OBJECTIVE: To assess the new quantitative bone scan parameters as markers of Charcot neuroosteoarthropathy (CNO) activity. RESEARCH DESIGN AND METHODS: Forty-two patients with acute (n = 21) and nonacute (n = 21) CNO underwent quantitative bone scanning. Patients with acute CNO were followed for 3-12 months and bone scans were repeated after treatment. New quantitative parameters were assessed and compared with markers of bone turnover and with skin temperature difference (STD). RESULTS: Significant correlations between quantitative bone scan parameters and bone turnover markers were observed (all P < 0.05). These parameters decreased after treatment of CNO, and its reduction to the baseline value correlated with differences of bone turnover markers and STD (all P < 0.05). CONCLUSIONS: Our study suggests that bone scanning can be used not only for diagnosis of CNO but also for monitoring disease activity by quantitative bone scan parameters.

PURPOSE OF THE STUDY: To evaluate the efficacy and usefulness of 99Tc-MDP bone scans for the diagnosis of complications associated with hydroxyapatite- coated total hip prostheses. MATERIAL AND METHODS: 99Tc-MDP bone scans of 42 hips were taken before revision surgery in 41 patients with ABG 1 prostheses. Of these, 26 patients (27 hips) were included in our study on the basis of the following criteria: at least 23 months between the index surgery and scintigraphic examination and the availability of good quality bone scans. The average patients' age at the time of index and revision operations was 45.3 years (range, 32-57; SD, 6.02) and 50.2 years (36-61; SD, 6.01), respectively. The average time between the index surgery and bone scintigraphy was 50.3 months (23-84; SD, 15.9). Two experienced specialists in nuclear medicine, who were unaware of any clinical conclusions, participated in the study. RESULTS: Pre- and intra-operative findings showed aseptic loosening in three cups (3/27, 11 %) and one stem (1/27, 4 %). Four hips were suspected to have an infection on the basis of the positive results of intra-operative culture. In the diagnosis of aseptic loosening, sensitivity, specificity, accuracy, and positive and negative predictive values of bone scintigraphy were 100 %, 57 %, 62 %, 23 %, and 100 %, respectively, for the ABG I cup and 100 %, 52 %, 54 %, 8 %, and 100 %, respectively, for the ABG I stem. These characteristics were 67 %, 96 %, 93 %, 67 %, and 96 %, respectively, for the ABG I cup, when plain X-ray was used for evaluation of aseptic loosening. Specificity, accuracy and negative predictive value of plain X-ray for ABG I stem aseptic loosening were 100 %, 96 %, and 96 %. Bone scintigraphy gave one false positive result of sepsis; on the other hand, none of the four hips with positive intraoperative culture was diagnosed as septic. DISCUSSION: The fact that 99Tc-MDP bone scintigraphy as a method of diagnosing periprosthetic complications has a high sensitivity and a relatively low specificity has often been discussed in the literature. Currently, there are only a few studies on bone scintigraphy investigations of hydroxyapatite-coated prostheses. Some authors report that a slight increase in peri-prosthetic radionuclide activity may persist for up to 8 years which can explain the problematic accuracy of the technique. CONCLUSIONS: 99Tc-MDP bone scintigraphy is an intermediate useful tool for detecting complications of total hip arthroplasty with hydroxyapatite- coating. A negative bone scan almost always suggested the absence of aseptic loosening. However, the value of a positive result was regarded as controversial for the diagnosis of both aseptic and septic complications.

The case of a 12-year-old girl with Jaffé-Lichtenstein-Uehlinger syndrome is presented. A bone scan pattern exhibits clinical symptoms, X ray images and histological findings are described in a case of polyostotic form of the fibrous dysplasia. A biopsy with histology was performed in consideration of bone scan findings and a confirmed final diagnosis.

The case of a vertebral osteoid osteoma localized in the arch and pedicle of the fourth lumbar vertebra in a young girl is described. The condition was manifested by the development of scoliosis, which was also the initial diagnosis. Because of painful muscle contractions and persistent pain at night, osteoid osteoma was suspected and then confirmed by scintigraphy. In order to minimize resection of the posterior stabilizing structures and to ensure removal of the complete nidus, surgery was carried out with the use of a surgical gamma probe. The method is based on administration of a radionuclide (99m Techneciumlabeled methylenediphosphonate) 2 to 3 h before the operation and intraoperative detection of the intensity of radiation directly in the operative wound. The values of impulses recorded over the nidus were more than ten-times higher than the background values, i.e., the impulses over the surrounding, unaffected bone. The nidus was exactly localized and removed as a whole. Posterolateral fusion without instrumentation was carried out. The patient was almost immediately free from night pain. After fixation with TLSO for 3 months, the patient started physical therapy. One year later she was without any complaints, fusion was healed and she had no relapse. The aim of this study was to draw attention to this less known and therefore less frequently employed method that may enable the surgeon to detect conditions otherwise difficult to localize. This method can also be used for other body sites if the lesion treated accumulates radionuclides.

The distribution and elimination of 99Tcm-complexes with methylene-diphosphonate (MDP) and with the calcium salt of diethylene-triamine-penta(methylene phosphonate) (DTPMP) were compared in rats. Both compounds exhibited high bone uptake and long-term retention of radioactivity in the skeleton. No significant accumulation of the complexes in non-osseous tissues was found. The pharmacokinetics of both chelates were similar, small differences in their distribution and elimination probably being due to different binding to plasma proteins. Two processes, namely bone uptake and kidney elimination, contributed to the disappearance of the complexes from the blood. The higher protein binding of 99Tcm-MDP probably caused its slower rate of urine elimination and insignificantly higher bone uptake compared with 99Tcm-DTPMP. On the other hand, the more rapid reduction in blood and muscle radioactivity with 99Tcm-DTPMP resulted in accelerated non-osseous tissue clearance compared with 99Tcm-MDP. This suggests that the time between administration and imaging may be shorter for 99Tcm-DTPMP than for 99Tcm-MDP. Furthermore, the much greater stability of 99Tcm-DTPMP may also reduce degradation of the complex and 99Tcm liberation in the body. For a general evaluation of both compounds, it will be necessary to determine lesion-to-bone ratios.

The effects of vitamin D-free diet with low Ca content on 24 hours' 99mTc-methylendiphosphonate retention (in the whole body and tibia), long-term the whole body 85Sr retention and 2 hours' 85Sr catchment in epiphysis, metaphysis and diaphysis of femur and tibia in rats (at the same time in certain experiments femur density and weight after incineration were established) were followed. The whole body 99mTc-MDP retention increases significantly as early as the third day of the diet administration, retention in tibia and the whole body is in further 2 experiments increased after 1, 2 and 4 weeks of the diet (density decreases from the first or the second week). The whole body 85Sr retention is significantly increased as early as after 2 days of the diet, the great difference against controls only slowly decreases after discontinuation of low-Ca diet. Further experiment followed 2 hours' 85Sr catchment in femur and tibia samples after 2, 5, 10 and 20 days of diet--catchment increase is significant after 20 days (bone density and ash weight is significantly decreased after 10 and 20 days of diet). Thus retention of both osteotrophic substances in bone is as a result of low Ca diet increased. This reaction is in case with 85Sr expected, in that of 99mTc-MDP it suggests a possible relation of the catchment and mineral component of osseous tissue. Increased retention of both the substances obviously fails to be related to local blood circulation which was in this situation in our previous experiments decreased.(ABSTRACT TRUNCATED AT 250 WORDS)

In this article, the authors present their experience with the diagnosis of bone metastases in patients with breast cancer using bone scintigraphy with 99mTc phosphonate and radioimmunological determination of carcinoembryonic antigen (CEA) and tissue polypeptic antigen (TPA). In a group of 395 patients, there was agreement between tumour markers (CEA, TPA) and the results of bone scintigraphy in 331 cases (84%)--negative in 193 cases (49%) and positive (i.e. in terms of bone scintigraphy results and the presence of at least one tumour marker) in 138 cases (35%). On the basis of this good agreement of the results between bone scintigraphy and CEA and TPA levels, the authors recommend the following algorithm in monitoring patients with breast cancer: follow-up of tumour markers at several-month intervals and, any increase in their levels will refer the patient to further examination using imaging techniques including bone scintigraphy.

We established blood flow (by means of 85Sr-microparticles) and 24 hours 99mTc-methylendiphosphonate (MDP) catchment and that of 85Sr (at different time intervals) in five portions of femur and 4 portions of tibia in rats. 1. We detected a marked differentiation in blood flow among all measured portions with the highest values in growth active distal femur metaphysis and proximal tibia metaphysis and with the lowest value in distal terminal tibia portion. 99mTc-MDP catchment is also highest in both metaphysis parts, and in the other parts it is differentiated similarly as the blood flow. 85Sr catchment in the early phase is very high in both metaphyses, but in several weeks it decreases to very low values; in the other parts the differences are small as well as the changes in the course of several weeks. 2. There is a considerable similarity in blood flow and 99mTc-MDP, in 85Sr catchment only high initial catchment in growth active metaphysis parts corresponds with blood flow. The time course of 99mTc-MDP and 85Sr catchment in the first hours after an intravenous injection is almost identical. The experiments showed great (up to 15-fold) local differences in blood flow among growth active and the other parts of femur and tibia in rats and a similar differentiation in 99mTc-MDP catchment, suggesting a certain relationship (but not quantitative dependency).

The present authors report on their own method of preparation of a frozen kit of methylenediphosphonate (MDP) which has been hitherto imported from the foreign currency area. The preparation after labelling with 99mTc serves for scintigraphy of the skeleton. The rate of MDP and the reducing reagent (SnCl2.2H2O)-5:1 proved to be the most advantageous. The preparation was compared with two more radiopharmaceuticals: hydroxyethylidenedisphosphanate (HEDP) and pyrophosphate (PYP). The results of a subjective evaluation of readability of scintigrams with grades 1-5 are unequivocally more favourable for MDP and HEDP (2.3 and 2.4) against PYP (3.1). MDP possessed the best rate of the number of impulses from the bone and the surrounding tissue--the background of the radiopharmaceuticals under study. The authors also examined the rapidity of uptake of MDP in the bone, kidney and its elimination from the plasma. The obtained results were in good agreement with commercial preparations tested at different laboratories. The use of MDP prepared in this manner is a contribution both to economy and clinical practice.
