Spondylartritidy (SpA), které jsou asociovány s m. Crohn, ulcerózní kolitidou nebo m. Whipple, se nazývají enteropatické a patří do velké skupiny séronegativních SpA. Na páteři jsou změny obdobné jako u ankylózující spondylitidy, z periferních kloubů jsou predilekčně zasaženy velké klouby (kyčle, ramena, kolena). Autoři demonstrují dva pacienty s rozvinutým klinickým a rentgenovým obrazem. 70letý muž s ulcerózní kolitidou měl na torakolumbální páteři obraz rozsáhlé diskovertebrální destrukce, s nehomogenní strukturou obratiu a na MR patologickou masu epidurálně, která byla pričinou spastické paraparézy. Histologický se jednalo jen o nespecifickou, granulomatózní zánětlivou tkáň. Při operaci byla prokázána navíc rozsáhlá instabilita ve všech segmentech hrudní a bederní páteře. U druhého 40letého nemocného s m. Crohn byl v popředí rozsáhlý nález na kyčelních kloubech, který si vyžádal totální endoprotézu oboustranně již v 35 letech věku. Rentgenový obraz m. Bechtěrev na krční páteři byl pokročilý, ale na torakolumbální páteři byly jen ojedinělé syndezmofyty. V diskusi jsou porovnávány nejnovější klinické a radiologické poznatky české a zahraniční literatury. Je pravděpodobné, že značná část tzv. nediferencovatelných spondylartritid má současně latentní formu m. Crohn. Velmi těsný vztah střevních zánětů a onemocnění kloubů a páteře je podle imunologických a jiných laboratorních vyšetření zjevný.

The cases of spondyloarthritis (SpA), which are associated with Crohn's disease, ulcerous colitis or Whipple's disease are called enteropathic and belong to a large group of serum-negative SpA. The changes on the spine are similar to those occurring in ankylosing spondylitis; in the peripheral joints the predilection localization involves the large joints (hip, shoulders, knees). The authors demonstrate two cases with advanced clinical and X-ray picture. The examination of a 70-year man with ulcerous colitis displayed a picture of extensive disco-vertebral destruction on thoracic and lumbar spine with non-homogenous structure of vertebrae and MR analysis showed a pathological mass in epidural localization, which was the cause of paraparesis. The histological examination revealed a non-specific, granulomatous inflammatory tissue. The operation uncovered an extensive instability in all segments of thoracic and lumbar spine. In the seconnd, 40-year patient with Crohn's disease, an extensive defect of the hip joints was found, which already required a total endoprothesis on both sides at the age of 35 years. The X-ray picture of Bechterev's dHisease on cervical spine revealed an advanced stage, whereas the thoracic-lumbar spine was affected by solitary syndesmophjiies. The discussion compares the most recent clinical and radiological knowledge published in the Czech and foreign literature. It is possible that a considerable part of cases of s.c. non-differentiated spondyloarthritis is accompanied by a latent form of Crohn's disease. A very close relation of intestinal inflammations to diseases of the joints and spine has become obvious from immunological and other laboratory examinations.
