Alergie na kešú – klinické poznámky a vyhodnocení souboru pacientů s IgE senzibilizací k hlavnímu alergenu Ana o 3 (2S albumin)

Článek zahrnuje poznatky o IgE mediované alergii na kešú ořechy s ohledem na pozorovaný nárůst nových případů. Zaměřuje se především na komponenty kešú a zvláště na klinicky rizikovou IgE senzibilizaci k zásobní bílkovině Ana o 3 (2S albumin). Alergická senzibilizace k této zásobní bílkovině semene je nejcitlivějším indikátorem primární alergie na kešú. Prezentujeme soubor 100 pacientů vyšetřených v letech 2013–2015 na našem pracovišti metodou ImmunoCAP s hodnotou sIgE rAna o 3 vyšší než referenční hodnota 0,35 kU/l. Alergická reakce na kešú proběhla u 10 dětských pacientů ve věku 2–7 let a měla v 80 % systémový charakter. Blíže jsou uvedeny výsledky ambulantního a labo-ratorního sledování těchto dětí až do konce roku 2018. Popisujeme také celý soubor jednoho sta pacientů s IgE senzibilizací k Ana o 3. Téměř všichni měli potravinovou alergii s atopickým ekzémem, alergickou rýmou, často také v kombinaci s průduškovým astmatem a systémové alergické reakce na potraviny byly zaznamenány téměř u třetiny pacientů.

The article includes findings on IgE mediated cashew nut allergy with respect to the observed increase in new cases. It focuses mainly on cashew components and especially on clinically risky IgE sensitization to storage protein Ana o 3 (2S albumin). Allergic sensitization to this seed storage protein is the most sensitive indicator of primary cashew allergy. We present a set of 100 patients examined in our workplace with ImmunoCAP method in 2013–2015 with sIgE rAna o 3 higher than the reference value of 0.35 kU/l. Allergic reaction to cashews occurred in 10 pediatric patients in age 2–7 years and was in 80 % systemic in nature. The results of the outpatient and laboratory monitoring of these children until the end of year 2018 are described in more detail. We also describe the entire set of one hundred patients with IgE sensitization to Ana o 3. Almost all had food allergy with atopic eczema, allergic rhinitis, often also combined with bronchial asthma and systemic allergic reactions to food have been reported in almost a third of patients.
