Cílem naší práce je prezentace prvních zkušeností s použitím nové parenterální hypolipidemické terapie v klinické praxi. Prezentujeme soubor pacientů, u kterých byla zahájena terapie inclisiranem a u kterých bylo provedeno sledování vývoje parametrů lipidového spektra od prosince 2021 do března 2023. Hypolipidemická terapie inclisiranem se v průběhu tohoto sledování ukázala jako efektivní a bezpečná s dobrou adherencí u sledovaných pacientů.
We present our first experience with the use of new parenteral hypolipidemic therapy in clinical practice. Our set of patients was started on hypolipidemic therapy with inclisiran and was followed by measuring the lipid spectrum parameters from December 2021 to March 2023. During this follow-up, hypolipidemic therapy with inclisiran proved to be effective and safe with good adherence to therapy.
Inklisiran je nové hypolipidemikum, založené na principu siRNA. Je podáván injekční formou 1x za půl roku k doposud užívané hypolipidemické léčbě. Inklisiran je zkoumán v rozsáhlém programu ORION. Popisujeme první zkušenosti s léčbou inklisiranem včetně kazuistiky pacientky s familiární hypercholesterolemií. Účinnost a snášenlivost inklisiranu při správné indikaci byla excelentní.
Inclisiran is a novel hypolipidemic drug based on the principle of siRNA. The drug is administered once a half year via subcutaneuos injection. Inclisiran is being studied in the ORION program. We describe a clinical case of a woman with familiar hypercholesterolemia. When indicated properly, the efficacy of inclisiran is excellent and the drug is well tolerated.

Inklisiran představuje novou možnost biologické léčby dyslipidemie využívající k potlačení produkce proprotein konvertázy subtilisin/kexin typu 9 (a tím k dosažení redukce hladin celkového, respektive LDL-cholesterolu) inhibici translace genetické informace pomocí cílené degradace specifické mRNA. Jedná se o revoluční technologii, která přináší možnost dlouhých aplikačních intervalů, což zvyšuje terapeutickou adherenci nemocných. Díky vysoce specifickému účinku se jedná o léčbu bezpečnou, která je velmi dobře tolerována. Inklisiran rozšiřuje stávající spektrum hypolipidemické léčby a umožnuje dosáhnout cílových hodnot LDL-cholesterolu většímu počtu vysoce rizikových pacientů.
Inclisiran represents a new option for the biological treatment of dyslipidemia, using to suppress the production of proprotein convertase subtilisin/kexin type 9 (thus achieving a reduction in total and LDL-cholesterol levels) by inhibiting the translation of genetic information through the targeted degradation of a specific mRNA. This is a revolutionary technology that brings the possibility of long application intervals, which increases the therapeutic adherence of patients. Thanks to its highly specific effect, it is a safe treatment that is very well tolerated. Inclisiran expands the existing spectrum of lipid-lowering treatments and allows a greater number of high-risk patients to reach LDL-cholesterol target values.

Neurological disorders are often debilitating conditions with no cure. The majority of current therapies are palliative rather than disease-modifying; therefore, new strategies for treating neurological disorders are greatly needed. mRNA-based therapeutics have great potential for treating such neurological disorders; however, challenges with delivery have limited their clinical potential. Lipid nanoparticles (LNPs) are a promising delivery vector for the brain, given their safer toxicity profile and higher efficacy. Despite this, very little is known about LNP-mediated delivery of mRNA into the brain. Here, we employ MC3-based LNPs and successfully deliver Cre mRNA and Cas9 mRNA/Ai9 sgRNA to the adult Ai9 mouse brain; greater than half of the entire striatum and hippocampus was found to be penetrated along the rostro-caudal axis by direct intracerebral injections of MC3 LNP mRNAs. MC3 LNP Cre mRNA successfully transfected cells in the striatum (∼52% efficiency) and hippocampus (∼49% efficiency). In addition, we demonstrate that MC3 LNP Cas9 mRNA/Ai9 sgRNA edited cells in the striatum (∼7% efficiency) and hippocampus (∼3% efficiency). Further analysis demonstrates that MC3 LNPs mediate mRNA delivery to multiple cell types including neurons, astrocytes, and microglia in the brain. Overall, LNP-based mRNA delivery is effective in brain tissue and shows great promise for treating complex neurological disorders.

Osteoarthritis (OA) is a frequent musculoskeletal disorder affecting millions of people worldwide. Despite advances in understanding the pathogenesis of OA, prognostic biomarkers or effective targeted treatment are not currently available. Research on epigenetic factors has yielded some new insights as new technologies for their detection continue to emerge. In this context, non-coding RNAs, including microRNAs, long non-coding RNAs, circular RNAs, piwi-interacting RNAs, and small nucleolar RNAs, regulate intracellular signaling pathways and biological processes that have a crucial role in the development of several diseases. In this review, we present current knowledge on the role of epigenetic factors with a focus on non-coding RNAs in the development, prediction and treatment of OA. Sdělení se věnuje mechanismu působení inclisiranu, jeho klinickému účinku a zamýšlí se i nad jeho postavením v současném spektru hypolipidemických přípravků.
Inclisiran represents a new option for the treatment of dyslipidemia. Its effect is a reduction of LDL (low-density lipoproteins) particles in the blood plasma due to the highly specific inhibition of the synthesis of the proprotein convertase subtilisin/kexin type 9 (PCSK9) directly in the cytoplasm of hepatocytes through interaction with the ribonucleic acid (RNA) used to translate the genetic information for the production of that protein. This review is devoted to the mechanism of action of inclisiran, its clinical effect and also reflects on its position in the current spectrum of hypolipidemic drugs.

Toscana virus (TOSV) (Bunyavirales, Phenuiviridae, Phlebovirus, Toscana phlebovirus) and other related human pathogenic arboviruses are transmitted by phlebotomine sand flies. TOSV has been reported in nations bordering the Mediterranean Sea among other regions. Infection can result in febrile illness as well as meningitis and encephalitis. Understanding vector-arbovirus interactions is crucial to improving our knowledge of how arboviruses spread, and in this context, immune responses that control viral replication play a significant role. Extensive research has been conducted on mosquito vector immunity against arboviruses, with RNA interference (RNAi) and specifically the exogenous siRNA (exo-siRNA) pathway playing a critical role. However, the antiviral immunity of phlebotomine sand flies is less well understood. Here we were able to show that the exo-siRNA pathway is active in a Phlebotomus papatasi-derived cell line. Following TOSV infection, distinctive 21 nucleotide virus-derived small interfering RNAs (vsiRNAs) were detected. We also identified the exo-siRNA effector Ago2 in this cell line, and silencing its expression rendered the exo-siRNA pathway largely inactive. Thus, our data show that this pathway is active as an antiviral response against a sand fly transmitted bunyavirus, TOSV.
