BACKGROUND: The association between duration of smoking abstinence before non-small-cell lung cancer (NSCLC) diagnosis and subsequent survival can influence public health messaging delivered in lung-cancer screening. We aimed to assess whether the duration of smoking abstinence before diagnosis of NSCLC is associated with improved survival. METHODS: In this retrospective, pooled analysis of cohort studies, we used 26 cohorts participating in Clinical Outcomes Studies of the International Lung Cancer Consortium (COS-ILCCO) at 23 hospitals. 16 (62%) were from North America, six (23%) were from Europe, three (12%) were from Asia, and one (4%) was from South America. Patients enrolled were diagnosed between June 1, 1983, and Dec 31, 2019. Eligible patients had smoking data before NSCLC diagnosis, epidemiological data at diagnosis (obtained largely from patient questionnaires), and clinical information (retrieved from medical records). Kaplan-Meier curves and multivariable Cox models (ie, adjusted hazard ratios [aHRs]) were generated with individual, harmonised patient data from the consortium database. We estimated overall survival for all causes, measured in years from diagnosis date until the date of the last follow-up or death due to any cause and NSCLC-specific survival. FINDINGS: Of 42 087 patients with NSCLC in the COS-ILCCO database, 21 893 (52·0%) of whom were male and 20 194 (48·0%) of whom were female, we excluded 4474 (10·6%) with missing data. Compared with current smokers (15 036 [40·0%] of 37 613), patients with 1-3 years of smoking abstinence before NSCLC diagnosis (2890 [7·7%]) had an overall survival aHR of 0·92 (95% CI 0·87-0·97), patients with 3-5 years of smoking abstinence (1114 [3·0%]) had an overall survival aHR of 0·90 (0·83-0·97), and patients with more than 5 years of smoking abstinence (10 841 [28·8%]) had an overall survival aHR of 0·90 (0·87-0·93). Improved NSCLC-specific survival was observed in 4301 (44%) of 9727 patients who had quit cigarette smoking and was significant at abstinence durations of more than 5 years (aHR 0·87, 95% CI 0·81-0·93). Results were consistent across age, sex, histology, and disease-stage distributions. INTERPRETATION: In this large, pooled analysis of cohort studies across Asia, Europe, North America, and South America, overall survival was improved in patients with NSCLC whose duration of smoking abstinence before diagnosis was as short as 1 year. These findings suggest that quitting smoking can improve overall survival, even if NSCLC is diagnosed at a later lung-cancer screening visit. These findings also support the implementation of public health smoking cessation strategies at any time. PURPOSE: Stratifying patients with cancer according to risk of relapse can personalize their care. In this work, we provide an answer to the following research question: How to use machine learning to estimate probability of relapse in patients with early-stage non-small-cell lung cancer (NSCLC)? MATERIALS AND METHODS: For predicting relapse in 1,387 patients with early-stage (I-II) NSCLC from the Spanish Lung Cancer Group data (average age 65.7 years, female 24.8%, male 75.2%), we train tabular and graph machine learning models. We generate automatic explanations for the predictions of such models. For models trained on tabular data, we adopt SHapley Additive exPlanations local explanations to gauge how each patient feature contributes to the predicted outcome. We explain graph machine learning predictions with an example-based method that highlights influential past patients. RESULTS: Machine learning models trained on tabular data exhibit a 76% accuracy for the random forest model at predicting relapse evaluated with a 10-fold cross-validation (the model was trained 10 times with different independent sets of patients in test, train, and validation sets, and the reported metrics are averaged over these 10 test sets). Graph machine learning reaches 68% accuracy over a held-out test set of 200 patients, calibrated on a held-out set of 100 patients. CONCLUSION: Our results show that machine learning models trained on tabular and graph data can enable objective, personalized, and reproducible prediction of relapse and, therefore, disease outcome in patients with early-stage NSCLC. With further prospective and multisite validation, and additional radiological and molecular data, this prognostic model could potentially serve as a predictive decision support tool for deciding the use of adjuvant treatments in early-stage lung cancer.

BACKGROUND: Testing for epidermal growth factor receptor (EGFR) mutations is an essential recommendation in guidelines for metastatic non-squamous non-small-cell lung cancer, and is considered mandatory in European countries. However, in practice, challenges are often faced when carrying out routine biomarker testing, including access to testing, inadequate tissue samples and long turnaround times (TATs). MATERIALS AND METHODS: To evaluate the real-world EGFR testing practices of European pathology laboratories, an online survey was set up and validated by the Pulmonary Pathology Working Group of the European Society of Pathology and distributed to 64 expert testing laboratories. The retrospective survey focussed on laboratory organisation and daily EGFR testing practice of pathologists and molecular biologists between 2018 and 2021. RESULTS: TATs varied greatly both between and within countries. These discrepancies may be partly due to reflex testing practices, as 20.8% of laboratories carried out EGFR testing only at the request of the clinician. Many laboratories across Europe still favour single-test sequencing as a primary method of EGFR mutation identification; 32.7% indicated that they only used targeted techniques and 45.1% used single-gene testing followed by next-generation sequencing (NGS), depending on the case. Reported testing rates were consistent over time with no significant decrease in the number of EGFR tests carried out in 2020, despite the increased pressure faced by testing facilities during the COVID-19 pandemic. ISO 15189 accreditation was reported by 42.0% of molecular biology laboratories for single-test sequencing, and by 42.3% for NGS. 92.5% of laboratories indicated they regularly participate in an external quality assessment scheme. CONCLUSIONS: These results highlight the strong heterogeneity of EGFR testing that still occurs within thoracic pathology and molecular biology laboratories across Europe. Even among expert testing facilities there is variability in testing capabilities, TAT, reflex testing practice and laboratory accreditation, stressing the need to harmonise reimbursement technologies and decision-making algorithms in Europe.

Naša kazuistika sa zaoberá pacientom, ktorému u nás diagnostikovali nemalobunkový pľúcny karcinóm (NSCLC) s potvrdenou aktivačnou mutáciou v géne pre receptor epidermálneho rastového faktora (EGFR) a následnou liečbou inhibítory tyrozínkináz (TKI) 2. generácie a po potvrdení progresie NSCLC sa zaoberá následnou rediagnostikou a liečbou pacienta.
The case report describes a patient diagnosed with non-small cell lung carcinoma (NSCLC) with a confirmed activating mutation in the gene for the epidermal growth factor receptor (EGFR). The case report also describes the subsequent treatment with second generation tyrosine kinase inhibitors (TKIs), and after confirming the progression of NSCLC, it deals with subsequent re-diagnosis and patient treatment.

Kazuistiky popisují případy mladého pacienta a mladé pacientky, jimž byl diagnostikován generalizovaný nemalobuněčný plicní adenokarcinom s mozkovými metastázami a ALK translokací. Mozkové metastázy jsou detekovány u 30 % pacientů s ALK pozitivním NSCLC v době stanovení diagnózy nádorového onemocnění. Následná diskuze se zabývá intrakraniální účinností jednotlivých ALK inhibitorů.
The present case reports describe a young male and a young female patient diagnosed with generalized non-small cell lung adenocarcinoma with brain metastases and ALK translocation. Brain metastases are detected in 30% of patients with ALK-positive NSCLC at the time of cancer diagnosis. The discussion section deals with the intracranial efficacy of individual ALK inhibitors.

Nemalobuněčný plicní karcinom s prokázanou mutací v genu ALK pro nás již nějakou dobu není novinkou, ale terapeutické možnosti se v posledních letech významně rozšířily. K přípravkům vyšší generace patří v neposlední řadě brigatinib s recentně udělenou úhradou, který prokázal terapeutickou účinnost a dobrou snášenlivost, jak ukazuje i sepsaná kazuistika.
ALK positive variant of non-small cell lung cancer has been known for some time already but the therapy options expanded significantly in the recent years. Brigatinib achieved reimbursement in the first-line setup based on its efficacy and tolerability data reflected in the case report.

Většina pacientů s nemalobuněčným karcinomem plic přichází k lékaři s lokálně pokročilým či metastazujícím onemocněním. Metastazující onemocnění jsou indikována k systémové léčbě. Pokud je to možné a jsou-li u pacientů s neskvamózní histologií vyloučeny mutace receptoru epidermálního růstového faktoru (epidermal growth factor receptor, EGFR) a anaplastické lymfomové kinázy (anaplastic lymphoma kinase, ALK) (eventuálně další řídicí mutace), je imunoterapie indikována do první linie léčby. Kombinovaná imunoterapie nivolumab, ipilimumab podávaná s úvodními dvěma cykly chemoterapie doložila signifikantně delší přežití ve srovnání s chemoterapií. Podle úhradových podmínek v České republice je jedinou možností léčby u nemalobuněčného karcinomu plic s negativitou ligandu programované buněčné smrti 1 (programmed cell death ligand 1, PD-L1). U pozitivních expresorů PD-L1 1-49 % je možnou alternativou standardních režimů s pembrolizumabem.
Most patients with non-small cell lung cancer come to the doctor with locally advanced or metastatic disease. Metastatic diseases are indicated for systemic treatment. If possible and if epidermal growth factor receptor (EGFR) and anaplastic lymphoma kinase (ALK) mutations (or other driver mutations) are excluded in patients with non-squamous histology, immunotherapy is indicated as the first line of treatment. Combined immunotherapy of nivolumab, ipilimumab administered with the initial two cycles of chemotherapy demonstrated significantly longer survival compared to chemotherapy alone. According to the reimbursement conditions in the Czech Republic, it is the only treatment option for non-small cell lung cancer with programmed cell death ligand 1 (PD-L1) negativity. For positive PD-L1 expressors 1-49% it is a possible alternative to standard regimens with pembrolizumab.

Durvalumab představuje monoklonální protilátku, která stimuluje imunitní systém v rozpoznávání a usmrcování nádorových buněk. Durvalumab prokázal svoji účinnost i bezpečnost v léčbě lokálně pokročilého neresekabilního nemalobuněčného plicního karcinomu po kurativní chemoradioterapii jako udržovací léčba na základě výsledků klinické studie PACIFIC, jejíž aktualizovaná data byla publikována v tomto roce. Současně získal durvalumab registraci pro léčbu generalizovaného malobuněčného plicního karcinomu s chemoterapií a následně jako monoterapie na základě studie CASPIAN, jejíž aktualizovaná data jsou opět předmětem tohoto sdělení. Úhrada z veřejného zdravotního pojištění v této druhé indikaci v tuto chvíli bohužel chybí. Současně jsou prezentovány povzbudivé výsledky pacienta s malobuněčným plicním karcinomem léčeného durvalumabem v našem komplexním onkologickém centru.
Durvalumab represents a monoclonal antibody which stimulates the immune system in the detection and killing of the tumor cells. Durvalumab has proven safety and effectivity in the treatment of the locally advanced non-resectable non-small cell lung cancer after curative chemoradiotherapy as maintenance therapy based on the results of the clinical trial PACIFIC. The updated results of this trial were published this year. At the same time durvalumab gained registration for the treatment of the metastatic small cell lung cancer with chemotherapy and as maintenance therapy based on the results of the CASPIAN trial, which updated results are again presented in this paper. Unfortunately the health insurance reimbursement for the second mentioned indication is missing so far. There is also presented very promising case report with patient with small cell lung cancer treated with durvalumab.

Kazuistika popisuje případ 76letého pacienta s diagnózou generalizovaného nemalobuněčného plicního adenokarcinomu. Pro průkaz rezistentní mutace EGFR T790M byl pacient léčen EGFR tyrozinkinázovým inhibitorem osimertinibem.
The case report describes a 76-year-old patient diagnosed with metastatic non-small cell lung adenocarcinoma. The patient was treated with osimertinib, an EGFR tyrosine kinase inhibitor, after detecting the EGFR T790M mutation.
