Myotónie a paramyotónia sú relatívne vzácne primárne svalové ochorenia, ktorých spoločnou črtou je patologicky zmenená dráždivosť membrány svalových vlákien. Nové poznatky z genetiky a molekulárnej biológie zásadným spôsobom objasňujú ich etiopatogenézu. Prevládajúca časť týchto nozologických jednotiek je podmienená poruchami štruktúry a funkcie iónových kanálov a patrí do skupiny chorôb s kontroverzným slovenským a českým názvom kanálopatie. V spolupráci s klinickou neurológiou umožňuje molekulárna biológia definovanie nových syndrómov a súčasne ponúka novú koncepciu klasifikácie týchto ochorení.

The objective of the study was to ascertain if the estradiol-progesterone substitution used for the preparation of endometrium which has been shown to elicit supraphysiological levels of circulating progesterone may also induce demonstrable changes in the ultrastructural morphology of human secretory endometrium which is characterised by three progesteron- dependent structures, the so called postovulatory triad: nuclear or nucleolar channel system, giant mitochondria and subnuclear glycogen. The study was approved by the institutional Ethic Committee and included 24 patients of one IVF centre who signed informed consent. Two sequentional endometrial biopsy specimens were taken on the 5th and 7th day after ovulation in one natural cycle and on the 5th and 7th day after the initiation of progesterone addition in another substituted cycles in the same patient. The surface and glandular epithelial cells were examined under transmission electron microscope (TEM). The appearance of nuclear channel system (NCS), giant mitochondria and glycogen was the same in both natural and substituted cycles, without abnormalities and deformations. Cells with NCS were found, mostly in clusters on discrete parts, in 55 to 70 % of ultrathin sections. The relative number of NCS positive specimens was not statistically different between the cycles or between the day +5 and the day +7 within different type of cycle. Some NCS positivity was detected more often (in 79 % and 66 %, resp.) in substituted cycles than in spontaneous ones. The percentage of NCS positive cells was also higher (median 17 % and 12 %, resp.) in substituted cycles but the differences were not statistically significant. With some caution regarding the inherent limited reproducibility of the TEM findings we can conclude that the oral hormonal substitution used in our setting does not markedly alter endometrial ultrastructural morphology.
Ligandem aktivované iontové kanály tvoří skupinu transmembránových proteinových struktur, které hrají klíčovou úlohu při vzniku a přenosu elektrických signálů. Pro zjištění mechanizmů, které se podílejí na vazbě ligandů, změnách pH a vysoké teploty vedoucích ke strukturálním změnám iontových kanálů, bylo v nedávné době využito nových technik. Cílem tohoto přehledu je sumarizovat komplexní mechanizmy podílející se na aktivaci glutamátových (NMDA, AMPA, kainátových) a vaniloidních (TRPV1) receptorů spojených s příslušnými iontovými kanály a rovněž shrnout výsledky studií provedených v nedávné době v Oddělení buněčné neurofyziologie Fyziologického ústavu AV ČR (http://www2.biomed.cas.cz /d331/index.html), které je součástí Centra neuropsychiatrických studií; http://www.pcp.lf3.cuni.cz/cns/.
Ligand-gated ion channels form a family of transmembrane proteins that play key role in generation and propagation of electrical signals. Recently, new techniques have become available for identification of the mechanisms involved in ligand binding, pH changes and elevated temperatures into structural rearrangements leading to channel gating. The aim of this article is to review the complexity of the gating mechanisms involved in the activation of glutamate (NMDA, AMPA, kainate) and vanilloid (TRPV1) receptor channels and summarize the recent results from studies performed in the Department of Cellular Neurophysiology of the Institute of Physiology AS CR (http://www2.biomed.cas.cz/d331/index.html) a member of the Centre of Neuropsychiatric Studies http://www.pcp.lf3.cuni.cz/cns/.
