Cemiplimab v léčbě nemalobuněčného plicního karcinomu

Terapie lokálně pokročilého či metastazujícího nemalobuněčného plicního karcinomu zaznamenala v posledních letech zásadní posun, když se v léčebných algoritmech jednoznačně etablovala imunoterapie. Imunoterapie se dostala postupně až do první linie paliativní léčby, poté co prokázala benefit ve srovnání s konvenční chemoterapií. Monoterapie pembrolizumabem, kombinace pembrolizumabu s chemoterapií či nově kombinace ipilimumabu a nivolumabu s dvěma cykly chemoterapie získaly úhradu z veřejného zdravotního pojištění v první linii paliativní léčby. Nově do léčby plicních karcinomů promlouvá také cemiplimab. Klinická studie EMPOWER-Lung 1 jednoznačně prokázala přínos a bezpečnost cemiplimabu v první linii paliativní léčby u pacientů s lokálně pokročilým nebo metastazujícím nemalobuněčným karcinomem plic s expresí PD-L1 ≥ 50 %. Současně studie EMPOWER-Lung 3 prokázala benefit cemiplimabu v kombinaci s chemoterapií. Cemiplimab získal registraci v monoterapii a v současnosti očekáváme úhradu z veřejného zdravotního pojištění.

The treatment strategy of the non-small cell lung cancer has changed significantly over the last few years, when immunotherapy has established its position in the treatment. Immunotherapy has moved from the later palliative lines into the first line of the palliative treatment after immunotherapy has proven its benefit over the chemotherapy. Pembrolizumab monotherapy, combination of the pembrolizumab with chemotherapy and ipilimumab with nivolumab and two cycles of chemotherapy have all gained the insurance reimbursement for first line of the palliative treatment. Recently cemiplimab has established its role in the lung cancer treatment when clinical trial EMPOWER-Lung 1 proven the effectivity and safety of the cemiplimab in the first line of the palliative treatment of the patients with locally advanced or metastatic non-small cell lung cancer with PD-L1 expression 50 % or more. At the same time based on the EMPOWER-Lung 3 trial cemiplimab showed the benefit in the combination with chemotherapy. At this moment cemiplimab gained the registration as monotherapy treatment and health care reimbursement is expected.
