Reduktivní plicní operace zprostředkují znovuzískání prostoru zabraného těžce nemocnou a výrazně expandovanou plicní tkání, který pak může vyplnit relativně nepoškozený okolní parenchym s efektním výsledkem zlepšení funkce. Operaci lze provést z mediální sternotomie, z jednostranné či oboustranné torakotomie nebo torakoskopicky, a to bud technikou resekce, laserem, anebo pneumoplikací. Podstatou úspěšnosti operace je resekce nebo vyřazení nemocné části plic, nikoliv operační přístup. Bez ohledu na použitou techniku má chirurgická léčba plicního emfyzému charakter paliace.
