the DES was correlated with the severity of subjective anxiety symptoms (p<0.0001), but not with the severity of depression symptoms. Patients did not have significantly higher mean score on the DES than healthy controls. CONCLUSIONS: Our results suggest that the level of psychological dissociation in panic disorder patients is associated with the severity of anxiety but not depressive symptoms.

Introduction. According to recent findings, certain clinical symptoms of patients suffering from affective and anxiety disorder can be related to dissociation. The aim of our study was to examine if the level of dissociation in bipolar affective disorder differed from the level of dissociation in healthy volunteers. Methods. 41 patients suffering from bipolar disorder (51.2% females), and 198 healthy controls ( 71.2% females) were included in the study. The patients with bipolar affective disorder in remission were recruited from the Outpatient department of the Department of Psychiatry of the University Hospital Olomouc. They were psychiatrically assessed and the state of the disorder was evaluated by an experienced psychiatrist. Only patients in remission, evaluated as 1 or 2 points of clinical global impression – severity scale, were included in the study. All participants were assessed with the Dissociative Experiences Scale (DES). Results. There were no differences in the two groups in demographic variables like age, gender and education. Patients had a significantly higher mean score on the DES and pathological DES than healthy controls. Conclusion. Our results suggest that the level of psychological dissociation in bipolar affective patients is higher than in healthy controls.

The aim of our study is to examine if the dissociation can influence the intensity of psychopathology in patients suffering from obsessive compulsive disorder and to compare the levels of dissociation in the groups of the patients and healthy subjects. Method. Fifty five patients suffering from obsessive compulsive disorder and 123 healthy controls were included into the study. The patients were psychiatrically assessed. The diagnosis was made using ICD-10 research criteria confirmed with structured interview MINI. The subjective intensity of anxiety and depressive symptoms was evaluated using Beck Anxiety Inventory and Beck Depression Inventory. The intensity of obsessions and compulsions was evaluated using Yale Brown Obsessive Compulsive Scale. All participants were assessed with the Dissociative Experiences Scale (DES). Results. Level of the psychological dissociation assessed with the DES was correlated with the severity of subjective anxiety (p<0.0001), depression (p<0.0001), and with the severity of obsessive-compulsive symptoms (p<0.005). Patients have significantly lower mean score on the DES than healthy controls (p<0.0001). Conclusion. Our results suggest that the level of psychological dissociation in OCD patients is lower than in healthy controls, and is associated with the severity of anxiety, depression and obsessive compulsive symptoms.

OBJECTIVES: The aim of our study is to measure very low frequency band (VLF), low frequency band (LF) and high frequency band (HF) components of R-R interval during orthostatic test in patients with panic disorder and a comparison with healthy controls. METHODS: We measured HRV in 31 patients with panic disorder and 20 healthy controls. Diagnosis was done according to the ICD-10 research diagnostic criteria confirmed with MINI (MINI international neuropsychiatric interview). Autonomic nervous system (ANS) has been evaluated during orthostatic change in three positions. Intensity of symptoms was assessed using psychiatric scales. RESULTS: There were highly statistically significant differences between panic patients and control group in all components of power spectral analysis in 2nd and 3rd VLF components and in HF components of 2nd. We have found highly statistically significant negative correlations between level of dissociation measured by DES and some parameters of ANS. We found negative correlations between the age of the patient and activity of ANS, and negative correlations between activity of ANS and duration and onset of disorder and dosage of antidepressants. CONCLUSION: These findings demonstrate a lower parasympathetic activity and higher sympathetic/parasympathetic ratio in panic disorder patients measured during the changes of postural position in comparison with healthy controls. Autonomic dysregulation is associated with panic disorder and has the relation with the level of dissociation, the age of patiens and age of onset of disorder.

V uplynulých dvaceti letech byla provedena řada studií zkoumajících autonomní nervový systém (ANS) u pacientů s panickou poruchou. Většina z těchto prací se zaměřila na sympatický nervový systém S hypotézou, že jde o jeho hyperaktivitu nebo dysregulaci. Nálezy těchto prácí jsou nekonzistentní a dosud nebylo dosaženo konsenzu, jak vlastně ANS u panické poruchy funguje. Metoda: HRV bylo měřeno u 31 pacientů s panickou poruchou a 20 zdravých kontrol. Pacienti užívali psychofarmaka. Autonomní netvový systém byl hodnocen v průběhu ortostatické změny ve třech polohách (1. - vleže 5 minut, 2. - stojí 5 minut, 3. - vleže 5 minut). Intenzita příznaků byla hodnocena pomocí CGI (Clinical Global Impression), BAI ((Beck Anxxiety Inventory) a BDI (Beck Depression Inventory) a DES (Dissociative Experience Scale). K hodnocení byl použit statistický software používající spektrální analýzu a Fourierovou transformaci. Výsledky: Rozdíly mezi HRV u panické poruchy ve srovnání s kontrolami byly nalezeny ve stoje a vleže v částech spektra VLF a HF. Zjistili jsme vysoce statisticky významné negativní korelace mezi mírou disociace a některými parametry ANS. Závěry: Autonomní vegetativní systém u pacientů s panickou potuchou vykazuje rozdíly při porovnání se zdravými kontrolami.
In past twenty years, many studies have been performed to examine autonomic nervous system (ANS) in panic disorder. Most of these projects have focused on the sympathetic nervous system with hypotheses of its hyperactivity or dysregulation. They have produced re- markably different findings such that no consensus has been reached on the pattern of sympathetic nervous system dysfunction in panic disorder. Methods: We measured HRV in 31 panic disorder patients and 20 healthy controls. Patients were treated with psychotropics. Autonomic nervous system has been evaluated during orthostatic change in three po- sitions (1st - supine 5 minutes, 2nd – standing 5 minutes, 3rd – supine 5 minutes). Intensity of symptoms was assessed using CGI (Clinical Global Impression), BAI (Beck Anxiety Inventory) and BDI (Beck Depression Inventory) and DES (Dissociative Experience Scale). The functioning of the ANS has been measured by the microcomputer diagnostic system that is using power spectral analysis which quantifies the heart rate variability. HRV was assessed using time domain, frequency domain, and nonlinear analyses. Results: There were highly statistically significant differences between panic patients and control group in all components of power spectral analysis in 2 nd and 3 rd VLF components and in HF components of 2 nd of experiment. We have found highly statistically significant negative correlations between level of dissociation measured by DES and some parameters of ANS. Conclusions: Autonomic dysregulation is associated with panic disorder showed differences in compariton of panic pacients and healthy controls..

V současné době je diskutován koncept disociace jako jev s komponentou „patologickou“ a „nepatologickou“. Patologická forma disociace je zřejmě u některých psychických poruch jedním z více příznaků celkového psychopatologického obrazu, u různých typů onemocnění se v míře zastoupení různí. Naopak nepatologická forma disociace je jevem běžným u normální zdravé populace. Ke skríningu patologické formy disociace slouží osmipoložkový dotazník DES-T. Naše vlastní zjištění koncept patologické formy disociace podporuje. Ověřili jsme vysoký výskyt patologické disociace u skupiny poruch disociativních a u pacientů s emočně nestabilní poruchou osobnosti. Na druhém pólu byla panická porucha, kde je výskyt patologické disociace srovnatelný s normální populací.
The concept of dissociation is currently discussed as a phenomenon containing both pathological and the non-pathological component. The pathological form of dissociation may in some mental disorders represent one symptom amongst many symptoms of the whole psychopathological picture. The quantity of it is different in a different type of the disorder. On the contrary the non-pathological form of dissociation is quite a common phenomenon in the normal healthy population. DES-T questionnaire containing 8 items is used for screening (to detect) the pathological form of dissociation. Our own findings support the concept of the pathological dissociation. We proved high prevalence of pathological dissociation in patiens with the Borderline Personality Disorder and in the group of the Dissociative disorders. The Panic Disorder appears to be the opposite – the finding of the pathological dissociation is comparable to that of the common population.

BACKGROUND: Recent findings confirm, following Bleuler's and Janet's tradition, the significant influence of stress-related events and dissociation in the pathogenesis of schizophrenia. Dissociative reaction is most often a consequence of traumatic experiences that lead to a loss of inhibitory control and may produce split fragments of the mind. Recent findings in studies on brain complexity and neural synchronization suggest the hypothesis that the specific functional fragmentation of neural subsystems could be linked to the dissociation and splitting in schizophrenia that may be reflected in dynamic neural complexity and assessed by measures reflecting these processes. MATERIAL/METHODS: In this context, the hypothesis tested in this study is that dynamic changes in electrodermal activity (EDA) as a measure of brain and autonomic activity could serve as an indicator of specific changes in neural complexity in schizophrenia patients. Therefore, bilateral EDA under rest conditions in 30 schizophrenic patients and 30 healthy subjects was measured. RESULTS: The results of nonlinear and statistical analysis of EDA records indicated increased neural complexity indexed by a point-wise correlation dimension (PD2) in schizophrenia patients compared with healthy controls. CONCLUSIONS: This result represents preliminary support for the hypothesis that increased neural complexity reflects the functional fragmentation of neural subsystems related to traumatic dissociation and splitting in schizophrenia.
