Pacientkou byla třináctiletá fena plemene bígl. Na jiném pracovišti jí byl klinicky před 3 roky diagnostikován Cushingův syndrom, diabetes mellitus a později tumor ve vagině a mnohočetné tumory mléčné žlázy. Na našem pracovišti byl v rámci předoperačního vyšetření před excizí tumoru ve vagině rentgenologicky zjištěn tumor v kaudálním laloku levé plíce, který se klinicky nemanifestoval. Při kontrolním vyšetření za 2,5 měsíce bylo patrné, že se tumor zvětšuje, a bylo doporučeno jeho chirurgické odstranění. Při thorakotomii byly nalezeny další dva tumory v témže laloku, tumor v kaudálním okraji levého kraniálního laloku, dva tumory na parietální pleuře a tumor v mediastinu. Histologicky byl diagnostikován pulmonální blastom. Imunohistochemické vyšetření prokázalo jen úzké spektrum cytokeratinů v epitelových buňkách. Silná pozitivita aktinu byla v epiteliích i v buňkách vazivové tkáně. V mezenchymálních buňkách byla silná pozitivita na vimentin. Primitivní buňky epiteloidního typu neexprimovaly neuron-specifickou enolázu. Reakce na průkaz proteinů p16 a p53 byly negativní.

The patient was a female beagle, 13 years old. At another clinic clinically was diagnosed 3 years ago Cushing’s syndrome, diabetes mellitus, and later a tumor in vagina and multiple mammary gland tumors. At our clinic was in frame of the presurgical examination before excision of the tumor in the vagina radiologically detected a tumor in the caudal lobe of the left lung, which did not manifest itself clinically. During the follow-up examination in 2.5 months, it was evident that the tumor was growing and its surgical removal was recommended. During thoracotomy, two more tumors were found in the same lobe, a tumor in the caudal margin of the left cranial lobe, two tumors on the parietal pleura and a tumor in the mediastinum. Pulmonary blastoma was diagnosed histologically. Immunohistochemical examination demonstrated only a narrow spectrum of cytokeratins in epithelial cells. Strong actin positivity was present in both epithelia and connective tissue cells. There was strong positivity for vimentin in the mesenchymal cells. Primitive cells of the epithelioid type did not express neuron-specific enolase. Reactions to the detection of p16 and p53 proteins were negative.
