Zažitá strategie léčby pacienta s diabetes mellitus 2. typu je založena na schodovitém systémuintenzifikace antidiabetické terapie končící intenzifikovanou inzulínovou léčbou v systémubazál-bolus s eventuální přítomností metforminu a/nebo gliflozinu, pokud nejsou kontrain-dikace nebo jiný důvod absence těchto perorálních antidiabetik. V posledních letech se alenejen na základě klinických dat, ale také i díky naší osobní zkušenosti ukazuje, že existujei možná alternativa tohoto „konečného řešení“. Existuje možnost jakéhosi kroku „zpět“, kdytuto kombinaci můžeme změnit na stejně efektivní, nebo i dokonce úspěšnější léčbu, kteráje ale pro pacienta uživatelsky příjemnější a mnohdy i bezpečnější. Kombinace léčby bazálníminzulínovým analogem 2. generace a GLP-1 analoga s aplikací 1× týdně je právě takovýmtokrokem.

The established therapeutic strategy in the type 2 diabetes mellitus patients is based on a step-wise system of the intensification of insulin treatment. The intensified insulin basal-bolustreatment that is possibly boosted with metformin and/or gliflozin in the absence of con-traindications or other reasons for the exclusion of oral antidiabetic agents represents its finalstep. Based on clinical data and our own experience, it has been recently shown that there isa possible alternative to this “final solution”. There is an option of a “step-back” approach, i.e.to replace this combination by the equally effective or even more successful treatment that ismore comfortable and often even safer for patients. The combined treatment with the 2ndgeneration basal insulin analogue and GLP-1 analogue administered once a week representssuch self-same step.
