(1) Objectives: Intestinal failure in home parenteral nutrition patients (HPNPs) results in oxidative stress and liver damage. This study investigated how a high dose of fish oil (FO) added to various lipid emulsions influences antioxidant status and liver function markers in HPNPs. (2) Methods: Twelve HPNPs receiving Smoflipid for at least 3 months were given FO (Omegaven) for a further 4 weeks. Then, the patients were randomized to subsequently receive Lipoplus and ClinOleic for 6 weeks or vice versa plus 4 weeks of Omegaven after each cycle in a crossover design. Twelve age- and sex-matched healthy controls (HCs) were included. (3) Results: Superoxide dismutase (SOD1) activity and oxidized-low-density lipoprotein concentration were higher in all baseline HPN regimens compared to HCs. The Omegaven lowered SOD1 compared to baseline regimens and thus normalized it toward HCs. Lower paraoxonase 1 activity and fibroblast growth factor 19 (FGF19) concentration and, on the converse, higher alkaline phosphatase activity and cholesten concentration were observed in all baseline regimens compared to HCs. A close correlation was observed between FGF19 and SOD1 in baseline regimens. (4) Conclusions: An escalated dose of FO normalized SOD1 activity in HPNPs toward that of HCs. Bile acid metabolism was altered in HPNPs without signs of significant cholestasis and not affected by Omegaven.
Volba správné metody léčby v případě litiázy střední velikosti (10–20 mm) není jednoduchá. Při využití extrakorporální rázové vlny může dojít ke vzniku velkého počtu fragmentů, které mohou obturovat močové cesty. Flexibilní ureterorenoskopie může představovat velmi dlouhý a nepřehledný výkon s rizikem infekčních komplikací. Perkutánní litotrypse (PCNL) je velmi efektivní, ale při využití standardního in strumentária 24–30 CH se však jedná o poměrně invazivní přístup s rizikem krvácení a delší dobou hospitalizace. Jako výborná metoda léčby litiázy střední velikosti se dle našich zkušeností jeví mini‑PCNL s velikostí přístupového kanálu 16 CH a využití takzvaného "Bernoulliho" nebo "vacuum cleaner" efektu k výplachu fragmentů z dutého systému ledviny, bez nutnosti použití dalších nákladných extrakčních nástrojů. Na videu bychom rádi prezentovali naše zkušenosti s mini‑PCNL pomocí instrumentária MIP od společnosti Karl Storz. Výhodou tohoto systému je malý přístupový kanál s minimální krevní ztrátou při výkonu a možnost jednostupňové dilatace, při které nedochází k traumatizaci dutého systému ledviny. Pro fragmentaci litiázy lze s velmi dobrým efektem využít standardního Ho:YAG laseru. Tuto techniku léčby jsme zatím použili u sedmi pacientů s litiázou průměrné velikosti 12 mm. Celková doba výkonu včetně zavedení ureterálního katétru a přetočení pacienta do pronační polohy byla v průměru 68 minut. Pouze u jednoho pacienta jsme časně po výkonu na CT pozorovali reziduální fragmenty do 2 mm. Ostatní pacienti byli ihned po výkonu bez reziduální litiázy. U žádného z pacientů nedošlo k poklesu hematokritu, který by si vyžádal podání krevní transfuze. Mini‑PCNL s velikostí přístupového kanálu 16 CH a využitím „Bernoulliho efektu“ pro evakuaci fragmentů z dutého systému se jeví jako velmi bezpečná a efektivní metoda léčby nefrolitiázy střední velikosti.
The choice of treatment method for medium size urolithiasis (10–20 mm) is not always easy. Extracorporal shockwave lithotripsy might create multiple large residual fragments, which might cause obstruction of the upper urinary tract. Flexible ureterorenoscopy might be a long, complicated procedure with the risk of infectious complications. Percutaneous lithotripsy (PCNL) is very effective, but the use of standard size PCNL instruments (24–30 CH) might be too invasive and require prolonged hospitalisation. Mini-PCNL (16 CH) with the use of „Bernoulli” or "vacuum cleaner" effect for irrigation of fragments from the upper tract seems to be an excellent choice of treatment. It is fast, effective and it does not require use of any expensive extraction devices. We would like to present our experience with mini-PCNL using Karl Storz MIP instruments. The advantage of this method is small diameter access, minimal blood loss and single step dilatation, without traumatisation of the upper urinary tract. The fragmentation is easily performed by standard Ho:YAG laser. We have used this technique in seven patients with the average stone size of 12 mm. Total procedure time, including insertion of ureterical catheter and rotation of the patient into the prone position was 68 minutes on average. We identified residual fragments smaller then 2 mm on a CT scan early after procedure in one patient. All the other patients were stone free right after the surgery. The procedure did not lead to significant blood loss, which would require blood transfusion in any of the patients. Mini-PCNL with a 16 CH access tract and the use of "Bernoulli effect" for fragment evacuation, is a safe and very effective treatment method for medium size stones.

Long-chain n-3 polyunsaturated fatty acids modulate immune cell functions. The primary objective of this study was to evaluate the impact of different lipid emulsions (LEs) with supplemented doses of fish oil (FO) on serum cytokine concentration and in vitro cytokine production in patients with intestinal failure on home parenteral nutrition (HPNPs). We hypothesized that FO supplementation would diminish lipopolysaccharide (LPS)-stimulated cytokine production. Twelve HPNPs receiving Smoflipid for at least 3 months were given FO (Omegaven) for a further 4 weeks. After this cycle, the patients were randomized to subsequently receive 1 cycle with Lipoplus and 1 cycle with ClinOleic for 6 weeks or vice versa plus 4 weeks of added Omegaven after each cycle in a crossover design. Comparison of the baseline LE regimens showed lower LPS-stimulated production of IL-1β in the HPNPs on Lipoplus than on the Smoflipid and ClinOleic regimens, as well as lower IL-8 compared to the Smoflipid regimen. Omegaven reduced IL-8 concentration in serum under the Lipoplus regimen and diminished LPS-stimulated production of IL-1β under the Smoflipid and ClinOleic. IL-6 and TNF-α production was depressed only in those on Smoflipid. Irrespective of the LE used, the HPNPs compared to the healthy controls showed higher IL-6, IL-8, and TNF-α concentrations in serum and LPS-stimulated production of IL-6 as well as lower n-6/n-3 long-chain polyunsaturated fatty acids in the erythrocyte phospholipids. LPS-stimulated production of IL-6 correlated negatively with the parenteral dose of eicosapentaenoic acid + docosahexaenoic acid. In conclusion, FO-supplemented parenteral nutrition suppresses in vitro cytokine production.

Cíl: Zhodnocení perioperačních funkčních a onkologických výsledků laparoskopických nefron‑šetřících operací pro nádor ledvin. Materiál a metoda: Hodnoceny jsou údaje z prospektivně vedené databáze jednoho centra z období od 1/2013 do 6/2018. Výkony prováděli tři operatéři. Finálně bylo vyhodnoceno 174 pacientů. Uvedený počet výkonů zahrnoval také jeden oboustranný výkon, děvet vícečetných jednodobých resekcí (2-5 lézí) a v osmi případech resekce solitární ledviny. Celkem bylo odstraněno 190 ložisek (vlevo 102x, vpravo 88x), u 64 žen a 110 mužů. Mediány souboru (IQR = interkvartilové rozpětí): věk 64 (55-70) let, index komorbidit Charlesonové 3 (2-4), kreatinin 78 (68-95) μmol/L, velikost léze 27 (20-35) mm, PADUA skóre 8 (7-9). Výsledky: Délka výkonu medián 118 (IQR 88-150) min, krevní ztráty medián 150 (IQR 80-300) ml, výkon bez ischemie (IT) 51x/190 lézí, délka IT medián 15 (IQR 12-17) min, 2x proběhla konverze na otevřenou RL, 2x konverze na laparoskopickou nefrektomii. Během 30 dnů bylo zaznamenáno 44 komplikací Claviena‑Dinda (CD): stupeň 1-17x, 2-13x, 3-10x, 4-1x, 5-3x; tj. CD ≥ 3 u 8 % pacientů. Ve čtyřech případech bylo zjištěno symptomatické pseudoaneuryzma, které vždy vyřešeno selektivní embolizací. Pooperační hospitalizace byla s mediánem 6 (IQR 5-7) dní. Histologicky bylo 45 lézí benigních, 145x maligních, z toho 122x pT1a, 16x pT1b, 3x pT2a, 4x pT3a. Pozitivní okraj byl zaznamenám u 11 %. Pouze jedna pacientka podstoupila otevřenou re‑resekci, ostatní byli sledováni. Při sledování byla zaznamenána jedna lokální recidiva v ledvině (při R0 výkonu), 1x rychlá vzdálená progrese (mozkové metastázy) a 1x současně vzdálená (plíce) i lokální progrese v perirenálním tuku (při R0). Trifekta hodnocená dle Montsouris (R0 výkon + IT ≤ 25 min + absence komplikace CD ≥ 3) dosažena u 74,1 %, dle Khalifeha a kol. (R0 + IT ≥ 25 min + žádná komplikace) u 59,2 %, dle Porpiglii a kol. (R0, IT ≥ 20 min, absence komplikace CD ≥ 3) u 69 %. Závěr: Laparoskopická RL je standardním řešením solidních expanzí ledvin poskytující příznivé výsledky. Míra dosažení komplexního klinického úspěchu (= trifekta) byla srovnatelná s literárními údaji.
Aim: Assessment of perioperative and onco‑logical results of laparoscopic nephron‑sparing procedures for renal tumors. Material a methods: We evaluate data from a prospectively collected database in one center between 1/2013 and 6/2018. Operations were per‑formed by 3 surgeons. There were 174 patients available for final analysis. The cohort included also 1 one‑stage bilateral case, 9 cases of multiple one‑stage partial nephrectomy (PN) (2–5 lesions) and in 8 cases PN of solitary kidney. Altogether, 190 renal masses were resected (left side 102×, right side 88×), in 64 women and 110 men. Cohort medians (IQR = interquartile range) were: age 64 (55–70) years, Charleson comorbidity index 3 (2–4), creatinine 78 (68–95) μmol/L, lesion diameter 27(20–35) mm, PADUA score 8 (7–9). Results: Length of surgery median 118 (IQR 88–150) min, blood loss median 150 (IQR 80–300) ml, no warm ischemia (WI) used in 51 of 190 lesions, in other length of WI median 15 (IQR 12–17) min, in 2 patients conversion to open PN was needed and in 2 patients conversion to laparoscopic nephrectomy (1× bleeding; 1× renal vein tumor thrombus). There were 44 complications according to Clavien‑Dindo (CD) classification within 30 post‑operative days: 17× grade 1, 13× grade 2, 10× grade 3, 1× grade 4, 3× grade 5, i.e. CD ≥ 3 in 8 % of patients Symptomatic pseudoaneurysm was diagnosed in 4 pts – all treated by selective embolization. Post‑operative hospital stay was median 6 (IQR 5–7) days. Histology found 45 benign and 145 malignant lesions, of the latter 12 2× pT1a, 16× pT1b, 3× pT2a and 4× pT 3a. Positive margin rate was 11% Only 1 patient underwent new PN via open approach, others were monitored. We detected 1 local kidney recurrence (in R0 surgery), 1 rapid distant progression (cerebral metastases) and 1 combined local (in perirenal fat) and distant (lungs) recurrence (in R0 surgery). Trifecta based on Montsouris (R0 + WI ≤ 25min + absence of CD ≥ 3 complication) was 74,1%, based on Khalifeh et al (R0 + WI ≤ 25 min + no complication) was 59,2% and based on Porpiglia et al (R0 + WI ≤ 20 min + absence of CD ≥ 3 complication) was 69 %. Conclusion: Laparoscopic PN is a standard management option of solid renal masses providing favorable outcomes. Trifecta rate was comparable to published results. Cíl studie: Posoudit vliv malnutrice u pacientů s karcinomem pankreatu (PC) na míru oxidačního stresu a na antioxidační systém organismu. Typ studie: Observační, strukturálně vyvážená studie případů a kontrol. Název a sídlo pracoviště: IV. Interní klinika, 1. LF UK a VFN v Praze, U Nemocnice 2, 128 01 Praha 2, Česká republika Materiál a metody: Do studie bylo zařazeno 68 pacientů (M/F = 36/32) s PC, kteří byli podle indexu NRI (nutrition risk index) rozděleni do dvou skupin na pacienty se středně těžkou až těžkou malnutricí (PC-MAL) a pacienty s lehkou malnutricí či bez malnutrice (PC-NOR). Každá skupina čítala 34 osob (M/F = 18/16) a mezi skupinami nebyl signifikantní rozdíl ve věku. Dále byla do studie zařazena na základě věku a pohlaví spárovaná kontrolní skupina (CON). Sledovaným subjektům byly odebírány vzorky po celonočním lačnění a kromě základních klinických a biochemických parametrů byly stanovovány markery oxidačního stresu (konjugované dieny v precipitovaných LDL; CD/LDL a oxidované LDL; ox-LDL/LDL), aktivity antioxidačních enzymů a koncentrace redukovaného glutathionu (GSH). Ke statistickému zpracování výsledků byl použit program STATISTICA (Stat Soft, CZ). Výsledky: Výsledky naší studie potvrzují zvýšený oxidační stres u pacientů s PC a to zvýšenými hladinami ox-LDL/LDL a CD/ LDL v porovnání s CON (p < 0,01). Signifikantně vyšší hladiny těchto markerů měli pacienti s malnutricí než bez malnutrice. Pozorovány byly rovněž výrazné změny v antioxidačním systému u pacientů s PC; kteří oproti CON skupině měli sníženou aktivitu katalázy (CAT, p < 0,01) glutationperoxidázy (p < 0,01), arylesterázovou (PON-A) i laktonázovou aktivitu (PON-L) paraoxonázy (p < 0,01) a koncentraci redukovaného glutathionu (p < 0,001) a zvýšené hladiny sérového amyloidu A (SAA, p < 0,001). Ovlivnění aktivit CAT, PON-A a PON-L a hladiny SAA bylo signifikantně větší u pacientů s podvýživou oproti PC pacientům bez příznaků malnutrice. Závěr: V této studii bylo prokázáno prohloubení oxidačního stresu a výraznější ovlivnění funkce antioxidačního systému organismu pacientů vlivem malnutrice.
Objective: To assess the influence of malnutrition in patients with pancreatic carcinoma (PC) on the oxidative stress and antioxidant system. Design: Observation, matched case-control study. Settings: This study was conducted at the 4th Department of Internal Medicine of General University Hospital in Prague, U Nemocnice 2, 128 01 Prague 2, Czech Republic Material and methods: In to our study 68 patients (M/F = 36/32) with PC were included and divided according to the nutrition risk index into two groups – patients with moderate or severe malnutrition (PC-MAL) and mild or no malnutrition (PCNOR). In both groups there were 34 patients (M/F = 18/16) with no difference in age between both groups. Furthermore, group of 34 sex- and age-matched healthy controls (CON) were enrolled into the study. The samplings were taken after overnight fast and apart from basic clinical and biochemical parameters markers of oxidative stress (level of conjugated dienes in precipitated LDL, CD/LDL and oxidized LDL, ox-LDL/LDL), activities of antioxidant enzymes and concentration of reduced glutathione (GSH) were assessed. For all statistical analysis the statistical program STATISTICA (Stat Soft, CZ) was used. Results: In our study we confirmed increased oxidative stress in PC, with higher levels of ox-LDL/LDL and CD/LDL compared to CON (p < 0.01). Significantly higher levels of these markers were in patients with malnutrition then without malnutrition. We observed also changes in antioxidant system of PC patients – these patients had decreased activity of catalase (CAT, p < 0.01), glutathione peroxidase (p < 0.01), arylesterase (PON-A) and also lactonase activity (PON-L) of paraoxonase (p < 0.01) and concentration of GSH (p < 0.001) and higher levels of serum amyloid A (SAA, p < 0.001). The changes in CAT, PON-A, PON-L and SAA levels were significantly higher in PC patients with malnutrition then without. Conclusion: In this study we proved the deepening of oxidative stress and the strongly impaired function of antioxidant system in PC patients due to malnutrition.

BACKGROUND/AIM: To evaluate the diagnostic accuracy and prognostic performance of urinary and plasma levels of placental growth factor (PLGF) and provide their comparison with the results of vascular endothelial growth factor A (VEGF-A) in patients with primary and recurrent urinary bladder cancer. MATERIALS AND METHODS: The enzyme-linked immunosorbent assay (ELISA) was used to assess urinary and plasma concentrations of PLGF and VEGF-A in 240 individuals. RESULTS: PLGF levels in urine and plasma were significantly higher in patients with primary bladder cancer than in healthy individuals (p=0.003, p=0.005, respectively). Area under the curve (AUC) of urinary PLGF was 0.68; AUC of plasma PLGF levels was 0.65. Patients with the urine levels of PLGF higher than 82.33 pg/ml had three times higher risk of recurrence. In patients with recurrent bladder cancer, the urinary concentrations of PLGF did not significantly differ from the concentrations in patients without current disease (p=0.61). However, plasma PLGF levels were significantly higher in patients diagnosed with tumor recurrence (p=0.001); AUC of plasma PLGF levels was 0.69. Moreover, patients with plasma levels higher than 10.09 pg/ml had a five-times higher risk of future tumor recurrence. The diagnostic accuracy of PLGF was comparable with VEGF-A. CONCLUSION: From a clinical point of view, PLGF could be considered a valid diagnostic test for the detection of primary and recurrent bladder cancer. In patients with recurrent bladder cancer, plasma PLGF levels can differentiate individuals at risk of tumor recurrence.
