Cardiovascular diseases on an atherosclerotic basis are a serious health problem. Atherosclerosis is a multifactorial pathological process with complex pathogenesis. Its origin and development is conditioned by a set of several risk factors. Changes in the spectrum of lipoproteins are one of the well-known, serious risk factors for cardiovascular disease. The results of several authors showed, that the examination of the ratio of some lipoproteins can provide more accurate information about the cardiovascular risk in a given individual than the individual lipid parameters alone. Non-alcoholic fatty liver disease (NAFLD) is currently the most common form of chronic hepatopathy. The aim of our study was to examine lipoprotein ratios - atherogenic index of plasma (AIP) and butyrylcholinesterase/ HDL-cholesterol ratio - in patients with NAFLD and to evaluate their changes in relation to the severity of hepatic steatosis. The study group consisted of 64 patients with hepatic steatosis, 32 men and 32 women. Patients were examined for fatty liver index (FLI), with FLI values above 60.0 signaling the presence of steatosis. From the biochemical parameters, we examined the serum concentrations of total cholesterol, HDL- and LDL-cholesterol, triacylglycerols and butyrylcholinesterase activity in patients with fatty liver disease. As the results of our study showed, both basic lipid parameters - total cholesterol and triacylglycerols - were increased in patients with hepatic steatosis compared to the values in the control group (Chol: 3,59 ± 0,16 vs. 5,14 ± 0,14 mmol/l; TAG: 0,85 ± 0,06 vs. 1,86 ± 0,14 mmol/l). Both investigated lipoprotein ratios - AIP and the BChE / HDL-cholesterol ratio were also significantly increased in patients with liver steatosis (AIP: -0,191 ± 0,04 vs. 0,157 ± 0,04; BChE/HDL-C: 3171 ± 123 vs. 4602 ± 291). The value of the BChE/HDL-cholesterol ratio correlated well with the total cholesterol/HDL cholesterol ratio (correl.coeff. 0,802) as well as with the atherogenic index of plasma (correl.coeff. 0,702). The findings of elevated AIP and BChE/HDL-C ratios confirmed the hypothesis of an increased risk of cardiovascular disease in patients with fatty liver disease. The finding of a significant positive correlation between AIP and BChE/HDL-C on the one hand and fatty liver index (FLI) on the other hand suggests that cardiovascular risk increases with the severity of steatotic liver injury.

It is very well known from the old epidemiological population studies, that HDL-cholesterol (HDL-C) was in a negative association with coronary events risk. New studies have occurred during the last 10 years that the mentioned association has in fact the U-shaped curve, i.e. lower and higher levels of HDL-C then the optimal one are in an exponential increasing risk. The association of high HDL-C levels with cardiovascular risk is not explained clearly. There might be a hypothesis concerning a change of function of large HDL particles riched of cholesterol esters, which become to be cholesterol donors for the arteries. Proportions of unsaturated fatty acids of high density lipoprotein (HDL) lipids and their relationships to the HDL-cholesterol level were compared in hypo- and hyperalphalipoproteinemic subjects. Both groups did not differ in the level of serum cholesterol. However, hypoalphalipoproteinemia was associated with hypertriglyceridemia, higher HDL-triacylglycerol level, and higher proportion of HDL-18:1 (oleic acid) and lower proportions of HDL-18:2 (linoleic acid) and 20:4 (arachidonic acid) than hyperalphalipoproteinemia. The HDL-20:4 was the only fatty acid correlating (negatively) with HDL-cholesterol. However, HDL-18:1 correlated positively and HDL-18:2 negatively with HDL-triacylglycerols, lipids related to the fall of HDL-cholesterol. These results suggest 1) an antagonism of 20:4 and 18:2 as structural components of HDL lipids in relation to the HDL-cholesterol level, and 2) an association of replacement of HDL 18:2 by 18:1 with the disorder of plasma triacylglycerol metabolism.

BACKGROUND: Increased levels of plasma lipoproteins are among some of the modifiable risk factors for cardiovascular disease (CVD). Dietary changes and increased physical activity are the most powerful non-pharmacological interventions for achieving optimal plasma lipid levels. OBJECTIVES: To investigate the effect of an intensive short-term lifestyle intervention on plasma lipid trajectories in overweight non-diabetic females. MATERIAL AND METHODS: A total of 202 healthy overweight (body mass index (BMI) >27.5 kg/m2) females underwent an intensive short-term (ten-week) intervention (at least 4 units of one-hour exercise activity weekly at optimal energy intake) aimed at lowering body weight. Plasma lipid (total cholesterol (TC), low-density-lipoprotein cholesterol (LDL-C), high-density-lipoprotein cholesterol (HDL-C), and triglycerides (TG)) levels were examined at baseline and every 2 weeks over the course of the ten-week intervention. RESULTS: There was a significant decrease in BMI (Δ -4.7%, p < 0.001) and body weight (Δ -4.9%, p < 0.001) after the intervention. Positive changes (decreases) in TC (Δ -8%, p < 0.001), TG (Δ -9%, p < 0.001) and LDL-C (Δ -11%, p < 0.001) were observed immediately after 2 weeks, but levels did not decrease further thereafter. In contrast, HDL-C did not increase as expected: after 2 weeks of intervention, we observed a significant decrease of about 6% (p < 0.001) followed by a slow return to baseline values. But even after 10 weeks of intervention, HDL-C values had not reached the values detected at baseline. CONCLUSIONS: In overweight females, HDL-C decreased after short-term intensive lifestyle intervention. To confirm the protective effect of increased physical activity, plasma lipids need to be examined over a longer time period.
