Diuretics belong into the group of basic antihypertensive drugs. A number of clinical trials provided evidence that long-term administration of diuretics leads not only to effective control of hypertension but has at the same time a favourable impact on cardiovascular morbidity and mortality. Along with beta-blockers diuretics are recommended as drugs of first choice in patients with arterial hypertension. The author presents a brief account of different groups of diuretics incl. their indications.

Diuretics are basic antihypertensives indicated in essential and secondary hypertension. Their pharmacodynamics is well known, their therapeutic spectrum is wide, their tolerance is excellent, the cost/benefit is lowest among all contemporary antihypertensives and after re-evaluation of the safety of small doses, diuretics have come to their revival.

Diuretics are one of the basic groups of antihypertensive drugs. They have also certain limitations and undesirable effects which are better defined than in other more recently developed antihypertensives. Undesirable effects can be prevented by early combination of saluretics with K+ sparing diuretics or other antihypertensive drugs and by prevention of metabolic disorders (in particular insulin resistance and dyslipoproteinaemia) and by prescription of small doses. Only prospective intervention studies will show whether the advantages of other groups of antihypertensives, as compared with diuretics, will influence the prognosis of patients with essential hypertension in a substantial way. Several studies along these lines are under way.

1. Large concentrations (in mM) of ethacrynic acid (0.1), furosemide (1.0), theophylline (5.0) and osmotic diuretics (100.0) sharply increased the flux of water along an osmotic gradient through the frog urinary bladder wall. Spironolactone (0.1), and hydrochlorothiazide (5.0) showed only a weak action on osmotic permeability. MercusalR, clopamide and triamterene did not affect water transport. 2. The presence of 0.2--1.0 mU/ml vasopressin (ADH) after pretreatment with a diuretic did not result in summation of the effects of both drugs used. 0.01--0.1 mM ethacrynic acid and 0.01 mM MercusalR significantly decreased the reaction to ADH. 1.0 mM furosemide, 0.1 mM spironolactone, 0.01 mM clopamide and 0.8 mM acetazolamide did not change the reaction to ADH. A reduction in the cellular response to ADH and a decrease in the osmotic permeability of the tubular wall may be responsible in part for the diuretic action of ethacrynic acid and MercusalR.

After their introduction in 1959, thiazide diuretics have become the cornerstone in the managment of heart failure and hypertension. They prevent sodium reabsorption in the upper segment of the distal tubule, increase the diuresis and, by diminishing the intravascular volume, they reduce the preload. With vigorous diuresis, cardiac output may drop under the critical level, and hypovolaemia, hypotension, syncopes and dehydratation with severe water-mineral disturbance may occur. A similar disorder takes place after aggressive administration of loop diuretics which act by the same mechanism at the ascending limb of the loop of Henle. Moreover, any chronic treatment with saluretics triggers the contraregulatory activation of RAAS. Thus, the dosing should be cautiously titrated, and with diminishing efficacy the diuretic is to be replaced by another one, or a second drug is to be added. With any potent diuretic agent, electrolyte depletion may occur; therefore, minimal active doses should be applied, potassium-sparing diuretics added or a pause should be inserted after/before furosemide with spirolactone. Hyponatraemia and hypokalaemia are common, and in combined treatment with ACE-inhibitors and spirolactone, hyperkalaemia is frequent. With long-term treatment, a rebound period of sodium reabsorption follows, and the diuretic effect is reduced or lost. To overcome this ",diuretic resistance", the dietary sodium is to be restricted, physical effort avoided and salt retaining drugs (NSAIDs and similar) eliminated. In the article, a short review of basic physiology concerning the water-electrolyte balance and the distribution of body-water is summarized.
