Súhrn: Hidradenitis suppurativa (HS) je chronický imunitne mediovaný zápal pilosebaceóznej jednotky a apokrinných potných žliaz s tvorbou bolestivých uzlov, abscesov, hnisajúcich fistúl a jaziev predilekčne v intertiginóznych priestoroch. U pacientov s HS je zvýšená incidencia zápalových ochorení čreva (IBD – inflammatory bowel disease), čo poukazuje na ich vzájomné priesečníky v patogenéze dysregulácie vrodenej a adaptívnej imunity. Nedávno sa výrazne zvýšil záujem o črevný mikrobióm kožných ochorení, predmetom záujmu je aj jeho úloha v patogenéze HS. Podľa literárnych údajov črevná dysbióza podporuje produkciu prozápalových cytokínov a následne tvorbu HS lézií. Syndróm prerastania baktérií v tenkom čreve (SIBO – small intestinal bacterial overgrowth) je stav, ktorý sa vyznačuje zvýšenou koncentráciou baktérií hrubého čreva v tenkom čreve. Eredikácia SIBO bola napr. schopná zlepšiť kožné prejavy pri psoriáze, v zmysle zníženia indexu jej závažnosti. V tomto kontexte existuje hypotéza, že eredikácia SIBO môže mať priaznivý vplyv na závažnosť prejavov zápalových imunitne mediovaných kožných ochorení, vrátane HS.
Hidradenitis suppurativa (HS) is a chronic immune-mediated inflammatory skin disease. It is characterized by an inflammation that affects apocrine gland bearing skin in axillae, groin and under the breasts with the formation of painful nodules, abscesses and purulent discharge, sinuses and scaring. Patients with HS have an increased incidence of inflammatory bowel disease (IBD), which also indicates their cross-sections in the pathogenesis of dysregulation congenital and adaptive immunity. Recently, there has been a significant increase of interest in the intestinal microbiome and skin disease, as much as its role in the pathogenesis of HS. According to literature, intestinal dysbiosis promotes the production of proinflammatory cytokines and consequently the formation of HS lesions. Overgrowth of bacteria in the small intestine (SIBO) is a condition characterized by an increased concentration of bacteria in the large intestine. Eradication of SIBO was able to improve psoriasis measured by the psoriasis severity index. In this context, it is hypothesized that eredication of SIBO may have a beneficial effect on the severity of manifestations in inflammatory immune-mediated skin diseases, including HS.
Hidradenitis suppurativa (HS) je chronické imunitne mediované zápalové ochorenie kože, charakterizované inflamáciou folikulov, ich oklúziou a tvorbou bolestivých uzlov, abscesov a supurujúcich fistúl, predilekčne v intertiginóznych priestoroch. U žien býva často postihnutá oblasť prsníkov a submamárny priestor, intermamárna lokalita môže predstavovať zriedkavý fenotyp prezentácie ochorenia. HS má výrazný dopad na kvalitu života pacientov, preto včasné stanovanie správnej diagnózy a zahájenie liečby je kľúčové pre zásadné zlepšenie priebehu a prognózy ochorenia, ako aj predchádzaniu rôznych komplikácií. Prejavy HS môžu predstavovať diagnostickú dilemu v ambulantnej praxi. V teréne chronických zápalových kožných prejavov, ako sú nodozity či drenujúce fistuly, môžu byť maskované aj kožné metastázy karcinómu prsníka, prípadne spinocelulárny karcinóm kože. Liečba HS u pacientov s anamnézou onkologického ochorenia je terapeutickou výzvou vyžadujúcou si medziodborovú spoluprácu.
Hidradenitis suppurativa (HS) is a chronic, immune-mediated, inflammatory skin disease characterized by follicular inflammation and occlusion as well as the formation of painful nodules, abscesses, and suppurative fistulas, with a predilection for intertriginous areas. In women, the breast area and submammary space are often affected, with the intermammary area being a rare phenotype of presentation of the disease. HS has a significant impact on the quality of life of patients; therefore, timely establishment of the correct diagnosis and initiation of treatment are crucial to substantially improve the course and prognosis of the disease, as well as to prevent various complications. The manifestations of HS may represent a diagnostic dilemma in the outpatient practice. In the setting of chronic inflammatory skin manifestations, such as nodules or draining fistulas, cutaneous metastases from breast cancer and/or squamous cell carcinoma may be masked. The treatment of HS in patients with a history of oncological disease is a therapeutic challenge requiring interdisciplinary cooperation.

Hidradenitis suppurativa (HS) chronické zánětlivé a neinfekční onemocnění často začínající již v adolescentním věku. Bohužel však bývá často nesprávně stanovena diagnóza či je onemocnění odhaleno pozdě. Brzký počátek projevů může být spojen s vážnějším průběhem této choroby. Včasná diagnostika a zahájení vhodné terapie je klíčové pro zmírnění průběhu onemocnění a vede taktéž ke včasnému záchytu komorbidit. Tento článek shrnuje informace o epidemiologii, klinických projevech, komplikacích, komorbiditách a možnostech léčby HS u pacientů do 19 let.
Hidradenitis suppurativa is a chronic inflammatory disease, that often starts in pediatric population, unfortunately there is a gap between onset and diagnosing this disease. The early onset of symptom is usually associated with a more serious course of the disease. Early diagnosis let us start appropriate therapy as soon as possible and help us to prevent exacerbations, complications and comorbidities. This article summarizes information about the epidemiology, clinical manifestations, comorbidities and treatment for patients under 19 years of age.

Cíl práce: Hodnocení pacientů zařazených do registru biologické/cílené léčby BIOREP v České republice za rok 2022. Metody: Bylo provedeno retrospektivní hodnocení pacientů k datu 31.12.2022 zařazených do registru BIOREP s diagnózou psoriázy, hidradenitis suppurativa a atopické dermatitidy léčených biologickou/cílenou léčbou v daném období. Výsledky: K 31. 12. 2022 bylo v registru BIOREP evidováno celkem 5 335 pacientů. S psoriázou bylo sledováno 4 067 pacientů (76,2 %), s hidradenitidou 443 pacientů (8,3 %), s atopickou dermatitidou 808 pacientů (15,1 %) a zbývajících 17 pacientů (0,3 %) s použitím „off-label“ léčby. Ve skupině léčených pro psoriázu bylo 62,9 % mužů, průměrný věk pacientů ke konci roku 2022 byl 52,1 roku, u psoriázy byl průměrný věk v době diagnózy 25,2 roku a při nasazení první biologické/cílené léčby 46,1 roku. Průměrná doba od diagnózy do zahájení první cílené léčby byla 20,9 roku. Ke konci roku 2022 byli pacienti léčeni cílenou léčbou v průměru 5,2 roku. Přidružené onemocnění se vyskytovalo u 70,4 % pacientů, nejčastější byla metabolická/endokrinní onemocnění (38,6 %) a kardiovaskulární onemocnění (37,3 %). Ze sledovaných komorbidit byla nejčetnější hypertenze (35,1 %), dyslipidemie (26,6 %) a diabetes mellitus prvního nebo druhého typu (12,1 %). Celkem 76,1 % pacientů trpělo nadváhou či obezitou a v registru bylo 32,2 % kuřáků. Souběžnou psoriatickou artritidu mělo 32,2 % pacientů. Při zahájení léčby a při poslední návštěvě bylo průměrné PASI 18,7 resp. 1,8, BSA bylo v průměru 29,4 %, resp. 2,4 % a průměrné DLQI 16,1, resp. 1,5. Po 12 měsících léčby dosáhlo PASI50 celkem 91,8 % pacientů, PASI75 82,3 % pacientů, PASI90 63,8 % pacientů a PASI100 40,1 % pacientů. V roce 2022 nově zahájilo léčbu celkem 514 pacientů, na konci sledovaného roku 2022 bylo aktivně léčeno celkem 3 480 pacientů a nejčastěji používaným preparátem byla Humira (17,2 %), Skyrizi (13,6 %) a Cosentyx (13,2 %). S diagnózou hidradenitis suppurativa bylo v registru na konci sledovaného období celkem 443 pacientů, z toho 54,4 % mužů, průměrný věk byl 44,6 let. V době diagnózy bylo pacientům v průměru 33,0 let a v době nasazení v průměru 42,2 let. Průměrná doba od diagnózy do zahájení první biologické léčby byla 9,3 let a pacienti byli léčeni v průměru 2,4 let. V roce 2022 svou první biologickou léčbu zahájilo celkem 94 pacientů. Na konci sledovaného roku bylo léčeno celkem 367 pacientů, průměrná délka léčby jedním léčivým přípravkem byla 2,3 let a nejčastěji použitým preparátem byla Humira (84,7 %). S atopickou dermatitidou bylo v registru BIOREP sledováno k 31. 12. 2022 celkem 808 pacientů, z toho 53,5 % mužů. V roce 2022 byl průměrný věk pacientů 36,2 let. V době diagnózy onemocnění byl průměrný věk pacientů 5,0 roků a při nasazení první cílené léčby 35,2 roku. Průměrná doba od diagnózy do zahájení první cílené léčby byla 30,2 roku. Ke konci roku 2022 byli pacienti léčeni v průměru 1,5 roku. Souběžnou alergickou rýmu mělo 64,1 % pacientů, potravinovou alergii 57,7 % a alergické astma 44,3 %. Oční postižení se vyskytlo u 28,7 % pacientů, nejčastější byla atopická konjunktivitida (15,3 %). Průměrné EASI při zahájení léčby a na poslední návštěvě bylo 31,6 resp. 4,4, BSA bylo v průměru 56,4 %, resp. 8,4 % a průměrné DLQI 18,4, resp. 3,8. Po 12 měsících léčby dosáhlo EASI50 celkem 98,3 % pacientů, EASI75 91,6 % pacientů, EASI90 57,5 % pacientů a EASI100 10,7 % pacientů. V roce 2022 zahájilo cílenou léčbu celkem 306 pacientů. Na konci sledovaného roku bylo léčeno celkem 757 pacientů, nejvíce pacientů bylo léčeno přípravkem Dupixent (91,8 %) a průměrná délka léčby jedním léčivým přípravkem byla 1,4 roku. Závěr: BIOREP je prvním registrem pacientů biologické léčby psoriázy v zemích střední a východní Evropy. Analýza potvrzuje vysokou aktivitu sledovaných onemocnění podle objektivního hodnocení a značné negativní ovlivnění kvality života pacientů při zahájení cílené léčby s poklesem hodnocených parametrů při léčbě. Současně prokazuje významnou prevalenci souběžných onemocnění a rizikových faktorů a dlouhé období nedostatečné léčby před nasazením cílené léčby.
Background and objectives: Evaluation of patients included in the registry of biological/targeted therapy BIOREP in the Czech Republic. Methods: A retrospective evaluation of patients to date 31 December 2022 enrolled in the BIOREP registry was performed. The aim of our study was to evaluate patients on biological/targeted treatment in the given period in individual categories and to analyze the population of patients with psoriasis, hidradenitis suppurativa and atopic dermatitis. Results: As of 31 December 2022, a total of 5,335 patients were registered in the BIOREP registry: 4,067 (76.2%) patients with psoriasis, 443 (8.3%) with hidradenitis, 808 (15.1%) with atopic dermatitis and 17 (0.3%) were treated off label with targeted treatment. Of the psoriasis group, 62.9% of patients were men. The average patient age at the end of 2022 was 52.1 years, the mean age at the time of diagnosis was 25.2 years, and the age at first biological/targeted treatment was 46.1 years. In 2022, the average time from diagnosis to the introduction of targeted treatment was 20.9 years, the average duration of biological/targeted treatment was 5.2 years. The comorbidities occurred in 70.4% of patients. The most common disorder were metabolic/endocrine diseases (38.6%) and cardiovascular diseases (37.3%). From the individual comorbidities, hypertension (35.1%), dyslipidemia (26.6%) and diabetes mellitus (12.1%) were the most common. More than 76% of patients were overweight or obese and 32.2% of patients were smokers. Psoriatic arthritis was observed in 32.2% of patients. At the start of treatment and at the last visit, the mean PASI was 18.7 and 1.8, respectively, the mean BSA 29.4% and 2.4%, respectively and DLQI 16.1 and 1.56, respectively. After 12 months of treatment, a total of 91.8%; 82.3.%; 63.8% and 40.1% of patients achieved PASI50, PASI75, PASI90 and PASI100, respectively. In 2022, a total of 514 patients newly started treatment and at the end of year 2022, a total of 3,480 patients were treated, the most patients used Humira (17.2%), Skyrizi (13.6%) and Cosentyx (13.2%). As of 31 December 2022, there were a total of 443 patients with hidradenitis suppurativa in the registry of mean age 44.6 years and 56.13% were men. The mean age at the time of diagnosis was 33.0 years and the age at first biological treatment was 42.2 years. The average time from diagnosis to the start of the biological treatment was 9.3 years and patients were treated for an average of 2.4 years. In 2022, a total of 94 patients started the biological treatment. At the end of 2022, a total of 367 patients were treated and the average duration of treatment was 2.3 years, the most common medication was Humira (84.7%). At the end of 2022, there were a total of 808 patients with atopic dermatitis in the registry of mean age 36.2 years, 53.5% were men. The mean age at the time of diagnosis was 5.0 years and the age at first targeted treatment was 35.2 years. The average time from diagnosis to the start of the targeted treatment was 30.2 years and patients were treated for an average of 1.5 years. A total of 64.1% of patients had concomitant allergic rhinitis, 57.7% food allergy and 44.4% allergic asthma. Ocular comorbidities occurred in 28.7% of patients, the most common were atopic conjunctivitis (15.3%). At the start of treatment and at the last visit, the mean EASI was 31.6 and 4.4, respectively, mean BSA was 56.4% and 8.4%, respectively, and mean DLQI 18.4 and 3.8 respectively. In 2022, a total of 306 patients started the biological treatment. At the end of 2022, a total of 757 patients were treated and the average duration of treatment was 1.4 years, the most common medication was Dupixent (91.8%). Conclusion: BIOREP is the first registry of patients with psoriasis treated with biologics in Central and Eastern Europe. The analysis confirmed the high activity of diseases according to objective evaluation and the significant negative impact on the quality of life before initiation of targeted treatment, with a decrease in the evaluated scores during treatment. At the same time, it demonstrates a high prevalence of comorbidities and risk factors and a long diseases duration before the introduction of targeted treatment.

Z jednotlivých komorbidit byla nejčetnější hypertenze (35,6 %), dyslipidemie (26,7 %) a diabetes mellitus (11,7 %). Celkem 75,6 % pacientů trpělo nadváhou či obezitou a v registru bylo 31,5 % kuřáků. Souběžnou psoriatickou artritidu mělo 34,5 % pacientů. Při zahájení léčby a při poslední návštěvě bylo průměrné PASI 18,9, resp. 1,9 a DLQI 16,5, resp. 1,6. V roce 2021 nově zahájilo léčbu celkem 347 pacientů. K 31. 12. 2021 bylo v registru celkem 328 pacientů s diagnózou hidradenitis suppurativa, z toho 56,1 % mužů, průměrný věk byl 44,8 roku. V době diagnózy bylo pacientům v průměru 33,3 roku a v době nasazení v průměru 42,7 roku. Průměrná doba od diagnózy do zahájení první biologické léčby byla 9,5 roku, pacienti byli léčeni v průměru 2,1 roku. V roce 2021 svou první biologickou léčbu zahájilo celkem 73 pacientů. Na konci sledovaného roku bylo léčeno celkem 281 pacientů a průměrná délka léčby jedním léčivým přípravkem byla 2,1 roku. K 31. 12. 2021 bylo v registru BIOREP celkem 403 pacientů s diagnózou atopické dermatitidy, z toho 54,6 % mužů. V roce 2021 byl průměrný věk pacientů 38,5 roku. V době diagnózy onemocnění byl průměrný věk 5,1 roku a při nasazení první biologické léčby 37,7 roku. Průměrná doba od diagnózy do zahájení první biologické léčby byla 32,6 roku. Ke konci roku 2021 byli pacienti léčeni biologickou léčbou v průměru 1,1 roku. Souběžnou alergickou rýmu mělo 67,5 % pacientů, potravinovou alergii 57,3 % a alergické astma 52,4 %. Oční postižení se vyskytovalo u 29,5 % pacientů, nejčastější byla atopická konjunktivitida (15,4 %). Průměrné EASI při zahájení léčby a na poslední návštěvě bylo 32,9, resp. 4,3 a DLQI 18,5, resp. 3,7. V roce 2021 biologickou léčbu zahájilo celkem 173 pacientů. Na konci sledovaného roku bylo léčeno celkem 382 pacientů a průměrná délka léčebné série byla 1,1 roku. Závěr: BIOREP je prvním registrem pacientů biologické léčby psoriázy v zemích střední a východní Evropy. Výsledky při porovnání s evropskými registry ukazují srovnatelnou nebo vyšší prevalenci souběžných onemocnění a rizikových faktorů a dlouhé období nedostatečné léčby před nasazením biologické léčby.

The mean age at the time of diagnosis was 33.3 years and the age at first biological treatment was 42.7 years. The average time from diagnosis to the start of the biological treatment was 9.5 years and patients were treated for an average of 2.1 years. In 2021, a total of 73 patients started the biological treatment. At the end of 2021, a total of 281 patients were treated and the average duration of treatment was 2.1 years. At the end of 2021, there were a total of 403 patients with atopic dermatitis in the registry of mean age 38.5 years, 54.6% were men. The mean age at the time of diagnosis was 5.1 years and the age at first biological treatment was 37.7 years. The average time from diagnosis to the start of the biological treatment was 32.6 years and patients were treated for an average of 1.1 years. A total of 67.5% of patients had concomitant allergic rhinitis, 57.3% food allergy and 52.4% allergic asthma. Ocular comorbidities occurred in 29.5% of patients, the most common were atopic conjunctivitis (15.4%). At the start of treatment and at the last visit, the mean EASI was 32.9 and 4.3, respectively and DLQI 18.5 and 3.7 respectively. In 2021, a total of 173 patients started the biological treatment. At the end of 2021, a total of 382 patients were treated and the average duration of treatment was 1.1 years. Conclusion: BIOREP is the first registry of patients with psoriasis treated with biologics in Central and Eastern Europe. Our results, when compared with European registries, show a comparable or higher prevalence of comorbidities, risk factors and a long period of inadequate treatment before starting biological therapy.

Hidradenitis suppurativa (HS) je zánětlivé chronické recidivující onemocnění projevující se tvorbou zánětlivých bolestivých nodulů, abscesů a píštělí, které se následně mohou jizvit. HS se nejčastěji objevuje v oblasti axil, třísel, pod prsy, v oblasti perinea a gluteálně (anogenitální oblast), méně často mohou být postiženy atypické lokalizace, např. krk a obličej, jako je tomu u popisovaného případu. HS má výrazně negativní vliv na kvalitu života, zvláště pokud je soustředěna ve viditelných lokalizacích. Při nedostatečném efektu dlouhodobé celkové antibiotické terapie, velmi často užívané v kombinaci s topickou a chirurgickou léčbou, lze pacientům splňujícím kritéria nabídnout biologickou léčbu. Zlatým standardem biologické terapie je adalimumab, u kterého lze využít i biosimilární přípravky. U popisovaného případu tato terapie vedla k dlouhodobé remisi zánětlivých projevů onemocnění.
Hidradenitis suppurativa (HS) is an inflammatory chronic recurrent disease manifested by the formation of inflammatory painful nodules, abscesses and fistulas, which can lead to scarring. HS most often occurs in the axillae, groin, under the breast, perineum and gluteal region (anogenital area), but less frequently, atypical sites such as the neck and face may be affected as in the case described. HS has a significant negative impact on the quality of life, especially if it is located in visible locations. In case of the insufficient efficacy of long‐term systemic antibiotic therapy, very often used in combination with topical and surgical treatment, biological treatment can be considered. The gold standard of biological therapy is adalimumab, for which biosimilar preparations can also be used. In the case described, this therapy led to a long‐term remission of the inflammatory manifestations of the disease.

Hidradenitis suppurativa je chronické, recidivující onemocnění s přítomností zánětlivých nodulů, abscesů, píštělí a jizvením, nejčastěji v místech kožních záhybů, kde se vyskytují apokrinní žlázy. Manifestuje se zpravidla po pubertě a významně ovlivňuje kvalitu života. Prevalence choroby se celosvětově různí, ale pohybuje se kolem 1 %. Přesná etiopatogeneze není známa, primární je dysregulace diferenciace epitelu vlasového folikulu, která vede k jeho hyperkeratóze a okluzi s následným rozšířením až rupturou infundibula, což vyvolá rozsáhlou zánětlivou reakci v okolí. Mezi nejvýznamnější rizikové faktory a potenciální spouštěče choroby se řadí obezita a kouření. Pacienti s hidradenitis suppurativa mají oproti zdravé populaci vyšší riziko kardiovaskulárních chorob, idiopatických střevních zánětů, psychiatrických a revmatických komorbidit. Léčba závisí na stadiu onemocnění a v mnohých případech se kombinují lokální antiseptika a antibiotická externa s celkovou léčbou antibiotiky, kortikosteroidy, retinoidy či biologickými léčebnými přípravky. Na to navazuje léčba chirurgická.
Hidradenitis suppurativa is a chronic, recurrent disease with the presence of inflammatory nodules, abscesses, fistulas, and scarring, most often in the areas of skin folds, where apocrine glands are located. It usually manifests after puberty and significantly affects the quality of life. The prevalence of the disease varies worldwide but is around 1%. The exact etiopathogenesis is not known, the primary is dysregulation of hair follicle epithelial differentiation, which leads to its hyperkeratosis and occlusion with subsequent enlargement and rupture of the infundibulum, which causes a widespread inflammatory reaction in the surrounding tissue. Obesity and smoking are among the most important risk factors and potential triggers of the disease. Patients with hidradenitis suppurativa have a higher risk of cardiovascular and inflammatory bowel disease, psychiatric and rheumatologic disorders, compared to healthy population. Treatment depends on the stage of hidradenitis suppurativa and in many cases topical antiseptics and external antibiotic are used in combination with systemic antibiotics, corticosteroids, retinoids or biologic therapy. This is followed by surgical treatment.
