The adhesive material used to bond orthodontic brackets to teeth should neither fail during the treatment period, resulting in treatment delays, untoward expenses or patient inconvenience nor should it damage the enamel on debonding at the end of the treatment. Although the effectiveness of a bonding system and any unfavorable effects on the enamel may be studied by conducting in-vivo studies, it is nearly impossible to independently analyze different variables that influence a specific bonding system in the oral environment. In-vitro studies, on the other hand, may utilize more standardized protocols for testing different bonding systems and materials available. Thus, the present review focused attention on in-vitro studies and made an attempt to discuss material-related, teeth-related (fluorotic vs non-fluorotic teeth) and other miscellaneous factors that influences the shear bond strength of orthodontic brackets. Within the limitations of this review, using conventional acid-etch technique, ceramic brackets and bonding to non-fluorotic teeth was reported to have a positive influence on the shear bond strength of orthodontic brackets, but higher shear bond strength found on using ceramic brackets can be dangerous for the enamel.

OBJECTIVES: The purpose of this study was to evaluate the in vitro sealing ability of three repair materials. Mineral trioxide aggregate (MTA; Group A), calcium phosphate cement (CPC; Group B), and light cured glass ionomer cement (GIC; Group C) when used to repair the perforation created in the pulpal floor of fifty extracted human permanent molars. MATERIALS AND METHODS: Preparation of access openings and furcation perforations were done, and the teeth divided into five experimental groups (A, B, C) including two controls (D, E) with ten samples in each group randomly. Following the repair procedure, the pulp chambers and access openings were filled with composite resin and immersed in 2% methylene blue solution for 48 hours. The teeth were sectioned longitudinally and the linear dye penetration measured under a stereomicroscope. RESULTS: The comparison of the linear length of micro-leakage (mm) among the experimental groups revealed no significant difference (p = 0.332). On calculating the percentage of depth of leakage to the total length of the perforation, it was observed that the mean leakage was 35.5% in Group A, 53.6% in Group B and the highest, 87.5% in Group C. The mean of leakage percentage was statistically significant by Kruskal-Wallis test (p = 0.003). The results indicated that the dye penetration used as furcation perforation repair material was least with mineral trioxide aggregate. Comparing the depth of penetration of dye, 50% of the Group A samples showed less than 25% of depth penetration. While 40% of Group B cases had more than 50% dye penetration. In our study, all Group C teeth had > or = 50% dye penetration. CONCLUSIONS: The present study indicated that GIC had the greatest dye penetration followed by CPC and MTA. Mineral trioxide aggregate and calcium phosphate cement had comparatively better sealing ability than glass ionomer cement.

The purpose of this study was to assess the oral health-related quality of life in Czech population. Methods: Data were collected from 1,380 subjects aged 30 to 69 years attending the Department of Dentistry, Medical Faculty of Charles University in Hradec Králové or attending three private dental practitioners collaborating on the study. Oral health-related quality of life was measured with the Oral Health Impact Profile (OHIP-14) questionnaire that was translated to Czech. The OHIP-14 scores were assessed in relation to chosen clinical and sociodemographic characteristics and oral health behaviour. Statistical analyses included descriptive analyses, the Mann-Whitney test, the Kolmogorov-Smirnov test and the Kruskal-Wallis test using the NCSS 2007 program. The &chi2 test of independence in contingency tables or Fisher's exact test was used for qualitative data. Results: Internal reliability for the 14 items overall was very high (Cronbach's ? = 0.924). The two most frequently scored items using the answer other than "never" during the last year were "painful aching" (62% of subjects) and "uncomfortable to eat" (44.4%), representing subdomain physical pain. The domain of social disability was reported least frequently. The OHIP-14 was significantly associated with dental status, dental behaviour, income and age. Conclusions: The findings of this study do suggest that the culturally adapted OHIP-14 version may be a good research instrument to be considered for use in measuring the impact of oral problems on the quality of life in Czech population.

Metodou průřezové epidemiologické studie provedené u náhodně vybraného vzorku dospělé populace byl sledován vliv kouření na orální zdraví, zejména na stav parodontu a tvrdých zubních tkání. Studie měla 2 části – dotazníkovou, realizovanou samotnými respondenty, a klinickou, kterou zajišťovali jednotně instruovaní zubní lékaři. Stav chrupu a parodontu byl hodnocen pomocí indexů kazivosti KPE a CPITN (Community Periodontal Index of Treatment Need). Statistické zpracování bylo provedeno v programu NCSS 2007 s využitím deskriptivní statistiky, ?2 testu a logistické regrese. Sledovaný soubor tvořilo 1473 respondentů (694 mužů a 779 žen) průměrného věku 44,3 let. Z uvedeného počtu bylo 310 pravidelných, 101 příležitostných a 230 bývalých kuřáků a 832 nekuřáků. Hodnocení stavu chrupu prokázalo, že kuřáci měli statisticky významně vyšší hodnoty KPE než nekuřáci (17,5 vs. 16,3), což bylo způsobeno zejména vyšším průměrným počtem zubů s kazem a zubů chybějících. Nekuřáci vykazovali vyšší prevalenci nálezů CPI 0 (zdravý parodont), kuřáci měli naopak ve všech sextantech vyšší prevalenci nálezů CPI 3, resp. CPI 4 (mělké, resp. hluboké choboty). Kouření zvyšovalo statisticky významně odds ratio pro počet zubů s kazem (OR=1,28; p=0,04) a pro potřebu komplexní péče parodontitidy s nálezem TN III, zde ovšem statisticky nevýznamně (OR=1,3; p=0,098). Byl hodnocen i vliv jiných proměnných na orální zdraví, především vliv věku, vzdělání, participace na preventivních prohlídkách a důsledného provádění ústní hygieny.
Using the method of the cross-sectional epidemiological study in randomized sample of an adult population an infl uence of smoking on the oral health (namely the periodontium and teeth) was followed. This study consisted of two parts, a selfreported questionnaire inquiry and clinical examination carried out ensured by uniformly instructed dentists. Dental status and periodontal status were assessed by DMF (Decayed, Missing, and Filled Teeth) and CPITN (Community Periodondal Index of Treatment Needs) indices. Statistical analysis was performed by NCSS 2007 programme exploited the descriptive statistics, ?2 test and logistic regression. The whole set contained 1473 respondents (694 males and 779 females) mean age 44.3 years. From this number there were 310 regular, 101 occasional and 230 former smokers, and 832 non-smokers. The assessment of dental status has revealed that smokers have a signifi cantly higher DMF index compared to non-smokers (17.5 vs. 16.3), which was caused mainly by a higher mean number of decayed and missing teeth. The non-smokers had a higher prevalence of CPI 0 (healthy parodont). On the contrary, the smokers had a higher prevalence of CPI 3 or CPI 4 fi ndings in all sextants (shallow or deep pockets). Smoking statistically enhanced the odds ratio of the mean number of decayed teeth (OR=1.3; p=0.04), and the odds ratio for complex periodontal treatment need with TN III fi nding (OR=1.3; p=0.098), however in this case non-signifi cantly. The infl uence of the other variables, mainly of age, education, participation in preventive check-ups and consistent dental hygiene were also evaluated.
Metodou průřezové epidemiologické studie provedené u dospělé populace České republiky byl sledován odlišný přístup kuřáků a nekuřáků k orálnímu zdraví. Studie měla 2 části — dotazníkovou a klinickou. Respondenti byli vybíráni náhodným výběrem, přičemž jediným vylučovacím kritériem byl věk. Statistické zpracování bylo provedeno v programu NCSS. Z celkového počtu 1473 respondentů (694 mužů a 779 žen) ve věku 30-69 let bylo 411 současných kuřáků (310 pravidelných a 101 příležitostných), 230 bývalých kuřáků a 832 nekuřáků. Výsledky dotazníkové části studie dokumentují odlišný přístup kuřáků a nekuřáků k orálnímu zdraví. Kuřáci jevili horší hygienické návyky, neúčastnili se preventivních prohlídek tak často jako nekuřáci a byli rovněž méně často spokojeni se vzhledem svého chrupu. Subjektivně vnímali zdravotní stav svých zubů, dásní a ústní sliznice jako horší v porovnání s nekuřáky. Informovanost o negativním vlivu kouření na orální zdraví byla u kuřáků statisticky významně nižší než u nekuřáků. Výsledky studie dokazují nutnost důslednějšího prosazování protikuřácké intervence v praxi zubních lékařů.
The aim of the study was to evaluate the different attitudes of smokers and non-smokers to the oral health. A cross-sectional population-based study was conducted on an adult sample of the Czech population. The study consists of two parts — self-reported questionnaire inquiry and clinical examination. Respondents were chosen randomly, age being the only excluding criterion. Statistical analyses were performed using NCSS 2004 software. Our set consisted of 1,473 respondents (694 males and 779 females) aged 30-69 years. There were 411 current smokers (310 regular and 101 occasional), 230 former smokers and 832 non-smokers among them. The results of the questionnaire part of the study revealed a different approach to oral health by smokers and non-smokers. Smokers exhibited poorer hygienic habits and did not take part in preventive check-ups as often as non-smokers. They were also less satisfied with the appearance of their dentition. Subjectively they apprehended the health status of their teeth, parodont and oral mucosa as worse compared to the non-smokers. Knowledge of the negative influence of smoking on dental health status was likewise lower in the group of smokers. The outcomes and conclusions of this study demonstrate that more rigorous assertion of anti-smoking intervention in dental practice is necessary.

This cross-sectional study was aimed at possible relationships between tobacco habits and selected behavior characteristics in an adult sample from India. Contemporaneous clinical examination comprised an intra-oral examination with specific emphasise to dental caries status in the form of DMFT (Decayed, Missing, and Filled Teeth) index. The study comprised 805 subjects in the age group from 30 to 69 years (72% of males and 28% of females). The participants were divided into regular smokers, occasional smokers, ex-smokers, tobacco chewers and non-tobacco users. The highest prevalence of oral mucosal lesions were found in tobacco chewers (22.7%) followed by regular smokers (12.9%), occasional smokers (8.6%), ex-smokers (5.1%) and non tobacco users (2.8%) (p < 0.001). The mean number of decayed teeth was highest in tobacco chewers (6.96) followed by regular smokers (6.44) and ex-smokers (5.5) (p < 0.001). The mean number of missing teeth was highest in the group of regular smokers (1.9) and lowest in non-tobacco users (1.53), but the results were not statistically significant (p = 0.529). The mean number of filled teeth were highest in the group of tobacco chewers (3.67) followed by regular smokers (3.29) (p < 0.001). DMFT value of tobacco chewers, regular smokers and ex-smokers is higher when compared to non-tobacco users (p < 0.001). The study documents that chewing tobacco and smoking can present significant risk factors for dental caries. However, the conclusions are burdened by some limitations. Further studies for investigation of the effect of tobacco using on dental caries are needed.

Tobacco is one of the most important risk factors for oral diseases, including oral cancer, oral mucosal lesions and periodontal diseases. There is substantial evidence suggesting that the risk of oral diseases increases with frequent use of tobacco and that quitting smoking results in reduced risk. In this article, the influence of cigarette smoking on the periodontium will be discussed, giving importance to the effects on immune responses, alveolar bone loss, periodontal pathogens and briefly outlining the negative effects of smoking on wound healing and periodontal treatment procedures.
