Asistovaná reprodukcia je pomerne rýchlo sa rozvíjajúci odbor medicíny, ktorý poskytuje liečbu neplodným párom. Od narodenia prvého dieťaťa zo „skúmavky“ uplynulo viac než 45 rokov, za ktoré obdobie boli vyvinuté rôzne metodiky za účelom zvýšenia úspešnosti liečby. Samotná spontánna koncepcia je podmienená mnohými faktormi, o ktorých máme aj v súčasnosti pomerne málo informácií. Vývoj asistovanej reprodukcie postupne odhaľuje jednotlivé významné procesy a stavy ktoré ovplyvňujú vznik a vývoj úspešnej tehotnosti. Diabetes mellitus a s ním spojené metabolické poruchy majú negatívny vplyv na kvalitu pohlavných buniek, fertilizáciu, nidáciu a vývoj tehotnosti. Identifikácia a úspešná liečba diabetu má preto dôležitý význam pred zahájením liečby neplodnosti.
Assisted reproduction is a relatively rapidly developing field of medicine focused on the treatment of infertile couples. More than 45 years have passed since the birth of the first “test- tube baby”. Since that time various methods were developed to increase the success rate of the treatment. Spontaneous conception itself is conditioned by many factors, about which we still have relatively little information. The development of assisted reproduction techniques gradually reveals important processes and conditions that influence the development of a successful pregnancy. Diabetes mellitus and associated metabolic disorders have a negative impact on the quality of gametes, fertilization as well as the development of pregnancy. Identification and successful treatment of diabetes plays a key role before starting infertility treatment.

This meta-analysis of randomized controlled trials (RCTs) was performed to summarize the effects of probiotics, prebiotics, and synbiotics on insulin resistance (IR), lipid profiles, anthropometric indices, and C-reactive protein (CRP) level for polycystic ovary syndrome (PCOS). We searched 8 databases from their inception until 1st October, 2020. The effect sizes were expressed as standardized mean difference (SMD) with 95% confidence intervals (95% CI). Subgroup analyses were undertaken for further identification of effects of probiotics, prebiotics, and synbiotics, based on the following aspects: (1) type of intervention (probiotics, prebiotics, or synbiotics); (2) study duration (≥ 12 weeks or < 12 weeks); (3) number of probiotic strains (multi strains or single strain); (4) probiotic dose (≥ 2 × 108 colony-forming units [CFU] or < 2 × 108 CFU). A total of 17 eligible RCTs with 1049 participants were included. Results showed that probiotic, prebiotic, and synbiotic intake decreased fasting plasma glucose (SMD, -1.35; 95% CI, -2.22 to -0.49; p = 0.002), fasting insulin (SMD, -0.68; 95% CI, -1.08 to -0.27; p = 0.001), homeostatic model of assessment for IR (SMD, -0.73; 95% CI, -1.15 to -0.31; p = 0.001), triglycerides (SMD, -0.85; 95% CI, -1.59 to -0.11; p = 0.024), total cholesterol (SMD, -1.09; 95% CI, -1.98 to -0.21; p = 0.015), low-density lipoprotein cholesterol (SMD, -0.84; 95% CI, -1.64 to -0.03; p = 0.041), very-low-density lipoprotein cholesterol (SMD, -0.44; 95% CI, -0.70 to -0.18; p = 0.001), and increased quantitative insulin sensitivity check index (SMD, 2.00; 95% CI, - 0.79 to 3.22; p = 0.001). However, probiotic, prebiotic, and synbiotic supplements did not affect anthropometric indices, high-density lipoprotein cholesterol, and CRP levels. Subgroup analysis showed that probiotic or prebiotic might be the optimal choice for ameliorating IR or lipid profiles, respectively. Additionally, the effect was positively related to courses and therapeutical dose. Overall, the meta-analysis demonstrates that probiotic, prebiotic, or synbiotic administration is an effective and safe intervention for modifying IR and lipid profiles.

Cíl studie: Zanalyzovat prenatální diagnostiku, management a výsledky těhotenství u plodů s ovariální cystou a porovnat naše výsledky s údaji v zahraniční literatuře. Metody: Retrospektivní analýza zahrnující plody s diagnostikovanou ovariální cystou v letech 2017–2021 na našem pracovišti. Byly hodnoceny velikost a charakter zobrazení cysty ve vztahu k výsledku těhotenství a pre- a postnatálnímu managementu. Výsledky: V letech 2017–2021 bylo na našem pracovišti vyšetřeno 5420 těhotenství a diagnostikováno 7 ovariálních cyst. Cysty byly rozděleny na menší a větší než 4 cm a jednoduché a komplexní. Tři cysty byly větší než 4 cm, 4 cysty menší než 4 cm, 2 cysty byly komplexní, zbývajících 5 jednoduchých. Prenatálně byl u všech cyst volen konzervativní management. Čtyři cysty (57 %) vymizely v průběhu těhotenství, 1 po porodu (14 %). Ve 2 (29 %) případech byla nutná postnatálně chirurgická intervence, 1× aspirace cysty a 1× ovarektomie. Obě cysty s nutností postnatální intervence byly ≥ 4 cm, jedna jednoduchá a jedna komplexní. Závěr: Ovariální cysty jsou vrozenou vadou s dobrou prognózou a většina z nich spontánně vymizí ještě v průběhu těhotenství či postnatálně. Velikost a typ cysty jsou v přímém vztahu s pravděpodobností vymizení cysty. Náš management a výsledky odpovídají údajům v zahraniční literatuře.
Objective: To analyse prenatal diagnosis, management, and pregnancy outcomes in fetuses with an ovarian cyst and compare our results with foreign literature. Methods: Retrospective analysis including fetal ovarian cysts diagnosed in the years 2017-2021 in our centre. The size and appearance of the cyst in relation to pregnancy outcome and pre- and post-natal management were evaluated. Results: In the years 2017-2021, we have examined 5 420 pregnancies and 7 ovarian cysts were detected. Cysts were divided into smaller and larger than 4 cm and simple and complex. 3 cysts were ≥ 40 mm, 4 cysts < 4 cm, 2 cysts were complex, the remaining 5 were simple. Prenatally, conservative management was chosen for all cysts. 4 cysts (57 %) disappeared during pregnancy, 1 after delivery (14 %), in 2 cases postnatal surgical intervention was necessary – 1× cyst puncture and 1× ovariectomy (28 %). Both cysts requiring postnatal intervention were larger than 4 cm, one simple and one complex. Conclusion: Ovarian cysts are a congenital defect with a good prognosis, majority of them resolves spontaneously during pregnancy or postnatally. The size and type of the cyst are directly related to the likelihood of the cyst disappearing. Our management and results are in agreement with world literature.

Cíl: Shrnutí nových poznatků v oblasti implantace embrya v závislosti na kvalitě endometria. Metodika: Rešerše literatury publikované do srpna 2022 v databázích WoS, Scopus, PubMed/Medline se zaměřením na „endometrial receptivity“, „polycystic ovary syndrome“, „endometriosis“, „SARS-CoV-2“. Výsledky: Receptivní stav endometria je výsledkem fyziologické remodelace, činnosti imunity modulované mikrobiomem. Tuto rovnováhu narušují myomy, polypy, saktosalpingy, adenomyóza, endometrióza, syndrom polycystických vaječníků, infekce. Diskutuje se dopad infekce SARS-CoV-2. Pro úspěšnou implantaci je klíčové časování embryotransferu. Konvenčně se pro tyto účely využívá ultrazvuk. Ve specifických případech hysteroskopie a biopsie endometria. Vzorek se hodnotí histologicky, imunohistochemicky, vyšetřuje se mikrobiom anebo transkriptom. Jako podpora implantace jsou využívány gestageny, u syndromu polycystických ovarií metformin. U opakovaného selhání implantace intrauterinní infuze mononukleárů, plazmy bohaté na destičky, subkutánní aplikace granulocyty stimulujícího růstového faktoru, intravenózní podání atosibanu, intrauterinní aplikace choriogonadotropinu. Závěr: Nejnovější výzkumy na poli transkriptomiky, proteomiky a reprodukční imunologie hlouběji odkrývají implantaci a otvírají novou etapu asistované reprodukce.
Objective: A summary of new knowledge on embryo implantation in dependence on quality of the endometrium. Methods: Literature review from August 2022 of the relevant publications in Web of Science, Scopus and PubMed/Medline databases, focused on “endometrial receptivity”, “polycystic ovary syndrome”, “endometriosis”, “SARS-CoV-2”. Results: The receptive state of the endometrium is a result of physiological remodeling and immune system activity modulated by the microbiome. This balance can be disturbed by myomas, polyps, sactosalpings, adenomyosis, endometriosis, polycystic ovary syndrome, infections. The effect of SARS-CoV-2 infection is being discussed. For a successful implantation, timing of transfer is crucial. The ultrasound examination is used conventionally. In specific cases, hysteroscopy and endometrium biopsy are recommended. Histological and immunohistochemical evaluation is performed together with examination of microbiome or transcriptome. To support the implantation, gestagenes are used, or metformin in the patients with polycystic ovary syndrome. In cases of a repeated implantation failure, the intrauterine infusion of mononuclear cells or platelet rich plasma is used, subcutaneous application of granulocyte colony stimulating growth factor, intravenous application of atosiban or intrauterine application of human chorionic gonadotropin. Conclusion: Recent research in the field of transcriptomics, proteomics and reproductive immunology uncovers the process of implantation more deeply and opens a new stage of the assisted reproduction.

Vitamin D inadequacy is widespread in females with polycystic ovarian syndrome (PCOS), with blood values of 25-hydroxy vitamin D (25OHD) down to or less than 20 ng/ml in two-third of PCOS patients. Decreased 25OHD concentrations have been linked to insulin resistance, ovulatory and period abnormalities, lower pregnancy outcomes, and other indications of PCOS, according to epidemiological studies. There might be some, but inadequate, research that vitamin D supplements can help women with PCOS with menstruation disruption and insulin sensitivity. Vitamin D inadequacy may bestow to the exacerbation of PCOS, and vitamin D intake may be helpful in the treatment of this condition. However, the present evidence is insufficient, and further experimental investigations are in need to establish vitamin D supplementation's potential advantages in this population. Many investigators claimed a pertinency between vitamin D concentrations and PCOS, nevertheless, these researches bear rattle factors of conjoined with other disorders or ailments, such as obesity, diabetes, and hirsutism. In the view of this, the present review detailed the role of vitamin D inadequacy and related biomolecule abnormalities, including sex hormones, and insulin and we listed their psychological and cardiovascular encounter. We concluded that the correlation between vitamin D intake and mended PCOS prominence is dubious and no definite findings attained by studies conducted across multicentral laboratories.

Polycystic ovary syndrome (PCOS) is associated with multiple risk factors for cardiovascular diseases, including insulin resistance, diabetes mellitus type 2, obesity, hypertension, and dyslipidaemia. Many studies have assessed the role of adipokines in the etiopathogenesis of PCOS, however, no single biomarker has been recognized to be in causal relation to the syndrome. Apelin has been identified as a new adipokine linked to obesity and insulin resistance. Some studies demonstrated that the apelin / apelin receptor could play a pivotal role in the pathogenesis of polycystic ovary syndrome, however the other yielded controversial results. Underlying mechanisms of possible involvement of apelin/apelin receptor complex are discussed.
