Clinical trials in oncology are important tools to identify and establish new effective drugs for cancer treatment. Since the development of the concept of precision oncology, a huge number of multi-centric biomarker-driven clinical trials have been performed and promoted by either academic institutions or pharmaceutical companies. In this scenario, the role of pathologists is essential in multiple aspects, with new challenges that should be addressed. In this position paper of the European Society of Pathology, the role of pathologists as contributors to the design of the clinical trial, as local collaborators, or as members of central review laboratories is discussed. Moreover, the paper emphasizes the important role of pathologists in guiding methods and criteria of tissue biomarker testing in the biomarker-driven clinical trials. The paper also addresses issues regarding quality control, training, and the possible role of digital pathology.

Good brain health plays a significant role in an individual's well-being and profoundly impacts the collective economy and society. Brain development does not stop at birth, and some aspects continue throughout childhood and adolescence, allowing the full development of cognitive functions. Different determinants related to physical health, healthy environments, safety and security, life-long learning and social connection as well as access to quality services influence the way our brains develop, adapt and respond to stress and adversity. Ongoing progress in neurobiology and cognitive neuroscience allows the design of better prevention and intervention strategies to help avoid brain deficits and/or limit their impact and maintain brain health. The European Brain Council (EBC) convened an expert meeting during the Federation of European Neuroscience Societies (FENS) Forum 2024 to address youth brain health challenges. In recent years, the importance of brain health has garnered significant attention across scientific, medical and policy-making communities. Although much focus has traditionally been on neurodegenerative conditions affecting the elderly, a paradigm shift towards prioritizing brain health in youth is both timely and necessary. This shift can profoundly impact individual lives and society, necessitating an interdisciplinary approach that brings neuroscience to the forefront of public health and informs evidence-based policy. The topic is of utmost importance as EBC launched this year a new campaign on No Health Without Brain Health rallying support with its member organizations and the wider brain community for the increased prioritization of brain health on EU health and research agendas.
Structural, architectural, contractile, or electrophysiological alterations may occur in the left atrium (LA). The concept of LA cardiopathy is supported by accumulating scientific evidence demonstrating that LA remodelling has become a cornerstone diagnostic and prognostic marker. The structure and the function of the LA and left atrial appendage (LAA), which is an integral part of the LA, are key elements for a better understanding of multiple clinical conditions, most notably atrial fibrillation, cardioembolism, heart failure, and mitral valve diseases. Rational use of various imaging modalities is key to obtain the relevant clinical information. Accordingly, this clinical consensus document from the European Association of Cardiovascular Imaging, in collaboration with the European Heart Rhythm Association, provides comprehensive, up-to-date, and evidence-based guidance to cardiologists and cardiac imagers for the best practice of imaging LA and LAA for the diagnosis, management, and prognostication of the patients.

The history of the Czech and Slovak experimental cardiology describes a completely unusual curve. The personality of J.E. Purkynje caused this field to reach unprecedented peak at the very beginning of its modern history. The development of experimental cardiology after the death of the great scholar was certainly not linear. Just when it seemed to be raising its head, the German occupation came. Its second hopeful awakening was delayed for a long time by forty years of isolation. The significant limitation of foreign contacts gradually led to the loss of hopefully developing contacts, to professional isolation and lagging behind the stormy development of world science. At the moment of greatest depression, in 1971, in Prague there was created a professional forum that was supposed to enable its intellectual survival and reduce the negative consequences of the "splendid isolation". The Society of Experimental Cardiology (SEC) was founded at the Czechoslovak Physiological Society of the Czechoslovak Medical Society J.E. Purkynje, with the main task of introducing theoretical and clinical cardiologists to the advances in world cardiology. The first meeting was held in 1973 and in 2023 we celebrated already the 50th anniversary of SEC. Moreover, nowadays we see the increasing interest of the young researchers, both experimental and clinical cardiologists, who consider SEC a very attractive platform for their education and professional growth. Článek popisuje, jak se v odpověď na celosvětovou epidemii obezity vyvíjela v posledních 40 letech obezitologie jako nový medicínský obor. Tato originální medicínská specializace odráží pokroky v pochopení interakce hereditárních a environmentálních faktorů v patogenezi obezity jako i stěžejní úlohy adipokinů a viscerální tukové tkáně při rozvoji kardiometabolických onemocnění a některých nádorů. Obezitologické společnosti, které vznikly v mnoha zemích po celém světě, pořádaly národní a mezinárodní obezitologické kongresy, připravily doporučené postupy pro léčbu a prevenci obezity a přispěly k vydávání specializovaných obezitologických časopisů. Aby se zlepšily znalosti v obezitologii u poskytovatelů zdravotní péče, byla publikována řada českých i mezinárodních učebnic o obezitě. Česká republika sehrává významnou úlohu v začlenění obezitologických center a redukčních klubů do sítě komplexní léčby obezity. Obezitologie je výjimečná v tom, že vyžaduje sjednocující přístup, který integruje znalosti různých lékařských oborů, například vnitřního a dětského lékařství, endokrinologie, diabetologie, výživy a dietologie, fyziatrie a fyziologie cvičení, psychiatrie, psychologie, bariatrické chirurgie, genetiky, kardiologie, revmatologie a dalších.
The paper describes how obesity medicine has developed as a new medical discipline in response to the global obesity epidemic over the last forty years. This novel medical discipline reflects advances in understanding the interaction between hereditary and environmental factors in the pathogenesis of obesity as well as the crucial role of adipokines and visceral adipose tissue in the development of cardiometabolic diseases and certain cancers. Obesity associations established in many countries across the world have organized national and international congresses on obesity, prepared guidelines on obesity management and prevention, and contributed to the publication of journals focused on obesity. Many Czech and international textbooks on obesity have been published to improve knowledge about obesity among health care providers. The Czech Republic has played an important role in the implementation of obesity management centres and weight reduction clubs into the network of comprehensive obesity management. Obesity medicine is unique in that the required holistic approach integrates knowledge from different medical disciplines such as internal medicine and pediatrics, endocrinology, diabetology, nutrition and dietetics, physiatrics and exercise physiology, psychiatry, psychology, bariatric surgery, genetics, cardiology, rheumatology and more.

The use of temporary mechanical circulatory support (tMCS) in cardiogenic shock patients has increased during the last decades with most management strategies relying on observational studies and expert opinion, including hemodynamic monitoring, device selection, and timing of support institution/duration. In this context, imaging has a pivotal role throughout the patient pathway, from identification to initiation, monitoring, and weaning. This manuscript summarizes the consensus of an expert panel from the European Society of Cardiology Association for Acute CardioVascular Care, the European Association of CardioVascular Imaging, and the European Extracorporeal Life Support Organization, providing the rationale for and practical guidance of imaging to tMCS based on existing evidence and consensus on best current practice.
