Exocrine pancreatic insufficiency (EPI), an important cause of maldigestion and malnutrition, results from primary pancreatic disease or is secondary to impaired exocrine pancreatic function. Although chronic pancreatitis is the most common cause of EPI, several additional causes exist. These include pancreatic tumors, pancreatic resection procedures, and cystic fibrosis. Other diseases and conditions, such as diabetes mellitus, celiac disease, inflammatory bowel disease, and advanced patient age, have also been shown to be associated with EPI, but the exact etiology of EPI has not been clearly elucidated in these cases. The causes of EPI can be divided into loss of pancreatic parenchyma, inhibition or inactivation of pancreatic secretion, and postcibal pancreatic asynchrony. Pancreatic enzyme replacement therapy (PERT) is indicated for the conditions described above presenting with clinically clear steatorrhea, weight loss, or symptoms related to maldigestion and malabsorption. This review summarizes the current literature concerning those etiologies of EPI less common than chronic pancreatitis, the pathophysiology of the mechanisms of EPI associated with each diagnosis, and treatment recommendations.

We present a case of a fish bone impacted in the papilla of Vater resulting in dyspepsia and mild elevation in liver function tests, which was subsequently treated endoscopically. Fish bones are one of the most commonly encountered swallowed foreign bodies. However, involvement of the biliary tract, such as the one described by us, represents an extremely rare complication of fish bone ingestion. The diagnosis of a foreign body in the biliary tract can be difficult, and early endoscopic or surgical extraction may be required to avoid complications such as biliary stone formation, obstructive jaundice, cholangitis or cholecystitis, and/or biliary sepsis. Prompt endoscopic treatment can avoid severe biliary complications or surgical therapy.

Úvod: Krvácení do horního gastrointestinálního traktu (GIT) patří mezi život ohrožující stavy, které vyžadují aktivní přístup a dynamický management akutní péče. Gastroskopie představuje zásadní diagnostický a terapeutický krok. Bohužel mnoho menších nemocnic nemá možnost provést gastroskopii mimo pracovní dobu. Glasgow-Blatchford skóre (GBS) představuje nástroj umožňující odhad vývoje stavu pacienta s krvácením do horního GIT. Evropská guidelines doporučují ambulantní řešení pacientů s GBS rovno 0 nebo 1. Cílem naší studie bylo posoudit použitelnost GBS na české populaci a zhodnotit bezpečnost vyšší hranice GBS pro řešení pacientů v ambulantním režimu. Metody: Retrospektivní sběr dat pacientů, kteří byli hospitalizováni a endoskopováni v Nemocnici Boskovice pro symptomy krvácení z horního GIT od října 2018 do prosince 2019. Výsledky: Data hodnotící celkový průběh onemocnění naznačují, že optimální GBS pro stanovení pacienta s nízkým rizikem je ≤ 3, ale s přihlédnutím k endoskopickým nálezům lze za bezpečné pro ambulantní léčbu považovat GBS ≤ 2. ≥ 10 pak předpovídá závažný celkový průběh nemoci a závažné endoskopické nálezy. Závěr: Dle našich dat lze zvýšit hodnotu GBS na 2 pro ambulantní řešení pacientů, což by mohlo snížit délku hospitalizace a tlak na urgentní endoskopie. Tyto závěry je nutné ověřit dalším výzkumem ideálně multicentrickou prospektivní studií s velkým množstvím analyzovaných pacientů.
Introduction: Upper gastrointestinal (GI) bleeding is a medical emergency that requires rapid assessment and dynamic management. Gastroscopy represents a crucial part of the diagnostic and therapeutic process. Unfortunately, in many hospitals emergency endoscopy is not easily available outside working hours. The Glasgow Blatchford score (GBS) predicts the outcome of patients at present. The European guidelines recommend outpatient management for a GBS of 0 or 1. The aim of our study was to validate the applicability of GBS in a population of Czech patients and to evaluate whether extending the GBS allows for early discharge while keeping the patient safe. Methods: Retrospectively collected data of patients who underwent gastroscopy in the endoscopy ward of Boskovice Hospital for symptoms of upper GI bleeding between October 2018 and December 2019. Results: Data based on the overall course of the disease suggest that the optimal GBS for determining a low-risk patient is 3, but concerning the endoscopic findings, GBS ≤ 2 should be considered safe for outpatient management. A GBS ≥ 10 predicts a severe overall course of the disease and a severe endoscopic finding. Conclusion: According to our data, the GBS could be extended to 2 for safe outpatient management, which might reduce the length of stay at the hospital and the pressure for urgent endoscopies. Further studies with more patients are necessary.

Cystická fibróza (CF) je genetická choroba postihující řadu orgánů vč. orgánů trávicího traktu. Zatímco postižení plic je nejvíce život ohrožující, pankreas je jedním z prvních a nejvíce postižených orgánů. Mutace v genu pro transmembránový regulátor vodivosti (CFTR) vede k poklesu tvorby pankreatické šťávy a zvýšené koncentraci makromolekul, což ve svém důsledku vede k precipitaci v duktech s jejich následnou obstrukcí a poškozením. Výsledný klinický projev závisí na kombinaci jednotlivých mutací, potenciální přítomnosti mutací v dalších chorobu modifikujících genech a případně na přítomnosti faktorů prostředí. CFTR mutace se historicky člení do 5 tříd - závažné mutace (třídy 1- 3) a mírné mutace (třídy 4 - 5). Stupeň postižení funkce CFTR závisí na kombinaci mutací obou alel CFTR a je obecně určován mírnější z přítomných mutací. Většina pacientů s CF vykazuje exokrinní pankreatickou insuficienci již od časného dětství, protože ke ztrátě funkčního parenchymu dochází již v průběhu nitroděložního vývoje či krátce po porodu. Tito pacienti pouze vzácně trpí pankreatickými komplikacemi typu rekurentní akutní pankreatitidy či chronické pankreatitidy, které se naopak mohou vyskytnout u menšiny pacientů s určitým stupněm reziduální pankreatické exokrinní funkce. Mutace v CFTR bývají nacházeny častěji též ve skupině pacientů s idiopatickou a alkoholickou pankreatitidou, nicméně důkazy nejsou jednoznačné. U pacientů s idiopatickou formou bývá též nalézána kombinace s mutacemi v genu pro pankreatický sekreční inhibitor (SPINK-1) a přítomnost různých faktorů prostředí. Malnutrice přímo souvisí se zhoršením prognózy pacientů s CF, a podávání trávicích enzymů proto tvoří základní prvek léčby CF. Snižování kyselosti žaludeční šťávy a suplementace vitaminů pak představují důležité pomocné nástroje léčby.
Cystic fibrosis (CF) is a genetic disease affecting many organs, including the gastrointestinal tract. While the pulmonary damage is the most life threatening, the pancreas is one of the first organs affected by CF and one of the most strongly affected. Mutation in the CF transmembrane conductance regulator (CFTR) gene results in a reduced volume of pancreatic juice and hyperconcentration of macromolecules leading to precipitation in the duct lumina, causing obstruction and damage. The clinical presentation of individual cases depends on a combination of different CFTR mutations, the potential presence of modifier gene mutations and environmental factors. CFTR mutations are historically divided into 5 classes - severe mutations (classes 1 - 3) and mild mutations (classes 4 - 5). The CFTR functional status depends on the combined effects of both CFTR allels and the severity of the phenotype depends on the milder mutation. The majority of CF patients exhibit exocrine pancreatic insufficiency in early childhood because functional acinar tissue has been lost in utero or soon after birth. These patients rarely suffer from pancreatic complications such as recurrent acute pancreatitis and/or chronic pancreatitis which, however, can occur in the minority of patients who possess residual pancreatic exocrine function. CFTR mutations are found more frequently in idiopathic and alcoholic chronic pancreatitis but the data is conflicting. A combination with serine protease inhibitor Kazal-type 1 (SPINK-1) mutations can be found in the idiopathic chronic pancreatitis group, as well as the presence of environmental factors. Malnutrition is directly related to a worse prognosis of CF patients and the delivery of active digestive enzymes is a cornerstone of treatment, with acid supression and vitamin supplementation playing an important additional role.
Pancreatic ductal adenocarcinoma (PDAC) is one of the most lethal solid malignancies with increasing incidence. The poor prognosis is due to the aggressive nature of the tumor, late detection, and the resistance to chemotherapy and radiotherapy. A radical surgery procedure is the only treatment that has been shown to improve the 5-year survival rate to 20-25%. However, the majority of patients (80-85%) are diagnosed with locally advanced or metastatic disease and just 15-20% patients are diagnosed in an early stage allowing them to undergo the potentially curative surgical resection. The early detection of PDAC without the use of invasive methods is challenging and discovery of a cost-effective biomarker with high specificity and sensitivity could significantly improve the treatment and survival in these patients. In this review, we summarize current and newly examined biomarkers in early PDAC detection.

Solidní ložiska pankreatu představují diferenciálně diagnostický problém s velkým množstvím maligních a benigních příčin. Metastázy jiných malignit představují méně než 5 % pankreatických lézí, z nichž metastázy melanomu patří mezi ty vzácnější. Diagnóza maligního melanomu je nejčastěji stanovena vyšetřením povrchu těla. Výjimky představují oční a slizniční forma melanomu, případy se spontánní regresí primárního ložiska a případy opomenutí ložiska při nedostatečně pečlivém vyšetření. Předkládáme případ pacientky se symptomatickým ložiskem v hlavě pankreatu, které bylo diagnostikováno jako metastáza melanomu za pomoci endosonografie s tenkojehlovou aspirační biopsií. Následné pečlivé fyzikální vyšetření odhalilo primární ložisko melanomu na pravé paži. V případě, že by vyšetřování začalo pečlivým fyzikálním vyšetřením, mohla být diagnóza melanomu stanovena dříve, bez nutnosti použití nákladných a invazivních vyšetřovacích modalit jako např. endosonografie s tenkojehlovou biopsií. Naše kazuistika tak poukazuje na důležitost sebevyšetřování kůže a vyšetřování kůže lékařem, počínaje praktiky. Vyšetření kůže by nemělo být opomíjeno ani v rámci diferenciální diagnostiky ložisek pankreatu, jelikož metastáza maligního melanomu je její nedílnou součástí.
The differential diagnosis of solid pancreatic masses is notably problematic, given the broad spectrum of possible malignant and benign etiologies. Metastases of other malignancies constitute less than 5% of pancreatic lesions, with metastases of melanoma being among the least common. Melanoma is generally diagnosed via careful examination of the body surface. Exceptions to this include ocular and mucosal forms, cases with spontaneous regression of the primary site, and cases missed because of insufficient physical examination. We present a case of a symptomatic solid pancreatic mass, diagnosed by endosonography with fine needle aspiration biopsy, as a rare metastasis of malignant melanoma. The primary skin tumor was thereafter detected after careful physical examination. Timely skin examinations could lead to earlier diagnosis without the need for expensive and invasive procedures such as endosonography with fine needle aspiration biopsy. This case report stresses the general importance of careful skin self-examinations and skin examinations by physicians, beginning with general practitioners. This case also highlights the potential importance of performing a skin examination (an inexpensive and simple tool) when investigating pancreatic masses, as metastatic melanoma should always be considered a possibility during differential diagnosis.
