Tuberkulóza je aj v súčasnosti pretrvávajúcim globálnym problémom postihujúcim milióny obyvateľov zeme. Pľúcna tuberkulóza tvorí 80−90% všetkých foriem tuberkulózy a jej prevalencia je v celosvetovom meradle na ústupe. Naproti tomu mimopľúcna forma ostáva viacmenej na stabilnej úrovni. Výskyt mimopľúcnej tuberkulózy je i na našom pracovisku ojedinelý. V článku informujeme o pacientovi, ktorý podstúpil resekciu pečene s nečakaným nálezom pečeňovej formy tohto ochorenia. Problémom tohto typu tuberkulózy je náročnosť v stanovení defi nitívnej diagnózy, keďže klinické symptómy a výsledky zobrazovacích vyšetrení môžu byť neisté a neurčité. Jej pomerne zriedkavý výskyt a fakt, že väčšina prípadov extrapulmonálnej tuberkulózy nie je zdrojom prenosu ochorenia, má za následok určité zníženie pozornosti zo strany odbornej verejnosti. Cieľom článku je upriamiť pozornosť na túto zriedkavú formu tuberkulózneho ochorenia. Jej predvídanie v rámci diferenciálnej diagnostiky rozhodne o správnosti stanovenia diagnostického a liečebného postupu.
Tuberculosis remains a global health issue aff ecting millions of people worldwide. Pulmonary form of tuberculosis presents 80−90% of cases with prevalence declining worldwide. On the other hand, extrapulmonary tuberculosis remains at the same level. Extrapulmonary tuberculosis cases are rare at our department. In our article we present a patient who underwent a liver resection with an unexpected fi nding of liver tuberculosis. In this form of tuberculosis it is difficult to establish a defi nite diagnosis since clinical symptoms and results of imaging tests maybe equivocal orin determinate. The rare occurrence and the fact that the majority of extrapulmonary tuberculosis cases are not transmitted to other patients lead to lower attention of health professionals. The goal of our article is to bring this rare form of tuberculosis to attention. Inclusion of this form of tuberculosis in differential diagnosis may help to establish correct a diagnosis and therapy.
Autoři předkládají dva případy mykobakteriálních infekcí. První kazuistika popisuje tuberkulózu vzácně lokalizovanou v játrech. Druhá kazuistika ukazuje kožní mykobakteriózu způsobenou druhem Mycohacterium chelonae a klade si za cíl upozornit na netuberkulózní, atypické neboli podmíněně patogenní mykobakterie, které mohou u imunosuprimovaných pacientů způsobit onemocnění různé lokalizace i závažnosti.
Two cases of mycobacterial infections are presented - one of rare hepatic tuberculosis and second of cutaneous mycobacteriosis caused by Mycobacterium chelonae. The aim of the second report is to point to nontuberculous, atypical, otherwise potentially pathogenic mycobacteria. These mycobacteria may cause diseases of various localization and severity in immunocompromised patients.

Autoři prezentují komplikovaný případ tuberkulózy u imunokompromitované pacientky po transplantaci jater na trvalé imunosupresivní léčbě. Pacientka byla přijata v klinicky závažném stavu na Kliniku nemocí plicních a TBC pro tuberkulózu peritonea a jater. V průběhu hospitalizace byla diagnostikována tuberkulóza střev, pravého oka, miliární tuberkulóza plic, tuberkulózní pleuritida. Při léčbě čtyřkombinací antituberkulotik se asi po 3 týdnech začal původně velmi závažný klinický stav postupně zlepšovat, kontrolní odběry na mykobakteria byly opakovaně negativní a pacientka byla propuštěna do ambulantní péče.
The authors present a complicated case of tuberculosis in an immunocompromised female patient after a liver transplantation with a permanent immunosuppressive therapy. The patient was admitted in serious condition to the Clinic of Respiratory Diseases and TBC due to tuberculosis of peritoneum and liver. During the hospitalization the tuberculosis of the intestine, the right eye, miliar pulmonary tuberculosis, and TBC pleuritis were diagnosed. On the treatment a quadrucombination of antituberculotics the originally serious health condition started to improve gradually, control samples were repeatedly negative for mycobacteria and the patient was discharged to the out-patient care.

BACKGROUND: Liver tuberculosis is a fairly rare manifestation of extra-pulmonary tuberculosis. We distinguish several forms of liver affection by tuberculosis. One of these is liver tuberculoma, the incidence of which is quite rare. The authors present a case of liver tuberculoma as an occupational disease. CASE REPORT: A 63-year-old veterinary doctor, who was diagnosed through a polymerase chain reaction with liver tuberculosis, was treated unsuccessfully with anti-tuberculosis drugs for a period of 8 months. After an earlier relapse of the focus in the liver it grew again and created an abscess (80 x 65 x 80 mm), together with a second satellite focus (20 mm). The patient was therefore indicated for a resection of 3 segments of the right liver lobe. The resection was without complications. The polymerase chain reaction, together with histology, proved the presence of a mycobacterium tuberculosis complex. Three year after the surgery, the patient is completely recovered, without any manifestations of the disease. CONCLUSION: Liver resection for liver tuberculoma is indicated in case of progression of the finding and long-term unsuccessful treatment with anti-tuberculosis drugs. It is a safe method with very good long-term results (Fig. 2, Ref. 12).
