Preinvasive lesions of the endometrium are histopathological and molecular alterations related to high-risk of uterine carcinoma development. Incidence has increasing tendency. Atypical endometrial hyperplasia is the preinvasive lesion of type I endometrial carcinoma, developing under a hyperestrogenic background. Carcinoma in situ of the endometrium is considered to be precursor lesion of type II endometrial carcinoma, first of all uterine serous carcinoma. Hysterectomy and bilateral oophorectomy is the main therapeutical modality in both preinvasive lesions of the endometrium. The hormonal therapy with progestogens is the possibility of fertility sparing approach in treatment of histological findings of atypical endometrial hyperplasia in young women. Even though preinvasive lesions of the endometrium are clearly defined, the possibilities of endometrial cancer screening are markedly limited.

OBJECTIVE: The verification of the predicive part of the vaginal ultrasonography in diagnostics of cancer of endometrium with the help of comparison of vaginal sonography finding (through the mediation EMI--endometrium/myometrium index) with relevant histological results. DESIGN: Clinical prospective study. SETTING: Department of Gynaecology and Obstetrics, Faculty of Medicine, Masaryk University and Faculty Hospital, Brno. METHODS: In this work we included 200 patients--on one hand the patients with the symptoms (pre- and perimenopauzal irregular bleeding and postmenopauzal bleeding), on the other hand the patients without symptoms, but with the suspicious ultrasonography find (suspicious or positive EMI). Each of the patients included in this study underwent the ultrasonography examination witch determined EMI and then the endometrial biopsy or the diagnostic hysteroscopy with sampling the material. The determined value EMI was compared with the histological result. RESULTS: On the basis of the results of this work we found out the possibility utilization of the vaginal ultrasonography as the prebioptic method in diagnostics cancer of endometrium through the mediation of determined EMI. The sensitivity of the method, however, is lowered by the next diseases of uterus (especially uterine myoma) that cause wrong negativity of EMI with connection with increased thickness of uterus in the region body of uterus. CONCLUSION: We can expect a high sensitivity of EMI in patients with diseases of endometrium, if there is by ultrasonography examination verified uterus with smooth border without uterine myoma. This examination acquires a special significance as the pre-bioptic and along with the preventive method the timely catch first of all in patients without symptoms, but with one or more risk factors for rise to cancer of endometrium.

BACKGROUND: Glucose transporter-1 (Glut-1) is a membrane glycoprotein that is, together with other glucose transporters, responsible for the regulation of glucose uptake. An increased expression of this protein seems to be a general feature of several malignant tumors that are able to reprogram their metabolism and switch from oxidative phosphorylation to aerobic glycolysis. METHODS: We performed comprehensive immunohistochemical analysis of Glut-1 expression in 336 endometrial samples, including tumors, nontumor lesions, and normal tissues. RESULTS: Expression of Glut-1 was found in 87% of endometrioid carcinomas (160/184 cases), 100% of serous carcinomas (29/29 cases), 100% of clear cell carcinomas (17/17 cases), 50% of polyps with atypical hyperplasia (8/16 cases), 12.5% of polyps with non-atypical hyperplasia (3/24 cases), 77% of hyperplasias with atypias (10/13 cases), 9% of hyperplasias without atypias (1/11 cases), 87% of secretory endometrium samples (13/15 cases), and in none of the nonsecretory endometrium samples (0/27 cases). In endometrioid carcinomas, Glut-1 was expressed in a marked geographical pattern. In nontumor lesions, its expression was more common in atypical hyperplasia and polyps with atypical hyperplasia compared with polyps with non-atypical hyperplasia and hyperplasias without atypia ( P = .00032). CONCLUSION: Our study confirms the high expression of Glut-1 not only in endometrioid carcinomas but also in other carcinomas of endometrium including clear cell and serous types. Glut-1 expression can be used as a surrogate marker in differential diagnosis between hyperplasia with and without atypia. Because of common Glut-1 expression in malignant tumors, therapeutic strategies influencing this protein or its signaling pathways can be beneficial.

OBJECTIVE: Solitary fibrous tumor (SFT) is an uncommon mesenchymal tumor. We present a case of SFT occurring in endometrium. DESIGN: Case report. SETTING: Department of Obstetrics and Gynecology, The Fingerland Department of Pathology, Medical Faculty of Charles University and Faculty Hospital Hradec Králové. CASE REPORT: We report a case of 57 years old woman with SFT arising from the endometrium, which was diagnosed and treated at our department. Histological finding was supported by typical immunohistochemical profile of the tumor. Aggressive nature of the tumor wasnt showed. The patient underwent abdominal hysterectomy with bilateral adnexectomy and is followed up in regular periods. CONCLUSION: Occurence of solitary fibrous tumor (SFT) in the female genital tract is extremely rare. To the best of our knowledge, we report the first case of SFT occurring in endometrium. Because of potencial aggressive behaviour of the tumor complete surgical excision and close follow-up is highly recommended.
